fréquence à un tissu vivant pour favoriser sa guérison.
La présente invention concerne un dispositif permettant d'appliquer un champ électromagnétique de haute fréquence à un tissu vivant pour favoriser sa guérison.
On sait depuis de nombreuses années qu'il est possible d'améliorer la vitesse de cicatrisation en appliquant des champs électromagnétiques de haute fréquence à un tissu blessé. On a considéré que les effets thérapeutiques étaient dus à l'échauffement du tissu par le champ, et l'appareil de soin de la technique antérieure était configuré de façon à produire des niveaux énergétiques de haute fréquence permettant d'échauffer le tissu à sa surface ou dans sa profondeur. Cette technique de chauffage est connue sous
le nom de diathermie. Le procédé consistant à rendre pulsé le champ de haute fréquence produit par l'appareil de diathermie est connu. Jusqu'ici, le champ électromagnétique pulsé a été produit à l'aide d'un équipement hospitalier ou de laboratoire comprenant
un générateur de signal électrique alimentant une bobine d'induction montée sur un support, lequel est placé au voisinage d'une aire
d'un patient à traiter. Cet appareil est encombrant et présente l'inconvénient selon lequel il est impossible de traiter de manière intensive et continue un patient sans l'hospitaliser.
Plus récemment, on a noté que la thérapie effectuée à l'aide d'un champ de haute fréquence appliqué ne se caractérisait
pas seulement en termes d'effets d'échauffement du tissu par le champ. Une discussion sur ce sujet est donné dans l'article écrit par le demandeur "Healing by Electromagnetism - Fact or Fiction",

Le demandeur a conçu un appareil portatif de moindre puissance permettant de produire le champ électromagnétique et pouvant être monté sur un patient. Cet appareil portatif est décrit dans la demande de brevet britannique publiée sous le n[deg.] 2 027 594 au nom du demandeur.
Cet appareil portatif comprend un oscillateur de haute fréquence excité par pile et une antenne dotée d'une souplesse permettant de la placer au-dessus d'une aire de tissu à traiter. L'appareil peut donc être fixé au patient et rester en marche d'une manière sensiblement continue. L'appareil portatif produit un
champ électromagnétique d'une fréquence appartenant typiquement
à l'intervalle de 3 à 30 MHz, la fréquence particulière n'ayant
pas grande importance quant à l'efficacité de la thérapie. Le
champ de haute fréquence est pulsé pour maximiser l'effet thérapeutique. Le champ a une intensité telle qu'il ne produit aucun échauffement notable du tissu. L'appareil portatif fonctionne donc
à des niveaux de puissance beaucoup moins élevés que l'encombrant appareil de diathermie, de façon à produire typiquement un champ
de haute fréquence de moins de 100 mW cm , cette valeur étant mesurée au niveau de la peau recouvrant le tissu, et s'appuyant
sur une hypothèse de base fondamentalement différente quant à la manière dont un champ de haute fréquence peut être utilisé pour réaliser un traitement, à savoir que le champ n'a pas à produire un échauffement du tissu pour amener une amélioration de la vitesse de guérison.
Un inconvénient du dispositif portatif est que son antenne émet de l'énergie électromagnétique'non seulement vers l'avant en direction du tissu mais également vers l'arrière à l'extérieur du patient, ce qui entraine qu'une partie importante
de l'énergie émise ne passe pas dans le tissu. De plus, l'énergie envoyée vers l'extérieur peut altérer le fonctionnement d'un équipement électronique du laboratoire ou de la maison. De plus, le substrat isolant souple de la technique antérieure présente un effet non souhaitable de barrière imperméable vis-à-vis de l'eau
et de l'air au dessus de la blessure, ce qui peut empêcher la guérison dans certaines circonstances.
Selon l'invention considérée sous un premier aspect,
il est proposé un dispositif permettant d'appliquer un champ magnétique de haute fréquence au tissu du patient pour favoriser la guérison, comprenant une antenne qui dirige le champ sur le patient et un circuit oscillant électrique conçu pour exciter l'antenne afin de réaliser la propagation de l'énergie électromagnétique en vue de faciliter la guérison du tissu sans produire aucun échauf-

