A laparoscope stitching instrument comprises a stitching needle (1), a main rod (3) and a handle (4), wherein the main rod (3) is a hollow rod, and the tail part of the main rod (3) is fixed on the handle; a first sewing needle fixing hole (31) is formed in the front end face of the main rod (3), a second sewing needle fixing hole (32) is formed in the front end face of the main rod (3), and the second sewing needle fixing hole (32) transversely penetrates through the main rod (3); the main rod (1) is provided with an inner fixed rod (5); the sewing needle (1) is a W-shaped needle, the height of the sewing needle is less than 5mm, and the tail section of the sewing needle is provided with a flat section; a wire wheel arranging groove is formed in the handle (4) and close to the lower portion of the main rod (3), a wire wheel fixing shaft is arranged in the wire wheel arranging groove, and a wire threading hole penetrating through the handle is formed in the wall of the wire wheel arranging groove. Description
Technical Field
The invention relates to a mobile phone instrument for treating abdominal cavity diseases, in particular to a laparoscope suture device which can be used for flexibly performing suture operation in an operation.
Background
Laparoscopes play a significant role in the diagnosis and treatment of abdominal diseases, but the mobility and flexibility of laparoscopic surgery, such as that of the basic suturing of laparotomy, is largely lost. In order to make up for the deficiency, the laparoscope is used for stopping bleeding or closing tissues, simple but dangerous thermal coagulation, electrocoagulation, laser, clips, staples and the like are adopted, but the careless operation can cause severe bleeding or necrosis of surrounding tissues, so that the patient dies. The invention aims to provide a laparoscopic suturing instrument which can flexibly suture and tie in an abdominal cavity.

a first sewing needle fixing hole is formed in the upper axis of the front end face of the main rod, a second sewing needle fixing hole is formed in the side face of the front end of the main rod, and the second sewing needle fixing hole transversely penetrates through the main rod; the main rod is provided with an inner fixed rod, the tail end of the inner fixed rod is provided with a rotary holding head, the tail section of the inner fixed rod is provided with an external thread, and the tail end of the main rod is provided with an internal thread matched with the external thread of the inner fixed rod;
the sewing needle is a W-shaped needle, the height of the sewing needle is less than 5mm, and a flat section is arranged at the tail section of the sewing needle;
the lower part of the handle close to the main rod is provided with a wire wheel arranging groove, a wire wheel fixing shaft is arranged in the wire wheel arranging groove, and the wall of the wire wheel arranging groove is provided with a wire through hole penetrating through the handle.
Furthermore, the insertion port of the second needle fixing hole is in a horn shape, and the fixing port of the second needle fixing hole is slightly larger than the tail diameter of the needle.
Furthermore, a thread guide groove is arranged below a needle eye of the sewing needle, and the height difference between the needle eye and the bottom of the needle is larger than 1 mm.

The invention is beneficial to the flexible entry and exit of the suture needle in the abdominal cavity, and the suture needle in the laparoscopic surgery can be flexibly sutured and knotted in all directions in the body; the handle and the main rod are integrated, various suturing and knotting operations are operated by the handle, the suture thread enters and exits from the front end and the rear end of the suture needle twice, the other end of the suture thread is held in the hand of an operator, and the needed suture thread is a common suture thread needed by a laparotomy, so that the loop technology, the suturing technology, the electrocoagulation technology and the like adopted by the existing laparoscope can be replaced, and a large amount of material cost is saved.

