Description
本发明涉及医疗器械领域。具体为一种肠道多腔灌排器。
通过辅助灌注设备和经肛门插入肠道的导管或专用探头进行肠道灌洗、结肠透析、灌肠给药治疗急慢性肾功能衰竭、重症肝病、乃至全身炎症反应综合征(或脓毒症)、下消化道炎症性等疾病,以及肠道清洁与辅助排便的新手段应用正在被临床医生所重新认识和重视。尤其是在治疗2020年由新型冠状病毒引发的脓毒症所导致的发热、干咳、腹泻等症状时,通过中药灌肠产生了显著疗效。同时这种作为一种结肠透析治疗方法,辅助血液透析、腹膜透析疗法可应用于慢性肾脏病(CKD)各阶段的治疗,并具有无创、、实施简便,且治疗费用较低等优势。。
在临床当中,针对患者的病情差异,灌肠治疗一般分为高位灌注和低位灌注。对于低位灌肠而言,由于灌肠液或药液只需送至直肠或乙状结肠或降结肠部即可,因此该方式较易实现,目前可进行低位灌肠的设施和装置也很多。
低位灌肠做法是:一般先插入一根单腔导管,通过该导管灌注液体至低位肠腔,然后让患者自行去卫生间排泄或通过导管引流。此种方法虽然解决了废液析出管堵塞问题,但在操作过程中,由于患者需要在肠腔内灌注并积蓄大量液体,致使患者的憋胀感较强,耐受性差。同时,现有针对结肠的灌排导管或专用探头或透析仪在临床使用中,没有很好地解决结肠高位和低位灌注的结合问题,特别是为了避免废液析出管堵塞,一般需要在进行高位结肠灌注前要进行低位结肠灌洗以排出粪便。对于高位结肠灌注或结肠透析治疗患者,往往在完成低位灌注清洗、排除粪便后,还需要换管才能进行高位结肠透析或给药治疗。这种以往的临床使用方法具有较为严重的缺陷,首先很多患者 会因这种人为的强烈的憋胀感而耐受性差以及频繁上厕所的麻烦,特别是那些年老体弱或行动不便的患者;其次需要经常或频繁地插管转换,增加了医护人员的治疗及护理操作强度,不利于治疗的快速顺利完成;第三,对于一些重症卧床患者如何进行封闭式排便目前也缺乏有效的手段;最后,由于反复换管操作,无疑会增加感染风险,同时还增加了灌注器具的消耗量,不利于节约,又增加了患者的经济负担。
发明内容
本发明解决的技术问题在于克服传统灌肠设备或导管由于插管转换所带来的种种不利因素,提供一次插管即可进行肠道低位灌洗、结肠透析、高位结肠给药的肠道多腔灌排器。

4、本发明提供的肠道多腔灌排器上肠道低位灌注连接装置和/或肠道高位灌注管上设有加压促流装置,其作用是当以重力作用无法灌注液体时通过辅助加压方式进行灌注。
5、本发明提供的肠道多腔灌排器的气囊可防止肠道高位灌注管在插入肠腔内遇到阻力时向回弯折,气囊在进入人体肠道后根据情况可不断地充气或放气以便于肠道高位灌注管导流的液体可到达肠腔高 位。气囊还通过调节其充盈程度以调节液体进入肠腔的部位以及液体在肠腔内的保留时间及液体排出肠腔时的流量和速度。

如图1-5所示,本发明提供的肠道多腔灌排器,包括主管1,所述主管1的一端设有第一开口,所述主管1的侧壁设有肠道低位灌注连接装置和排废连接装置,在本实施例中,肠道低位灌注连接装置为肠 道低位灌注支管2,排废连接装置为排废支管3。在其它实施例中,肠道低位灌注连接装置和排废连接装置也可以是开始在主管侧壁上的连接孔。
