Description

情報処理システム、情報処理方法、プログラム
 本発明は、視神経脊髄炎スペクトラム障害を含む視神経疾患の疾患関連情報を可視化するための情報処理システム、情報処理方法、プログラムに関する。
 視神経脊髄炎スペクトラム障害(以下、「NMOSD」とも呼ぶ。)は、日本での有病率が10万人あたり2~4人といわれており、重度の再発を繰り返し致命的となり得る中枢神経系の自己免疫疾患である。身体の免疫システムが、健康な細胞(一般的には視神経、脊髄および脳)を攻撃して再発や重篤な傷害をもたらし、その結果、眼の痛みや失明、重度の筋力低下、麻痺、しびれ、腸や膀胱の機能低下および呼吸不全を引き起こす恐れがある。
 例えば、特許文献1においては、視神経脊髄炎の症状を改善するまたは軽減する抗炎症性製剤に関する発明が記載されている。

 上述のとおり、NMOSDは重度の再発を繰り返す疾患である。しかしながら患者が診療時に、再発に関連する可能性のある症状を医師に適切に伝えられていないことが多く、患者・医師ともに不安を感じている。したがって、診療日間における患者の定量・定性情報を医師に提供でき、NMOSDの再発兆候を早期発見可能とすることができるようなデジタルツールが求められている。
 本発明は、患者から医師への日常のNMOSD関連症状等の情報伝達と、将来的には患者が入力したデータから再発兆候の早期発見を可能とするために、視神経脊髄炎スペクトラム障害を含む視神経疾患の疾患関連情報を可視化するための情報処理システムを提供することを目的とする。
 上記課題を解決するための本発明の主たる発明は、対象者の視神経疾患関連情報を少なくとも含む疾患関連情報を取得する疾患関連情報取得部と、前記視神経疾患関連情報を含む前記疾患関連情報に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加する可視化情報付加部と、前記可視化情報を前記第1図に付加した第2図を含む表示情報を生成する表示情報生成部と、を備える。
 その他本願が開示する課題やその解決方法については、発明の実施形態の欄及び図面により明らかにされる。
 本発明によれば、容易にNMOSDの再発兆候を発見可能とするために、視神経疾患関連情報を含む疾患関連情報を可視化するための情報処理システムを提供することができる。

 以下、本開示の実施の形態について、図面を参照しながら説明する。なお、本明細書および図面において、実質的に同一の機能構成を有する構成要素については、同一の符号を付することにより重複説明を省略する。
 <システム構成>
 図1は、本開示の一実施形態に係る情報処理システム(以下、「本システム」とも呼ぶ)の構成例を示す図である。本システムは、対象者の視神経疾患間情報(例えば、対象者端末において実行されたアプリケーションによるフリッカー検査の検査結果情報の少なくとも一部を含む)を少なくとも含む疾患関連情報に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加した第2図を対象者端末(例えば、医師端末)に表示する。
 図1は、本実施形態に係る情報処理システムの全体構成例を示す図である。同図に示すように、本実施形態に係る情報処理システム1は、サーバ装置10と対象者端末20とユーザ端末30が通信ネットワークNWを介して互いに通信可能に接続されている。通信ネットワークNWは、例えば、インターネットやLAN(Local Area Network)であり、公衆電話回線網、専用電話回線網、携帯電話回線網、イーサネット(登録商標)、無線通信路等により構築される。
 サーバ装置10は、情報処理システム全体を管理するコンピュータである。サーバ装置10は、例えば、ワークステーションやパーソナルコンピュータ、クラウドコンピューティングにより論理的に実現される仮想コンピュータ等である。
