Abstract

The present invention addresses the problem of providing a method for detecting fibrosis, the method having low invasiveness and high versatility, and finding a marker, of a fibrotic disease, having high discrimination accuracy and applicable to a relatively wide range of organs. Provided are: a combination marker for identifying a fibrotic disease, the marker containing LOX-1 protein or a fragment thereof and LAB or a part thereof; and a method for determining the presence or absence of a fibrotic disease or the presence or absence of risk of onset thereof in a subject by using the marker.

[1-1]被検体における血管内皮障害を伴う線維化疾患の罹患の有無又は発症リスクの有無を判定するための方法であって、被検体由来の試料中における、レクチン様酸化低密度リポタンパク質受容体1タンパク質(LOX-1タンパク質)及びアポリポタンパク質B含有LOX-1リガンド(LAB)の量を測定する工程;前記LOX-1タンパク質の量と前記LABの量の比率を算出する工程を含み、前記比率が、前記被検体が血管内皮障害を伴う線維化疾患を罹患している又はその発症リスクを有することを示す、前記方法。

1.線維化疾患判定用組合せマーカー

 本発明の第1の態様は線維化疾患の鑑別用組合せマーカーである。本発明の組合せマーカーは、LOX-1タンパク質又はその断片、及びLAB又はその一部をバイオマーカーとして使用する。被検体の生体試料中に含まれるそのマーカーの量を第2態様に記載の判定方法を用いて測定することで、被検体の線維化疾患の罹患の有無又は発症リスクの有無を判定することができる。

Six patients who were ACA-positive were selected from those with a history of outpatient visits to the Department of General Medicine at Tokyo Medical and Dental University, and whose ACA concentration detected by quantitative anti-centromere antibody testing (SRL Co., Ltd.) on serum samples was 10.0 U/mL or higher. These patients were later confirmed to be ACA-positive through the measurements described below.

 ACA陰性患者としては、東京医科歯科大学総合診療科の外来に通院歴があり、糖尿病、脂質異常症(LDL120以上又は治療薬内服あり)及び肥満(脂肪肝)のいずれか一以上を有する患者19名を用いた。なお、これらの患者については、後述の測定においてACA陰性であることが確認された。 As ACA-negative patients, we used 19 patients who had a history of outpatient visits to the Department of General Medicine at Tokyo Medical and Dental University and who had one or more of the following conditions: diabetes, dyslipidemia (LDL ≥ 120 or taking medication), or obesity (fatty liver). These patients were confirmed to be ACA-negative in the measurements described below.

 各患者から、インフォームドコンセントを得た上で血液を採取した。LOX-1タンパク質量及びLAB量の測定用には、専用スピッツに血液を採取した。 Blood samples were collected from each patient after obtaining informed consent. Blood was collected in dedicated test tubes for measuring LOX-1 protein and LAB levels.

2.測定
 各患者について、外来診察時に通常の採血検査(東京医科歯科大学病院内で実施)を実施し、血小板数、AST、ALT、ヒアルロン酸、IV型コラーゲンを測定した。
2. Measurements For each patient, routine blood tests (conducted at Tokyo Medical and Dental University Hospital) were performed during outpatient consultations to measure platelet count, AST, ALT, hyaluronic acid, and type IV collagen.

 ACAの濃度は、抗セントロメア抗体定量試験(株式会社エスアールエル社)により測定した。ACAの濃度が10.0U/mL以上であった場合を陽性、10.0U/mL未満であった場合を陰性と判断した。
 LOX-1タンパク質量及びLAB量はプリメディカ社において測定を行った。
The concentration of ACA was measured using an anti-centromere antibody quantitative test (SRL Co., Ltd.). A concentration of 10.0 U/mL or higher was considered positive, and a concentration below 10.0 U/mL was considered negative.
LOX-1 protein and LAB levels were measured at Primedica.

(1)LAB量の測定
 LAB量の測定は特開2021-042986号に準じて行った。測定方法は概略以下の通りである。
(1) Measurement of LAB amount The LAB amount was measured in accordance with Japanese Patent Publication No. 2021-042986. The measurement method is roughly as follows.