在使用时,肠道低位灌注支管2与低位灌注液袋21通过第一注液管22连接,第一注液管22上设有低位注液阀24,其中的低位灌注液袋21、第一注液管22和低位注液阀24为常规所使用的医用部件。所述肠道低位灌注支管2和排废支管3与所述主管1的内腔相通,主管1的内腔可作为低位灌注时的药液和排废时的废液流经的通道。所述主管1的另一端设有第二开口及可堵住所述第二开口的密封堵头13,密封堵头13与主管1的另一端可拆卸地连接。在本实施例,二者通过螺纹连接,且在密封堵头13与第二开口之间设有密封垫14。所述密封堵头13设有孔,所述肠道多腔灌排器还包括一端设有注液口的肠道高位灌注管4,所述肠道高位灌注管4设有注液口的一端可穿过所述孔131和所述主管1从所述第一开口伸出。在使用时,肠道高位灌注管4通过第二注液管41与高位灌注液袋43相连,肠道高位灌注管4设有高位注液阀44。
如图6所示,排废连接装置设有负压引流装置34和/或所述肠道低位灌注连接装置设有加压促流装置23和/或肠道高位注液管上设有加压促流装置45。在本实施例中,负压引流装置34采用引流球,加压促流装置23采用加压球。在灌注时,可通过加压促流装置23,45促使低液袋内的液体流向患者体内。在排废时,可通过负压引流装置34产生负压,使废液和异物顺利排出。
在本发明中,高位灌注液袋43中装有药液,低位灌注液袋21内可以是药液,也可以是清洗液。低位灌排可以是排出患者体内的粪便等代谢产物,也可以是灌注药液进行治疗。高位灌注是肠道高位灌注管4通过密封堵头13的孔131到达肠道高位后灌注药液进行治疗。
通过本发明提供的肠道多腔灌排器,可通过主管1单独实现低位灌注,也可通过肠道高位灌注管4单独实现高位灌注,也可以通过主管1和肠道高位灌排管4同时实现低位灌注和高位灌注、排空。同时,肠道高位灌注管4从主管1的一端穿入主管1内并从主管1的另一端 穿出,克服了以往技术从主管侧壁穿出而容易发生弯曲的弊端,而且操作简便。
如图1-6所示,所述排废支管3设有排废阀31,不仅可控制排废支管3的通断,还可控制排废的流量,防止患者在灌注的时候产生憋胀感。如图6所示所述肠道多腔灌排器还包括与所述排废支管3连接的废液管32和接收所述废液管32的废液的废液收集装置33,废液收集装置33使得排出的肠内异物和废液被直接收集起来,在本实施例中,废液收集装置33采用废液袋。作为优选,所述主管1设有第一开口的一端的轮廓为凸或凹或凸凹结合的曲面。在本实施例中,主管1设有第一开口的一端的轮廓凸凹结合的曲面,可以与辅助活塞插管器5配合使用不会对肛门或直肠段造成损害,。所述主管1设有第一开口的一端的侧壁设有废液析出口11。废液通过废液析出口11流入主管1,特别时当第一开口阻塞时辅助废液引流。
如图6所示,肠道低位灌注支管和/或肠道高位灌注管分别设有压力检测口25,46,用于检测管道的压力。所述废液管也可以设压力检测口。
作为优选,所述主管1的外壁设有限位挡12,所述肠道低位灌注支管2和排废支管3位于所述限位挡12与所述第二开口之间。在将主管1插入患者的肛门时,限位挡12可防止主管1插入位置过深。
如图4所示,所述肠道高位灌注管设有气囊42。所述肠道多腔灌排器还包括向所述气囊42注气的注气装置,所述注气装置包括注气管和注气器。气囊42可防止肠道高位灌注管4在插入肠腔内遇到阻力时向回弯折,所述气囊42在进入人体肠道后根据情况可不断地充气或放气,通过调节其充盈程度以调节液体进入肠腔的部位以及液体在肠腔内的保留时间及液体或废液排出肠腔时的流量和速度。
如图1和2所示,所述肠道多腔灌排器还包括辅助活塞插管器5,所述辅助活塞插管器5包括可以从主管1的第一开口伸出的活塞头51,所述活塞头51的轮廓为圆锥形且圆锥形的顶部为光滑的曲面。所述辅助活塞插管器5还包括活塞杆52和活塞手柄53,辅助活塞插管器5可防止主管1插入肛门或直肠段时发生肠粘膜损伤。
采用本实施例的肠道多腔灌排器进行治疗时,进行以下操作:
