Abstract
Description
一方、FVaの変異により、血栓症などの疾患が生じることが知られており、これはAPCへの耐性変異で、APCによるFVaの不活性化が阻害されることにより、TM/APC経路による抗凝固作用が障害される。FV Leiden変異とよばれ、欧米白人における主要な血栓性素因として知られる。
外因系の血液凝固活性化試薬は組織因子または組織トロンボプラスチンであることができる。
ヘパリン様物質は、未分画ヘパリン、低分子ヘパリン、フォンダパリヌクスナトリウム並びにヘパラン硫酸及びデキストラン硫酸からなる群から選択される硫酸化多糖類の一種以上であることができる。
トロンボモジュリンの終濃度は9nM~200nMであることができる。
トロンボモジュリンは可溶性トロンボモジュリンであることができる。
本発明の方法の一態様では、プロテインC阻害剤を添加して前記血液凝固能の評価を行い、プロテインC阻害剤を添加しない場合の結果を対比することでプロテインCおよびプロテインSの機能評価を行うことができる。
本発明の方法の一態様では、前記血液試料がクエン酸で抗凝固処理された血液試料であり、カルシウムと、外因系の血液凝固活性化試薬並びにトロンボモジュリンおよびヘパリン様物質を添加することで血液凝固の評価を行うことができる。
本発明の方法の一態様では、外因系の血液凝固活性化試薬及び接触因子阻害剤を添加して前記血液凝固能の評価を行うことができる。
本発明の方法の一態様では、接触因子阻害剤が、FXII阻害剤及びカリクレイン阻害剤のいずれかであることができる。
本発明の方法の一態様では、血液凝固の解析結果を、トロンボモジュリンを添加しない場合の血液凝固解析結果と対比することでプロテインCおよびプロテインSの機能を評価することができる。
本発明の方法の一態様では、トロンボモジュリンを添加しない場合(すなわち、組織因子または組織トロンボプラスチンとヘパリン様物質が添加された血液)の血液凝固時間が60秒~700分となるように設定し、そこにトロンボモジュリンを添加して血液凝固の評価を行うことができる。
血液試料に、外因系の血液凝固活性化試薬並びにトロンボモジュリンおよびヘパリン様物質を添加して血液凝固能の評価を行うことを特徴とする。
トロンボモジュリンとともにヘパリン様物質を添加することで、プロテインCおよびプロテインSの機能を反映した血液凝固能の解析が可能となる。
トロンボモジュリンが9nM以下であると、組織因子または組織トロンボプラスチンとへパリン様物質を添加した場合にも、トロンボモジュリンの抗凝固効果が明確に現れないことがある。また、200nMの高濃度では、ヘパリン様物質の添加が無くとも、トロンボモジュリンは抗凝固能を示すことができるものの、トロンボモジュリンは高価であり、検査用途に用いる上での問題となる。
組織因子及び組織トロンボプラスチンは、比活性が異なるが、例えば、全血に添加された場合の血液凝固時間が、50-600秒になるように添加されることが好ましい。
低濃度の組織因子または組織トロンボプラスチンとヘパラン硫酸とトロンボモジュリンの存在下による血液凝固の解析であると、生体内の血液凝固反応に近く好ましい。
血液凝固時間の測定が比較的短い場合は(例えば10分以下)、プラスチック容器との接触による接触因子の活性化の影響は少ないが、血液凝固の測定時間が長い場合(例えば30分以上)には、接触因子の活性化による血液凝固時間の短縮の影響が現れる場合がある。
FXII阻害剤およびFXI阻害剤は、FXIIおよびFXIの活性化に対する阻害剤 または 活性化FXII(FXIIa)及び活性化FXI(FXIa)の酵素活性に対する阻害剤であってもよい。
