Die vorliegende Erfindung betrifft eine Vorrichtung zum Unterbinden und Durchtrennen von Eileitern, die dadurch gekennzeichnet ist, dass sie einen rohrförmigen Teil aufweist, in dem zwei Koagulationszangen und wahlweise eine Betrachtungsoptik mit Lichtquelle, ein elektrochirurgisches Schneidmesser oder eine Biopsiezange eingesetzt bzw. einsetzbar sind und in dem eine in einem Anschlussstutzen mündende Gasleitung zur Erzeugung eines Bauchpneus verläuft, und dass Bedienungsorgane für die Koagulationszangen und das Schneidmesser sowie Anschlüsse für die Zufuhr hochfrequenten Stromes zu den Koagulationszangen und zum elektrochirurgischen Schneidmesser vorgesehen sind.

Die dargestellte, als laparoskopisch-chirurgisches Kombinations-Instrument ausgebildete Vorrichtung weist gemäss Fig. 1 einen rohrförmigen Werkzeugschaft 1 aus Kunststoff auf, dessen hinteres, durch einen Bund 2 (Fig. 11) verstärktes Ende mit einem ringförmigen Führungsstück 3 aus Metall verschraubt ist. Das Führungsstück 3 ist in ein hohlzylindrisches Griffstück 4 aus Kunststoff eingesetzt. Das Griffstück 4 weist zwei voneinander abgekehrte Griffe 5 und 5' auf. Im Hinterteil des hohlzylindrischen Griffstückes 4 ist ein hohlzylindrischer Schieber 6 aus Kunststoff mit Handgriff 7 axial verschiebbar geführt. Der Hinterteil des Schiebers 6 dient als Führung für einen Drücker 8.
Im ringförmigen Führungsstück 3 ist ein Rohr 9 axial verschiebbar geführt, auf dessen hinteres Ende eine Mutter 10 aufgeschraubt ist, welch letztere sich gegen eine Ringschulter 11 des hohlzylindrischen Schiebers 6 abstützt. Das Rohr 9 weist im Bereich des Werkzeugschaftes 1 gegenüber dessen Lichtweite einen kleineren Durchmesser auf, so dass zwischen dem Rohr 9 und der Innenwand des Werkzeugschaftes 1 ein Hohlraum 12 verbleibt. Eine Druckfeder 13 stützt sich einenends gegen das Führungsstück 3 und andernends gegen ein in den Schieber 6 eingesetztes Widerlager 14, so dass durch Nachvorndrücken des Schiebers 6 mittels des Handgriffes 7 entgegen der Wirkung der Druckfeder 13 das Rohr 9 entsprechend nach vorn geschoben wird.
Das Rohr 9 ist mit einem Isolierfutter 15 ausgekleidet und umschliesst eine innere Stange 16. Diese innere Stange 16 durchsetzt die Mutter 10 mit Spiel in einer Bohrung 17 und ist in einem an das innere Ende des Futterrohres 15 anliegenden Isolierring 18 geführt. Das Hinterende der inneren Stange
16 ist mit einem Widerlager 19 verschraubt, auf welches der zylindrische Drücker 8 aus Kunststoff aufgeschoben ist. 20 ist eine Druckfeder, die sich einerseits gegen die Teile 10, 11 des Schiebers 6 und anderseits gegen die Teile 19 des Drückers 8 abstützt. Bei Verschieben des Drückers 8 entgegen der Wirkung der Feder 20 wird die innere Stange 16 nach vorn verschoben.
Das Rohr 9 ist mit zwei aus dem Vorderende des Werkzeugschaftes vorstehenden Koagulationszangen 21 verbunden.
Sowohl die Koagulationszangen 21 wie auch die innere Stange 16 durchsetzen einen vorn im Schaft 1 fest angeordneten Einsatz 9', in dem die Zangen 21 und auch die innere Stange 16 geführt sind. Die Koagulationszangen 21 werden durch Vorschieben des Rohres 9 mittels des Schiebergriffes 7 aus dem Werkzeugschaft 1 heraus in ihre Offenstellung bewegt, wie dies durch die strichpunktierten Linien in Fig. 3 und 4 veranschaulicht ist. Das Schliessen der Koagulationszangen 21 erfolgt unter Freigabe des Griffes 7 durch Zurückfedern des Schiebers 6, wobei die Zangen in den Einsatz 9' eingezogen und dabei entgegen der Wirkung ihrer Eigenfederung geschlossen werden.