(一)准备工作
1.分别对高位灌注液袋43和低位灌注液袋21加入灌肠液(包括需配置的药液)。具体包括:
a)拧开高位灌注液袋43的盖子431和低位灌注液袋21头部的盖子211;
b)将与高位灌注液袋43连接的第二注液管41上的高位注液阀44和与低位灌注液袋21连接的第一注液管22上的低位注液阀24卡住处于关闭状态;
c)将配置好的灌肠液加入高位灌注液袋43和低位灌注液袋21中,液体的温度应保持在36-38℃之间;
d)完成后将液袋上的盖子431和211扣紧,保证不漏液。
2.将装有灌肠液(或药液)的高位灌注液袋43和低位灌注液袋21储液袋挂在床旁的输液架上,推荐悬挂高度离床垫30-50厘米处;同时,将废液袋通过挂钩放置低于床垫的10-20厘米处。注意:高度过高会因为灌注压力过大给患者造成不适。
3.管路连接:将高位灌注液袋43、低位灌注液袋21、辅助活塞插管器5、废液袋等通过各种接头进行连接并保持密封不漏液;检查各连接管路是否打折、打结,保证管路通畅。
4.将患者在治疗床上取左侧卧位,肛门指诊排除禁忌症,将主管1外侧和辅助活塞插管器5及活塞头均匀涂抹润滑剂,然后将辅助活塞插管器5套接在主管内,缓慢地将主管1和辅助活塞插管器5经肛门送入直肠内。插管时应不断询问患者感受,并根据患者耐受和实际情况决定插管深度,但最深不超过主管的限位挡12。
5.完成插管后,应将辅助活塞插管器5从主管内撤出并做医疗废物处置。
6.辅助活塞插管器5拔出后,将主管1密封堵头13拧紧,若要进行肠道高位灌注,则将肠道高位灌注管4通过密封堵头13上孔131穿入并插入10-15CM左右停止。
(二)肠道灌洗程序
1.关闭排废阀31,排废阀31的旋钮开关角度的大小直接影响排废的流量和速度。
2.灌排过程控制。打开与低位灌注液袋21连接的第一进液管22上的低位注液阀24,开始肠道灌洗;期间根据患者耐受情况,间歇式不断地开启/关闭排废阀31,直至低位灌注袋21内的液体用尽。
(三)肠道给药透析程序
1.关闭排废阀31。
2.灌排过程控制。
a)打开与高位灌注液袋43连接的第二注液管41上的高位注液阀44。
b)当第二注液管41有液体流出时,缓慢地推动肠道高位灌注管4继续插入至30-50CM左右停止。
c)根据患者耐受情况,不断地开启/关闭排废阀31,直至高位灌注液袋43内的液体用尽。期间如需要药液保留一段时间,分别关闭第二注液管41上的高位注液阀44、排废阀31,然后再分别打开。
(四)结束工作
1.上述治疗结束后,将肠道高位灌注管4缓慢退出,退至主管1腔内至密封堵头13处而无法拔出时停止。
2.关闭排废阀31,缓慢拔出主管1。
3.保证废液袋封闭,将使用过的辅助活塞插管器5、各连接管路和液袋等做医疗废物处理。
通过本发明的肠道多腔灌排器可实现以下三种模式:
(1)低位肠道灌洗模式:
a、关闭高位注液阀44和排废阀31,打开低位注液阀24,利用重力或者加压促流装置45将清洗液灌注入直肠内;
b、灌注完毕后,关闭低位注液阀24,打开排废阀31,利用重力或负压引流装置或引流球34,配合病人排便,废液或异物通过主管1和排废支管3排到废液收集装置33;
(2)高低位肠道灌注透析序贯模式:
a、关闭高位注液阀44和排废阀31,打开低位注液阀24,利用重力或者加压促流装置23将清洗液灌注入直肠内;
b、灌注完毕后,关闭低位注液阀24,打开排废阀31,利用重力或负压引流装置,配合病人排便,废液及异物通过主管1和排废支管3排到废液收集装置33;
c、关闭排废阀31,打开高位注液阀44,将药液灌注入直肠内,并贮留一定时间;