The present invention will be further described with reference to the following examples.
As shown in fig. 1, a laparoscopic suturing instrument comprises a suturing needle 1, a main rod 3 and a handle 4, wherein the main rod 3 is a hollow rod, and the tail of the main rod 3 is fixed on the handle 4; the front end of the main rod 3 is provided with a first sewing needle fixing hole 31, the front end of the main rod 3 is provided with a second sewing needle fixing hole 32, and the second sewing needle fixing hole 32 transversely penetrates through the main rod 3. As shown in fig. 2, the insertion port 321 of the second needle fixing hole 32 is flared, and the fixing port 322 of the second needle fixing hole is slightly larger than the tail diameter of the needle 1, so that the needle 1 can be conveniently inserted into the second needle fixing hole 32 during operation.
The main rod 1 is provided with an inner fixing rod 5, the tail end of the inner fixing rod 5 is provided with a rotary holding head, the inner fixing rod 5 is used for fixing the sewing needle 1, the tail section of the inner fixing rod 5 is provided with an external thread, and the tail end of the main rod 1 is provided with an internal thread matched with the external thread of the inner fixing rod; or the tail end of the main rod 3 is welded and fixed on the handle 4, the handle 4 is provided with a matching hole which is coaxial with the inner hole of the main rod 3 and can accommodate the inner fixing rod 5 to pass through, and the tail end of the matching hole is provided with an internal thread matched with the external thread of the inner fixing rod.
As shown in figure 3, the sewing needle 1 is a W-shaped needle, the height is less than 5mm, and the tail section of the sewing needle is provided with a flat section 11; the flat section 11 is convenient for the inner fixing rod 5 to press the sewing needle 1, fix the sewing needle 1 on the second sewing needle fixing hole 32 and prevent the sewing needle 1 from rotating after being fixed. A thread guide groove is arranged below the needle eye of the sewing needle 1, and the height difference between the needle eye and the bottom of the needle is more than 1 mm; the arrangement of the wire guide groove is convenient for threading the needle eye and the lead; the height difference between the needle eye and the bottom of the needle is convenient for lifting the suture line in the operation process. The thread guide groove extends to the inner side bent position of the sewing needle 1 close to the needle eye, so that the sewing needle 1 can be inserted into the sewing part conveniently, and the influence of the sewing thread on the needle insertion is reduced. In this example, the length of the needle 1 is 3.5cm, the diameter of the needle is 1.1mm, and the eye of the needle is an eye of an 8-gauge needle. In the actual operation of the arc needle in the prior art, human tissues extrude the suture to be tightly attached to the needle body of the suture needle after the needle is inserted, the suture is difficult to lift in the operation process, and the operation difficulty and the operation time are increased. The structure of the W-shaped needle can prevent human tissues from extruding the suture to be tightly attached to the needle body of the suture needle, the suture is easy to lift and pull in the operation process, and the suture time is shortened.
As shown in fig. 1, a wire wheel mounting groove 41 is arranged on the handle 4 and close to the lower part of the main rod 3, a wire wheel fixing shaft 42 is arranged in the wire wheel mounting groove, and a wire threading hole 43 penetrating through the handle is arranged on the wall of the wire wheel mounting groove. When the device is used, a suture is drawn out from the suture wheel, the suture is drawn out of the handle 4 through the threading hole 43, the suture wheel is placed in the thread wheel placing groove 41, the shaft hole of the suture wheel is sleeved on the thread wheel fixing shaft 42, the shaft diameter of the thread wheel fixing shaft 42 is smaller than the aperture of the shaft hole of the suture wheel, and the suture wheel can freely rotate in the thread wheel placing groove 41.
The exhaust and blood discharge hole seat 33 is arranged at the position of the main rod 3 above the handle, the exhaust and blood discharge hole seat 33 is provided with a through hole communicated with the hollow through hole of the main rod, and the exhaust and blood discharge hole seat 33 is provided with a cover cap 34. The exhaust and blood discharge hole seat is convenient for discharging blood gas in the main rod 3.
The sewing and knotting technique of the invention is as follows:
1. entering the abdominal cavity: the sewing needle 1 threaded with the sewing thread is inserted into the first sewing needle fixing hole 31 on the main rod 3, so that the sewing needle 1 and the main rod 3 are in a straight line, after the sewing needle enters the abdominal cavity through a 6-7 mm sleeve, the sewing needle 1 is taken down from the first sewing needle fixing hole 31 through a thread clamp, the sewing needle 1 is inserted into the second sewing needle fixing hole 32, the inner fixing rod 5 is rotated to firmly fix the sewing needle 1 in the second sewing needle fixing hole 32, and the sewing needle 1 and the main rod 3 are in an angle of 90 degrees so as to be beneficial to smooth sewing.
2. And (3) sewing: and (3) holding the suturing device by the right hand to enable the suturing needle 1 to penetrate through the suturing part, then further withdrawing the needle to enable the suture to be exposed from the needle hole by 4-5 mm, holding the thread clamping forceps by the operator with the left hand to jack and clamp the suture at the front end, withdrawing the suturing needle, completing the suturing and threading, and releasing the broken end of the suture by the thread clamping forceps.
3. And (3) knotting: after the suture needle 1 with the thread rotates around the thread clamp for 1 circle (square knot making) or 2 circles (surgical knot making), the thread clamp clamps the broken end of the thread, and the stitching instrument and the thread clamp are correspondingly tensioned to complete one knot. The second or third junction is completed in the same way.
4. Trimming: the stitching instrument and the thread clamp draw the straight thread and the thread scissors cut off. The stitching instrument does not withdraw from the abdominal cavity, and secondary stitching and knotting are carried out.
5. And (3) exiting: the inner fixing rod 5 is unscrewed, the suture needle 1 is taken down from the second suture needle fixing hole 32 through the thread clamp, the suture needle 1 is inserted into the first suture needle fixing hole 31, and then the main rod 3 and the suture needle 1 are drawn out of the abdominal cavity.
The present invention can perform:
1. thread transferring and knotting: when the suture is not sutured, the suture is delivered and knotted by a suturing device, and the broken end of the tissue is ligated. The method is the same as above. Can replace the ferrule technology adopted by the prior laparoscope, and save a large amount of material cost.
2. Intermittent sewing: the sewing and knotting skill is intermittent sewing.
3. 8-shaped sewing: after the stitching device stitches and threads, the suture is knotted without cutting the suture, the second needle stitching and threading are carried out, after the thread clamp passes through the suture at the needle hole, the other broken end of the suture is clamped and pulled out, the stitching needle exits the tissue, the two ends of the suture are knotted, and the 8-shaped stitching is completed.
4. Continuous sewing and chain sewing: the same 8-shaped suture is adopted, but the directions of the entering and exiting suture needles are different.
Claims (6)
1. A laparoscope stitching instrument comprises a stitching needle (1), a main rod (3) and a handle (4), wherein the main rod (3) is a hollow rod, and the tail part of the main rod (3) is fixed on the handle (4); the method is characterized in that:
a first sewing needle fixing hole (31) is formed in the end face of the front end of the main rod (3), a second sewing needle fixing hole (32) is formed in the end face of the front end of the main rod (3), and the second sewing needle fixing hole (32) transversely penetrates through the main rod (3); the main rod (1) is provided with an inner fixed rod (5), the tail end of the inner fixed rod (5) is provided with a rotary holding head, the tail section of the inner fixed rod (5) is provided with an external thread, and the tail end of the main rod (1) is provided with an internal thread matched with the external thread of the inner fixed rod;
the sewing needle (1) is a W-shaped needle, the height is less than 5mm, and the tail section of the sewing needle is provided with a flat section (11);
a wire wheel arranging groove (41) is arranged on the handle (4) and close to the lower part of the main rod (3), a wire wheel fixing shaft (42) is arranged in the wire wheel arranging groove, and a threading hole (43) penetrating through the handle is arranged on the wall of the wire wheel arranging groove.
