 この種の内視鏡としては、先端側から順に、先端部、湾曲部、及び、可撓管部が配設された軟性鏡が広く用いられている。このような軟性鏡の挿入部を被検体内に挿入する場合、術者等は、一方の手によって操作部を把持しつつ、他方の手によって可撓管部を把持して被検体内に挿入部を押し込む。その際、術者等は、操作部を把持した手によって、当該操作部に配設されている操作ノブを操作することにより、湾曲部を所望の方向に湾曲させることが可能となっている。
 ところで、例えば、泌尿器や鼻腔等を対象とした内視鏡下での観察や処置を行う場合、挿入部は横臥した患者等の被検体に対し、略水平方向から挿入される。このような対象部位に対する挿入性を向上させるため、例えば、日本国特開2009-189684号公報には、操作部(操作把持部)に設けられる把持部を、操作部本体(操作部)の延在方向に対して所定角度傾斜させて、当該操作部本体の基端側から延在させた略L字型をなす操作部が開示されている。また、日本国特開2009-189684号公報の技術において、把持部の基端には、当該把持部の延在方向に沿って延在する折れ止部が設けられ、この折れ止部によってユニバーサルケーブルの一端側が保護されている。
 しかしながら、上述の日本国特開2009-189684号公報に開示された技術のように、操作部をL字型に形成した場合、硬質な把持部及び折れ止部が、操作部本体の延在方向に対して所定角度を有して延在するため、収納ケースや、滅菌装置の洗浄槽等に対して内視鏡をコンパクトに収納することが困難となる虞がある。
 また、操作部をL字型に屈曲形成した場合、操作部の重心位置が把持部から離間する傾向にあり、このように重心位置が把持部から離間すると、把持部を把持した術者等の手首に負担がかかる等、良好な把持性及び操作性を確保することが困難となる虞がある。
 本発明は上記事情に鑑みてなされたもので、コンパクトな収納性と、良好な把持性及び操作性と、を両立することができる内視鏡を提供することを目的とする。
 本発明の一態様による内視鏡は、挿入部の基端側に連設する操作部本体と、前記操作部本体の基端側に連設され、前記操作部本体が延在する第1の長手軸に対して屈曲する方向である第2の長手軸に沿って延在するリア把持部と、前記リア把持部の基端側から延出するケーブル部と、前記リア把持部の基端側において前記ケーブル部を被覆するチューブ部と、前記チューブ部の方向が、前記操作部本体に対する前記リア把持部の屈曲方向内側に指向する第1の方向と、前記操作部本体に対する前記リア把持部の屈曲方向外側に指向する第2の方向と、を含む複数の方向に変更可能となるよう、前記チューブ部を前記リア把持部の基端側に連結する可動連結部と、を備えたものである。

 以下、図面を参照して本発明の形態を説明する。図面は本発明の一実施形態に係り、図1は内視鏡システムの概略構成図、図2はユニバーサルケーブルを第1の方向に指向させた内視鏡の側面図、図3はユニバーサルケーブルを第2の方向に指向させた内視鏡の側面図、図4はリア把持部の内部構造を示す要部断面図、図5は図4のV-V線に沿う断面図、図6は図4のVI-VI線に沿う断面図、図7はリア把持部を把持する際の好適な把持例を示す説明図、図8はフロント把持部を把持する際の好適な把持例を示す説明図、図9は内視鏡を洗浄装置の洗浄槽に収納した状態を示す説明図である。
 図1に示す内視鏡システム1は、内視鏡2と、内視鏡2に照明光を供給する光源装置3と、内視鏡2が撮像した映像信号に対して画像処理を行うカメラコントロールユニット(CCU)を含むビデオプロセッサ4と、ビデオプロセッサ4から出力される映像信号を表示するモニタ5と、を備えて構成されている。
 図1~図3に示す本実施形態の内視鏡2は、例えば、泌尿器用の内視鏡である。この内視鏡2は、被検体内の対象部位に挿入可能な長尺な挿入部10と、挿入部10の基端側に連設された操作部11と、操作部11から延出されたケーブル部としてのユニバーサルケーブル12と、を有して構成されている。
 挿入部10は、硬質な先端部15と、湾曲部16と、可撓性を有する可撓管部17と、が先端側から順に連設されて要部が構成されている。