d、关闭高位注液阀44,打开排废阀31,利用重力或加压促流装置23,配合病人排便,废液通过主管1和排废支管3排到废液收集装置33内。
以上实施例仅为本发明的示例性实施例,不用于限制本发明,本发明的保护范围由权利要求书限定。本领域技术人员在本发明的实质和保护范围内,对本发明做出的各种修改或等同替换也落在本发明的保护范围内。
Claims (17)
- 一种肠道多腔灌排器,其特征在于,包括主管,所述主管的一端设有第一开口,所述主管的侧壁设有肠道低位灌注连接装置和排废连接装置,所述肠道低位灌注连接装置和排废连接装置与所述主管的内腔相通,所述主管的另一端设有第二开口及可堵住第二开口的密封堵头,所述密封堵头设有孔,所述肠道多腔灌排器还包括一端设有注液口的肠道高位灌注管,所述肠道高位灌注管设有注液口的一端可穿过所述孔和所述主管从所述第一开口伸出。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述肠道低位灌注连接装置为肠道低位灌注支管,所述排废连接装置为排废支管。
- 根据权利要求1所述的一次性使用肠道多腔灌排器,其特征在于,所述排废连接装置设有负压引流装置。
- 根据权利要求3所述的肠道多腔灌排器,其特征在于,所述负压引流装置为泵或引流球。
- 根据权利要求3所述的一次性使用肠道多腔灌排器,其特征在于,所述的肠道低位灌注连接装置和/或肠道高位灌注管上设有加压促流装置。
- 根据权利要求5所述的肠道多腔灌排器,其特征在于,所述加压促流装置为泵或加压球。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述密封堵头与所述主管可拆卸地连接。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述肠道多腔灌排器还包括与所述排废连接装置连接的废液管和接收所述废液管流出废液的废液收集装置。
- 根据权利要求8所述的肠道多腔灌排器,其特征在于,所述废液管上设有可控制流量的排废阀。
- 根据权力要求8所述的肠道多腔灌排器,其特征在于,所述废液管和/或肠道低位灌注支管和/或肠道高位灌注管设有压力检测口。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述主管设有第一开口的一端的轮廓为凸或凹或凸凹结合的曲面。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述主管设有第一开口的一端的侧壁设有废液析出口。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述主管的外壁设有限位挡,所述肠道低位灌注连接装置和排废连接装置位于所述限位挡与所述第二开口之间。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述肠道高位灌注管设有气囊。
- 根据权利要求14所述的肠道多腔灌排器,其特征在于,所述肠道多腔灌排器还包括向所述气囊注气的注气装置,所述注气装置包括注气管和注气器。
- 根据权利要求1所述的肠道多腔灌排器,其特征在于,所述肠道多腔灌排器还包括辅助活塞插管器,所述辅助活塞插管器包括可以从主管的第一开口伸出的活塞头,所述活塞头的轮廓为圆锥形且圆锥形的顶部为光滑的曲面。
- 根据权利要求16所述的肠道多腔灌排器,其特征在于,所述辅助活塞插管器还包括活塞杆和活塞手柄。
