Abstract

A business form including a base ply having a window therethrough defined by a boundary, a thinner backing ply adhered to the base ply rear face about the base ply window, and a card made of different material than the base ply, the card being disposed within the window boundary and releasably adhered to the backing ply. The base ply front face aid the card are readily printable during a single pass through a printer.
A method of creating such a business form with an card is also shown, including the steps of (1) providing a first ply having a the first face adapted for receiving printing, (2) die cutting a window opening into the first ply, (3) securing a second ply to the second face of the first ply so as to cover the first ply window opening, and (4) releasably adhering a card to the second ply first face with the card disposed in the window opening. The card may be tipped onto the second ply either before or after the second ply is secured to the first ply. The method may be repeated on a continuous first ply, which may be cut with either perforations or full cuts to define form lengths.

Claims (66)

1. A business form including a protectible separable card, comprising:
a paper ply having a front face and a rear face, said front face being adapted for receiving printed information thereon;
a cut boundary in said paper ply defining a window opening through said paper ply;
a patch ply having a portion of one face secured to the rear face of the paper ply about the window boundary;
a card of different material than said paper ply adhered to the patch ply one face by a releasable adhesive, said adhered card being substantially surrounded by said paper ply window opening boundary.
2. The form of claim 1, wherein the patch ply is thinner than the paper ply.
3. The form of claim 1, wherein said paper ply, said window opening, said patch ply, and said card all have a length and width, and said patch ply length and width are greater than the window opening length and width and less than the paper ply length and width, and said card length and width are slightly less than the window opening length and width.
4. The form of claim 31, wherein said patch ply substantially covers the window opening.
5. The form of claim 31, wherein said patch ply has a greater adherence to the adhesive than the card whereby said adhesive stays with said patch ply when said card is removed therefrom.
6. The form of claim 36, wherein said card has a release coating on its face releasably secured to said patch ply.
7. The form of claim 31, wherein said card has a greater adherence to the adhesive than the patch ply whereby said adhesive stays with said card when said card is removed from said patch ply.
8. The form of claim 31, wherein said patch ply has adhesive substantially covering said patch ply one face, with said adhesive on said one face portion securing said patch ply to said paper ply rear face and the adhesive on the other portion of the patch ply which covers the window opening being the releasable adhesive adhering the card to the patch ply.
9. The form of claim 31, wherein the card and the paper ply differ in thickness.
10. A method of creating a business form with a card, comprising the steps of:
providing a first ply with first and second faces, with said first face being adapted for receiving printing;
die cutting a window opening into the first ply;
securing a first face of a second ply that is thinner than said first ply to the second face of the first ply so as to substantially cover the first ply window opening; and releasably adhering a card to said second ply first face with said card disposed in said window opening.
11. The method of claim 48, further comprising the step of die cutting an opening in the second ply, said second ply opening being smaller than said card, wherein said adhering step adheres the card to the second ply around the second ply opening.
12. The method of claim 48, further comprising the step of providing a release material on the face of said card adhered to said second ply whereby said adhesive therebetween will remain with the second ply when said card is removed therefrom.
13. The method of claim 48, wherein said securing step includes applying adhesive to said second face of said first ply and then bringing said second ply into contact with said second face of said first ply.
14. The method of claim 58, further comprising the step of applying a releasable second adhesive over a portion of said first face of said second ply, and said step of bringing said second ply into contact with said second face of said first ply positions said second ply with said portion with said second adhesive surrounded by said window.
15. The method of claim 48, wherein said securing step includes applying adhesive to said first face of said second ply and then bringing said second ply into contact with said second face of said first ply.
16. A business form, comprising:

