Vynález se týká bariatrického implantátu opatřeného stentovou částí a určeného k léčbě morbidní obezity. Bariatrický implantát je opatřený stentovou částí propojenou s přítlačným diskem a také trubicovou částí z nepropustného materiálu.The present invention relates to a bariatric implant provided with a stent portion for the treatment of morbid obesity. Dosavadní stav technikyBACKGROUND OF THE INVENTION
Doposud známé bariatrické implantáty uplatňují při svém použití pouze jeden účinek, kterým je snižována obezita pacienta. Například řešení podle dokumentu US 2007/0239284 nebo podle WO 2007/142833 využívá pouze tlaku na stěnu žaludku prostřednictvím kroužků nebo stentu. Tento tlak potom u pacienta vyvolává pocit sytostí žaludku. Řešení podle WO 03/094785 nebo US 2007/0293716 využívá zase trubice implantované do dvanáctníku nebo tenkého střeva, která zabraňuje vstřebávání potravy. Dokument US 2011213469 řeší rozšíření pro zabránění pohybu stentu ve spojení s nepropustnou trubicí pro nepropouštění potravy a řeší také provedení samotných stentů ve tvaru balonu. Toto provedení však neřeší dokonale přítlak stentu ke stěně žaludku. Dokument US 2012/0271217 řeší stent o různých průměrech, nahoře opatřený zajišťovacím rozšířením. Tato provedení však neřeší zcela přítlak stentu a omezení přísunu potravy ve dvanáctníku a tenkém střevu. Podobně je tomu v případě dokumentu WO 2011/116025, který řeší nasouvání kuželových stentů a drátěné růžice do trávicího ústroji. Komplikovaným řešením je rovněž řešení podle WO 2009/097582, uvádějící nasouvání vaku s drátěným roztažným prvkem do trávicího ústrojí. Aplikace tohoto provedení je značně náročná. Nasouvání stentů různých průměrů do trávicího ústrojí je známo potom z dokumentu US 2010/0256775. Spis CZ 303466 řeší stent zajištěný ve vrchní části rozšířením a ve spodní části navazující na děrovanou trubici. Tento stent však nelze použít jako bariatrický implantát, neboť zajištění stentové části je pro tento účel nedostatečné a ke splnění svého účelu trubice musí být plně nepropustná, nikoliv děrovaná. Stent je tudíž vhodný jako výztuha tkáně, avšak jako bariatrický implantát je zcela nepoužitelný. Dokument EP 2 386 275 řeší stent o různých průměrech, kde stentová část o menším průměru je uvnitř opatřena tenkým filmem a je opatřen přítlačným prstencem, avšak přítlačná část stentu do žaludku zde zcela řešena není.The prior art bariatric implants exert only one effect in their use, which reduces the obesity of the patient. Similarly, the document WO 2011/116025, which addresses the insertion of conical stents and wire rosettes into the digestive tract. A complicated solution is also a solution according to WO 2009/097582, which puts a bag with a wire extensible element into the digestive tract. Application of this embodiment is very demanding. The insertion of stents of different diameters into the gastrointestinal tract is then known from US 2010/0256775. CZ 303466 solves a stent secured in the upper part by extension and in the lower part connected to the perforated tube. However, this stent cannot be used as a bariatric implant, since securing the stent portion is insufficient for this purpose and the tube must be fully impermeable, not perforated, to fulfill its purpose. The stent is therefore suitable as a tissue reinforcement, but as a bariatric implant it is completely unusable. EP 2 386 275 discloses a stent of different diameters, wherein the smaller diameter stent portion is provided with a thin film inside and is provided with a thrust ring, but the stent thrust portion is not completely addressed here.

Podstata vynálezuSUMMARY OF THE INVENTION
Vytyčeného cíle je dosaženo bariatrickým implantátem podle vynálezu, jehož podstata spočívá v tom, že je tvořen stentovou částí propojenou s přítlačným diskem a propojenou také s trubicovou částí z nepropustného materiálu, přičemž stentová část má oválný tvar. Při zavedení implantátu jeho stentová část je umístěna v žaludku, přičemž je držena přítlačným diskem, přitlačovaným ke stěně, spojující žaludek a dvanáctník a trubicová část spočívá ve dvanáctníku a tenkém střevu. Stentová část přitlačením ke stěně žaludku vyvolává u pacienta pocit nasycení a trubicová část vlivem svojí nepropustnosti neumožňuje vstřebávání potravy ve dvanáctníku a tenkém střevu. Bariatrický implantát je tvořen stentovou částí 2 se vstupním otvorem 1, kde tato síťovina je provedena vzájemně propletenými svazky plastových vláken 7. Stentová část 2 má oválný tvar se vstupním otvorem 1, po jehož obvodu je umístěno extrakční lanko 4 s očkem, které slouží k zachycení a manipulaci s bariatrickým implantátem. Na opačné straně stentové části 2 je proveden přítlačný disk 5 a stentová část 2 přechází do trubicové části 3, na konci opatřené záchytným lankem 6 s očkem, které slouží k uchycení při zavádění implantátu a také k upravení délky ve střevě pacienta. Stentová část 2 a přítlačný disk 5 jsou vytvořeny z polyetyletylketonu a trubicová část 3 je vytvořena z Dacronu. Materiál stentové části 2 obsahuje rentgenokontrastní látku k umožnění viditelnosti na rentgenu. Stentová část 2 je umístěna v žaludku pacienta a trubicová část 3 je zavedena do dvanáctníku a tenkého střeva.
