发明背景
1.发明领域。本发明总体上涉及医疗装置和方法。更具体地,本发明涉及人工角膜的结构以及用于将植入物插入角膜的方法。
角膜盲从史前时代起就是人类痛苦的一个来源,在圣经中已有记载。在过去的200年里,已经尝试过角膜的合成替代品(“人工角膜”)。遗憾的是,绝大多数这些尝试都以失败告终。人工角膜的常见问题包括眼内感染和植入物挤出。
大多数人工角膜是以贯穿方式植入的,其中人工角膜与眼睛中的房水直接接触。由于用于人工角膜的合成材料无法与周围的角膜组织完全结合在一起,因此来自眼睛表面的细菌可沿人工角膜与周边角膜组织之间的微小开口行进,从而导致眼内感染(眼内炎),而这可能导致丧失眼睛。

人工角膜挤出的第二个潜在原因是角膜生理的干扰。举例而言,一些人工角膜是由例如聚甲基丙烯酸甲酯等对氧和葡萄糖均不可渗透的材料制成。聚甲基丙烯酸甲酯没有可有效测量的透氧性,且因此具有0Barrer的dK。随着时间的推移,角膜组织缺乏充足氧合及营养可导致角膜融化,继而导致挤出。

本发明克服了至少一些上述伴随现有技术人工角膜的问题。本发明的人工角膜被制作用于植入角膜的薄层内,而无需穿透至眼前房中。通过避免这样的穿透,大大降低了眼部感染(眼内炎)的风险。此外,本发明的人工角膜设计得足够柔韧和耐用,以便其在角膜薄层袋进路切口的尺寸小于人工角膜时可通过该小的切口植入该袋中。这样的植入是有利的,这既是因为其进一步抑制了植入物周围的细菌侵入前房内,又是因为其帮助锚固植入物,而这使得装置即使在无缝合线或粘合剂的情况下仍然能够被自身固定。然而,植入物通常将至少比角膜组织稍硬,这将使得植入物在植入之后保持光学上有利的形状。由于哺乳动物角膜的典型杨氏模量在0.2与0.29MPa之间,因此人工角膜的杨氏模量应优选地大于0.29MPa,以便在植入到角膜中之后保留其形状。然而,人工角膜不应过于坚硬以致无法在诸如眨眼等正常生理活动期间与角膜一起弯曲。发明人已经确定,为了利用本发明实现这一点,杨氏模量优选地应不大于100MPa。
现有技术人工角膜不能一致地匹配角膜的天然形状。这是很重要的,因为人工角膜镜片与角膜表面之间的错配会导致临床上显著的问题。在Boston人工角膜的情况中,镜片在载体供体角膜的水平面之上的隆起导致患者持续性的异物感,并且需要连续使用绷带式接触镜来防止眼睑内侧结膜的磨损。另一方面,Alphacor的镜片位于受体角膜的水平面之下300微米,其形成的缺陷上持续累积诸如黏液等碎屑,并因此限制患者的视力改善。图7示出了现有技术人工角膜及本发明的剖面,以及如何将它们装入受体或载体角膜内。
在本发明的特定方面中,使用具有非常精确的尺寸的切口将人工角膜植入角膜中。尺寸的精确度将允许人工角膜完全配合于角膜之中,以使得人工角膜的表面将与人工角膜镜片的表面平齐,并且在镜片与角膜组织之间将不会有间隙。形成这种高精确度水平的角膜切口的能力直到最近利用飞秒激光和机械角膜袋制作器才成为可能。在优选的方面中,将利用飞秒激光来形成角膜切口,该飞秒激光器通常具有约+/-3微米的公差。在备选的优选方面中,还可利用机械角膜袋制作器来形成角膜袋切口,该制作器一般会具有+/-50微米或更好的公差。还可使用手工制作的袋来植入本发明的人工角膜,但是由于人手无法制作具有+/-50微米精确度的切口,因此将不可能保证镜片会与受体角膜的表面平齐。
在优选的方面中,为本发明的人工角膜的镜片形成开口的切口还将精确地匹配于镜片的角度。即,邻接镜片的角膜切口将在+/-30度内、更优选地在+/-10度内与镜片的角度相匹配。例如,如果镜片的侧面与由承垫和镜片的接合所形成的平面成90度的角度,则邻接的角膜切口也应与承垫和镜片的接合所形成的平面成90度。
在备选的优选方面中,将会制作角膜切口以切除具有与人工角膜的三维形状相似的形状的一定体积的角膜组织(图9A)。人工角膜与角膜组织的这种过盈配合或互锁配合可如图9B中所示帮助将装置保留在角膜内。