FXII阻害剤は、FXII阻害活性を有する物質であれば特に制限されないが、コーントリプシンインヒビター(CTI)やペプチド阻害剤(J Med Chem. 2017 Feb 9;60(3):1151-1158)などを使用することができる。FXII阻害剤は、例えばCTIであれば終濃度10~100μg/mLで添加されることが好ましい。
カリクレイン阻害剤は、特に制限されないが合成カリクレイン阻害剤(PKSI-527:富士フイルム和光純薬)やアプロチニンなどが望ましい。カリクレイン阻害剤(PKSI-527)であれば、終濃度0.1μM~100μMで添加されることが好ましい。
ヘパラン硫酸は血管内皮細胞上に存在し、血管内における生理的な抗凝固作用を担っている。よって、ヘパリン様物質としてヘパラン硫酸を用いることで、生理的な血液凝固を再現できるので、より好ましい。ヘパラン硫酸は、終濃度が0.4-15ug/mLで添加されることが望ましい。
例えば、トロンボモジュリンの有り無しの血液凝固時間やROTEM/TEGの波形を比較し、その延長度を評価することで、プロテインCおよびプロテインSの機能の評価が可能となる。
例えば、FVライデンは、FVの遺伝子の変異によりAPC(活性化プロテインC)でFVaが不活性化されない異常症で血栓の要因になるが、トロンボモジュリン非存在下の凝固時間の測定では反映されないので、可溶性トロンボモジュリン存在下と非存在下の血液凝固時間の差が小さくなる。
また、プロテインS(PS)は、血液中ではフリーのPSと補体制御因子C4BPと複合体(PS・C4BP複合体)の両方が存在している。APCの補酵素として働くのはフリーのPSである。炎症などでC4BPが増加するとフリーのPSが低下して、APCの抗凝固能が低下する。このような病態では、可溶性トロンボモジュリン存在下と非存在下の血液凝固時間の差が小さくなる。
このようにトロンボモジュリン存在下と非存在下の血液凝固を対比することで、PC・PSの機能が解析することが可能である。
この場合、外因系の活性化試薬を多く添加すると凝固時間は短縮し、ヘパリン様物質を多く添加すると凝固時間は遅延するので、両者のバランスで60秒~700秒の間になるように調整することができる。
APCの阻害剤(またはPCの阻害剤)としては、PCの活性化を阻害する物質やAPCの酵素活性を阻害する物質などが含まれ、特に制限されないが、例えば、APC阻害のDNAアプタマーを使用することができる。APC阻害アプタマーの例として、HS02-44G(Chemistry & Biology Volume 16, Issue 4, 24 April 2009, Pages 442-451)が挙げられるが特に限定されない。
血液凝固測定試薬は、さらに、FXII阻害剤及びカリクレイン阻害剤などの接触因子阻害剤を含んでもよい。
血液凝固測定試薬は、さらに、プロテインC阻害剤を含んでもよい。
各成分の好ましい使用時の濃度(終濃度)は、上述したとおりであり、使用時(血液試料添加時)に好ましい終濃度に希釈して使用される。
各成分は、あらかじめ混合されていてもよいが、血液凝固測定試薬キットに別々の試薬成分として含まれ、使用時にそれぞれ測定に適した濃度で混合されるようになっていることが好ましい。血液凝固測定試薬は、血液凝固測定のための使用説明書を含んでもよい。
血液凝固の解析にROTEM(IL社)を用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
全血300μLに、50倍希釈した外因系活性化試薬であるExtem試薬(IL社) 20μLおよび 塩化カルシウム試薬であるStartem試薬(IL社)20μL(終濃度12mM)と可溶性トロンボモジュリン(rTM;商品名リコモジュリン; 旭化成ファーマ社)を終濃度で0、20nM、50nM、200nMになるように添加して血液凝固を解析した。
結果を図2に示す。