 対象者端末20は、ユーザ50(例えば医師など)により疾患に関する情報(以下、「疾患関連情報」という。)の確認対象となる対象者40が有するデバイスであって、スマートフォン、パーソナルコンピュータ、タブレット端末等が含まれる。なお、対象者端末20は、アプリケーションプログラムをインストールさせることができ、対象者端末20の演算装置によって、サーバ装置10によって実行される全部または一部機能を実行させることもでき、また、サーバ装置10に備えられる記憶装置103に格納される情報のうち全部または一部を、対象者端末20に内蔵される記憶部に格納させることもできる。また、その逆として、サーバ装置10の演算装置によって、対象者端末20によって実行される全部または一部機能を実行させることもでき、また、対象者端末20に備えられる記憶装置103に格納される情報のうち全部または一部を、サーバ装置10に内蔵される記憶部に格納させることもできる。
 ユーザ端末30は、ユーザ50(例えば医師など)が対象者の疾患関連情報の確認を行うデバイスであって、パーソナルコンピュータ、スマートフォン、タブレット端末等が含まれる。なお、ユーザ端末30は、アプリケーションプログラムをインストールさせることができ、ユーザ端末30の演算装置によって、サーバ装置10によって実行される全部または一部機能を実行させることもでき、また、サーバ装置10に備えられる記憶装置103に格納される情報のうち全部または一部を、ユーザ端末30に内蔵される記憶部に格納させることもできる。また、その逆として、サーバ装置10の演算装置によって、ユーザ端末30によって実行される全部または一部機能を実行させることもでき、また、ユーザ端末30に備えられる記憶装置103に格納される情報のうち全部または一部を、サーバ装置10に内蔵される記憶部に格納させることもできる。
 次に、上記情報処理システム1の構成の詳細について説明する。
 <ハードウェア構成>
 図2は、本実施形態においてサーバ装置10を実現するコンピュータのハードウェア構成例を示す図である。サーバ装置10は、演算装置101、メモリ102、記憶装置103、通信インタフェース104、入力装置105、出力装置106を備える。これらはバス(図示せず)を通じて相互に電気的に接続される。なお、上記のように、サーバ装置10に含まれる記憶装置103等の装置及び演算装置101によって実行される機能の全部または一部を、対象者端末20やユーザ端末30等の他の装置に備えさせる、または、実行させることができる。
 演算装置101は、サーバ装置10全体の動作を制御し、各要素間におけるデータの送受信の制御、およびアプリケーションの実行および認証処理に必要な情報処理等を行う。演算装置101は、例えば、CPU(Central Processing Unit)やGPU(Graphics Processing Unit)等のプロセッサであり、記憶装置103に格納されメモリ102に展開されたプログラム等を実行して各情報処理を実施する。
 メモリ102は、DRAM(Dynamic Random Access Memory)等の揮発性記憶装置で構成される主記憶と、フラッシュメモリまたはHDD(Hard Disc Drive)等の不揮発性記憶装置で構成される補助記憶と、を含む。メモリ102は、演算装置101のワークエリア等として使用される。また、サーバ装置10の起動時に実行されるBIOS(Basic Input/Output System)および各種設定情報等を格納する。
 記憶装置103は、各種のデータやアプリケーションプログラム等の各種プログラムを格納する。例えば、ハードディスクドライブやソリッドステートドライブ、フラッシュメモリ等である。
 通信インタフェース104は、サーバ装置10を通信ネットワークNWに接続するためのインタフェースであり、例えばイーサネット(登録商標)に接続するためのアダプタ、公衆電話回線網に接続するためのモデム、無線通信を行うための無線通信機、シリアル通信のためのUSB(Universal Serial Bus)コネクタやRS132Cコネクタ等である。ネットワークに接続する。
 入力装置105は、データを入力する、例えば、キーボードやマウス、タッチパネル、ボタン、マイクロフォン、カメラ(撮像部)等である。
 出力装置106は、データを出力する、例えば、ディスプレイやプリンタ、スピーカ等である。
 <ソフトウェア構成>
 図3は、本実施形態においてサーバ装置10のソフトウェア構成例を示す図である。同図に示すように、サーバ装置10は、情報管理部110、疾患関連情報取得部111、可視化情報付加部112、疾患検査部113、表示情報生成部114の各機能部と、対象者情報記憶部131、疾患関連情報記憶部132の各記憶部とを備える。なお、必要に応じて、ユーザ(例えば医師)に関する情報(例えばユーザID、所属、連絡先など)を記憶するユーザ情報記憶部(不図示)を備えていてもよい。
