Example 1
Referring to fig. 1, the embodiment of the present invention provides a foreskin stapler capable of performing suturing first and then cutting, the foreskin stapler includes a housing 1, an ejector pin member 2, a cutter member 3, a handle 5 and an elastic reset member:
the shell 1 defines a containing cavity 100, one side of the shell 1 forms an open end 10 for suturing and girdling operation, and the open end forms an open opening for the suturing nails 7 of the thimble component and the cutter to extend out so as to complete the suturing and girdling operation on the foreskin;
the thimble element 2 is pivoted in the containing cavity 100 of the shell and is provided with a thimble tube 21 and a thimble tube 22 arranged at the rear side of the thimble tube 21, the front end of the thimble tube 21 is arranged towards the open face end 10, and the front end of the thimble tube is used for installing the suturing nail 7;
the cutter element 3 has an annular cutter and a top cutter tube 32 formed at the rear side of the annular cutter, the annular cutter (not shown) is coaxially pivoted in the ejector pin barrel 21, as shown in fig. 1, the top cutter tube 32 is pivoted through the ejector pin tube 22;
the handle 5 penetrates through the shell, and the handle 5 is in transmission connection with the ejector pin tube 22 and the ejector blade tube 32 through a first transmission mechanism and a second transmission mechanism respectively so as to drive the ejector pin tube 22 and the ejector blade tube 32 to move towards the direction of the open end independently, and finally the annular cutter and the ejector pin tube extend out of the shell; and the first and second transmission mechanisms are integrally configured to cause the top needle cylinder 21 to be advanced out of the housing relative to the circular cutter so as to perform a circular cutting operation after completion of the sewing work.
Compared with the existing foreskin anastomat, the foreskin anastomat provided by the invention abandons a transmission circular cutting suture method of cutting firstly and then suturing, adopts a brand-new thought of cutting firstly and then suturing, fundamentally prevents the condition of bleeding caused in the time period between circular cutting and suturing, and effectively prevents the bleeding.
The elastic reset piece is arranged between the handle and the shell, so that the handle can automatically return after the trigger work is completed.
The first transmission mechanism and the second transmission mechanism have various structural realization modes, for example, a transmission gear transmission mode, a connecting rod transmission mode, a mode of combining a gear and a connecting rod and the like can be adopted, and the technical personnel in the field can freely select the transmission gear transmission mode, so long as the transmission gear transmission mode can realize that the annular cutter extends out after the thimble tube finishes sewing work to finish circular cutting work, namely, the annular cutter and the thimble tube are ensured to extend out in order; illustratively, in some embodiments of the present invention, the first transmission mechanism and the second transmission mechanism particularly adopt the following structural forms: the handle 5 has a free end 51 protruding out of the housing, a main section 52 arranged through the housing and a hinged end 53 for hinging with the interior of the housing; the hinged end 53 is in transmission connection with the rear side of the cutter ejecting pipe 32 through a swing arm 8 so as to push the cutter ejecting pipe 32 to move horizontally; in this embodiment, one end of the swing arm 8 is connected to the handle, the other end forms a sliding cavity 81 extending axially, and the ejector blade tube forms a sliding shaft 321 which is axially slidably disposed inside the sliding cavity, and the swing arm is driven to swing by the rotation of the handle, and further the ejector blade tube is driven to move horizontally; specifically, the swing arm is formed with symmetrical sliding cavities 81 on both sides, and correspondingly, sliding shafts 321 are formed on both sides of the cutter-ejecting tube, and the horizontal forward movement of the cutter-ejecting tube is driven by the cooperation of the handle and the swing arm through the sliding cavities and the sliding shafts.
The handle 5 is provided with a first gear section 55 at the front side of the main body section 52 in the shell, the overhead needle tube 22 is provided with a second gear section 221, and the first gear section 55 and the second gear section 221 are in meshing transmission through a first gear 9 and a second gear 11; in this manner, the engagement of the first gear segment 55 with the first gear 9, the engagement of the first gear 9 with the second gear 11, and the engagement of the second gear 11 with the second rack segment 221 triggers the horizontal movement of the overhead needle tube 22 toward the front side;
as described above, since the ejector tube 32 is protruded from the ejector tube 11 and the swing arm 8 is formed at the rear side of the second transmission mechanism (the first and second gear sections 55 and 221, the first and second gears 9 and 11), the annular cutter is not protruded out of the housing when the ejector cylinder 21 is protruded out of the housing for seaming because the advancing speeds thereof are not uniform.
It can be understood that when the annular cutter is pushed, the annular cutter is directly driven to advance by the firing handle, and the moving angle of the annular cutter is equal to the rotating angle of the firing handle; due to the functional requirements of sewing and cutting firstly, when the trigger handle rotates for a certain angle, the position of the annular cutter is in the plane position of the front end of the anastomat main body or a proper distance backwards, and the position of the annular cutter does not exceed the position of the front end; in this embodiment, when the trigger handle is triggered, the circumcision can be completely cut off only if the annular cutter exceeds the front end by 3 mm.