可溶性トロンボモジュリン 0,20,50,200 nM添加した血液の凝固時間(CT)は、それぞれ275, 323, 342, 570秒であった。可溶性トロンボモジュリンが50nMにおいても血液凝固時間の延長効果は限定的であった。1.5倍以上の延長には可溶性トロンボモジュリンが200nMの添加が必要であった。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
50倍希釈のEXTEM試薬の代わりに、50倍希釈のウサギ能由来組織トロンボプラスチン(TP)試薬(終濃度0.9μg/mL)を血液に添加した以外は実施例1と同様の血液凝固の解析を行った。
結果を図3に示す。

血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
実施例2同様の実験で、ウサギ脳由来組織トロンボプラスチン(TP:終濃度0.9μg/mL)と可溶性トロンボモジュリン(rTM:終濃度20nM)に加えて、ヘパラン硫酸(iduron社、平均分子量 5500)を終濃度 1μg/mL、1.5μg/mLおよび2μg/mLになるように添加して血液凝固波形を解析した。
結果を図4に示す。

さらに、ヘパラン硫酸を2μg/mL添加した場合においては、少量の可溶性トロンボモジュリンによって、凝固時間が大きく遅延すると同時にクロットの強度も抑制された。
これらの結果は、外因系の血液凝固活性化試薬とヘパリン様物質を添加することで、トロンボモジュリンの抗凝固能を大幅に増強することを示している。
実施例3と同様の実験で、ウサギ脳由来組織トロンボプラスチン(TP;終濃度0.9μg/mL)に、ヘパラン硫酸(終濃度1μg/mL)とともに、接触因子阻害剤としてFXII阻害剤(CTI; 終濃度20μg/mL)とカリクレイン阻害剤(PKSI-527;終濃度40μM)を添加して、可溶性トロンボモジュリン(rTM;終濃度20nM)の存在下及び非存在下における血液凝固の解析を行った。
Startem試薬の代わりに塩化カルシウム溶液20μL(Ca;終濃度12mM)を用いた。
結果を図5に示す。

希釈したウサギ脳由来組織トロンボプラスチンとヘパラン硫酸と接触因子阻害剤を添加した血液に、低濃度(20nM)の可溶性トロンボモジュリンを添加することで、凝固時間が顕著に延長されることが示された。
実施例3と同様の実験で、ヘパラン硫酸の代わりに、未分画ヘパリン(持田製薬株式会社;終濃度0.1U/mL)を添加し、可溶性トロンボモジュリン(rTM) 終濃度0nM, 20nMおよび30nMの血液凝固波形を解析した。
結果を図6に示す。

実施例5と同様の実験で、未分画ヘパリンの代わりに低分子ヘパリン(商品名フラグミン キッセイ薬品工業株式会社(販売)、ファイザー株式会社(製造販売元)製 終濃度0.2U/mL)を用いて血液凝固の解析を行った。
結果を図7に示す。

比較としてウサギ脳由来組織トロンボプラスチン試薬(TP;0.9μg/mL)とStartem試薬(塩化カルシウム:終濃度12mM)とヘパラン硫酸(終濃度1μg/mL)に可溶性トロンボモジュリン(rTM;終濃度20nM)または可溶性トロンボモジュリン(rTM;終濃度20nM)とAPC阻害アプタマー(HS02-44G ;終濃度1μM;Chemistry & Biology Volume 16, Issue 4, 24 April 2009, Pages 442-451)を添加して血液凝固の解析を行った。
結果を図10に示す。

また、組織トロンボプラスチンとヘパラン硫酸を添加した血液に、さらに可溶性トロンボモジュリン(20nM)とAPC阻害アプタマーを添加した時のCTは208秒であった。
この結果から、154秒から208秒へのCTの遅延は、可溶性トロンボモジュリンの抗トロンビン作用によるものと考えられる。319秒から208秒への短縮は、活性化プロテインCを介した抗凝固作用によるものと考えられる。よってAPC阻害アプタマー添加と非添加の凝固時間を比較することで、PC/PSの機能の評価が可能となる。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
100倍希釈したウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.45μg/mL)とStartem試薬(塩化カルシウム;終濃度12mM)と、ヘパラン硫酸(終濃度1μg/mL)を添加して血液凝固解析した。
比較としてウサギ脳由来組織トロンボプラスチン試薬(0.9μg/mL)とStartem試薬(塩化カルシウム;終濃度12mM)とヘパラン硫酸(終濃度1μg/mL)に可溶性トロンボモジュリン(rTM;終濃度100nM)または可溶性トロンボモジュリン(rTM;終濃度100nM)とAPC阻害アプタマー(HS02-44G ;終濃度1μM)を添加して血液凝固の解析を行った。
結果を図11に示す。

また、組織トロンボプラスチンとヘパラン硫酸を添加した血液に、さらに可溶性トロンボモジュリン(100nM)とAPC阻害アプタマーを添加した時のCTは387秒であった。APCを介した抗凝固効果を特異的に反映している。
ヘパラン硫酸存在下に、トロンボモジュリンとAPC存在下と非存在下の血液凝固をそれぞれ対比することで、詳細な血液凝固の解析が可能となる。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度16.6μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸を(終濃度 0, 1, 3, 10,15, 20, 30μg/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 50nM)した場合について血液凝固解析した。
ヘパラン硫酸 3ug/mL以下では可溶性トロンボモジュリン50nMの凝固時間の延長作用は確認されなかったのに対し、10および 15μg/mLでは、可溶性トロンボモジュリンの添加により凝固時間は顕著に(1.5倍以上)延長されることが確認された。一方、ヘパラン硫酸を20または30μg/mL添加した場合においては、トロンボモジュリンの添加によりCT値は得られず、血液凝固時間は1時間以上となった。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。