 なお、上記各機能部は、サーバ装置10が備える演算装置101が記憶装置103に記憶されているプログラムをメモリ102に読み出して実行することにより実現され、上記各記憶部は、サーバ装置10が備えるメモリ102および記憶装置103が提供する記憶領域の一部として実現される。
 対象者情報記憶部131は、対象者40に関する情報を記憶する。図4は、対象者情報記憶部131の構成例を示す図である。同図に示すように、対象者情報記憶部131が記憶する対象者情報には、対象者40を特定する対象者IDに対応付けて、基本情報(氏名、住所、性別、年齢等の個人情報)、身長、体重等の身体に関する情報、視神経疾患(特に、NMOSD)に関連する病歴、進行等の情報が含まれる。
 疾患関連情報記憶部132は、対象者40の諸疾患に関する検査結果情報などの諸疾患に関する情報を含む疾患関連情報を記憶する。図5は、疾患関連情報記憶部132の構成例を示す図である。同図に示すように、疾患関連情報記憶部132は疾患関連情報を記憶し、疾患関連情報には、対象者を特定する対象者IDに対応付けて、対象者が患っている疾患の種類を示す疾患種別情報と、各疾患種別情報に紐づき、対象者が患っている疾患が発症している身体の対象部位を示す疾患対象部位情報と、各疾患種別情報に紐づき、対象者が患っている疾患に対する検査結果情報と、を含む。なお、本実施例では、対象者情報記憶部131と疾患関連情報記憶部132とは便宜的に互いに分けて構成されるように例示されているが、疾患関連情報記憶部132に含まれる情報の全部または一部を対象者情報記憶部131に統合してもよい。
 疾患関連情報は、対象者40の諸疾患に関する情報であり、特にNMOSDに関連して、視神経疾患関連情報を含んでいる。疾患関連情報は、例えば、対象者端末20において実行されるアプリケーション(Webブラウザを含む)を利用して対象者40が諸疾患について検査を行うことで生成された検査結果情報であってもよい。視神経疾患関連情報は、例えば、フリッカー検査の検査結果情報であってもよい。検査結果情報は、対象者端末20へサーバ装置10の疾患検査部113から提供される検査を実行した結果の情報であってもよいし、本システム外のアプリケーションと連携して、当該外部アプリケーションから提供される検査を実行した結果を取得した情報であってもよく、対象者端末20を介して取得してもよいし、サーバ装置10から直接アクセスして取得してもよい。
 情報管理部110は、各種記憶部から所定の情報を読み出して、外部(例えば、対象者端末20やユーザ端末30)へ送信したり、外部から所定の情報を受け付けて、対応する記憶部へ記憶したりする。より具体的な例としては、例えば、ユーザ操作によりユーザ端末30から送信される要求に応じて、対象者情報記憶部131や疾患関連情報記憶部132などから所定の情報を読み出してユーザ端末30へ送信したり、対象者端末20において実行されたアプリケーション(Webブラウザを含む)により生成された検査結果情報を受け付け、疾患関連情報記憶部132に記憶したりしてもよい。
 疾患関連情報取得部111は、ユーザ端末30からの要求に応じて、疾患関連情報記憶部132に記憶される疾患関連情報(特に視神経疾患関連情報)を取得する。
 可視化情報付加部112は、疾患関連情報(特に視神経疾患関連情報)に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加する。第1図に可視化情報が付加されたものは、第2図という。生成される可視化情報は、疾患関連情報に基づき判定される疾患程度情報に応じて、異なる態様であってもよい。疾患程度情報は、例えば、疾患関連情報に検査結果情報が含まれる場合には、検査結果情報に示される疾患の程度(例えば、検査結果としてAないしFといったように表現される疾患の程度に関する情報など)を示す情報であってもよいし、または、検査結果情報に示される数値と所定の基準値との比較結果に応じた疾患の程度(例えば、複数の基準値を予め設定して疾患の程度を示す分類を区切り、比較結果に応じて該当する疾患程度分類情報など)を示す情報であってもよい。分類は、検査結果同様にA-FやI-Vのような順序のある文字列であってもよいし、安全-注意-危険のような意味合いに段階的な順序がある文字列であってもよく、これらに限らず、サーバ装置10の管理者やユーザが分類を適宜設定するようにしてもよい。