nel configuré de manière que l'énergie se propage vers l'avant, pour traiter le tissu, dans une proportion notablement supérieure à celle dirigée vers l'arrière, afin de minimiser l'émission de l'énergie
à l'extérieur du patient.
A cet effet, l'antenne peut comprendre un élément émetteur conçu pour être excité par le circuit oscillant afin de propager ladite énergie, et un moyen réflecteur qui réfléchit,dans cette direction avant, l'énergie émise par l'élément émetteur dans la direction, arrière. De préférence, le moyen réflecteur est conçu de façon que l'énergie renvoyée par lui s'ajoute de manière cons-

L'antenne peut comprendre, en plus du moyen réflecteur,
ou à la place de ce dernier, un élément absorbant destiné à absorber l'énergie émise vers l'arrière.
L'antenne peut comporter un élément "parasite", c'està-dire une partie d'antenne directionnelle non reliée directement
à l'émetteur, afin de modifier le diagramme directionnel naturel d'émission de l'énergie à partir de l'élément émetteur.
De préférence, l'élément émetteur comprend une configuration conductrice de l'électricité se présentant sous forme d'une boucle qui entoure un espace autorisant le passage de fluides afin de permettre à la blessure du tissu de respirer. Si cela est souhaitable, l'espace peut comporter un pansement. La configuration conductrice peut être constituée par une boucle de câble coaxial.
Du point de vue d'un deuxième aspect, l'invention propose un dispositif se révélant supérieur au dispositif portatif
de la demande de brevet citée où l'antenne est configurée de façon à émettre un champ électromagnétique spatialement uniforme sur au moins une majeure partie d'une aire circonscrite par l'antenne placée en utilisation en regard du tissu à traiter.
Selon ce deuxième aspect de l'invention, il est possible d'effectuer des traitements dont le caractère peut être mieux prédit et quantifié que cela n'était possible jusqu'ici, du fait que, avec le dispositif portatif de la technique antérieure, l'intensité du champ auquel le tissu est soumis varie sur l'étendue spaciale de l'antenne de sorte que des régions différentes du tissu reçoivent des traitements de modalités différentes.
De préférence, l'antenne à champ uniforme est configurée sous forme d'un élément du type feuille souple pour se conformer au corps des patients. Par exemple, l'antenne peut comprendre un circuit imprimé souple qui porte en outre les composants d'un oscillateur de basse tension, du type alimenté par pile.
Selon les deux aspects de l'invention, l'inductance propre de l'antenne peut être utilisée comme partie d'un circuit LC définissant la fréquence d'oscillation de l'oscillateur de haute fréquence qui excite l'antenne, d'une manière décrite plus en détail dans la demande de brevet citée.
La description suivante, conçue à titre d'illustration de l'invention, vise à donner une meilleure compréhension de ses caractéristiques et avantages; elle s'appuie sur les dessins annexés, parmi lesquels :
- la figure 1 est une vue en plan simplifiée d'un dispositif selon l'invention;
- la figure 2 est une vue en coupe du dispositif présenté sur la figure 1, prise suivant la ligne A'-A;

- la figure 4 est une représentation simplifiée d'un autre dispositif selon l'invention permettant de produire un champ uniforme.
On se reporte d'abord aux figures 1 et 2. Le dispositif comporte une boucle de c8ble coaxial 1 disposée suivant une forme appropriée permettant de la placer au-dessus d'une blessure, d'une plaie ou d'une aire endommagée analogue de tissu 2. La boucle est souple de manière à s'adapter à la surface de la peau du patient. A une extrémité, le cable 1 possède un conducteur interne 3 et un