可溶性トロンボモジュリンを9nMの添加した場合には、ヘパラン硫酸を添加しない場合は、凝固時間の延長が限定的であったが、ヘパラン硫酸を0.5μg/mL添加することで、凝固時間は1.5倍以上に延長された。

可溶性トロンボモジュリンを10nMの添加した場合には、ヘパラン硫酸が0.3μg/mL以下では、凝固時間の延長が限定的であったが、ヘパラン硫酸を0.4μg/mL以上添加することで、凝固時間は1.5倍以上に延長された。

可溶性トロンボモジュリンが5nMの添加した場合には、ヘパラン硫酸を添加した場合にも、凝固時間は1.5倍以上に延長されなかった。

可溶性トロンボモジュリンを400nM 以上添加した場合には、ヘパラン硫酸を添加しなくとも、凝固時間は1.5倍以上に延長された。

血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.011μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 0,0.2,0.3, 0.5μg/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 10nM)した場合について血液凝固を解析した。
トロンボプラスチン0.011μg/mLとヘパラン硫酸(0.2μg/mL以上)を添加した場合に、可溶性トロンボモジュリン 10nMを添加すると、血液凝固時間が1時間以上となり、いずれもCT値が得られなかった。

ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.0166μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 0.3, 0.5, 1μg/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 10nM)した場合について血液凝固解析した。
トロンボプラスチン0.0166μg/mLを添加した場合に、可溶性トロンボモジュリン 10nMを添加しても、1.5倍以上のCT値の延長は確認されなかった。ヘパラン硫酸1.0μg/mL添加した場合には、血液凝固時間が1時間以上となりCT値は得られなかった。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度16.6μg/mL)と塩化カルシウム(Ca;終濃度12mM)と未分画ヘパリン(持田製薬)(終濃度 0, 0.1,0.2, 0.3 U/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 100nM)した場合について血液凝固解析した。
トロンボプラスチン16.6μg/mLで未分画ヘパリン(0及び0.1U/mL)のCT値は、46および53秒であった。さらに、可溶性トロンボモジュリン 100nMを添加した場合においてもCT値の顕著な延長は確認されなかった。
一方、トロンボプラスチン16.6μg/mLと未分画ヘパリン(0及び0.3U/mL)を添加した際のCT値は69秒であった。さらに、可溶性トロンボモジュリン 100nMを添加した場合において、顕著なCT値の延長が確認された。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度16.6μg/mL)と塩化カルシウム(Ca;終濃度 12mM)とフォンダパリヌクスナトリウム(商品名;アリクストラ; 終濃度 0, 0.5, 2, 3, 4, 5μg/mL)とCTI(終濃度20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 100nM)した場合について血液凝固解析した。
トロンボプラスチン16.6μg/mLとアリクストラ(0,0.5μg/mL)を添加した際のCT値はいずれも1分以下であり、さらに、可溶性トロンボモジュリン 100nMを添加した場合においてCT値の顕著な延長は確認されなかった。
一方、トロンボプラスチン16.6μg/mLとアリクストラ(2, 3, 4, 5μg/mL)を添加した際のCT値はいずれも64秒~78秒で、さらに、可溶性トロンボモジュリン 100nMを添加した場合において、いずれも1.5倍以上のCT値の延長が確認された。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.64μg/mL)と塩化カルシウム(Ca;終濃度 12mM)とアリクストラ(終濃度 0および 0.5μg/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 30nM)した場合について血液凝固解析した。
アリクストラ存在下に可溶性トロンボモジュリンを添加した場合に、血液凝固が顕著に延長された。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.055μg/mL)と塩化カルシウム(Ca;終濃度 12mM)とアリクストラ(終濃度 0, 0.05, 0.2 μg/mL)とCTI(終濃度 20μg/mL)とPKSI-517(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 20nM)した場合について血液凝固解析した。
トロンボプラスチン0.055μg/mLでアリクストラを添加しなかった場合のCT値は、575秒であった。さらに、可溶性トロンボモジュリン 20nMを添加した場合においてもCT値の顕著な延長は確認されなかった。
一方、トロンボプラスチン0.055μg/mLとアリクストラ(0.05μg/mL)を添加した際のCT値はいずれも619秒であった。さらに、可溶性トロンボモジュリン 20nMを添加した場合においては、約1.7倍のCT値の延長が確認された。
さらに、トロンボプラスチン0.055μg/mLとアリクストラ(0.2μg/mL)を添加した際のCT値はいずれも799秒であった。さらに、可溶性トロンボモジュリン 20nMを添加した場合において、血液凝固時間は1時間以上となり、CT値は得られなかった。

ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度16.6μg/mL)と塩化カルシウム(Ca;終濃度12mM)と低分子ヘパリン(商品名 フラグミン;ファイザー社)(終濃度 0, 0.2,1 U/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(終濃度 100nM)した場合について血液凝固解析した。
トロンボプラスチン16.6μg/mLでフラグミン(0及び0.2U/mL)のCT値は、46および50秒であった。さらに、可溶性トロンボモジュリン 100nMを添加した場合においてもCT値の顕著な延長は確認されなかった。
一方、トロンボプラスチン16.6μg/mLとフラグミン(1U/mL)を添加した際のCT値は83秒であった。さらに、可溶性トロンボモジュリン 100nMを添加した場合においては、約1.5倍のCT値の延長が確認された。

血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ能由来組織トロンボプラスチン試薬(TP;終濃度0.64μg/mL)と塩化カルシウム(Ca;終濃度12mM)と低分子ヘパリン(フラグミン;ファイザー社)(終濃度 0及び0.2U/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 30nM)した場合について血液凝固解析した。
フラグミンを(0.2U/mL)を添加することで、可溶性トロンボモジュリンのCT値は、顕著に延長された。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.055μg/mL)と塩化カルシウム(Ca;終濃度12mM)と低分子ヘパリン(フラグミン;ファイザー社)(終濃度 0, 0.05,0.15単位/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 20nM)した場合について血液凝固解析した。
トロンボプラスチン0.055μg/mLでフラグミン(0、0.05、0.15単位/mL)のCT値は、575、614,886秒であった。さらに、可溶性トロンボモジュリン 20nMを添加した場合において、フラグミン未添加の場合にCT値の顕著な延長は確認されなかったが、フラグミンを0.05単位/mL添加することで、可溶性トロンボモジュリン添加後に約1.6倍に血液凝固が延長された。フラグミン0.15単位/mLを添加した場合は、可溶性トロンボモジュリンを添加した場合、血液凝固時間は1時間以上となりCT値は得られなかった。
血液凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
デイドイノビン(リコンビナントティッシュファクター:シスメックス株式会社)を透析後50倍希釈した試薬を4%(終濃度0.569μg/mL)と塩化カルシウム(Ca;終濃度 12mM)とヘパラン硫酸(終濃度 0又は2μg/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 30nM)した場合、さらに可溶性トロンボモジュリン30nMに加えて、APC阻害アプタマー(HS02-44G ;終濃度1μM)を添加した場合について血液凝固解析した。
ヘパラン硫酸の添加により、可溶性トロンボモジュリンの抗凝固能は顕著に増強された。また、活性化プロテインCを阻害することで凝固時間が短縮された。短縮幅はプロテインCとプロテインSを介した抗凝固能を反映していると考えられる。

凝固の解析にROTEMを用いた。
血液はテルモ社製の3.2%クエン酸ナトリウムを含む採血管を用い採取した。
デイドイノビンを透析後50倍希釈した試薬を4%(終濃度0.569μg/mL)と塩化カルシウム(Ca;終濃度 12mM)とアリクストラ(終濃度 0又は0.5μ/mL)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)を添加して、さらに可溶性トロンボモジュリンの添加しない場合と添加(rTM;終濃度 30nM)した場合、さらに可溶性トロンボモジュリン30nMに加えて、APC阻害アプタマー(HS02-44G ;終濃度1μM)を添加した場合について血液凝固解析した。
ウサギ脳由来組織トロンボプラスチンとデイドイノビンのPT値の測定
半自動血液凝固測定装置CA104(シスメックス社)を用いて、実施例に使用した組織因子のPT値を測定した。
ウサギ脳由来組織トロンボプラスチン(415 μg/mL)はヒトアルブミン100 μg/mL 入りBuffer(5mM Hepes, 0.15M NaCl(pH7.4))で希釈した。デイドイノビンは蒸留水に透析後(711.3 μg/mL)、 ヒトアルブミン100 μg/mL 入りBuffer(5mM Hepes, 0.15M NaCl(pH7.4))で希釈した。
標準血漿50μLを37℃1分インキュベーション後、塩化カルシウム溶液(25mM)50μLと希釈したウサギ脳由来組織トロンボプラスチン又はデイドイノビン溶液50μLと混和し凝固時間(PT値)を測定した。標準血漿は、デイドサイトロールレベル1(シスメックス株式会社)、血液凝固試験用標準ヒト血漿(シスメックス株式会社)、Pooled Normal Plasma(George King Bio-Medical,Inc.)を用いた。
結果を表19および表20に示す。