 遠心分離により上清を採取することで血漿を得た。
 各患者由来の血漿をsLOX-1固相化プレートの各ウェルに100μLずつ分注し、室温にて2時間インキュベートした。sLOX-1固相化プレートとしては、96穴マイクロプレートにおいて、配列番号3で示すアミノ酸配列からなるヒト由来の組換え可溶型LOX-1(sLOX-1)タンパク質液(5μL/mL)を各ウェルに100μLずつ添加することによりsLOX-1タンパク質を固相化し、ブロッキング液(3%ブロックエース、2%スクロースとなるように蒸留水で希釈)にてブロッキングを行ったプレートを用いた。
Plasma was obtained by collecting the supernatant after centrifugation.
Plasma from each patient was dispensed in 100 μL portions into each well of an sLOX-1 immobilized plate and incubated at room temperature for 2 hours. The sLOX-1 immobilized plate used was a 96-well microplate in which sLOX-1 protein was immobilized by adding 100 μL of human recombinant soluble LOX-1 (sLOX-1) protein solution (5 μL/mL) consisting of the amino acid sequence shown in SEQ ID NO: 3 to each well, and then blocked with a blocking solution (diluted with distilled water to make a 3% Block Ace, 2% sucrose).

 HRP標識抗Apo抗体溶液を血漿除去後のsLOX-1固相化プレートに各ウェルに100μLずつ分注し、1000rpmで1分間撹拌した後、室温で1時間インキュベートした。HRP標識抗Apo抗体溶液としては、HRP標識化ニワトリ抗ヒトアポリポタンパク質モノクローナル抗体(HUC20:Creative Biolabs社:apoB-48及びapoB-100のいずれにも結合)のPBS溶液(濃度:1μg/mL)を420倍になるように希釈用PBS溶液(0.4%ブロックエース含有)で希釈した溶液を用いた。 HRP-labeled anti-Apo antibody solution was dispensed into each well of a plasma-removed sLOX-1 immobilized plate in 100 μL increments. The mixture was stirred at 1000 rpm for 1 minute and then incubated at room temperature for 1 hour. The HRP-labeled anti-Apo antibody solution used was a PBS solution (concentration: 1 μg/mL) of HRP-labeled chicken anti-human apolipoprotein monoclonal antibody (HUC20: Creative Biolabs: conjugates to both apoB-48 and apoB-100), diluted 420-fold with dilution PBS solution (containing 0.4% Block Ace).

 HRPに基づくシグナルは、発光液を各ウェルに100μLずつ分注し、プレートリーダー(Infinite(R) 200 PRO:テカンジャパン株式会社)で検出し、その光強度に基づいて定量化した。発光液としては、SuperSignalTM ELISA Pico Chemiluminescent Substrate(Thermo Fisher Scientific社)に付属のPeroxide SolutionとLuminol/Enhancer Solutionを1:1で混合した溶液を用いた。 The HRP-based signals were detected by dispensing 100 μL of luminescent solution into each well and using a plate reader (Infinite (R) 200 PRO: Tecan Japan Co., Ltd.), and quantified based on the light intensity. As the luminescent solution, a 1:1 mixture of Peroxide Solution and Luminol/Enhancer Solution included with SuperSignal ELISA Pico Chemiluminescent Substrate (Thermo Fisher Scientific) was used.

(2)LOX-1タンパク質量の測定
 LOX-1タンパク質量としてはsLOX-1タンパク質の定量値を使用した。sLOX-1タンパク質量の測定は、Inoue et al, 2010, Clinical Chemistry 56(4)(doi:10.1373/clinchem.2009.140707)に記載の手順に準じて行った。手順の概略は、sLOX-1の代わりに抗ヒトLOX-1抗体を用い、HRP標識抗Apo抗体の代わりに抗ヒトLOX-1抗体を使用した以外はLAB量の測定と同様である。
(2) Measurement of LOX-1 protein amount The quantitative value of sLOX-1 protein was used as the LOX-1 protein amount. The measurement of sLOX-1 protein amount was performed according to the procedure described in Inoue et al, 2010, Clinical Chemistry 56(4) (doi:10.1373/clinchem.2009.140707). The general procedure is the same as for the measurement of LAB amount, except that anti-human LOX-1 antibody was used instead of sLOX-1 and anti-human LOX-1 antibody was used instead of HRP-labeled anti-Apo antibody.

