Abstract

Un dispositivo de intubado laringeotraqueal que comprende una máscara laríngea, una pieza conectora, un tubo endotraqueal y una guía de intubación, donde la máscara laríngea es un tubo hueco adaptable a una garganta, con dicha pieza conectora conectada en su extremo proximal, y dicha pieza conectora está provista de un ramal principal con una válvula elástica y al menos una derivación, ambas en comunicación fluida con la máscara laríngea, y de una mascarilla en su extremo distal, una guía de intubación que se inserta dentro de la máscara laríngea, un tubo endotraqueal que se introduce utilizando la guía de intubación una vez que la máscara laríngea fue retirada, y al menos una derivación para conectar un sistema de ventilación. El procedimiento de intubación comprende los pasos de: insertar la máscara laríngea con la pieza conectora en su extremo proximal; inflar la almohadilla y el balón en el dorso de la almohadilla; iniciar la ventilación; insertar la guía de intubación con cámara por la válvula elástica y dentro de la máscara laríngea; maniobrar la guía de intubación hasta que su extremo distal esté en posición delante de la cuerdas vocales; atravesar las cuerdas vocales con la punta de la guía de intubación; desconectar el visor de la guía; suspender la ventilación; desinflar la almohadilla y el balón; retirar la máscara laríngea dejando la guía de intubación en su posición; insertar el tubo endotraqueal alrededor de la guía de intubación hasta pasar las cuerdas vocales; inflar el manguito para sellar el tubo endotraqueal a la tráquea; retirar la guía de intubación; y continuar con la ventilación.A laryngeal tracheal intubation device comprising a laryngeal mask, a connecting piece, an endotracheal tube and an intubation guide, where the laryngeal mask is a hollow tube adaptable to a throat, with said connecting piece connected at its proximal end, and said piece connector is provided with a main branch with an elastic valve and at least one bypass, both in fluid communication with the laryngeal mask, and a mask at its distal end, an intubation guide that is inserted into the laryngeal mask, a tube endotracheal tube that is inserted using the intubation guide once the laryngeal mask has been removed, and at least one shunt to connect a ventilation system. The intubation procedure comprises the steps of: inserting the laryngeal mask with the connecting piece at its proximal end; inflate the cushion and the balloon on the back of the cushion; start ventilation; insert the tube guidewire through the elastic valve and into the laryngeal mask; maneuver the intubation guide until its distal end is in position in front of the vocal cords; cross the vocal cords with the tip of the intubation guide; disconnect the viewfinder from the guide; suspend ventilation; deflate the cushion and balloon; remove the laryngeal mask leaving the intubation guide in position; insert the endotracheal tube around the intubation guide until it passes the vocal cords; inflate the cuff to seal the endotracheal tube to the trachea; remove the intubation guide; and continue ventilation.