 先端部15にはCCD,CMOS等の撮像素子を内蔵した撮像ユニットが保持され、この撮像ユニットには挿入部10内に挿通された信号線が電気的に接続されている。また、先端部15には照明光学系が保持され、この照明光学系には挿入部10内に挿通されたライトガイドバンドルが光学的に接続されている。また、先端部15には処置具チャンネル口が開口され、この処置具チャンネル口には、挿入部10内に挿通された処置具チャンネルが連通されている(何れも図示せず)。
 湾曲部16は、例えば、操作部11において行われる湾曲操作に応じて、上下(UP/DOWN)の2方向のみに湾曲可能となっている。なお、湾曲部16は、上下(UP/DOWN)及び左右(LEFT/RIGHT)の4方向に湾曲可能に構成されたものであっても良い。
 可撓管部17は、受動的に湾曲可能な可撓性を有する管状部材によって構成されている。
 図2,3に示すように、操作部11は、挿入部10の基端側に連設する操作部本体20と、挿入部10と操作部本体20との間に介装された先端回転部21と、操作部本体20の基端側に連設されたリア把持部22と、を有して構成されている。
 操作部本体20は、例えば、略楕円筒形状をなす部材によって構成され、挿入部10の挿入軸Oと同軸に設定された第1の長手軸O1に沿って延在されている。
 この操作部本体20の先端側の領域には、術者等が挿入軸O方向に沿って操作部11を把持するための第1の把持部として、フロント把持部25が設定されている。
 また、操作部本体20の基端側には、湾曲部16を湾曲操作するための湾曲操作レバー26が設けられている。湾曲操作レバー26は、例えば、レバー部26aと、このレバー部26aの自由端側に指当部26bが一体形成された片持ちのL型形状をなし、レバー部26aの固定端側が操作部本体20の側部に軸支されている。この湾曲操作レバー26は、操作部本体20内に設けられた周知のワイヤ牽引機構(図示せず)に連結されている。これにより、湾曲操作レバー26が回動操作されると、挿入部10内に挿通されたワイヤが牽引あるいは弛緩され、湾曲部16が湾曲動作される。
 さらに、操作部本体20の基端側には、当該操作部本体20を挟んで指当部26bと反対側の部位に、突起状の指掛部27が設けられている。
 先端回転部21の先端側は、折れ止部30を介して挿入部10の基端側に連設されている。
 また、先端回転部21の側部には処置具挿入口31が突設され、この処置具挿入口31が、先端回転部21の内部において、処置具チャンネルに連通されている。
 さらに、先端回転部21の基端側は、操作部本体20の先端側に対し、回転機構32を介して回動可能に連結されている。ここで、本実施形態の回転機構32は、例えば、操作部本体20の第1の長手軸O1周りに、180°の回動範囲にて、先端回転部21を回動させることが可能となっている。この先端回転部21の回動により、処置具挿入口31は、挿入部10と一体的に回動され、操作部11の把持状態に応じて最適な位置に変更可能となっている(図2,3参照)。なお、この回転機構32による回動範囲は、180°に限定されるものではなく、例えば、360°以下の任意の角度範囲を設定することが可能である。
 リア把持部22は、例えば、略楕円筒形状をなす部材によって構成され、第1の長手軸O1に対する屈曲方向である第2の長手軸O2に沿って延在されている。
 本実施形態において、リア把持部22は、例えば、第1の長手軸O1に対する第2の長手軸O2の劣角側のなす角度αが105°の鈍角となるよう、操作部本体20の基端側に屈曲配置されている。これにより、リア把持部22は、フロント把持部25とは異なる方向にて操作部11を把持することが可能な第2の把持部として機能する。
 この場合において、フロント把持部25或いはリア把持部22の何れを把持した場合にも同等の操作性による湾曲操作を実現すべく、湾曲操作レバー26は、湾曲操作が行われていない中立状態にあるとき、指当部26bが、第1の長手軸O1と第2の長手軸O2との優角側のなす角度βの略中央に位置するよう、操作部本体20とリア把持部22との屈曲方向外側に突出されている。一方、指掛部27は、第1の長手軸O1と第2の長手軸O2との劣角側のなす角度αの略中央に突出するよう、操作部本体20とリア把持部22との屈曲方向内側に配置されている。