在本发明的特定方面中,人工角膜具有中心镜片,该中心镜片将被加工至非常精密的公差以将预选镜片高度保持在±50μm或更小的公差内。由于50μm是角膜之上的上皮的平均厚度,且角膜上皮将能够通过作出减薄或增厚的响应以抵消人工角膜的表面水平面与天然角膜的表面水平面之间的任何差异。因此,通过将角膜袋的深度仔细控制在相似的或更精密的公差内,可使人工角膜的镜片高度与天然角膜的高度相匹配,以保护眼睛和人工角膜上的泪膜并提高患者舒适度。镜片高度通常将保持在200μm至400μm之间,以允许足够厚度的天然角膜组织覆盖人工角膜的承垫,从而降低侵蚀穿过组织的风险。在角膜厚度异常——如常见于因内皮衰竭而患有角膜水肿的患者身上的情况中,镜片高度可高达800微米以补偿这种增加的厚度。
因此,根据本发明,可逆变形的人工角膜包括单片体,该单片体具有由环形承垫所包围的中心镜片。所谓“单片”,是指中心镜片和承垫是无接缝、接头等的,单一的材料连续体。例如,人工角膜可由单一的材料坯或块来形成,该材料通常为一类常用于形成人工晶状体(IOL)的聚合物水凝胶,优选地为甲基丙烯酸羟乙酯和甲基丙烯酸甲酯的共聚物,诸如可从Contamac Ltd.,Essex,U.K.购得的C126,或者诸如可从Benz Research购得的疏水性丙烯酸材料。聚合物水凝胶材料还可同时具有疏水和亲水性质,诸如经受过等离子体表面处理的甲基丙烯酸羟乙酯和甲基丙烯酸甲酯的共聚物。备选地,人工角膜可由包含互穿网络或胶原基水凝胶的材料经模塑、加工或激光切割制得。
当水合时,单片体将具有4mm至10mm范围内的直径。中心镜片将具有3mm至9mm范围内的直径,以及200μm至800μm范围内的镜片高度(D,图4)。镜片高度的制造公差将会为+/-50微米或更小,以允许精确配合于周围的受体角膜组织。环形承垫将具有0.5mm至4.5mm范围内的环宽度,以及从50μm至200μm范围内的中位厚度。优选地,将选择聚合物水凝胶使其在充分水合时具有0.3MPa至100MPa范围内的模量。优选地,抗拉强度应至少为1.5MPa,且断裂伸长率应至少为100%。合适的主体材料应至少部分地可渗透氧,通常具有至少3Barrer的透氧性(dK)。具有优良透氧性(例口,至少60的dK)的示例性材料包括:Lotrafilcon A、Lotrafilcon B、BalafilconA、Comfilcon A、Senofilcon A、Enfilcon A和Galyfilcon A。
在示例性实施方式中,角膜植入物的环形承垫外接中心镜片的后缘。此外,中心镜片的前表面通常呈凸形,以在植入角膜中时提供大体上等于天然角膜或与之一致的屈光力,该屈光力通常在30屈光度至70屈光度的范围内。通常,中心镜片的前表面将呈凸形,而后表面将呈凹形。后部镜片和承垫的曲率半径通常将与天然角膜的曲率范围一致,在6.2mm至10mm的范围内。
在本发明的其他特定方面中,环形承垫将具有多个孔洞,以允许营养物和氧经此通过。由于承垫将被植入角膜的相邻层状表面之间,因此营养物能够经此通过以保持角膜组织的健康是很重要的。通常,孔洞将会占据从10%至90%的承垫环面积,通常占据该面积的约33%。在示例性实施方式中,孔洞是绕环形承垫均匀安设的圆孔,但它们亦可采取一些其他几何形状,诸如环形承垫的外缘之中的锯齿状物。
在本发明的另一方面中,用于在角膜中植入人工角膜以替代角膜的受损中心区域的方法包括:在角膜中形成中前部开口,该开口具有被周边侧壁所包围的后表面。该开口将优选地具有均匀的深度,通常在200μm至800μm的范围内,其中将选择该深度使之与植入物的中心镜片周壁的高度相匹配,优选地在±50μm内。将人工角膜植入该中前部开口内,以使得中心镜片的周边厚度或壁高将会与中前部开口的周边侧壁匹配至±50μm的公差内,从而提供以上讨论的优点。
