Abstract
A body part fixation brace system includes a body brace configured to support a first side of a patient. A restraining member is configured to rest on an opposite, second side of the patient. An adjustable strap assembly is configured to connect the restraining member to the body brace with a patient on a table between the body brace and the restraining member. The adjustable strap assembly includes an adjustable strap mounted to one of the body brace and the restraining member. The adjustable strap includes multiple fastening members arranged front-to-back along a length of the adjustable strap defining discrete locking locations. A fastening mechanism is mounted to the other of the body brace and the restraining member. The fastening mechanism includes engaging members that are configured to releasably mate with individual ones of each of the fastening members carried by the adjustable strap to define a selected strap length.
Description
BODY PART FIXATION BRACE SYSTEMS AND METHODS OF FIXING PARTS OF BODIES USING BODY PART FIXATION BRACES
TECHNICAL FIELD
[0001] The present specification generally relates to body part fixation brace systems and, more specifically, to body part fixation brace systems for use during radiation treatment that include adjustable straps. The present specification also relates to table assemblies for body radiation therapy and methods of fixing a position of a patient on a support table.
BACKGROUND
[0002] During stereotactic radiation therapy (SBRT) or similar radiation therapies, the patient must be immobilized with limited diaphragm movement to improve the effectiveness of the particle radiation therapy. In addition, placement and position of the patient should be consistent over several visits to maintain patient and tumor positioning during each treatment session. Current technologies may be successful in immobilizing patients during therapy; however, they can be cumbersome for use and uncomfortable. For example, some systems use large bars that extend across the patient’s stomach and chest and are secured down with restraining members. These bars can interfere with linear accelerator equipment that is targeting the tumor. In addition, while the current devices may be radiolucent, they can still compromise the imaging and be in the way of the various machinery.
[0003] What is needed are alternative body part fixation braces that reduce interference with the equipment and improve patient comfort.
SUMMARY
[0004] According to a first embodiment, a body part fixation brace system includes a body brace configured to support a first side of a patient. A restraining member is configured to rest on an opposite, second side of the patient. An adjustable strap assembly is configured to connect the restraining member to the body brace with a patient on a table between the body brace and the restraining member. The adjustable strap assembly includes an adjustable strap mounted to one of the body brace and the restraining member. The adjustable strap includes multiple fastening members arranged front- to-back along a length of the adjustable strap defining discrete locking
locations. A fastening mechanism is mounted to the other of the body brace and the restraining member. The fastening mechanism includes engaging members that are configured to releasably mate with individual ones of each of the fastening members carried by the adjustable strap to define a selected strap length.

A restraining member is configured to rest on an opposite, second side of the patient. An adjustable strap assembly is configured to connect the restraining member to the body brace with a patient on a table between the body brace and the restraining member. The method includes locating the patient on the support table between a body brace mounted to the support table and a restraining member configured to rest on a body part of the patient. A strap length of an adjustable strap assembly is adjusted that connects the restraining member to the body brace. The adjustable strap assembly includes an adjustable strap mounted to one of the body brace and the restraining member. The adjustable strap includes multiple hooks arranged front-to-back along a length of the adjustable strap. Each hook of the multiple hooks defines a discrete fastening location. A buckle is mounted to the other of the body brace and the restraining member. The buckle includes a latch that is configured to releasably mate with one of multiple hooks carried by the adjustable strap to define a selected strap length.

DETAILED DESCRIPTION
[0017] Embodiments described herein are generally directed to body part fixation brace systems that limit diaphragm movement to improve therapy. The body part fixation braces include a body brace that is configured to be mounted to a table and a restraining member that is configured to rest on a patient’s abdomen. An adjustable strap assembly is configured to connect the restraining member to the body brace with a patient on the table between the body brace and the restraining member. The adjustable strap assembly includes an adjustable strap mounted to one of the body brace and the restraining member. The adjustable strap includes multiple hooks as discrete fastening members arranged in a front-to-back array along a length of the adjustable strap. A buckle is a fastening member that is mounted to the other of the body brace and the restraining member. The buckle includes a latch as an engaging member that is configured to releasably mate with one of multiple hooks carried by the adjustable strap to define a selected strap length.