Die innere Stange 16 ist mit einem zusammen mit dieser auswechselbaren elektrochirurgischen Schneidmesser 22 verbunden. Letzteres führt beim Betätigen des Drückers 8 entgegen der Wirkung der Druckfeder 20 eine Schneidbewegung nach vorn aus, wie dies durch die strichpunktierten Linien in Fig. 3 und 4 veranschaulicht ist.
Das elektrochirurgische Schneidmesser 22 ist gegen eine (in den Fig. 5 und 6 dargestellte) Biopsiezange 23 zusammen mit einer zugehörigen inneren Stange 16 auswechselbar.
Sowohl das Schneidmesser 22 als auch die Biopsiezange 23 sind zusammen mit der zugehörigen inneren Stange 16 nach Abziehen des Drückers 8 nach vorn herausnehmbar bzw. von vorn her einsetzbar.
Anstelle des Schneidmessers 22 oder der Biopsiezange 23 ist ein mit einer Beleuchtungseinrichtung versehenes optisches Gerät 24 in das Instrument einsetzbar (siehe Fig. 9 und 10).
In Fig. 1 ist 25 ein mit dem Kanal 12 in Verbindung stehender Anschlusstutzen für die Zuführung von CO2-Gas, das zur Erzeugung eines Bauchpneus in die Bauchhöhle eingepumpt wird.
In die Handgriffe 5 und 5' (Fig. 11) des Griffstückes 4 sind Steckerbüchsen 26. 27 eingeschraubt, durch welche zugleich das ringförmige Führungsstück 3 gegenüber dem Griffstück 4 festgelegt ist. Die Steckerbüchse 26 dient für den Anschluss eines Steckers 28 (Fig. 1) für die Zuführung eines hochfrequenten Stromes zu den Koagulationszangen 21.
In die Steckerbüchse 27 ist ein Stecker 29 für die Zuführung eines hochfrequenten Stromes zum elektrochirurgischen Schneidmesser 22 bzw. zur Biopsiezange 23 einschiebbar.
Die Steckerbüchse 27 durchsetzt das Führungsstück 3 und ragt in das Rohr 9. Sie ist im Bereich der Teile 3 und 9 mittels einer Kunststoffhülse 30 elektrisch isoliert. Die auf dem verjüngten Teil 27' der Steckerbüchse 27 angeordnete Kunststoffhülse 30 durchsetzt das Führungsstück 3 in einer Bohrung 31 und das Rohr 9 in einem Längsschlitz 32, durch welchen zugleich der Schiebeweg des Rohres 9 begrenzt ist. In einer Axialbohrung 33 des verjüngten Teiles 27' der Steckerbüchse 27 ist ein Kontaktstift 34 axial verschiebbar, welcher das Futterrohr 15 des Rohres 9 in einem Längsschlitz 35 durchsetzt und durch eine Druckfeder 36 gegen die innere Stange 16 gedrückt wird. Der mittels des Steckers 29 zugeführte Hochfrequenzstrom wird über den Kontaktstift 34 und die innere Stange 16 dem elektro-chirurgischen Schneidmesser 22 bzw. der Biopsiezange 23 zugeführt.
Das Koagulieren und elektrische Schneiden wird mit einer Neutral-Elektrodenplatte durchgeführt, d. h. der eine Pol (Neutral-Elektrodenplatte) wird an die Haut der Patientin angeschlossen, während der zweite Pol (Schneid- oder Koagulationsstrom) über die separaten Anschlüsse 28 und 29 (Fig.