在所述方法的特定实施方式中,除了形成中前部开口之外,将会在中前部开口的周边侧壁的至少一部分形成层状袋,并且将会把植入物的承垫部分插入该层状袋中,以便将植入物锚固在开口中。通常,层状袋将完全外接中前部开口,且环形承垫将环绕植入物完全延伸。在其他示例性实施方式中,在中前部开口的前表面的周缘周围形成层状袋,并且将进入层状袋的环形承垫安设在角膜植入物的中心镜片的后缘周围。
在示例性实施方式中,形成具有比中心镜片的直径更小的直径的中前部开口,该直径通常为中心镜片直径的70%-99%,使得部分具弹性的角膜组织可环绕植入物周壁紧密地密封以在移除缝合线之后帮助防止植入物的挤出,抑制上皮细胞的向内生长,抑制细菌的进入,以及防止从前房损失流体。人工角膜可通过两种不同技术中之一植入中前部开口内。在第一种技术中,约束人工角膜以减小其宽度,并在后侧方向上通过前中部开口的上表面将其引入。在中前部开口内可将人工角膜从约束中释放开来,以便使其呈现其不受约束的几何形状从而占据中心开口的体积,通常伴随着将环形承垫插入层状袋中。备选地,可从眼睛的侧面形成通入中前部开口的单独的横向开口,并且经此引入受约束的人工角膜。
在特定的实施方式中,本发明的人工角膜将适合于支持活角膜上皮在镜片前面的周缘上的生长。在周边前表面上形成活上皮将会有利地在镜片前面的边缘周围提供生物密封,从而防止细菌通过抬高的镜片与角膜基质的接合处进入角膜袋。优选地,镜片的中心将会在植入之后保持无角膜内皮,这将允许镜片主体的中心面(其对于光学性能十分关键)即使在患者的眼睛无法形成光学上良好的平滑上皮时仍保持光学上的光滑。在优选的实施方式中,患者的角膜上皮将能够以范围从0.1mm至1mm内的宽度向镜片的前面的周缘上生长。
促进活角膜上皮在前部镜片周缘上的生长可通过在镜片前面的周缘上涂覆或共价键合(通常在上述范围的宽度内)某些促进这种生长的生物分子(诸如细胞外基质蛋白或生长因子)而得以实现。合适的生物分子包括胶原蛋白、纤连蛋白、层粘连蛋白、纤连蛋白粘附促进肽序列(H-trp-gln-pro-pro-arg-ala-arg-ile-OH)(FAP)和表皮生长因子。在其他优选方面中,可将镜片的周缘制成多孔或粗糙质地,以允许角膜上皮细胞更容易地结合至前镜片面的周缘表面。
在不经上述特殊表面处理的情况下,许多可用于制造人工角膜的材料一般将不支持角膜上皮细胞的生长。在这样的情况下,可由周缘经处理的此类非促生长型材料来形成人工角膜以促进生长。在由诸如胶原蛋白或胶原蛋白衍生物等固有地支持上皮生长的材料形成人工角膜的情况下,可在中心镜片表面上涂覆将不支持上皮生长的聚合物,例如硅氧烷或甲基丙烯酸酯,以便保持中心镜片表面无上皮。

具体实施方式
参照图1-图3,根据本发明原理的人工角膜10包括由环形承垫14所包围的中心镜片12。完全穿过环形承垫形成有多个孔洞16,通常为圆孔,以允许在植入之后让营养物经此通过。如图所示,孔洞16包含环形承垫14的总面积的约33%,但是由孔洞所提供的总敞开面积或总空洞面积可以是总面积的10%至90%范围内的任何值。
人工角膜10形成为单片结构,即,不具有接缝或接头且优选地由单一的材料坯或块形成的结构。例如,人工角膜10可由诸如上述甲基丙烯酸羟乙酯和甲基丙烯酸甲酯的共聚物之类的合适的水凝胶聚合物块(当其充分脱水以允许加工时)加工而成。可使用常用于制造人工晶状体的技术对材料进行加工。示例性加工技术利用旋转式金刚石车床,诸如由Benz Research and Development制造和销售的旋转式金刚石车床。一旦加工出人工角膜10,就可将其抛光、清洗、水合及灭菌,以供后续使用和植入。