 また、湾曲操作レバー26の近傍において、リア把持部22の背面には、凹部28が設けられ、この凹部28には各種内視鏡機能のスイッチ類29が設けられている。
 ユニバーサルケーブル12は、挿入部10の内部を通じて先端側から操作部11に至り、さらに操作部11から延出する各種信号線やライトガイド等を内部に挿通する複合ケーブルである。また、送気送水装置(不図示)から延出される送気送水用チューブ等を挿通してもよい。
 このユニバーサルケーブル12は、リア把持部22の基端側から延出されている。また、ユニバーサルケーブル12の延出端には、光源装置3に接続可能な内視鏡コネクタ35が設けられている。さらに、内視鏡コネクタ35の側面部からは信号ケーブル36aが延出され、この信号ケーブル36aの延出端には、ビデオプロセッサ4に接続可能な電気コネクタ部36bが設けられている。
 ここで、リア把持部22の基端側には、ユニバーサルケーブル12を被覆するチューブ部として、折れ止部40が設けられている。本実施形態において、この折れ止部40は、ユニバーサルケーブル12を被覆している領域の硬度を徐々に変化させることにより、ユニバーサルケーブル12のリア把持部22側における座屈等を防止するためのものである。このため、折れ止部40は、リア把持部22側の硬度が高く(すなわち、硬質であり)、ユニバーサルケーブル12が露出する延出端側に向かうほど硬度が低くなるよう(すなわち、柔らかくなるよう)設定されている。
 この折れ止部40は、可動連結部41を介して、リア把持部22の基端側に連結されている。そして、可動連結部41が介在することにより、折れ止部40の方向は、操作部本体20に対するリア把持部22の屈曲方向の内側に指向する第1の方向(図2参照)と、操作部本体20に対するリア把持部22の屈曲方向の外側に指向する第2の方向(図3参照)と、に変更可能となっている。
 この場合、第1の方向は、例えば、リア把持部22の第2長手軸O2の方向に対して屈曲方向の内側に20度から40度の範囲で傾斜していることが望ましく、本実施形態では、第2の長手軸O2に対する第1の方向の傾斜角度θ1は約35度に設定されている。
 また、第2の方向は、例えば、リア把持部22の第2の長手軸O2の方向に対して屈曲方向の外側に80度以上傾斜していることが望ましく、本実施形態では、第2の長手軸O2に対する第2の方向の傾斜角度θ2は約90度に設定されている。これにより、本実施形態における第2の方向は、例えば、第1の長手軸O1に対し、±15度程度の誤差範囲で略並行となる方向に設定されている。
 次に、リア把持部22に対する可動連結部41の連結構造について具体的に説明する。
 図4に示すように、リア把持部22の内部には、操作部本体20側から延出する各種信号線、ライトガイド、及び、送気送水チューブ等のケーブル類を挿通するためのガイド筒45が固設されている。
 また、リア把持部22の基端側には開口部22aが開口され、この開口部22aには、ガイド筒45の基端側に固設されたブラケット46が臨まされている。
 また、開口部22a内には、略円筒形状をなす軸部材47が配設されている。軸部材47の外周部には外向フランジ47aが設けられ、この外向フランジ47aがビス48(図5参照)によってブラケット46に締結されることにより、軸部材47はリア把持部22内に保持されている。
 一方、可動連結部41の内部には、軸部材47の外周側に嵌合する外筒部材50が設けられている。また、軸部材47の内周側には、内筒部材51が嵌合されている。内筒部材51の端部には軸部材の端面と係合可能な係合部51aが設けられ、この内筒部材51がビス52によって外筒部材50に締結されている(図6参照)。これにより、可動連結部41は、軸部材47の中心軸を回動軸O3として回動可能となるよう、リア把持部22に連結されている。
 この場合において、第2の方向の傾斜角度θ2を第1の方向の傾斜角度θ1よりも大きく設定するため、回動軸O3は、第2の長手軸O2に対して屈曲方向外側に傾斜するよう設定されている。