1) den beiden Koagulationszangen 21 bzw. dem Elektro Schneidmesser 22 zugeführt wird. Gemäss einer Variante der Vorrichtung in Fig. 7 und 8 können anstelle der zwei Koagulationszangen 21 mit Neutral-Elektrode zwei voneinander isolierte Koagulationszangen 21' für die bipolare Koagulation eingesetzt werden, d. h. die Neutral-Elektrode wird bei dieser Methode überflüssig. Es wird je ein Pol des Koagulationsstromes über die Anschlüsse 28' und 29' gemäss Fig. 9 den Koagulationszangen 21' (Fig. 7 und 8) zugeführt.
Mit der beschriebenen Vorrichtung kann nach dem Anbringen eines Einstiches durch die Bauchdecke vorerst ein Bauchpneu zur Freigabe des Operationsfeldes erzeugt werden, wobei mit Hilfe der mit der Beleuchtungseinrichtung ausgestatteten Optik 24 eine gute Beobachtung möglich ist. Nun können durch Betätigung des Handgriffes 7 die beiden Koagulationszangen 21 nach vorn geschoben, geöffnet, über den Eileiter geschoben und durch Loslassen des Griffes 7 zurückgezogen und geschlossen werden. Beim Schliessen der Zangen wird der von ihnen erfasste Eileiter an zwei voneinander distanzierten Stellen durch Koagulieren verschlossen.
Nach Einsetzen des Schneidmessers 22 oder der Biopsiezange 23 anstelle der Optik 24 kann bei abermaligem Erfassen des Eileiters mittels der Zangen 21 und Betätigen des Drückers 8 der Eileiter mittels des Schneidmessers 22 zwischen den beiden Verschlussstellen durchgetrennt und gegebenenfalls mittels der Biopsiezange 23 ein Biopsie-Präparat entnommen werden.
The present invention relates to a device for ligating and severing fallopian tubes, which is characterized in that it has a tubular part in which two coagulation forceps and optionally a viewing lens with a light source, an electrosurgical cutting knife or biopsy forceps are inserted or can be used and in which a gas line which opens into a connecting piece runs for generating an abdominal tire, and that operating elements for the coagulation forceps and the cutting knife as well as connections for the supply of high-frequency current to the coagulation forceps and the electrosurgical cutting knife are provided.

The device shown, designed as a laparoscopic-surgical combination instrument, has, according to FIG. 1, a tubular tool shaft 1 made of plastic, the rear end of which, reinforced by a collar 2 (FIG. 11), is screwed to an annular guide piece 3 made of metal. The guide piece 3 is inserted into a hollow cylindrical handle 4 made of plastic. The handle 4 has two handles 5 and 5 'facing away from one another. In the rear part of the hollow-cylindrical handle 4, a hollow-cylindrical slide 6 made of plastic with a handle 7 is guided so as to be axially displaceable. The rear part of the slide 6 serves as a guide for a lever 8.
In the annular guide piece 3, a tube 9 is guided axially displaceably, on the rear end of which a nut 10 is screwed, the latter being supported against an annular shoulder 11 of the hollow cylindrical slide 6. In the area of the tool shank 1, the tube 9 has a smaller diameter than its clearance, so that a cavity 12 remains between the tube 9 and the inner wall of the tool shank 1. A compression spring 13 is supported at one end against the guide piece 3 and at the other end against an abutment 14 inserted into the slide 6, so that by pushing the slide 6 forward by means of the handle 7, the tube 9 is pushed forward accordingly against the action of the compression spring 13.
The pipe 9 is lined with an insulating lining 15 and encloses an inner rod 16. This inner rod 16 penetrates the nut 10 with play in a bore 17 and is guided in an insulating ring 18 resting against the inner end of the casing 15. The rear end of the inner bar
16 is screwed to an abutment 19 on which the cylindrical pusher 8 made of plastic is pushed. 20 is a compression spring which is supported on the one hand against the parts 10, 11 of the slide 6 and on the other hand against the parts 19 of the pusher 8. When the pusher 8 is moved against the action of the spring 20, the inner rod 16 is moved forward.
The tube 9 is connected to two coagulation tongs 21 protruding from the front end of the tool shaft.