任选地,可以改变(或保持不改变)中心镜片的前表面的周缘30,以支持宽度通常在0.1mm至1mm范围内的环形区域上的上皮生长。如在上文的发明内容中所述,当中心镜片材料固有地抑制上皮生长时,可通过涂覆或沉积促上皮生长材料来改变环形区域(以虚线32界定)。使环形区域的质地粗糙或将环形区域制成多孔状也可促进环形区域上的上皮生长。当中心镜片材料固有地促进上皮层时,可用抑制上皮生长的材料,诸如硅氧烷或甲基丙烯酸酯,来涂覆或沉积虚线32内部的中心镜片的前部区域。
在参照图4和图5的以下表1中列出了人工角膜10的尺寸的典型范围和示例值。这些尺寸是针对在其充分水合状态下的人工角膜而给出的:

虽然环形承垫14优选地包括诸如圆孔16之类的孔洞,但还期望至少保持与外周壁22和中心镜片12(图1-图3)相邻的环形承垫的最内侧区域为实心。紧邻镜片的承垫的这一实心部分将会有助于在镜片后面的后部角膜已被切除的情况下(这在后部角膜非常不透明时可能是必要的)防止细菌进入和上皮细胞侵入。优选地,将会保持实心材料的宽度W(图3)在0.25mm至0.75mm的范围内。因此,将会从该实心区域径向向外安设承垫的孔洞或其他敞开区域。
在其他实施方式中,如图8A中所示,围绕中心镜片112的承垫114可以是间断式的,或者可以包含具有空心部分或支架结构的材料。其他变体包括围绕中心镜片212的间断式承垫214(图8B)、围绕中心镜片312的具有伸长切除部的环形承垫314(图8C)以及围绕中心镜片412的支架承垫414(图8D)。
现在参照图6A-图6F,将描述本发明的人工角膜10向角膜C中的植入。如图6A中所示,图示了具有不透明的或其他光学干扰区域O的角膜C,该区域O通常存在于角膜C的中心区域内。如图6B中所示,为了引入人工角膜10,将角膜的中心区域CR切离并移除。切割可通过常规方式来完成,通常使用飞秒激光,可选地与机械环钻相结合,以在矢状方向上切割出圆柱形袋。在植入根据本发明的人工角膜的过程中,仔细地控制袋的深度,尤其是周缘周围的深度是非常重要的。通常将该深度控制在约200μm至400μm,从而在袋的下方为具有500至600um平均厚度的角膜留下足够的角膜后部厚度。在角膜厚度异常的情况下——如常见于因内皮衰竭所致的角膜水肿的情况中那样,袋的深度可高达800um以补偿增加的角膜厚度。如图6B中所示,将形成具有后壁PW的袋。在袋P形成之后,如图6C中所示,在中心袋P的周边基底周围形成环形袋AP。备选地,首先形成层状袋(其中后壁PW和环形袋AP是连续的),随后移除中心区域CR以形成前部开口。将会选择主袋和环形袋的尺寸使之与上述人工角膜10的尺寸相配。例如,如果人工角膜具有表1中所列的优选尺寸,则袋的深度应为200μm,袋的直径应为3.5mm,而在中心袋P周围延伸的环形袋AP的外径应为8.5mm。
如图6D中所示,一旦形成了中心袋P和环形袋AP,就可以以折叠或以其他方式约束人工角膜10,并在矢状方向上将其插入中心袋P。如图6E中所示,随着人工角膜10被插入,可释放约束以使得环形承垫14径向向外张开并进入环形袋AP。继而可任选地将人工角膜12缝合就位,通常是使用可吸收尼龙缝合线。通常,本发明的人工角膜即使在没有缝合线的情况下也将会自保留在角膜内。
备选地,如图6F中所示,可通过为提供通道而形成的横向切口L1引入人工角膜10。例如,当使用共同拥有的美国专利号7,223,275中所描述的袋制作器来形成袋时,可选择使用横向切口。
在图7A中可见,Boston人工角膜的镜片112高出供体载体角膜的表面之上,这将会因磨损患者眼睑的内衬而造成刺激。在图7B中可见,Alphacor的镜片212居于角膜表面之下,这产生了将会累积黏液和碎屑的缺损,因而使患者的视力模糊。在图7C中,本发明的人工角膜的镜片12的边缘与周围角膜处于相同的水平面,此外在周围角膜组织与镜片之间没有间隙,这是因为角膜开口的大小设为稍小于镜片直径,从而提供了适贴配合。还应注意,镜片侧面的角度α与相邻切口的角度相匹配。
