FIG. 1 illustrates a flow diagram of an assessment plan generation method according to some embodiments of the invention. The evaluation plan generating method in the exemplary embodiment of fig. 1 may be executed on a client side or a server side, and may include steps S110 to S130, which will be described in detail below.
Referring to fig. 1, in step S110, evaluation data of each item to be evaluated is acquired from the current evaluation result of the target object.
In an example embodiment, taking an endowment community as an example, the target object is a target resident or a target patient who enters the endowment community, after the target resident or the target patient enters the endowment community, the medical staff evaluates the target resident, and obtains current evaluation data, such as an evaluation score, of each item to be evaluated from the content of an evaluation scale of the target resident, such as a fall evaluation scale, a pressure sore evaluation scale and the like, wherein the items to be evaluated may include a fall risk evaluation item, a pressure sore risk evaluation item, a venous thrombosis risk evaluation item and the like. In addition, when the situation of the residents changes, for example, the residents tend to fall, the residents are evaluated in real time, and the evaluation data of each to-be-evaluated item of the residents are acquired according to the evaluated evaluation scale.
It should be noted that, in the example embodiment of the present invention, the item to be evaluated may further include: the evaluation items of daily life ability, medication condition, fall rehabilitation condition and the like are not particularly limited in the present invention.
Further, in the exemplary embodiment, the evaluation data of the item to be evaluated of the target object is the evaluation score of the evaluation item, but the exemplary embodiment of the present invention is not limited thereto, and for example, the evaluation data of the item to be evaluated may further include data of gender, age, and the like of the target object, which is also within the scope of the present invention.
In step S120, the evaluation data of the item to be evaluated is matched with a preset evaluation plan rule corresponding to the item to be evaluated.
In an example embodiment, the evaluation data, for example, the evaluation score of the to-be-evaluated item of the target object is matched with the preset evaluation plan rule corresponding to the to-be-evaluated item, for example, if the preset evaluation plan rule of the pressure sore risk evaluation item is that "if the total score of pressure sore risk evaluation is greater than 18 scores, then pressure sore risk evaluation is performed once after three months", and if the evaluation score of the pressure sore risk evaluation item of the target object is 20 scores, then the evaluation data of the pressure sore risk evaluation item of the target object is matched with the preset evaluation plan rule evaluation; and if the evaluation score of the pressure sore risk evaluation item of the target object is 15 points, the evaluation data of the pressure sore risk evaluation item of the target object is not matched with the preset evaluation plan rule evaluation.
In step S130, a subsequent evaluation plan of each of the items to be evaluated of the target object is generated based on the matching result.
In an example embodiment, a subsequent evaluation plan of the target object is determined based on a matching result of evaluation data of each item to be evaluated of the target object and a preset evaluation plan rule. Further, a subsequent evaluation date of the corresponding item to be evaluated is determined based on the matched preset evaluation plan rule, and a subsequent evaluation plan of the item to be evaluated of the target object is generated based on the subsequent evaluation date and the matched preset evaluation plan rule. For example, if the preset evaluation rule matched by the fall risk evaluation item of the target object is 30 days later for fall risk evaluation, the preset evaluation rule matched by the pressure sore risk evaluation item is 90 days later for pressure sore risk evaluation, and the preset evaluation rule matched by the venous thrombus risk evaluation item is 15 days later for venous thrombus risk evaluation, the evaluation plan of the to-be-evaluated item of the target object may be as shown in table 1 below:
TABLE 1 evaluation plan of target object
Next evaluation scale Next evaluation interval
Fall risk assessment 30 days
Pressure sore risk assessment 90 days
Venous thrombosis risk assessment 30 days
According to the evaluation plan generating method in the exemplary embodiment of fig. 1, on one hand, the evaluation data of the item to be evaluated of the target object is matched with the corresponding preset evaluation plan rule, and the corresponding evaluation plan rule can be automatically matched according to the evaluation data of the target object; on the other hand, a subsequent evaluation plan of the item to be evaluated of the target object is generated based on the matching result, and the evaluation plan of the target object can be generated in a standardized and automatic manner, so that the labor cost can be reduced; on the other hand, the subsequent evaluation plan of the target object can be automatically generated, so that the evaluation plan of the target object can be dynamically adjusted, and the nursing quality is favorably improved.
Further, in an example embodiment, a preset evaluation plan rule corresponding to an item to be evaluated in the rule engine is determined based on the identification information of the item to be evaluated. For example, a plurality of preset evaluation plan rules may be stored in the rule base, and the preset evaluation plan rule corresponding to the item to be evaluated may be searched from the rule base according to the identification information, such as a name or an index, of the item to be evaluated. And matching the evaluation data of the item to be evaluated with the searched preset evaluation plan rule through the rule engine. The preset fall assessment plan rules in the rule base are shown in table 2 below:
TABLE 2 Fall assessment planning rules
Evaluating result scores Next evaluation scale Next evaluation interval
< 4 points Fall risk assessment 90 days
Not less than 4 points Fall risk assessment 30 days
Not less than 11 points Fall recovery assessment 30 days
The rule engine is an engine that determines whether or not it can execute an action specified in a rule by matching a real-time condition of an operation time with a specified filter condition included in the rule. After the preset evaluation plan rule corresponding to the item to be evaluated is found, the found preset evaluation plan rule is matched according to the evaluation data of the item to be evaluated, such as the evaluation score. For example, if the evaluation score of the pressure sore risk evaluation item of the target object is 20 points, the preset evaluation plan rule of the found pressure sore risk evaluation item is "if the total score of the pressure sore risk evaluation is greater than 18 points, pressure sore risk evaluation is performed once after three months", and the evaluation data of the pressure sore risk evaluation item of the target object is matched with the preset evaluation plan rule evaluation.