[0018] Referring to FIG. 1, a body part fixation brace system 10 includes a body brace 12 and a restraining member 14 (e.g., a paddle). The body brace 12 includes a planar body 18 having a front face 20, a rear face 22, a top 24, a bottom 26 and opposite sides 28 and 30. In the illustrated embodiment, the sides 28 and 30 are slanted toward each other from the bottom 26 to the top 24 so that the body 18 is somewhat tapered toward the top 24 whereas in other embodiments, the sides are not slanted and the body is not tapered. The body 18 is configured to be mounted to a support table using mounting elements which, in this embodiment, are openings or holes 32, 34, 36 and 38 through which fasteners can be inserted and threaded through corresponding engaging elements on the support table. In other embodiments, the body 18 may be mounted using other suitable mechanisms such as clips or straps. The body brace 12 may be shaped and configured to rest against a back of the patient. Any suitable shape configured to rest against the back of the patient may be used. In this regard, the body brace 12 may be a posterior sub-assembly of the body part fixation brace system 10.
[0019] Adjustable straps 40, 42, 44 and 46 of an adjustable strap assembly 47 extend outward from the body 18, for example, from the sides 28 and 30 of the body 18. In some embodiments, the adjustable straps 40, 42, 44 and 46 are connected to the body 18 by brackets 48, 50, 52 and 54. In other embodiments, the adjustable straps 40, 42, 44, 46 may be directly mounted to the body 18. While four adjustable straps 40, 42, 44 and 46 are shown that firmly secure the body 18 to the patient, any number of adjustable straps may be used, such as two or even one or more than
four, such as six. The adjustable straps 40, 42, 44 and 46 each include a base structure 56, which may or may not be strip-form and an array of front- to-back hooks 58 as fastening members aligned in a row, and each hook 58 may be elongated across a width of the base structure 56. In some embodiments, the hooks 58 may be formed as a monolithic part of the base structure 56 (e.g., such that the hooks 58 and the base structure 56 form a unitary body), such as by molding the hooks 58 with the base structure 56. Each hook 58 includes a stem portion 69 and a head portion 62 that extends outward from the stem portion 69 and overhangs the base structure 56. The head portion 62 and the stem portion 69 may together define a single, discrete locking location along a selected strap length such that the number of locking locations is defined by the number of hooks 58. By “discrete,” it is meant that there is a single fastening location at spaced-apart intervals along the length of the adjustable straps. That is, the number of locking locations is equal to the number of hooks. Further, the spacing between adjacent hooks 58 may be equidistant such that the density of hooks 58 per unit length remains constant along a length of the base structure 56. In other embodiments, the density of hooks 58 may change along lengths of the base structure 56 such that one length of the base structure 56 may have a higher density of hooks 58 than another length. In other embodiments, other hook shapes may be used for the hooks 58.
[0020] The adjustable straps 40, 42, 44 and 46 may be formed of a flexible, non-stretchy (non- extensible) material, such as a plastic or woven fabric material, so that they may bend into various positions and hug the contours of the body during use. In other embodiments, a stretchy (extensible) material may be used. The base 18 and the adjustable straps 40, 42, 44 and 46, including the base structure 56 and the hooks 58, may also be formed using a radiolucent material (i.e., formed of a radiolucent material), like nylon, to reduce interfering with the particle radiation and imaging.