lateur de haute fréquence dont la plus grande partie est constituée par un circuit 5 représenté schématiquement, le circuit 5 étant monté sur un substrat de support 6 en matière plastique qui relie des borda de la boucle de câble coaxial et est fixé à ceux-ci.
Le substrat de support 6 porte également une alimentation électrique 7 sous forme de pile destinée à l'oscillateur. Le circuit 5 coopère avec le câble 1 pour produire dans le câble des impuls ions de haute fréquence à une vitesse de répétition prédéterminée. La haute fréquence peut appartenir à l'intervalle de 3 à 30 MHz et
est, de manière souhaitable, de 27,12 MHz, car il s'agit d'une fréquence légalement attribuée aux utilisations médicales.
Toutefois, la valeur précise de la haute fréquence n'est pas cruciale en ce qui concerne l'efficacité du traitement. La boucle de cable coaxial 1 fait fonction d'élément émetteur d'une antenne et propage les impulsions d'énergie de haute fréquence. Le diagramme directionnel d'émission naturel est sensiblement uniforme suivant la section radiale du cable coaxial et, par conséquent, l'énergie de haute fréquence tend à être émise par le câble 1 à
la fois vers l'avant; en direction du tissu 2 à traiter, et vers l'arrière, dans le sens opposé au patient.
Toutefois, en plus de l'élément émetteur 1, l'antenne comporte un réflecteur 8 qui peut être constitué d'un treillis métallique souple ou d'un feuillet métallique, réfléchissant l'énergie de haute fréquence dirigée vers l'arrière qui a été
émise par le câble 1. Le réflecteur 8 est, de manière convenable, séparé du cable coaxial 1 par une couche d'écartement diélectrique 9, si bien que l'énergie de haute fréquence réfléchie par le réflecteur 8 s'ajoute constructivement à l'énergie émise par le cable 1 vers l'avant, ceci améliorant la densité de l'énergie de haute fréquence reçue par le tissu 2.
L'antenne peut comporter un élément "parasite", c'est-àdire une partie d'antenne directionnelle non reliée directement à l'émetteur, désigné par la référence 10 et comprenant une boucle

de modifier le diagramme directionnel naturel d'émission des hautes fréquences par la boucle de cable 1, de façon à réduire l'émission dans les directions transversales à la direction vers l'avant, et
à ainsi placer un lobe principal de l'antenne en direction de l'avant. En outre, l'antenne peut comporter un matériau absorbant
la haute fréquence (non représenté) destiné à sélectivement absorber le rayonnement de haute fréquence émis vers l'arrière ou sur le coté, de façon à modifier le diagramme directionnel de l'antenne, ainsi que cela peut être souhaitable dans certaines circonstances.
Il faut noter que la boucle de cable 1 entoure un espace ouvert 13 qui peut s'étendre au-dessus d'une blessure et qui permet à la blessure de respirer. L'espace peut comporter un pansement
(non représenté).
De façon souhaitable, l'antenne est excitée de façon
à produire une densité énergétique de haute fréquence qui, mesurée

est de préférence de l'ordre de quelques microwatta par centimètre carré. Ceci ne produit aucun échauffement notable du tissu. La vitesse de répétition des impulsions de haute fréquence est choisie de façon à rendre maximale l'efficacité du traitement. Typiquement, la vitesse de répétition d'impulsion appartient à la gamme de 0,1
à 10 000 Hz.
Dans le domaine des hautes fréquences, le cable coaxial présente une inductance entre son conducteur interne 3 et son conducteur de blindage 4. Cette inductance est utilisée comme composant de détermination de la fréquence dans le circuit oscillant 5. Ceci offre l'avantage selon lequel le circuit 5 ne doit pas comporter d'inductances. L'oscillateur comprend essentiellement un circuit LC alimenté par un transistor, dont le composant d'inductance L est défini par l'inductance du cable coaxial, tandis que les composants de l'oscillateur qui ne sont pas le composant L,