 可視化情報は、例えば、図6に示されるように、疾患種別情報または疾患対象部位情報のいずれかに基づく疾患の位置を示す図形(図6においては星マーク)であってもよい。そして、例えば、疾患程度情報が示す疾患の程度に応じて、図形の大きさや形状が異なったり(例えば、疾患の程度が重いと大きくなる、軽いと丸印、重いと星など)、色が異なったり(例えば、疾患の程度が軽いと青色、疾患の程度が重いと赤色など)してもよい。また、可視化情報は、図形に代えて、疾患の位置に色付けがされてもよく、疾患程度情報に基づき、いわゆるヒートマップ状に複数の色により色付けされてもよい(例えば、疾患がなければ無色または白色、疾患の程度が軽いと青色、疾患の程度が重いと赤色など)。
 さらに、可視化情報は、疾患種別情報または疾患対象部位情報のいずれかに基づく疾患の位置近傍に付与されるテキスト情報であってもよく、サーバ装置10において予め記憶されている所定のテキスト情報(例えば、図6に例示されるように「要確認」のような確認を促すテキスト情報など)から選択されたテキスト情報であってもよく、特に疾患程度情報に基づき、テキスト情報の内容を変更してもよい(例えば、経過観察、要確認など)。また、可視化情報は、疾患種別情報に基づく疾患名を示すテキスト情報であってもよいし、検査結果情報に含まれる情報(例えば検査結果の数値や疾患の程度など)を参照したテキスト情報であってもよい。なお、図6に示されるように、図形とテキスト情報は組み合わせて表示されてもよい。
 疾患検査部113は、上述の検査結果情報を取得するために、サーバ装置10が検査方法を提供する場合に備えている機能部であり、諸疾患に対応する検査を提供する。検査は、例えば、フリッカー検査等のフリッカー刺激(例えば、点滅する光による刺激等)を利用した検査であってもよい。フリッカー検査等とは、例えば、対象者端末20において、フリッカー刺激として点滅する光(例えば、対象者端末20に表示される所定位置の画素)の点滅周波数を増大或いは減少させ、または、フリッカー刺激として点滅する光の高輝度時と低輝度時の輝度の差(コントラスト)を変化させて、対象者40に提示し、点滅の状態が変化した際(点滅が見えるようになった際、または、見えないようになった際)に、対象者40の入力操作に応じて、その際の設定情報(例えば、点滅周波数や切替周波数、切換時の高低それぞれの輝度値など)を疾患関連情報記憶部132に記憶するような検査であってもよい。なお、対象者端末20上のアプリケーションにより所定の検査が実行される場合には、疾患検査部113の一部または全部の機能を対象者端末20の演算処理部に備えるようにしてもよい。
 表示情報生成部114は、対象者端末20の表示部に所定の表示をするための表示情報(第1図に可視化情報を付加した第2図も含む)を生成する。なお、Webブラウザを介したWebサービスではなく、対象者端末20上のアプリケーションにより所定の表示が制御される場合には、表示情報生成部114の一部または全部の機能を対象者端末20の演算処理部に備えるようにしてもよい。
 このように、特に、フリッカー検査に基づく視神経疾患関連情報を含む疾患関連情報に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加した第2図をユーザ端末30に表示させることで、医師等のユーザ50は、容易にNMOSDの疾患の状態が把握できる。将来的には患者が入力したデータから再発兆候の早期発見が可能となる。
 図7は、本実施形態の情報処理システムにおいて実行される処理の流れを示す図である。
 まず、本処理の前処理として、サーバ装置10の情報管理部110は、対象者40(例えば、患者)に関する情報入力を受付け、対象者情報を対象者情報記憶部131に記憶する。対象者情報は、ユーザ50によりユーザ端末30を介して入力されてもよいし、対象者40により対象者端末20を介して入力されてもよい。
 また、本処理の前処理として、対象者40は、諸疾患に関する検査(特にフリッカー検査)を対象者端末20において実行するなどして、疾患関連情報(特に、視神経疾患関連情報)を疾患関連情報記憶部132に記憶する。