[0021] The restraining member 14 may include a planar body 60 having a front face 63, a rear face 64, a top 66, a bottom 68 and opposite sides 70 and 72. In the illustrated embodiment, the sides 70 and 72 are slanted toward each other from the bottom 68 to the top 66 so that the body 60 is somewhat tapered toward the top 66 while in other embodiments the sides 70 and 72 may not slant toward each other. The body 60 may be shaped and configured to be placed against the abdomen of a patient. Any suitable shape for being placed against the abdomen may be used. In this regard, the restraining member 14 may be an anterior sub-assembly of the body part fixation device 10. The front face 63 may further include a cushion, such as padding, that can rest between the planar body 60 and the patient during use.
[0022] Strap connectors 74, 76, 78 and 80 are fastening mechanisms that extend outward from the sides 70 and 72 of the body 60. The strap connectors 74, 76, 78 and 80 may include a connector arm 82 that includes a projecting portion 84 and a curved portion 86 that extends outward from the projecting portion 86 and is curved back toward the body 60. Any suitable shape for the strap connectors 74, 76, 78 and 80 may be used, such as straight (e.g., without a curved portion).
[0023] Referring also to FIG. 2, a buckle 88 is an engaging member that is movably connected to each curved portion 86. The buckles 88 comprise a latch 90 that is configured to releasably mate with only one of the hooks 58 carried by the adjustable straps 40, 42, 44 and 46 to define a selected strap length. The latch 90 is connected to a rod 92 that is movably received within a slot 94 of the curved portion 86 such that the buckles 88 are movable within the slots 94 between a lowered, latched position and a raised, unlatched position as represented by arrow 95. Any suitable latching arrangement may be used, such as the latches may not be moveable relative to the curved portions 86. In some embodiments, the buckles 88 may be connected to latches that can be used to lock and unlock the buckles.
[0024] In FIG. 2, a side view of a portion of one of the adjustable straps 40 is shown. While adjustable strap 40 is shown in FIG. 2, any and all of the adjustable straps 40, 42, 44 and 46 may include the features described with regard to adjustable strap 40. The adjustable strap 40 may include identifiers 100, each identifier 100 being unique to a particular hook 58 and its respective locking location. In this embodiment, the identifiers 100 are centered beneath their respective hook 58 on a side edge 102 of the base structure 56. In FIG. 3, identifiers 100 are located on a same face 104 as their respective hooks 58. In FIG. 4, identifiers 100 are located on the stem portions 60. In some embodiments, there are provided multiple locations where the identifiers 100 are located including combinations of FIGS. 2-4. In the embodiment of FIGS. 2-4, the identifiers 100 are letters followed by sequential numbers. The letter can indicate which strap 40, 42, 44 and 46 and the number can indicate which hook 58. However, any type of identifiers 100 may be used, such as letters, symbols, combinations of letters and symbols, etc.
[0025] As can be appreciated, each identifier 100 is associated with a unique locking position and selected strap length. Once the strap length and locking position are determined, the identifier 100 may be recorded and used for a subsequent treatment procedure.
[0026] Referring to FIG. 5 and also to FIGS. 1 and 2, the body part fixation brace 10 is illustrated in use. A radiation apparatus 101, such as one for SBRT, includes a support table 112 for
supporting a patient P thereon. The body brace 12 is mounted to the support table 112 with the bottom 26 facing the foot of the support table 112 using mounts 32, 34, 36 and 38 (FIG. 1). The patient P is placed supine on the support table 112 and on the body brace 12. The restraining member 14 is placed on the patient and strap lengths of the adjustable straps 40, 42, 44 and 46 are adjusted by the buckles 88 engaging their respective hooks 58 at their desired positions by moving the buckles 88 from the unlatched position to the latched position. As can be seen, the body 18 of the body brace 12 defines a first patient contact area Ai (FIG. 1) that is greater than a second patient contact area A2 defined by the body 60 of the restraining member 14. The associated identifiers 100 may then be recorded when the desired strap lengths are reached. Once the treatment is finalized, the buckles 88 may be moved from the locked position to the unlocked position and the restraining member 14 may be removed from the patient P.