In some preferred embodiments, the second gear 11 engaged with the second gear segment 221 is circumferentially divided into a gear area 111 and a stop area 112, the gear area 111 is used for meshing transmission with the second gear segment 221 and the first gear 9, and the stop area 112 is used for stop cooperation with the second gear segment 221, in this embodiment, the stop area 112 is a circular arc section formed in the circumferential direction of the second gear 11; thus, referring to fig. 1 again, when the suturing nail 7 reaches the front end plane of the stapler, the firing handle continues to rotate, the first gear segment 55 drives the first gear 9 to continue to rotate, the first gear 9 drives the second gear 11 to continue to rotate until the stopping area 112 of the second gear 11 just abuts against the first rack segment 221 of the thimble element 2, that is, the next tooth on the second gear 11 is not engaged and pushed at this time, only the arc surface thereof is tangent and kept, and thereafter the second gear 11 always keeps tangent with the thimble element 2, and continues to give a stable force to the thimble element 2, so that the thimble element does not retreat, and the suturing nail 7 at the front end is continuously subjected to an extrusion force, thereby reducing the occurrence of a bleeding situation.
As shown in fig. 1, the housing 1 includes a horizontal housing 101 disposed horizontally and a vertical housing 102 extending downward relative to the horizontal housing, the handle 5 is disposed at the front side of the vertical housing 102, and the handle 5 has a natural state of forming an angle with the vertical housing and a trigger state of swinging toward the vertical housing;
the left end of the horizontal shell 101 forms an open end 10, and the thimble piece 2 and the cutter piece 3 are horizontally pivoted in the horizontal shell; the right end of the horizontal shell is provided with an opening for the tortoise head seat and the ejector rod to penetrate through the thimble piece and the cutter piece;
according to the foreskin anastomat provided by the invention, the invention provides the following suture modes: the suturing nail 7 and the annular cutter are pushed to move forwards through the firing handle 5, but due to the fact that the advancing speeds of the two parts are different, the suturing nail 7 can contact with the foreskin tissue firstly, and after the suturing nail 7 is in place, the annular cutter can contact with the foreskin tissue again for cutting;
the invention provides the following movement modes:
the manual trigger handle pushes the suturing nail 7 and the annular cutter to move forwards, the trigger handle pushes the gear set to rotate, the gear set is meshed with and pushes the thimble tube, the suturing nail 7 is placed at the front end of the thimble tube, and the suturing nail 7 is pushed to move forwards at the same time; the trigger handle pushes the top knife pipe to move forwards, the front end of the top knife pipe is connected with the annular cutter in a passing mode, and the annular cutter is pushed to move forwards at the same time.
In addition, the whole cutting and suturing motion of the foreskin anastomat is realized by squeezing the handle, and due to the structure of the hand of a human body, the rotation angle of the percussion handle is within 45 degrees, and 35 degrees are selected as a percussion power source.
When the percussion handle pushes the suturing nail 7, the first gear section on the handle drives the first gear to rotate, the first gear drives the second gear to rotate, the second gear drives the thimble tube to rotate through the first gear section, and according to the gear transmission principle, the linear speed of the gear meshing part (the tangent position of the reference circle) is equal.
At this time: the firing handle can be set to have a firing angular velocity W, and the pinion can have an angular velocity WSmallThe moving speed of the staple 7 is VNeedle
According to the size specification selection of the anastomat, the diameter of a reference circle of a big gear of the handle is 30mm, the diameter of a reference circle of a small gear is 10mm, and the same modules are meshed;
the following can be obtained: 30/2W 10/2WSmall=VNeedle
15W=5WSmall=VNeedle
Referring to fig. 2, according to the height requirement of the staple 7 of the present invention, the staple 7 reaches the front end plane position when the pushing distance is 4.5mm, the time taken for pushing the staple 7 is t, and the rotation angle of the firing handle is θ;
Vneedle*t=15W*t=4.5,
Figure BDA0002919017890000051
Therefore, it is not only easy to use
Figure BDA0002919017890000052
To obtain: theta is 17.189 DEG
Furthermore, when the annular cutter is pushed, the annular cutter is directly driven to advance by the firing handle, and the moving angle of the annular cutter is equal to the rotating angle of the firing handle; due to the functional requirements of sewing and cutting firstly, when the firing handle rotates by 17.189 degrees, the position of the annular cutter is at the plane position of the front end of the anastomat main body or a proper distance backwards, and the position of the annular cutter does not exceed the front end position. In the case, when the trigger handle is triggered, the circumcision can be completely cut off only when the annular cutter exceeds the front end by 3mm (the size is not limited);
after the firing handle rotates for 17.189 degrees (the movement distance of the top knife is L at the moment), the firing handle also needs to rotate for an angle theta, when the rotation center is vertical to the central line of the top knife, the angle beside the firing handle is theta 1, the vertical height is H,
from the triangle formula we can get:

Figure BDA0002919017890000054
from (ii) can be obtained: l ═ Htan θ 1 ((c))
Substituting the third step into the first step to obtain:

h decreases as tan θ 1 increases, and reaches a minimum of infinity close to 0 (in which case the actual meaning is not large), and H reaches a maximum of 0

When in this case θ is 35 ° -17.189 ° -17.811 °, H is equal to about 9.338 mm.
Therefore, H should be within 9.338mm to achieve cutting.
According to the suturing device provided by the invention, after the suturing nail 7 reaches the front end plane, the annular cutter cuts, and at the same time, the suturing nail 7 is always in a pressing state, so that blood can be better prevented from flowing out.