3.算出
 各マーカーの値は以下の方法で算出した:
 LOX-index(R):LOX-1タンパク質量(ng/L)×LAB量(μg/L);
 LOX/LAB:LOX-1タンパク質量(ng/L)/LAB量(μg/L);
 Fib4-index:{年齢(歳)×AST(IU/L)}/{血小板数(10^9/L)×√ALT(IU/L)}。
3. Calculation The value of each marker was calculated using the following method:
LOX-index (R) : LOX-1 protein amount (ng/L) × LAB amount (μg/L);
LOX/LAB: LOX-1 protein amount (ng/L)/LAB amount (μg/L);
Fib4-index: {age (years) x AST (IU/L)} / {platelet count (10^9/L) x √ALT (IU/L)}.

<実施例2.本発明のマーカーの評価>
(目的)
 患者の試料から得られた各マーカーの測定結果及び各患者の病態との比較に基づき、本発明のマーカーの鑑別性能を評価する。
<Example 2. Evaluation of the marker of the present invention>
(the purpose)
The differential performance of the markers of the present invention is evaluated based on the measurement results of each marker obtained from patient samples and their comparison with the pathological condition of each patient.

(方法)
1.病態の検査
 血流依存性血管拡張反応(FMD)の検査はUNEXEF18VG(ユネクス社)を用いて行った。
(method)
1. Examination of the pathological condition: Flow-dependent vasodilation (FMD) was tested using UNEXEF18VG (UNEX Corporation).

 線維化の重症度は、病理生検での肝臓の線維化組織の観察に基づき、新犬山分類に従った判断となるが、肝生検が不可である症例においては、CTやMRIを用いた形態学的評価、MRエラストグラフィーによる肝硬度の測定を代替手段として用いた。また、腹部超音波エラストグラフィーによる肝線維化の評価や、血液検査データとしてELFテスト、4型コラーゲン7S、M2BPGi、オートタキシン、ヒアルロン酸等のデータを総合的に組合せた評価を通して重症度の評価を行った。
 その他の所見は、問診、患者の体表面の観察、血圧検査に基づいて得た。
The severity of fibrosis was determined according to the New Inuyama classification based on the observation of fibrotic liver tissue in pathological biopsy. However, in cases where liver biopsy was not possible, morphological evaluation using CT or MRI, and measurement of liver stiffness by MR elastography were used as alternative methods. Furthermore, the severity was assessed through evaluation of liver fibrosis using abdominal ultrasound elastography and a comprehensive evaluation combining blood test data such as ELF test, type IV collagen 7S, M2BPGi, autotaxin, and hyaluronic acid.
Other findings were obtained based on the medical history, physical examination of the patient, and blood pressure measurement.

2.統計解析
 二群間の比較にはウィルコクソンの2標本検定を用いた。
 2つのマーカーの相関の解析には、スピアマン順位相関係数に基づく無相関検定を用いた。
2. Statistical Analysis: The Wilcoxon two-sample test was used to compare the two groups.
To analyze the correlation between the two markers, we used a test of no correlation based on Spearman's rank correlation coefficient.

 LOX/LABとFIB-4 indexに関する単位オッズ比の解析には、ACAの陽性陰性を目的変数、LOX-1/LAB比とFIB-4 indexを説明変数としたロジスティック回帰分析を行った。
 いずれの統計解析もJMP(バージョン18、JMP Statistical Discovery社)を用いて行った。
For the analysis of unit odds ratios related to LOX/LAB and FIB-4 index, logistic regression analysis was performed with ACA positive/negative as the dependent variable and the LOX-1/LAB ratio and FIB-4 index as independent variables.
All statistical analyses were performed using JMP (version 18, JMP Statistical Discovery).

(結果)
 結果を表1及び図1~7に示す。
 図1に示す通り、LOX-1タンパク質量はACA陰性群に比べ、ACA陽性群において有意に高かった。しかしながら、いずれの群においても値の分布域の重なりが大きく、この値のみによって両群を明確に区別することは難しかった。なお、分布の様子は、LOX-1タンパク質量の自然対数を取った場合も変化はなかった(図2)。
(result)
The results are shown in Table 1 and Figures 1-7.
As shown in Figure 1, LOX-1 protein levels were significantly higher in the ACA-positive group compared to the ACA-negative group. However, there was a large overlap in the distribution ranges of the values in both groups, making it difficult to clearly distinguish between the two groups based solely on these values. Furthermore, the distribution remained unchanged even when the natural logarithm of the LOX-1 protein levels was taken (Figure 2).