 また、可動連結部41の回動範囲を規定するため、軸部材47の外向フランジ47aにはストッパピン49が設けられている。一方、外筒部材50には、軸部材47の外向フランジ47aに指向する突起部50aが設けられている。そして、この突起部50aの一側がストッパピン49に当接する回転位置から、突起部50aの他側がストッパピン49に当接する回転位置までの範囲が、可動連結部41の回動範囲として規定されている。
 なお、リア把持部22と可動連結部41との摺動部や、可動連結部41と折れ止部40との接合部等には、これらの水密を確保するためのOリング55が適宜介装されている。
 次に、このように構成された内視鏡2を把持する際の好適な把持例について、図7,図8を参照して説明する。
 例えば、図7に示すように、操作部本体20(第1の長手軸O1)が略水平となるよう、術者等がリア把持部22を把持する場合、折れ止部40は、可動連結部41の回動操作を通じて第2の方向に指向される。なお、このような把持方法では、リア把持部22を把持する手の上方から処置具等の操作を行うべく、処置具挿入口31は、先端回転部21の回動を通じて、屈曲方向の外側(第1の長手軸O1を基準として、リア把持部22と反対側)に配置される。
 そして、折れ止部40を第2の方向に向けることにより、ユニバーサルケーブル12は、操作部本体20に対するリア把持部22の屈曲方向の外側を起点として下方に延在され、操作部11の重心Cが、リア把持部22の第2の長手軸O2の近傍に位置する。
 従って、操作部11を把持する手に不要なモーメントが作用することを抑制でき、手首等にかかる負担を軽減して良好な把持性を実現することができる。
 加えて、操作部11を把持する手に不要なモーメントが作用することを抑制できるため、操作部本体20を水平方向に維持することが容易となる。従って、操作部11から被検体までの区間において、挿入部10(可撓管部17)が不必要に撓むことを防止でき、湾曲操作レバー26への操作に対して良好な応答性にて湾曲部16を湾曲動作させることができる。すなわち、挿入部10の不必要な撓みに起因して当該挿入部10内に挿通されたワイヤに意図しない張力や撓み等が発生し、湾曲操作レバー26への操作に対する湾曲部16の湾曲動作の応答性が低下することを抑制することができる。
 また、挿入部10が不必要に撓むことを防止できるので、挿入部10に対して捩り操作を行う際の先端側へのトルクの追従性が向上し、さらに、挿入部10の耐久性が向上することが期待できる。
 また、折れ止部40がリア把持部22を把持する手の前腕に沿って延在するため、当該前腕の直近傍を起点としてユニバーサルケーブル12を延在させることができる。
 従って、術中等に操作部11の方向を変化させる場合等にも、ユニバーサルケーブル12の取り回しが容易となり、より良好な操作性を実現することができる。すなわち、例えば、第1の長手軸O1を中心として術中等に操作部11の姿勢を回転させた場合においても、ユニバーサルケーブル12が暴れ難くなる。従って、ユニバーサルケーブル12が術者の腹部等に接触することを防ぎ、ユニバーサルケーブル12内の断線等による故障を予防することができる。
 これに対し、例えば、図7中に二点鎖線で示すように、可動連結部が介在することなく、折れ止部40が第2の長手軸O2方向に指向している場合、操作部11の重心C’は、第2の長手軸O2から離間する。従って、操作部11を把持する手に不要なモーメントが作用し、手首等に負担がかかる。加えて、発生したモーメントによって、操作部本体20を水平方向に維持することが困難となるため、操作部11から被検体までの区間において、挿入部10に不必要な撓みが発生しやすくなり、湾曲操作レバー26への操作に対する湾曲部16の湾曲動作が緩慢となる。そればかりか、挿入部10の撓みに起因し、例えば、挿入部10に対して捩り操作を行う際の先端側へのトルク追従性の低下や、挿入部10の耐久性の低下等が発生する可能性が高まる。また、リア把持部22を把持する手の前腕から折れ止部40が離間するため、ユニバーサルケーブル12は当該前腕から離間した位置を起点として延在され、ユニバーサルケーブル12の取り回しが困難となる。
