Description
Technical Field
The invention relates to the field of clinical care, in particular to an epidural catheter fixing device.
Background
In order to further standardize the diagnosis and treatment related to labor analgesia, improve the diagnosis and treatment level of labor of a lying-in woman, optimize and perfect the whole management process of labor analgesia in hospitals, improve the quality of perinatal period medical services, and gradually popularize the painless labor technology in China by the national Weijian committee of health care. According to the multi-center survey of 9 months in 2019, 76.3% of hospitals develop intraspinal delivery analgesia, wherein the intraspinal delivery analgesia rate is more than or equal to 50% and accounts for 40.9%. The epidural delivery analgesia technology is the safest and most reliable analgesia method at present. Epidural catheter displacement is defined clinically as: move inwards >1cm and outwards >2 cm. The epidural catheter displacement sometimes occurs in the puerpera due to the reasons of more movement, easy sweating, fixed waist fetal heart monitoring and the like during childbirth. It was found that the catheter displacement rate in epidural postoperative analgesia was 13.75% and up to 50% in labor analgesia. The epidural catheter displacement not only causes unilateral block or incomplete block to influence the analgesic effect, but also causes total spinal anesthesia in severe cases. Not only increases the risk of the patient, but also reduces the satisfaction of the patient.
In foreign studies, Burns et al compared 3 different catheter fixation modalities in labor analgesia and found that Tegaderm application in combination with epidural catheter filter shoulder fixation was superior to Epi-Fix fixation, but still had a 30% shift rate. Odor et al compared the effectiveness of 3 different epidural catheter securement devices on catheter displacement during epidural analgesia, and found that the LokIt Plus securement device significantly reduced the rate of catheter displacement, but still had about 10% catheter displacement. Chadwick et al use foam suturing to secure catheters with good fixation and reduced catheter displacement rates, but may cause localized red rashes and abscesses, and the necessity for clinical use of anesthesia is considerable.
After the puncture is successful, the catheter at the skin outlet is fixed by a small disc groove arranged in an intraspinal anesthesia bag, and then the disc, the surrounding skin and the catheter are fixed by a 40cm multiplied by 4.7cm non-woven fabric adhesive plaster in the anesthesia bag. However, the epidural catheter is easy to displace due to insufficient viscosity of the non-woven fabric adhesive plaster and small effective fixing area of the non-woven fabric adhesive plaster and the skin. The peridural catheter is added with a cotton pad at the position of the skin, and the catheter is fixed by using adhesive plaster in a waist-hard combined anesthesia bag, so that the incidence rate of catheter displacement is 12%; xiaoshengrong and the like firstly fix a catheter at a skin outlet by using a small groove of an inner disc of an anesthesia bag, then fix the catheter by using a 10cm X11.5 cm HP transparent semipermeable membrane dressing and a non-woven fabric adhesive plaster produced by 3M company, tightly screw an epidural catheter and an analgesic pump connector and then fix the epidural catheter to a patient part by using an indwelling needle application, and the catheter does not shift; the Licheng female adopts a skin open-circuit needle head in an epidural puncture bag to penetrate through the center of the band-aid, an epidural catheter is led out from a needle eye, the band-aid slides to a puncture part along the catheter and is stuck on the skin, a transparent sterile surgical film of 7cm multiplied by 11cm is stuck outside the band-aid and the epidural catheter, finally, a long adhesive plaster with the width of about 3cm is used for fixing the catheter on the back of a patient, and the displacement occurrence rate of the catheter is 4%; the disposable air-permeable plaster equipped in the waist-hard combined anesthesia bag is fixed from the puncture point vertical to the vertebral column to the right or left to the horizontal plane to the navel, and the displacement incidence rate of the catheter is 1.3%. However, the above studies are directed to postoperative epidural pain relief, and there is no study on epidural catheter fixation in delivery pain relief.