 一方、図3に示す通り、LAB量はACA陰性群に比べ、ACA陽性群において有意に低かった。しかし、LAB量、つまり酸化LDL量が低いのみでは、生活習慣病のリスクが低い健常個体と区別することができない。 On the other hand, as shown in Figure 3, LAB levels were significantly lower in the ACA-positive group compared to the ACA-negative group. However, low LAB levels, i.e., low oxidized LDL levels, alone are not sufficient to distinguish between healthy individuals with a low risk of lifestyle-related diseases and those with low LAB levels.

 そこで、LAB量とLOX-1タンパク質量の積で表されるLOX-index(R)の値を比較したところ、調べられたいずれの患者も脳梗塞・心筋梗塞発症リスクについて高リスクには分類されず、両群に有意差は無かった(図4)。また、ACA陽性群の個体はLAB量が低く、いずれもLOX-index(R)に基づく脳梗塞・心筋梗塞発症リスクは低リスク又は中高リスクに分類された。 Therefore, when we compared the LOX-index (R) values, which are the product of LAB levels and LOX-1 protein levels, none of the patients examined were classified as high-risk for stroke or myocardial infarction, and there was no significant difference between the two groups (Figure 4). In addition, individuals in the ACA-positive group had low LAB levels, and all of them were classified as low-risk or medium-high-risk for stroke or myocardial infarction based on the LOX-index (R) .

 さらに、細胞外マトリックス関連因子であり、一般的な肝線維化マーカーの一つとされるヒアルロン酸(図5A)やIV型コラーゲン(図5B)について調べたところ、いずれのマーカーにおいても、ACA陽性群とACA陰性群に有意な差は無かった。この結果は、これらのマーカーの線維化判別のための特異度が高くないという一般的な知見に合致する。 Furthermore, when we examined hyaluronic acid (Figure 5A) and type IV collagen (Figure 5B), which are extracellular matrix-related factors and are considered common liver fibrosis markers, we found no significant difference between the ACA-positive and ACA-negative groups for either marker. This result is consistent with the general understanding that these markers do not have high specificity for discriminating fibrosis.

 確立されている肝線維化マーカーとしてFIB-4 indexが知られている。そこで、FIB-4 indexによる比較を行ったところ(図6)、ACA陰性群に比べ、ACA陽性群において有意に高かった。しかしながら、FIB-4 indexが高値として判定される2.67以上(図6中、破線以上)であった個体はACA陽性個体6個体のうち2個体に留まった。 The FIB-4 index is a well-established marker for liver fibrosis. Therefore, a comparison using the FIB-4 index (Figure 6) showed that the ACA-positive group had significantly higher FIB-4 scores compared to the ACA-negative group. However, only 2 out of 6 ACA-positive individuals had an FIB-4 index of 2.67 or higher (above the dashed line in Figure 6), which is considered high.

 一方、本発明のマーカーであるLOX/LABの場合(図7)、ACA陰性群に比べ、ACA陽性群において有意に高く、両群の値の分布域も明確に区別可能であった。また、LOX/LABはFIB-4 indexと有意に相関し(p=0.0326)、このマーカーが信頼度の高いマーカーであることが示された。 On the other hand, in the case of LOX/LAB, the marker of the present invention (Figure 7), the values were significantly higher in the ACA-positive group compared to the ACA-negative group, and the distribution ranges of the values in both groups were clearly distinguishable. Furthermore, LOX/LAB showed a significant correlation with the FIB-4 index (p = 0.0326), demonstrating that this marker is a highly reliable indicator.

 FIB-4 indexとLOX/LABの判定精度をさらに明らかにするために、ACA陽性度を目的変数とし、FIB-4 indexの値及びLOX/LABの自然対数の値を説明変数とした場合の各マーカーの単位オッズ比を算出した。その結果を表1に示す。 To further clarify the accuracy of the FIB-4 index and LOX/LAB, the unit odds ratios for each marker were calculated using ACA positivity as the dependent variable and the FIB-4 index value and the natural logarithm of LOX/LAB as independent variables.  表1に示す通り、log(LOX/LAB)の単位オッズ比はFib4-indexより大きかった。単位オッズ比は、目的変数を説明変数の多項式で表したロジスティック回帰式におけるその説明変数の係数を直接反映する値である。このことから、ACA陽性度に対する寄与度がFib4-indexと比較してlog(LOX/LAB)で高いことが示された。また、log(LOX/LAB)の単位オッズ比の信頼区間が1をまたいでいないことから、log(LOX/LAB)はFib4-indexとの比較において、ACA陽性度に有意に寄与していることが示された。 As shown in Table 1, the unit odds ratio for log(LOX/LAB) was larger than that of the Fib4-index. The unit odds ratio directly reflects the coefficients of the explanatory variables in a logistic regression equation where the dependent variable is expressed as a polynomial of the explanatory variables. This indicates that log(LOX/LAB) has a higher contribution to ACA positivity compared to the Fib4-index. Furthermore, since the confidence interval for the unit odds ratio of log(LOX/LAB) does not cross 1, it is shown that log(LOX/LAB) significantly contributes to ACA positivity compared to the Fib4-index.