 次に、ステップS101として、疾患関連情報取得部111により、ユーザ端末30からの要求に応じて、疾患関連情報記憶部132に記憶される疾患関連情報(特に視神経疾患関連情報)を取得する。
 次に、ステップS102として、可視化情報付加部112により、疾患関連情報(特に視神経疾患関連情報)に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加する。
 次に、ステップS103として、表示情報生成部114により、対象者端末20の表示部に所定の表示をするための表示情報(第1図に可視化情報を付加した第2図も含む)を生成する。
 次に、ステップS104として、情報管理部110により、ユーザ50の要求を受けて生成された表示情報をユーザ端末30へ送信する。
以上のように、本実施形態によれば、容易にNMOSDの再発兆候を発見可能とするために、視神経疾患関連情報を含む疾患関連情報を可視化するための情報処理システムを提供することができる。
 ここで、上記実施形態の情報処理システムの変形例について説明する。上記情報処理システムにおいては、サーバ装置10と対象者端末20とユーザ端末30が通信ネットワークNWを介して互いに通信可能に接続されている構成であったが、さらに対象者及びユーザとは異なる第三者が有する第三者端末(不図示)も通信ネットワークNWを介して接続されている構成であってもよい。第三者端末は、第三者が有するデバイスであって、パーソナルコンピュータ、スマートフォン、タブレット端末等が含まれる。第三者とは、例えば、対象者40の家族や介護者などの対象関係者、医療機器企業の従業員、製薬企業の従業員、医薬品開発業務受託機関の従業員、臨床試験実施者などの外部組織関係者であってもよい。
 本変形例によると、第三者端末においても、上記疾患関連情報(特に、視神経疾患関連情報)、または、ヒトの身体の少なくとも一部の形状を表す第1図に上記可視化情報を付加した第2図の少なくともいずれかを受信し、当該受信情報を第三者が第三者端末を介して確認可能に表示するようにしてもよい。
 より具体的には、例えば、対象者端末20または第三者端末の少なくともいずれかで実行されるアプリケーションにおいて対象者40の家族や介護者などの対象関係者の対象関係者IDと対象者40の対象者IDを対応付ける処理を実行することで、対象者40の疾患関連情報や第2図を対象関係者の第三者端末においても受信可能となり、対象関係者においても対象者40の疾患状態を確認可能となるようにしてもよい。これにより、例えば対象者40が要介護者となった場合などに、介護をしている対象関係者においても疾患状態を容易に確認可能となる。
 また、例えば、第三者が製薬会社の従業員などの外部組織関係者である場合には、対象者端末20またはユーザ端末30の少なくともいずれかで実行されるアプリケーションにおいて外部組織関係者の外部組織関係者IDに対して閲覧許可処理を実行することにより、許可された対象者40に関する疾患関連情報等を外部組織関係者の第三者端末においても受信可能となり、外部組織関係者においても許可された対象者40の疾患状態を確認可能となるようにしてもよい。この場合、外部組織関係者の第三者端末においては、許可された対象者40の氏名または住所の少なくともいずれかの情報を含まない情報(例えばサーバ装置10などで匿名化処理された情報)が送信されるように構成されていてもよい。これにより、研究等の目的において疾患に関する情報を必要としている外部組織関係者においても疾患に関する種々の情報が容易に収集可能となる。そして特に、許可された対象者40の氏名または住所の少なくともいずれかの情報を含まない情報が送信されるように構成されている場合には、匿名性等の担保も両立することが可能となる。
 なお、上記の情報処理システムは、多発性硬化症(MS)等の他の神経疾患にも適用可能である。

     前記視神経疾患関連情報を含む前記疾患関連情報に基づき生成される可視化情報を、ヒトの身体の少なくとも一部の形状を表す第1図に付加する可視化情報付加機能部と、
     前記可視化情報を前記第1図に付加した第2図を含む表示情報を生成する表示情報生成機能部と、をコンピュータにおいて実現する、プログラム。