a base ply with a first face and a second face and having a window therethrough defined by a boundary;
a backing ply adhered to the base ply second face substantially about the base ply window; and a card made of different material than the base ply, said card being disposed within the window boundary and releasably adhered by an adhesive to said backing ply.
17. The form of claim 1, wherein the backing ply is thinner than the base ply.
18. The form of claim 1, wherein said card is made of a durable synthetic material.
19. The form of claim 3, wherein said material is plastic.
20. The form of claim 1, wherein said backing ply further has an opening therethrough aligned with said base ply window, said opening exposing a printable portion of said card.
21. The form of claim 1, wherein said backing ply substantially covers the window opening.
22. The form of claim 1, wherein said backing ply has a greater adherence to the adhesive than the card whereby said adhesive stays with said backing ply when said card is removed therefrom.
23. The form of claim 1, wherein said card has a greater adherence to the adhesive than the backing ply whereby said adhesive stays with said card when said card is removed from said backing ply.
24. The form of claim 1, wherein said base ply, said window, said backing ply, and said card all have a length and width, and said backing ply length and width are greater than the window length and width and less than the base ply length and width, and said card length and width are slightly less than the window length and width.
25. The form of claim 1, wherein said base ply front face and said card are adapted for printing during a single pass through a printer.
26. The form of claim 1, wherein said base ply is paper stock and said card is plastic.
27. The form of claim 1, wherein the card and base ply material differ in thickness.
28. The form of claim 1, comprising a plurality of said forms cut from a continuous base ply.
29. The form of claim 13, wherein said base ply includes transverse perforations separating said plurality of forms.
30. The form of claim 13, wherein said base ply includes transverse cuts separating said plurality of forms into individual cut sheets.
31. A business form, comprising:
a first sheet having a front face and a rear face, said front face being adapted for receiving printed information thereon;
a cut boundary in said first sheet defining a window opening through said first sheet;
a second sheet secured to the rear face of the first sheet about the window boundary, said second sheet substantially covering said window opening;
a separate card tipped onto the second sheet through said window opening; and a releasable adhesive securing a separate card to said second sheet with said card surrounded by said first sheet window opening boundary.
32. The form of claim 16, wherein the second sheet is thinner than the first sheet.
33. The form of claim 16, wherein said second sheet has a greater adherence to the adhesive than the card whereby said adhesive stays with said second sheet when said card is removed therefrom.
34. The form of claim 18, wherein said card has a release coating on its face secured to said second sheet by the adhesive.
35. The form of claim 16, wherein said card has a greater adherence to the adhesive than the second sheet whereby said adhesive stays with said card when said card is removed from said second sheet.
36. The form of claim 16, wherein said first sheet, said window opening, said second sheet, and said card all have a length and width, and said second sheet length and width are greater than the window opening length and width and less than the first sheet length and width, and said card length and width are slightly less than the window opening length and width.
37. The form of claim 16, wherein said card and said first sheet are different materials.
38. The form of claim 22, wherein the card material and the second sheet material differ in thickness.
39. The form of claim 22, wherein said first sheet is made of a paper material and said card is made of a durable synthetic material.
40. The form of claim 24, wherein said card material is plastic.
41. The form of claim 16, wherein said card has one face adhered to said second sheet and a second oppositely facing face adapted to receive printing thereon.
42. The form of claim 26, wherein said second sheet first face and said card second face are adapted for printing during a single pass through a printer.
43. The form of claim 16, comprising a plurality of said forms cut from a continuous first sheet.
44. The form of claim 28, wherein said first sheet includes transverse perforations separating said plurality of forms.
45. The form of claim 28, wherein said first sheet includes transverse cuts separating said plurality of forms into individual cut sheets.
46. The form of claim 31, wherein said patch ply further has an opening therethrough aligned with said base ply window, said patch ply opening exposing a printable portion of said card.
47. The form of claim 31, wherein said card is made of a durable synthetic material.
48. The form of claim 41, wherein said card material is plastic.
49. The form of claim 31, wherein said card has one face adhered to said patch ply and a second oppositely facing face adapted to receive printing thereon.
50. The form of claim 43, wherein said paper ply front face and said card second face are adapted for printing during a single pass through a printer.
51. The form of claim 31, comprising a plurality of said forms cut from a continuous paper ply.
52. The form of claim 45, wherein said paper ply includes transverse perforations separating said plurality of forms.
53. The form of claim 45, wherein said paper ply includes transverse cuts separating said plurality of forms into individual cut sheets.
54. The method of claim 48, wherein said second ply is thinner than said first ply.
55. The method of claim 48, wherein said card is smaller than said window opening and said second ply is larger than said window opening but smaller than said first ply, and said securing step is accomplished by tipping said second ply onto said first ply.
56. The method of claim 51, wherein said securing step is done before the adhering step and said card is tipped through said window opening.
57. The method of claim 48, wherein said securing step is done before the adhering step.
58. The method of claim 48, wherein said adhering step is done before the securing step, whereby said securing step disposes the adhered card in the window opening.
59. The method of claim 48, wherein said method is repeated continuously on a continuous first ply moved in a downstream direction, and said adhering step is accomplished downstream of said securing step.
60. The method of claim 48, wherein said method is repeated continuously on a continuous first ply, and said adhering step is done prior to said securing step.
61. The method of claim 60, wherein said adhesive applying step applies adhesive over substantially the entire first face of the second ply, and said releasably adhering step includes bringing said card into contact with a selected portion of said second ply first face.
62. The method of claim 60, wherein said securing step is done before said adhering step.
63. The method of claim 60, wherein said adhering step is done before the securing step, whereby said securing step disposes the adhered card in the window opening.
64. The method of claim 48, wherein said first ply is continuous, and further comprising the step of cutting the first ply to define form lengths, wherein said die cutting, securing and releasably adhering steps are performed on each such form length.
65. The method of claim 64, wherein said step of cutting the first ply to define form lengths comprises cutting transverse perforations across the first ply to define separable forms.
66. The method of claim 64, wherein said step of cutting the first ply to define form lengths comprises cutting across the first ply to define separate cut sheet forms.