In addition, in the example embodiment, when the situation of the target object changes, for example, when it is found that the patient tends to fall when walking, the target object needs to be evaluated in real time, and after the evaluation is completed, since the original evaluation plan of the target object does not adapt to the new situation, the evaluation plan which has the same evaluation content as the evaluation plan of the target object and has an evaluation date after the evaluation date of the evaluation plan of the target object is cancelled. For example, in 2018, 30/9, nurse B finds that resident a tends to fall when walking, performs fall risk assessment for the resident a once in real time, and after the assessment is completed, the total score of fall risk assessment items of the resident a is 13, and automatically generates the next assessment plan for the resident a according to the rule engine: evaluating the fall rehabilitation condition, wherein the planned evaluation date is 10 months and 30 days in 2018; meanwhile, the fall risk assessment of which the assessment date of the previously generated assessment plan is 11, 25 and 2018 is cancelled, and when the assessment plan of the resident nail is inquired, the fall rehabilitation condition assessment of which the date is 10, 30 and 2018 can be inquired.
Further, in order to make the evaluation plan more matched with the actual situation of the target object, in the example embodiment, the priority of each item to be evaluated of the target object is determined, and the evaluation plan of each item to be evaluated of the target object is generated based on the preset evaluation plan rule and the priority matched with each item to be evaluated of the target object. For example, when the evaluation dates of both items to be evaluated are 30 days, the items to be evaluated having a high priority are preferentially evaluated.
Further, in order to generate an evaluation plan of a target object more accurately, priorities of respective items to be evaluated of the target object are determined based on historical evaluation data of the target object. For example, if the score of a certain item to be evaluated, such as a pressure sore risk evaluation item, of the target object is high all the time in history, the priority of the certain item to be evaluated is set to be high, and if the score of the certain item to be evaluated, such as a fall risk evaluation item, is low all the time in history, the priority of the certain item to be evaluated is set to be low.
In addition, in the exemplary embodiment, the evaluation data of the item to be evaluated may include parameters such as age and sex in addition to the evaluation score. And when the item to be evaluated of the target object is matched with the preset evaluation plan rule, matching the evaluation score based on the gender and the age of the target object and the item to be evaluated with the preset evaluation plan rule. For example, when evaluating the medication condition of a target subject, the age of the target subject is 86 years and the sex of the target subject is obtained from evaluation data of the target subject, insulin is contained in the medication, and the matched preset evaluation planning rule is that the age is more than 85 years and insulin is contained in the medication, and the user condition evaluation is performed once after one month.
FIG. 2 illustrates a flow diagram of matching evaluation data to preset evaluation plan rules, according to some embodiments of the invention.
Referring to fig. 2, in step S210, a preset evaluation plan rule set corresponding to the item to be evaluated is obtained based on the identification information of the item to be evaluated.
In an example embodiment, a preset evaluation plan rule set corresponding to each item to be evaluated is stored in a rule base in advance, and an evaluation plan rule set corresponding to an evaluation item object is acquired from the rule base based on identification information of the item to be evaluated. The evaluation plan rules in the preset evaluation plan rule set may be the same evaluation plan rules as the items to be evaluated, or may be evaluation plan rules associated with the items to be evaluated. For example, the preset evaluation plan rule set of fall risk evaluation items may include the following evaluation rules: the falling risk assessment score is more than or equal to 4 points, and the next falling risk assessment is carried out after 30 days; the evaluation score of the fall risk is more than or equal to 11 points, and the evaluation of the fall rehabilitation condition is carried out every 15 days.
In step S220, the evaluation data of the item to be evaluated is matched with the preset evaluation plan rule in the preset evaluation plan rule set.
In an example embodiment, the evaluation data, e.g., the evaluation score, according to the item to be evaluated is matched with the preset evaluation plan rule in the preset evaluation plan rule group. For example, the items to be evaluated are fall risk evaluation items, and the fall risk evaluation items of the target object are matched with the following two evaluation rules when the score of the fall risk evaluation items is 12 minutes: the falling risk assessment score is more than or equal to 4 points, and the next falling risk assessment is carried out after 30 days; the evaluation score of the fall risk is more than or equal to 11 points, and the evaluation of the fall rehabilitation condition is carried out every 15 days.
FIG. 3 shows a flow diagram of an assessment plan generation method according to further embodiments of the present invention.
Referring to fig. 3, in step S310, an evaluation result is submitted after the completion of the care evaluation on the target object, for example, in the case of the endowment community, the target object is a target resident or a target patient who enters the endowment community, after the target resident or the target patient enters the endowment community, the medical staff performs the care evaluation on the target resident, and after the evaluation is completed, the evaluation result is submitted to the server.
In step S320, a next evaluation date of each item to be evaluated of the target object is determined according to the rule engine based on the evaluation result of each item to be evaluated of the target object. For example, a preset evaluation plan rule corresponding to each item to be evaluated is acquired from a rule base of the rule engine, the corresponding evaluation plan rule is matched based on the evaluation score of each item to be evaluated, and the evaluation date of each item to be evaluated is determined based on the matched preset evaluation plan rule.
In step S330, a subsequent evaluation plan for each item to be evaluated of the target object is generated based on the matched preset evaluation plan rule and the evaluation date. For example, on 27 months 8 and 8 in 2018, a nurse a performs fall risk assessment on a resident a in an endowment community, after the assessment is completed, the total score of fall risk assessment items of the resident a is 2, and a next assessment plan is automatically generated for the resident a according to a rule engine: fall risk assessment with planned assessment date of 2018, 11 month, 25 days.
In step S340, when the condition of the target object changes, the target object is re-evaluated, and a next evaluation plan is automatically generated according to the rule engine, and the previously generated evaluation plan is cancelled.