[0027] Referring to FIG. 6, in another embodiment, a strap feeding device 120 is a fastening mechanism that includes motor assemblies 122 that are configured to take-up indexed adjustable straps 124 of a body brace 126. For example, the adjustable straps 124 may include discrete fastening members 132 in the form of slots, notches, etc. that are configured to be engaged with engaging members 134 of a wheel 136, as shown by FIG. 7. The motor 122 can be used to rotate the wheel 136, which moves the engaging members 134 from one engaging member 134 to the next and indexes the adjustable strap 124 a selected amount as represented by arrow 138. Once the adjustable straps 124 are connected to the motor assemblies 122, for example, by threading them through openings 128, the motor assemblies can be activated to take-up the straps 124 and adjust a strap length. A sensor 130, such as a force sensor, may be provided that can monitor force or tension on the strap 124 and inhibit over-tightening of the straps 124 based on a predetermined magnitude of force and may provide an indication to a controller that controls operation of the motor assemblies 122 to turn off the motor assemblies 122 at a predetermined magnitude of force. The sensor 130 may also be used to tighten the straps 124 to a selectable level of tightness/force.
[0028] Referring to FIG. 8, an exemplary method 140 of fixing a position of a patient on a support table is schematic shown. Referring also to FIG. 5, the method 140 includes, at step 142, locating the patient P on the support table 112 between the body brace 12, 126 mounted to the support table and a restraining member 14, 120 configured to rest on a body part of the patient P. In particular, this step can include placing the patient P on the support table 112 and on the body brace 12, 126. Then, the restraining member 14 and 120 can be placed on the abdomen of the patient P as shown by FIG. 5. The adjustable strap assembly 47, 127 may then be connected
together by engaging fastening members 58, 132 of the adjustable straps 42, 44, 46, 48, 124 with engaging members 88, 134 of the fastening mechanism as discussed above. At step 144, a strap length of the adjustable strap assembly 47, 127 connecting the restraining member 14, 120 to the body brace 12, 126 is adjusted by engaging a selected fastening member 58, 132 with the respective engaging member 88, 134, which can include, at step 146, identifying the selected strap length using an identifier 100 (FIG. 2) associated with the one of the fastening members 58, 132. At step 148, the patient P may be released by disengaging the fastening members 14, 120 with the engaging members 88, 134.
[0029] The above-described body part fixation braces provides adjustable straps that have single hooks that are arranged front-to-back in a single row along the lengths of the adjustable straps. Each hook defines its own locking location for a mating buckle to engage to provide a selected strap length. Such adjustability and flexibility of the adjustable straps can provide a more comfortable condition for the patient than current fixation braces. Further, each hook has a corresponding indicator, which can be recorded and used whenever the body part fixation braces are used for the same patient. The body part fixation braces can also be formed of a radiolucent material so that they do not interfere with the treatment or imaging. While an abdominal fixation brace is shown in the figures, the body part fixation braces may be configured for use at other parts of the body, such as the head or limbs.

Claims
1. A body part fixation brace system comprising: a body brace configured to support a first side of a patient; a restraining member configured to rest on an opposite, second side of the patient; and an adjustable strap assembly that is configured to connect the restraining member to the body brace with a patient on a table between the body brace and the restraining member, the adjustable strap assembly comprising: an adjustable strap mounted to one of the body brace and the restraining member, the adjustable strap comprising multiple fastening members arranged front- to-back along a length of the adjustable strap defining discrete locking locations; and a fastening mechanism mounted to the other of the body brace and the restraining member, the fastening mechanism comprising engaging members that are configured to releasably mate with individual ones of each of the fastening members carried by the adjustable strap to define a selected strap length.
2. The body part fixation brace system of claim 1, wherein the selected strap length is a first selected strap length, the fastening members are arranged front-to-back along the length of the adjustable strap such that engagement of one of the engaging members with a different one of the fastening members defines a second selected strap length.