Selon une variante au dispositif décrit, le substrat de support 6, qui porte le circuit 5 et l'alimentation électrique 7 par pile, peut être détaché de l'antenne pour être réutilisé avec une nouvelle antenne sur un type différent de blessure.
L'antenne peut comporter sur sa face antérieure un matériau autocollant destiné à la fixation sur la peau. Le matériau autocollant peut s'étendre à l'extérieur du périmètre défini par la boucle de câble 1..
On notera que l'antenne décrite présentement est souple et peut s'adapter à la forme extérieure du patient. Alors que, ainsi que le montre la figure 1, le câble coaxial 1 se présente sous forme de boucle elliptique, il peut être disposé suivant d'autres formes choisies pour s'adapter aux contours dé parties particulières de l'anatomie, par exemple la mâchoire ou le nez, les membres ou le tronc. Il est possible de configurer une paire d'antennes de façon qu'elle ressemble à une paire de lunettes de
protection, pour le traitement des yeux.
De plus, il est possible d'exciter à l'aide d'un circuit oscillant commun plus d'une antenne. Le circuit oscillant
peut être conçu de façon à exciter les antennes suivant un ordre prédéterminé.
Selon un autre mode de réalisation, les conducteurs de l'élément émetteur et de l'élément "parasite" peuvent se présenter sous forme d'une boucle disposée sur une plaquette de circuit imprimé souple, et le réflecteur peut Etre réalisé par une couche de métallisation, ou une configuration métallisée, sur la plaquette, De préférence, l'antenne est configurée de façon à présenter, visà-vis du tissu, une densité d'énergie de haute fréquence qui est sensiblement spacialement uniforme sur au moins une majeure partie de l'aire de l'antenne tournée vers le tissu.
Les dispositifs présentés sur les figures 3 et 4 illustrent des modes de réalisation qui permettent d'obtenir un champ électromagnétique de haute fréquence spacialement uniforme pour le traitement d'un patient, le champ étant uniforme sur au moins une majeure partie de l'étendue spaciale de l'antenne qui,
en utilisation, est tourné vers le patient. Les enseignements tirés de la manière d'obtenir le champ spacialement uniforme décrite ci-après en relation avec les figures 3 et 4 peuvent être utilisés en combinaison avec les enseignements obtenus en relation avec les figures 1 et 2, ou séparément.
Sur la figure 3, est présenté un substrat de circuit imprimé souple 15 qui porte une antenne comprenant une configuration de métallisation 16, telle que représentée, qui est connectée à un circuit intégré 17 monté sur le substrat. Le circuit intégré 17 est alimenté par une alimentation à pile 18, également montée sur le substrat. On notera que le circuit 17 et la pile 18 peuvent correspondre au circuit 5 et à la pile 7 de la figure 2. En utilisation, le substrat 15 est disposé au-dessus du tissu vivant à traiter et est classiquement fixé à la peau du patient par un moyen adhésif non représenté. Le circuit intégré 17 comprend les composants d'un oscillateur et d'un circuit de cadencement montés de façon à exciter la configuration de métallisation 16 au moyen d'impulsions de haute fréquence.
Comme cela est décrit en relation avec la figure 2, il est possible d'utiliser l'inductance de l'antenne comme composant de détermination de la fréquence dans l'oscillateur. La fréquence de l'énergie de haute fréquence se situe typiquement dans la gamme de 3 à 30 MHz et est commodément 27,12 MHz. On verra que la configuration de métallisation 16 entoure une aire donnée du substrat
et traite donc une aire correspondante du tissu du patient se trouvant au-dessous du substrat 15. La configuration de métallisation 16 est formée de manière qu'il y ait émission, en direction du tissu, d'un champ électromagnétique de haute fréquence qui présente une intensité sensiblement uniforme sur au moins une majeure partie du substrat 15. De façon souhaitable, l'intensité du champ ne dépasse
-2 -2 pas 100 mW cm et s'exprime de préférence en microwatts par cm . Une telle intensité ne produit pas d'échauffement notable du tissu. La configuration de métallisation comporte plusieurs parties conductrices linéaires 16a orientées parallèlement entre elles et transversalement à la longueur du substrat 15. On choisie le taux de répétition des impulsions de haute fréquence de manière à rendre maximale l'efficacité du traitement.
Le substrat souple 15 présente l'avantage de pouvoir s'adapter à la surface de la peau d'un patient et, ainsi, de pouvoir être enroulé autour d'un membre par exemple.
Le dispositif présenté sur la figure 4 est analogue à celui de la figure 3, mais les parties conductrices 16a sont disposées longitudinalement au substrat 15. Parmi les variantes qui sont naturellement possibles, on indiquera que, par exemple, au lieu d'utiliser un circuit imprimé souple, il est possible de former la configuration de parties conductrices définissant l'antenne à partir d'un conducteur formant un serpentin suivant l'une ou l'autre des configurations des figures 3 et 4, un revêtement électriquement isolant maintenant le conducteur sous la forme d'un serpentin.
Bien entendu, l'homme de l'art sera en mesure d'imaginer, à partir des dispositifs dont la description vient d'Être donnée à titre simplement illustratif et nullement limitatif, diverses autres variantes et modifications ne sortant pas du cadre 'de l'invention.
REVENDICATIONS
1. Dispositif destiné à appliquer un champ électromagnétique de haute fréquence à un tissu d'un patient pour favoriser
la guérison, caractérisé en ce qu'il comprend une antenne (1, 16) destinée à diriger le champ sur le patient et un circuit oscillant (5, 17) conçu pour exciter l'antenne afin de propager de l'énergie électromagnétique pour favoriser la guérison du tissu mais sans produire aucun échauffement notable de celui-ci, l'antenne possédant un diagramme directkxaidconfiguré de sorte que l'énergie est propagée dans la direction avant suivant une proportion sensiblement plus grande que dans la direction arrière opposée afin de minimiser l'émission du rayonnement à l'extérieur du patient.