虽然以上是本发明优选实施方式的完整描述,但亦可使用各种备选方案、修改方案和等效方案。因此,不应将以上描述视为对本发明范围的限制,该范围是由随附权利要求所限定。

Claims (27)

1.一种可逆变形的人工角膜,包括:
直径为4mm与10mm之间的单片体,该单片体包含由承垫所包围的中心镜片,其中所述中心镜片具有3mm至9mm范围内的直径以及200μm至800μm范围内的镜片高度,并且所述承垫具有0.5mm至4.5mm范围内的宽度以及50μm至200μm范围内的中位厚度;
其中所述单片体包含聚合物水凝胶,该聚合物水凝胶在充分水合时具有0.3MPa至100MPa范围内的杨氏模量。
2.如权利要求1中所述的人工角膜,具有至少1.5MPa的抗拉强度。
3.如权利要求1中所述的人工角膜,具有至少100%的断裂伸长率。
4.如权利要求1中所述的人工角膜,其中所述中心镜片的所述镜片高度变化小于±50μm。
5.如权利要求1中所述的人工角膜,其中所述中心镜片与所述承垫之间的角度变化小于±10°。
6.如权利要求1中所述的人工角膜,其中所述承垫外接所述中心镜片的后缘。

14.如权利要求1中所述的人工角膜,其中所述聚合物水凝胶包含的材料选自由以下各项构成的组:亲水性丙烯酸材料、疏水性丙烯酸材料、硅氧烷聚合物水凝胶、胶原蛋白聚合物水凝胶以及互穿网络水凝胶。
15.如权利要求1中所述的人工角膜,其中所述聚合物水凝胶具有至少3Barrer的透氧性。
16.如权利要求1中所述的人工角膜,其中所述聚合物水凝胶包含的材料选自由以下各项构成的组:Lotrafilcon A、Lotrafilcon B、Balafilcon A、Comfilcon A、Senofilcon A、Enfilcon A和Galyfilcon A。
17.如权利要求1中所述的人工角膜,其中所述中心镜片的前面的环形周边区域促进上皮生长,而位于所述环形周边区域内的所述前面抑制上皮生长。
18.如权利要求17中所述的人工角膜,其中所述环形周边区域覆盖有促上皮生长材料,是粗糙的,或者是多孔的。
19.如权利要求17中所述的人工角膜,其中位于所述环形周边区域内的所述中心镜片的所述前面覆盖有抑制上皮生长的材料。
20.一种用于将人工角膜植入具有受损中心区域的角膜中的方法,所述方法包括:
在所述角膜中形成中前部开口,该中前部开口具有由周边侧壁所包围的后表面,其中所述开口具有200μm至800μm范围内的均匀周边深度;以及
在所述中前部开口中植入人工角膜,其中所述植入物具有中心镜片,该中心镜片具有周壁,该周壁具有在所述中前部开口的周边深度的±50μm内的均匀镜片高度。
21.如权利要求20中所述的方法,还包括:
环绕所述中前部开口的周边侧壁的至少一部分形成层状袋;以及
向所述层状袋中插入外接所述角膜植入物的承垫,以将所述植入物锚固在所述开口中。
22.如权利要求21中所述的方法,其中所述层状袋是环绕所述中前部开口的后表面的周缘和环绕围绕所述角膜植入物的中心镜片的后缘的承垫而形成的。
23.如权利要求20中所述的方法,其中所述中前部开口形成为具有小于所述中心镜片直径的直径,其中所述角膜组织贴靠所述中心镜片的周壁密封,以抑制上皮细胞的向内生长、细菌的进入以及流体的损失。
24.如权利要求20中所述的方法,其中经所述前部开口在矢状方向上插入所述植入物。
25.如权利要求20中所述的方法,还包括形成经角膜进入所述中前部开口的横向通道,以及经所述横向通道插入所述角膜植入物。
26.如权利要求20中所述的方法,其中所述人工角膜在无缝合线或粘合剂的情况下锚固。
27.如权利要求20中所述的方法,其中首先为整个中心角膜制作层状袋,并继而在所述层状袋的区域上形成所述中前部开口。