3. The body part fixation brace system of claim 2, wherein the adjustable strap comprises an identifier associated with the one of the fastening members that provides an indication of location of the one of the engaging members.
4. The body part fixation brace system of claim 3, wherein the adjustable strap comprises a radiolucent material.
5. The body part fixation brace system of claim 3, wherein the identifier is a first identifier, the adjustable strap comprises a second identifier associated with the different one of the fastening members that provides an indication of location of the different one of the fastening members.
6. The body part fixation device system of claim 1, wherein the fastening mechanism comprises a strap feeding device comprising a motor that is operatively connected to the adjustable strap that is configured to adjust a length of the adjustable strap.
7. The body part fixation device of claim 6 further comprising a sensor that is configured to provide a signal that is indicative of tension in the adjustable strap.
8. The body part fixation brace system of claim 1, wherein the body brace defines a first patient contact area that is greater than a second patient contact area defined by the restraining member.
9. The body part fixation brace system of claim 1, wherein the body brace comprises mounting elements configured to engage with corresponding engaging elements of the table.
10. A table assembly for body radiation therapy, comprising: a support table configured to support a patient thereon; a body brace comprising mounts fixed to the support table; a restraining member configured to rest on a body part of a patient; an adjustable strap assembly that is configured to connect the restraining member to the body brace with the patient on the support table between the body brace and the restraining member, wherein the adjustable strap assembly comprises: an adjustable strap mounted to one of the body brace and the restraining member, the adjustable strap comprising multiple fastening members arranged front- to-back along a length of the adjustable strap defining discrete locking locations defining discrete locking locations; and a fastening mechanism mounted to the other of the body brace and the restraining member, the fastening mechanism comprising engaging members that are configured to releasably mate with individual ones of the fastening members carried by the adjustable strap to define a selected strap length.
11. The table assembly of claim 10, wherein the restraining member comprises the fastening member and the body brace comprises the adjustable strap.
12. The table assembly of claim 11, wherein the fastening member comprises a latch that is configured to releasably mate with one of the fastening member of the adjustable strap to define the selected strap length, the fastening members comprising hooks.
13. The table assembly of claim 12, wherein the selected strap length is a first selected strap length, the hooks are arranged front-to-back along the length of the adjustable strap such that engagement of the latch with a different one of the hooks defines a second selected strap length.
14. The table assembly of claim 12, wherein the adjustable strap comprises an identifier associated with the one of the hooks that provides an indication of location of the one of the hooks.
15. The table assembly of claim 14, wherein the adjustable strap comprises a radiolucent material.
16. The table assembly of claim 15, wherein the identifier is a first identifier, the adjustable strap comprises a second identifier associated with the different one of the hooks that provides an indication of location of the different one of the hooks.
17. The table assembly of claim 10, wherein the fastening mechanism comprises a strap feeding device comprising a motor that is operatively connected to the adjustable strap that is configured to adjust a length of the adjustable strap.
18. The table assembly of claim 17, wherein the motor controls operation of the motor based on a signal from a sensor that is indicative of tension the adjustable strap.
19. A method of fixing a position of a patient on a support table, the method comprising: locating the patient on the support table between a body brace mounted to the support table and a restraining member configured to rest on a body part of the patient; and adjusting a strap length of an adjustable strap assembly connecting the restraining member to the body brace, the adjustable strap assembly comprising: an adjustable strap mounted to one of the body brace and the restraining member, the adjustable strap comprising multiple hooks arranged front-to-back along a length of
the adjustable strap, each hook of the multiple hooks defining a discrete fastening location; and a buckle mounted to the other of the body brace and the restraining member, the buckle comprising a latch that is configured to releasably mate with one of multiple hooks carried by the adjustable strap to define a selected strap length.
20. The method of claim 19 further comprising: identifying the selected strap length using an identifier associated with the one of the multiple hooks; and latching the latch to the one of the multiple hooks associated with the identifier.