However, the precise value of the high frequency is not crucial with regard to the effectiveness of the treatment. The coaxial cable loop 1 acts as the transmitting element of an antenna and propagates the pulses of high frequency energy. The directional diagram of natural emission is substantially uniform along the radial section of the coaxial cable and, therefore, the high frequency energy tends to be emitted by the cable 1 to
both forward; towards the tissue 2 to be treated, and backwards, in the opposite direction to the patient.
However, in addition to the transmitting element 1, the antenna includes a reflector 8 which may consist of a flexible metallic mesh or a metallic sheet, reflecting the high frequency energy directed towards the rear which has been
emitted by the cable 1. The reflector 8 is suitably separated from the coaxial cable 1 by a dielectric spacer layer 9, so that the high frequency energy reflected by the reflector 8 is added constructively to the energy emitted by the cable 1 towards the front, this improving the density of the high frequency energy received by the fabric 2.
The antenna may include a "parasitic" element, that is to say a directional antenna part not directly connected to the transmitter, designated by the reference 10 and comprising a loop

to modify the natural directional diagram of emission of the high frequencies by the loop of cable 1, so as to reduce the emission in the directions transverse to the direction towards the front, and
thus placing a main lobe of the antenna towards the front. In addition, the antenna may include an absorbent material

The repetition speed of the high frequency pulses is chosen so as to maximize the effectiveness of the treatment. Typically, the pulse repetition speed is in the range of 0.1

In the high frequency domain, the coaxial cable has an inductance between its internal conductor 3 and its shield conductor 4. This inductance is used as a component for determining the frequency in the oscillating circuit 5. This has the advantage that the circuit 5 must not include inductors. The oscillator essentially comprises an LC circuit supplied by a transistor, the component of inductance L of which is defined by the inductance of the coaxial cable, while the components of the oscillator which are not the component L,

According to a variant of the device described, the support substrate 6, which carries the circuit 5 and the power supply 7 by battery, can be detached from the antenna to be reused with a new antenna on a different type of injury.
The antenna may include on its front face a self-adhesive material intended for fixing to the skin. The self-adhesive material can extend outside the perimeter defined by the cable loop 1.
It will be noted that the antenna described here is flexible and can adapt to the external shape of the patient. While, as shown in Figure 1, the coaxial cable 1 is in the form of an elliptical loop, it can be arranged in other shapes chosen to adapt to the contours of particular parts of the anatomy, for example the jaw or nose, limbs or trunk. You can configure a pair of antennas to look like a pair of sunglasses.