 次に、例えば、図8に示すように、操作部本体20(第1の長手軸O1)が略垂直となるよう、術者等がフロント把持部25を把持する場合、折れ止部40は、可動連結部41の回動操作を通じて第2の方向に指向される。なお、このような把持方法では、フロント把持部25を把持する手の前方から処置具等の操作を行うべく、処置具挿入口31は、先端回転部21の回動を通じて、屈曲方向の内側(第1の長手軸O1を基準としてリア把持部22と同じ側)に配置される。
 これにより、ユニバーサルケーブル12は、リア把持部22の屈曲方向の外側から、フロント把持部25を把持した手の手首の近傍を経由して下方に延在され、操作部11の重心Cが、フロント把持部25の第1の長手軸O1の近傍に位置する。
 従って、操作部11を把持する手に不要なモーメントが作用することを防止でき、手首等に係る負担を軽減して良好な把持性を実現することができる。
 加えて、操作部11を把持する手に不要なモーメントが作用することを抑制できるため、操作部本体20を垂直方向に維持することが容易となる。従って、操作部11から被検体までの区間において、挿入部10(可撓管部17)が不必要に撓むことを防止でき、湾曲操作レバー26への操作に対して良好な応答性にて湾曲部16を湾曲動作させることができる。
 また、操作部11を把持する手の近傍をユニバーサルケーブル12が経由することにより、術中等に操作部11の方向を変化させる場合等にも、ユニバーサルケーブル12の取り回しが容易となり、より良好な操作性を実現することができる。すなわち、例えば、第1の長手軸O1を中心として術中等に操作部11の姿勢を回転させた場合においても、ユニバーサルケーブル12が暴れ難くなる。また、ユニバーサルケーブル12は操作部11を把持する手の近傍を通るため、ユニバーサルケーブル12が誤って挿入部10に触れることを防ぐことができる。
 これに対し、例えば、図8中に二点鎖線で示すように、可動連結部が介在することなく、折れ止部40が第2の長手軸O2方向に指向している場合、操作部11の重心C’は、第1の長手軸O1から離間する。従って、操作部11を把持する手に不要なモーメントが作用し、手首等に負担がかかる。加えて、発生したモーメントによって、操作部本体20を垂直方向に維持することが困難となるため、操作部11から被検体までの区間において、挿入部10に不必要な撓みが発生しやすくなり、湾曲操作レバー26への操作に対する湾曲部16の湾曲動作が緩慢となる。そればかりか、挿入部10の撓みに起因し、例えば、挿入部10に対して捩り操作を行う際の先端側へのトルク追従性の低下や、挿入部10の耐久性の低下等が発生する可能性が高まる。また、ユニバーサルケーブル12が操作部11の前方を経由して延在するため、ユニバーサルケーブル12の取り回しが困難となり、さらに、処置具挿入口31に対する作業性が低下する。
 次に、このように構成された内視鏡2を収納する際の好適な収納例について、図9を参照して説明する。
 例えば、図9に示すように、内視鏡2を洗浄機の洗浄槽60に収納する場合、折れ止部40は、可動連結部41の回動操作を通じて第1の方向に指向される。なお、この場合、例えば、処置具挿入口31は、先端回転部21の回動を通じて、屈曲方向の内側に配置される。
 これにより、洗浄槽60の壁面等と折れ止部40との干渉を防止することができ、折れ止部40が第2の長手軸O2方向、あるいは第2の方向に指向している場合(図9中の二点鎖線参照)等に比べ、洗浄槽60内に内視鏡2をコンパクトに収納することができる。
 このような実施形態によれば、操作部本体20が延在する第1の長手軸O1に対して屈曲する方向である第2の長手軸O2に沿って延在するリア把持部22を備えた内視鏡2において、リア把持部22の基端側から延出するユニバーサルケーブル12を被覆する折れ止部40を、可動連結部41を介してリア把持部22の基端側に連結し、可動連結部41により、折れ止部40の方向を、操作部本体20に対するリア把持部22の屈曲方向の内側に指向する第1の方向と、操作部本体20に対するリア把持部22の屈曲方向の外側に指向する第2の方向と、に変更可能とすることにより、コンパクトな収納性と、良好な把持性及び操作性と、を両立することができる。
