"Dispositif d'éveinage"
La présente invention a pour objet un dispositif d'éveinage destiné, en particulier, à l'ablation des veines variqueuses (saphène interne ou externe) des membres inférieurs et comprenant un élément de traction souple (stripper), de section transversale inférieure à celle de la veine, destiné à être introduit, par une de ses extrémités, dans la partie de veine à arracher pour faire saillie, par rapport au membre, de part et d'autre de cette partie de veine, une première extrémité dudit stripper étant utilisée afin d'exercer une traction sur ce dernier pour l'ablation de la partie de veine susdite, tandis que sa seconde extrémité est garnie, d'une part,
d'une olive d'éveinage de section transversale correspondant à celle de la partie de veine à arracher et destinée à l'ablation de cette dernière lorsqu'une traction sera exercée sur la première extrémité du stripper et, d'autre part, une mèche de gaze disposée à l'arrière de l'olive susdite, en considérant le sens de la traction exercée sur le stripper, et qui est destinée à occuper la cavité laissée libre par la partie de veine arrachée.
Dans les dispositifs connus, qui sont actuellement offerts aux chirurgiens, la mèche de gaze précitée n'est pas fournie. De sorte que cette dernière doit être préparée et assujette au stripper lors de l'opération, ce qui entraîne divers inconvénients, à savoir : difficulté de réaliser au coup par coup des mèches de gaze dont la section transversale est, d'une part, parfaitement adaptée à la section transversale de l'olive d'éveinage utilisée, et, d'autre part, sensiblement constante sur toute la longueur de la mèche, ce qui peut entraîner, lorsque ces sections ne correspondent pas idéalement,

des complications postopératoires dues notamment à des hématomes localisés ou diffus, un manque de confort pour le patient en particulier lorsqu'il marche ainsi qu'un accroissement de la durée de l'intervention et de l'hospitalisation.
L'invention a pour but de remédier à ces inconvénients et de procurer un dispositif d'éveinage complet, parfaitement adapté à chaque intervention, qui permet de faciliter cette dernière, tout en améliorant le confort postopératoire du patient et en réduisant de manière sensible les risques postopératoires, ce qui permet, dans la plupart des cas, d'abréger la durée d'hospitalisation.
A cet effet, suivant l'invention, la mèche de gaze du dispositif d'éveinage est préfabriquée et a une section transversale sensiblement constante correspondant à la section transversale de l'olive d'éveinage, des moyens de fixation de ladite mèche au dispositif d'éveinage étant prévus à une des extrémités de cette dernière.
Suivant une forme de réalisation avantageuse de l'invention, la mèche préfabriquée susdite comporte les moyens de fixation précités.
Suivant un autre mode de réalisation avantageux de l'invention, le stripper et l'olive d'éveinage sont réalisés en deux pièces, les moyens de fixation de la mèche précités étant agencés pour que l'extrémité précitée de la mèche soit immobilisée soit dans l'olive d'éveinage, soit à la périphérie de cette dernière.
Suivant une forme de réalisation particulièrement avantageuse de l'invention, le dispositif d'éveinage comprend 1 stripper et l'otive d'éveinage précités, réalisés en deux pièces ainsi qu'une seconde olive porte-mèche de gaze agencée pour pouvoir être immobilisée, sur le stripper, à l'arrière de l'olive d'éveinage en considérant le sens de la traction sur ledit stripper, les moyens de fixation de la mèche susdits étant agencés pour que l'extrémité précitée de 1 a mèche soi t immobilisée soit dans l'olive porte-mèche, soit à la périphérie

une mèche de gaze 14 disposée à l'arrière de l'olive d'éveinage 9 et qui est destinée à occuper la cavité laissée libre dans le membre par la partie de veine 5 qui a été arrachée. La mèche de gaze 14 est préfabriquée et a une section transversale qui est sensiblement constante. Cette section correspond à la section transversale de l'olive d'éveinage 9 et est choisie dans une matière et de manière à ce que, lorsque la mèche de gaze est imbibée de sang, cette mèche, dont la longueur est supérieure à la longueur de la partie de veine 5 à arracher, occupe l'entièreté du volume de la cavité susdite. Ladite mèche de gaze 14 est pourvue de moyens de fixation 15 au dispositif d'éveinage, qui sont agencés à une de ses extrémités.
Comme montré aux figures l, 4 et 5, la mèche de gaze préfabriquée 14 comporte avantageusement les moyens de fixation 15 précités.
Dans la forme de réalisation du dispositif d'éveinage illustré à la figure 1, les moyens de fixation 15 de la mèche de gaze 14 sont constitués par un lien souple 16 fixé à l'extrémité 16'de ta mèche 14 afin de former une boucle 17 fermée destinée à s'insérer dans deux encoches 18 diamétralement opposées qui sont réalisées dans l'olive d'éveinage 9 à partir de sa périphérie, ces encoches 18 étant profilées de manière à bloquer automatiquement ledit lien 16, par rapport à l'olive, lorsqu'une traction est exercée sur le stripper l suivant la flèche 8, ce blocage pouvant être obtenu par une variation de la largeur de l'encoche 18.
Dans la forme de réalisation du dispositif d'éveinage illustré à la figure 2, l'olive d'éveinage 9 et le stripper l sont réalisés en une seule pièce. Les moyens de fixation précités 15 sont agencés pour que l'extrémité 16'de la mèche de gaze 14 soit immobilisée dans l'olive d'éveinage 9, en noyant cette extrémité 161 dans la matière constituant ladite olive.

Les moyens de fixation 15 précités sont agencés pour que l'extrémité 16'de la mèche de gaze 14 soit immobilisée à l'extrémité 4 du stripper l en noyant cette extrémité 16'dans 1a matière constituant la butée 10 dudit stripper.
On pourrait également prévoir un dispositif d'éveinage, tel qu'illustré à la figure 3, dans lequel l'extrémité 16'de la mèche de gaze 14 serait immobilisée dans l'olive 9 ou à la périphérie de cette dernière.
Dans la forme de réalisation du dispositif d'éveinage illustrée à la figure 4, celui-ci comprend l'olive d'éveinage 9, qui est amovible par rapport au stripper l et qui peut être maintenue sur ce dernier grâce à sa butée 10, et une olive porte-mèche 20 qui est également amovible par rapport au stripper et qui peut être maintenue sur ce dernier grâce à une butée 21. L'extrémité 16'de la mèche 14 est immobilisée dans l'olive 20, par exemple en la noyant dans la matière constituant cette dernière.
La section transversale de l'olive porte-mèche 20 est inférieure ou tout au plus égale à la section correspondante de l'olive d'éveinage 9, la section transversale de la mèche de gaze 14 correspondant à la section transversale de l'olive 9.
Le dispositif d'éveinage suivant l'invention et représenté à la figure 5 comprend une mèche de gaze 14 qui est insérée dans une gaine 22, qui est souple, poreuse et fixée à ladite mèche 14. Cette gaine, qui est avantageusement réalisée en gaze, présente l'avantage d'éviter que des débris de la mèche se dispersent dans la cavité précitée créée par l'ablation de la partie de la veine 5. Les moyens de fixation 15 de la mèche 14 sont avantageusement portés par la gaine 22 et ménagés à proximité d'une des extrémités de cette dernière. Ces moyens

15 sont avantageusement constitués par un anneau de sertissage 23, porte par la gaine 22, agencé pour coopérer, afin de réaliser son sertissage automatique, avec une rainure correspondante 24 ménagée à la périphérie de l'olive d'éveinage 9.