(3)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 1μg/mL)とPKSI-527(終濃度 40μM)
(4)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 1μg/mL)とCTI(終濃度20μg/mL)とPKSI-527(終濃度 40μM)

(3)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 1μg/mL)とPKSI-527(終濃度 40μM)
(4)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とヘパラン硫酸(終濃度 1μg/mL)とCTI(終濃度20μg/mL)とPKSI-527(終濃度 40μM)
血液凝固の解析にROTEMを用いた。

以下の(1)~(4)の試薬を添加して血液凝固の添加を行った。

(2)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)とデキストラン硫酸(名糖産業 デキストラン硫酸エステルナトリウムイオウ18 平均分子量5000、終濃度 60μg/mL)と可溶性トロンボモジュリン(rTM;終濃度 40nM)
(3)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)とデキストラン硫酸(名糖産業 デキストラン硫酸エステルナトリウムイオウ18 平均分子量5000、終濃度 60μg/mL)と可溶性トロンボモジュリン(rTM;終濃度 40nM)とAPC阻害アプタマー(HS02-44G ;終濃度1μM)
(4)ウサギ脳由来組織トロンボプラスチン試薬(TP;終濃度0.664μg/mL)と塩化カルシウム(Ca;終濃度12mM)とCTI(終濃度 20μg/mL)とPKSI-527(終濃度 40μM)とデキストラン硫酸(名糖産業 デキストラン硫酸エステルナトリウムイオウ18 平均分子量5000、終濃度 60μg/mL)と未分画ヘパリン(終濃度 0.5U/mL)

Claims (15)
- 血液試料に、外因系の血液凝固活性化試薬並びにトロンボモジュリンおよびヘパリン様物質を添加して血液凝固能の評価を行うことを特徴とする、インビトロにおける血液凝固の解析方法。
- ヘパリン様物質が、未分画ヘパリン、低分子ヘパリン、ペンタサッカライド及び硫酸化多糖からなる群から選択される一種以上である、請求項1に記載の方法。
- ヘパリン様物質が、ヘパラン硫酸であり、終濃度が0.4~15μ/mLである、請求項1または2に記載の方法。
- 外因系の血液凝固活性化試薬が組織因子または組織トロンボプラスチンである、請求項1に記載の方法。
- さらに、接触因子阻害剤を添加する、請求項1~4のいずれか一項に記載の方法。
- 血液の凝固時間が60~700秒となるような濃度で組織因子または組織トロンボプラスチンとヘパリン様物質を添加し、そこにトロンボモジュリンを添加する、請求項1~5のいずれか1項に記載の方法。
- トロンボモジュリンが可溶性トロンボモジュリンである、請求項1~6のいずれか一項に記載の方法。
- トロンボモジュリンの終濃度が9nM~200nMである、請求項1~7のいずれか一項に記載の方法。
- プロテインC阻害剤を添加して前記血液凝固能の評価を行い、プロテインC阻害剤を添加しない場合の結果を対比することでプロテインCおよびプロテインSの機能評価を行う、請求項1~8のいずれか一項に記載の方法。
- 前記血液試料がクエン酸で抗凝固処理された血液試料であり、カルシウムと、外因系の血液凝固活性化試薬並びにトロンボモジュリンおよびヘパリン様物質を添加することで血液凝固の評価を行う、請求項1~9のいずれか一項に記載の方法。
- 血液試料に、さらに接触因子阻害剤を添加する、請求項1~10のいずれか一項に記載の方法。
- 接触因子阻害剤が、FXII阻害剤及びカリクレイン阻害剤のいずれかまたは両方である、請求項11に記載の方法。
- 血液凝固の解析結果を、トロンボモジュリンを添加しない場合の血液凝固解析結果と対比することでプロテインCおよびプロテインSの機能を評価する、請求項1~12のいずれか一項に記載の方法。
- 外因系の血液凝固活性化試薬並びにトロンボモジュリンおよびヘパリン様物質を含む血液凝固測定試薬。
- さらに、接触因子阻害剤を含む、請求項14に記載の血液凝固測定試薬。