Both the coagulation forceps 21 and the inner rod 16 pass through an insert 9 'which is fixedly arranged at the front in the shaft 1 and in which the forceps 21 and also the inner rod 16 are guided. The coagulation forceps 21 are moved into their open position by advancing the tube 9 by means of the slide handle 7 out of the tool shaft 1, as is illustrated by the dash-dotted lines in FIGS. 3 and 4. The coagulation forceps 21 are closed by releasing the handle 7 by the springing back of the slide 6, the forceps being drawn into the insert 9 'and thereby being closed against the effect of their own springiness.
The inner rod 16 is connected to an electrosurgical cutting knife 22 which can be exchanged together with it. When the pusher 8 is actuated, the latter performs a cutting movement forwards against the action of the compression spring 20, as is illustrated by the dash-dotted lines in FIGS. 3 and 4.
The electrosurgical cutting knife 22 can be exchanged for a biopsy forceps 23 (shown in FIGS. 5 and 6) together with an associated inner rod 16.
Both the cutting knife 22 and the biopsy forceps 23 can be removed or inserted from the front together with the associated inner rod 16 after the trigger 8 has been pulled off.
Instead of the cutting knife 22 or the biopsy forceps 23, an optical device 24 provided with an illumination device can be inserted into the instrument (see FIGS. 9 and 10).
In FIG. 1, 25 is a connection piece, which is in communication with the channel 12, for the supply of CO2 gas, which is pumped into the abdominal cavity to generate an abdominal tire.
Sockets 26, 27 are screwed into the handles 5 and 5 '(FIG. 11) of the handle 4, by means of which the annular guide piece 3 is fixed with respect to the handle 4 at the same time. The plug socket 26 is used to connect a plug 28 (FIG. 1) for feeding a high-frequency current to the coagulation forceps 21.
A plug 29 for feeding a high-frequency current to the electrosurgical cutting knife 22 or to the biopsy forceps 23 can be pushed into the socket 27.
The socket 27 penetrates the guide piece 3 and protrudes into the tube 9. It is electrically insulated in the area of the parts 3 and 9 by means of a plastic sleeve 30. The plastic sleeve 30 arranged on the tapered part 27 'of the socket 27 penetrates the guide piece 3 in a bore 31 and the tube 9 in a longitudinal slot 32, through which the sliding path of the tube 9 is limited. In an axial bore 33 of the tapered part 27 'of the socket 27, a contact pin 34 is axially displaceable, which penetrates the casing 15 of the pipe 9 in a longitudinal slot 35 and is pressed against the inner rod 16 by a compression spring 36. The high-frequency current supplied by means of the plug 29 is supplied to the electrosurgical cutting knife 22 or the biopsy forceps 23 via the contact pin 34 and the inner rod 16.
The coagulation and electrical cutting is performed with a neutral electrode plate, i.e. H. one pole (neutral electrode plate) is connected to the patient's skin, while the second pole (cutting or coagulation current) is connected via the separate connections 28 and 29 (Fig.
1) the two coagulation forceps 21 or the electric cutting knife 22 is fed. According to a variant of the device in FIGS. 7 and 8, instead of the two coagulation forceps 21 with a neutral electrode, two coagulation forceps 21 'isolated from one another can be used for bipolar coagulation, i. H. the neutral electrode is superfluous with this method. One pole of the coagulation current is fed to the coagulation forceps 21 '(FIGS. 7 and 8) via the connections 28' and 29 'according to FIG. 9.
With the device described, after a puncture has been made through the abdominal wall, an abdominal tire can initially be generated to clear the operating field, with the aid of the optics 24 equipped with the lighting device being possible for good observation. Now, by operating the handle 7, the two coagulation forceps 21 can be pushed forward, opened, pushed over the fallopian tube and pulled back and closed by letting go of the handle 7. When the forceps are closed, the fallopian tube grasped by them is closed by coagulation at two distant points.
After inserting the cutting knife 22 or the biopsy forceps 23 instead of the optics 24, if the fallopian tube is grasped again with the forceps 21 and the trigger 8 is pressed, the fallopian tubes can be severed between the two closure points with the cutting knife 22 and, if necessary, a biopsy specimen can be removed with the biopsy forceps 23 will.