According to another embodiment, the conductors of the emitting element and of the "parasitic" element can be in the form of a loop arranged on a flexible printed circuit board, and the reflector can be produced by a layer of metallization, or a metallized configuration, on the wafer, Preferably, the antenna is configured so as to present, with respect to the fabric, a high frequency energy density which is substantially spatially uniform over at least a major part of the area of the antenna facing the fabric.
The devices presented in FIGS. 3 and 4 illustrate embodiments which make it possible to obtain a spatially uniform high frequency electromagnetic field for the treatment of a patient, the field being uniform over at least a major part of the spatial extent. of the antenna which,
in use, is turned towards the patient. The lessons learned from the manner of obtaining the spatially uniform field described below in connection with Figures 3 and 4 can be used in combination with the lessons obtained in connection with Figures 1 and 2, or separately.
In Figure 3, there is shown a flexible printed circuit substrate 15 which carries an antenna comprising a metallization configuration 16, as shown, which is connected to an integrated circuit 17 mounted on the substrate. The integrated circuit 17 is supplied by a battery supply 18, also mounted on the substrate. It will be noted that the circuit 17 and the battery 18 can correspond to the circuit 5 and to the battery 7 of FIG. 2. In use, the substrate 15 is placed above the living tissue to be treated and is conventionally fixed to the skin of the patient. by adhesive means not shown. The integrated circuit 17 includes the components of an oscillator and a timing circuit mounted so as to excite the metallization configuration 16 by means of high frequency pulses.
As described in connection with FIG. 2, it is possible to use the antenna inductance as a component for determining the frequency in the oscillator. The frequency of the high frequency energy is typically in the range of 3 to 30 MHz and is conveniently 27.12 MHz. We will see that the metallization configuration 16 surrounds a given area of the substrate
and therefore treats a corresponding area of the patient's tissue lying below the substrate 15. The metallization configuration 16 is formed so that there is emission, towards the tissue, of a high frequency electromagnetic field which has a substantially uniform intensity over at least a major part of the substrate 15. Desirably, the intensity of the field does not exceed
-2 -2 steps 100 mW cm and is preferably expressed in microwatts per cm. Such an intensity does not produce significant heating of the tissue. The metallization configuration comprises several linear conductive parts 16a oriented parallel to each other and transversely to the length of the substrate 15. The repetition rate of the high frequency pulses is chosen so as to maximize the efficiency of the treatment.
The flexible substrate 15 has the advantage of being able to adapt to the surface of the skin of a patient and, thus, of being able to be wrapped around a limb for example.
The device presented in FIG. 4 is similar to that of FIG. 3, but the conductive parts 16a are arranged longitudinally to the substrate 15. Among the variants which are naturally possible, it will be indicated that, for example, instead of using a circuit flexible print, it is possible to form the configuration of conductive parts defining the antenna from a conductor forming a coil according to either of the configurations of FIGS. 3 and 4, an electrically insulating coating keeping the conductor under the shape of a serpentine.
Of course, those skilled in the art will be able to imagine, from the devices the description of which has just been given by way of illustration only and in no way limitative, various other variants and modifications not departing from the scope of the 'invention.
CLAIMS
1. Device for applying a high frequency electromagnetic field to a patient's tissue to promote
healing, characterized in that it comprises an antenna (1, 16) intended to direct the field on the patient and an oscillating circuit (5, 17) designed to excite the antenna in order to propagate electromagnetic energy to favor healing of the tissue but without producing any noticeable heating thereof, the antenna having a directkxaid diagram configured so that the energy is propagated in the front direction in a proportion substantially greater than in the opposite rear direction in order to minimize the radiation to the outside of the patient.
2. Device designed to be worn by a patient and intended to apply a high frequency electromagnetic field