 また、ACA陽性個体における血管内皮障害の程度、線維化の重症度及びその他の所見を以下の表2に示す。 Furthermore, the degree of vascular endothelial damage, the severity of fibrosis, and other findings in ACA-positive individuals are shown in Table 2 below.

 表2に示す通り、ACA陽性群中、Fib4-indexが最も高かった個体(図中、黒三角で示す個体)のFMDの値は3.8%と低く、血管内皮障害を有していた。また、この個体は肝線維化に伴って生じる特発性門脈圧亢進症、及び限局皮膚硬化型全身性強皮症を呈していた。このように、肝臓において線維化が進行して肝硬変が起きると、しばしば門脈圧が亢進し、さらに病状が進展すると、動脈に静脈から血液が逆流することにより体の他の部位における動脈の血圧が上昇する。この個体においても肺動脈高血圧症がみられた。 As shown in Table 2, the individual with the highest Fib4-index among the ACA-positive group (indicated by a black triangle in the figure) had a low FMD value of 3.8%, indicating vascular endothelial dysfunction. Furthermore, this individual presented with idiopathic portal hypertension and localized cutaneous systemic scleroderma, both associated with hepatic fibrosis. Thus, when fibrosis progresses in the liver and cirrhosis occurs, portal pressure often increases, and as the disease progresses further, blood reflux from veins into arteries leads to elevated arterial blood pressure in other parts of the body. Pulmonary hypertension was also observed in this individual.

 また、LOX/LAB値が高かった2個体のうち、図中、黒四角で示す個体においては、FMDの値がさらに低く、強皮症を呈していた。また、図中、黒丸で示す個体は、上述の個体と同様、シェーグレン症候群が観察され、線維化疾患が進行していることが示唆された。 Furthermore, among the two individuals with high LOX/LAB values, the individual indicated by the black square in the figure had an even lower FMD value and exhibited scleroderma. The individual indicated by the black circle in the figure, similar to the aforementioned individual, showed Sjögren's syndrome, suggesting the progression of fibrotic disease.

 これらの2個体はFIB-4 indexが2.67を下回っていることから、病理検体による詳細な検査は行われていない。しかし、これらの個体の所見に基づいて、本発明のマーカーが従来のマーカーより広く線維化が進行した病態を捕捉可能であることが示唆された。 Since these two individuals had FIB-4 index levels below 2.67, detailed pathological examinations were not performed on their specimens. However, based on the findings of these individuals, it was suggested that the marker of the present invention can capture a wider range of fibrotic conditions than conventional markers.

 また、LOX-1タンパク質は全身の血管内皮細胞に発現するタンパク質であり、LABもその部位にかかわらず血中に存在するリポタンパク質である。したがって、本発明のマーカーによれば、線維化が起きている身体部位によらず、全身の線維化を広く検出可能であることが示唆された。 Furthermore, LOX-1 protein is expressed in vascular endothelial cells throughout the body, and LAB is a lipoprotein present in the blood regardless of its location. Therefore, the markers of this invention suggest that fibrosis can be broadly detected throughout the body, regardless of the specific body part where it occurs.

 以上から、本発明のマーカーが信頼度が高く、また、従来の確立されたマーカーと比較しても、その汎用性及び判別精度共に優れたマーカーであることが示唆された。

 被検体における線維化疾患の罹患の有無又は発症リスクの有無を判定するための方法であって、
 被検体由来の試料中におけるレクチン様酸化低密度リポタンパク質受容体1タンパク質(LOX-1タンパク質)の量及び/又はアポリポタンパク質B含有LOX-1リガンド(LAB)の量に基づく判定値を算出する工程を含み、
 前記判定値が、前記被検体が線維化疾患を罹患している又はその発症リスクを有することを示す、
前記方法。