 ここで、上述の実施形態においては、可動連結部41によって、折れ止部40の方向を、リア把持部22の屈曲方向の内側に指向する第1の方向と、リア把持部22の屈曲方向の外側に指向する第2の方向と、に変更する一例について説明したが、その他の方向に変更することも可能である。例えば、図10に示すように、折れ止部40の方向を、リア把持部22の屈曲方向の外側に指向し、且つ、屈曲方向の側方に傾けた第3の方向に変更することも可能である。このような第3の方向に折れ止部40の方向を変更することにより、上述した各効果だけでなく、更に、リア把持部22を把持する手や腕と、折れ止部40及びユニバーサルケーブル12と、の干渉を効果的に抑制することができる。また、光源装置3やビデオプロセッサ4が、例えば術者の側方に配置されている場合、光源装置3やビデオプロセッサ4の方向に折れ止部40を向けることもできる。これによって、ユニバーサルケーブル12を光源装置3やビデオプロセッサ4までの最短距離を通過させることができる。従って、手術中に操作部11を進退させたり、捻ったりする際に、ユニバーサルケーブル12の長さが足りなくなって、操作しにくくなることを予防できる。
 また、上述の実施形態においては、リア把持部22の基端から延出する回動軸O3周りに折れ止部40を回動させる可動連結部41の構成の一例について説明したが、この可動連結部41に代えて、例えば、図11に示すように、所謂スタンドチューブ等のフレキシブルチューブからなる可動連結部65を採用することも可能である。なお、説明を簡略化するため、図11に示す例では、可動連結部65を外部に露出して示しているが、操作部11内の水密性を確保するため、可動連結部65の外側はゴムチューブ等によって覆われている。
 また、上述の実施形態においては、リア把持部22の基端から延出する回動軸O3周りに折れ止部40を回動させる可動連結部41の構成の一例について説明したが、この可動連結部41に代えて、例えば、図12,13に示すように、第2の長手軸O2に対して直交する回動軸O4周りに折れ止部40を回動させる可動連結部66を採用することも可能である。
 なお、本発明は、以上説明した実施形態及び各変形例に限定されることなく、種々の変形や変更が可能であり、それらも本発明の技術的範囲内である。

Claims (9)

  1.  挿入部の基端側に連設する操作部本体と、
     前記操作部本体の基端側に連設され、前記操作部本体が延在する第1の長手軸に対して屈曲する方向である第2の長手軸に沿って延在するリア把持部と、
     前記リア把持部の基端側から延出するケーブル部と、
     前記リア把持部の基端側において前記ケーブル部を被覆するチューブ部と、
     前記チューブ部の方向が、前記操作部本体に対する前記リア把持部の屈曲方向の内側に指向する第1の方向と、前記屈曲方向の外側に指向する第2の方向と、を含む複数の方向に変更可能となるよう、前記チューブ部を前記リア把持部の基端側に連結する可動連結部と、を備えたことを特徴とする内視鏡。
  2.  前記可動連結部は、前記チューブ部の方向を、前記屈曲方向の外側に指向し、且つ、前記屈曲方向の側方に傾いた第3の方向に変更可能であることを特徴とする請求項1に記載の内視鏡。
  3.  前記第1の方向は、前記第2の長手軸に対し、前記屈曲方向の内側に20度から40度の範囲で傾斜した方向に設定されていることを特徴とする請求項1に記載の内視鏡。
  4.  前記第2の方向は、前記第2の長手軸に対し、前記屈曲方向の外側に80度以上傾斜した方向に設定されていることを特徴とする請求項1に記載の内視鏡。
  5.  前記可動連結部は、前記第2の長手軸に対して前記屈曲方向の外側に傾斜した回動軸周りに回動可能であることを特徴とする請求項1に記載の内視鏡。
  6.  前記可動連結部は、前記第2の長手軸に対して直交する回動軸周りに回動可能であることを特徴とする請求項1に記載の内視鏡。

  8.  前記リア把持部よりも先端側において前記操作部本体に設けられ、前記第1の長手軸に沿って把持可能なフロント把持部を有することを特徴とする請求項1に記載の内視鏡。
  9.  前記挿入部と前記操作部本体との間に介装され、前記操作部本体に対し、前記第1の長手軸周りに回動可能な先端回転部と、
     前記先端回転部に設けられた処置具挿入口と、を備えたことを特徴とする請求項1に記載の内視鏡。
