Abstract

Die Erfindung betrifft ein Phakoemulsifikationssystem, aufweisend: einen Irrigationsfluidbehälter zur Aufnahme von Irrigationsfluid, eine Irrigationsleitung, durch welche das Irrigationsfluid fließen kann, eine Strömungspumpe, durch welche das Irrigationsfluid strömen kann, eine Aspirationsleitung, durch welche Aspirationsfluid fließen kann, eine Aspirationspumpe zum Absaugen von Aspirationsfluid, eine Vorrichtung zum Erfassen eines Drucksignals basierend auf einem Druck in der Aspirationsleitung oder eine Vorrichtung zum Erfassen eines Volumenstromsignals basierend auf einem Volumenstrom in der Aspirationsleitung, und eine Steuerungsvorrichtung, welche eingerichtet ist, das Drucksignal oder das Volumenstromsignal zu empfangen und aufgrund des Drucksignals oder des Volumenstromsignals die Strömungspumpe so anzusteuern, dass eine Änderung des Druckes oder des Volumenstromes in der Irrigationsleitung erfolgt, wobei die Steuerungsvorrichtung eingerichtet ist, die Drehzahl der Strömungspumpe mit einer Frequenz im Bereich von 0 bis 10 Hertz einzustellen, wobei die Strömungspumpe einen dynamischen Überdruck von bis zu 0,5 bar aufbaut.

Description

Phakoemul si fikationss vstem
Die Erfindung betrifft ein Phakoemulsifikationssystem. Zur Behandlung einer Augenl insentrübung, welche in der Medizin als Grauer Star bezeichnet wird, gibt es mehrere chirurgische Techniken. Die am weitesten verbreitete Technik ist die Phakoemulsi fikation, bei der eine dünne Nadel in die Augenlinse eingeführt und mit Ultraschall zu Schwingungen angeregt wird. Die vibrierende Nadel emulsifiziert in ihrer nächsten Umgebung die Linse derart, dass die entstehenden Linsenpartikel durch eine Leitung mittels einer Pumpe abgesaugt werden können. Dabei wird ein Spülfluid (Irrigati- onsfluid) zugeführt, wobei das Absaugen der Partikel und des Fluides durch eine Aspirationsleitung erfolgt, welche üblicherweise innerhalb der Nadel angeordnet ist. Ist die Linse vollständig emulsifiziert und entfernt worden, kann in den leeren Kapselsack eine neue künstliche Linse eingesetzt werden, so dass ein derart behandelter Patient wieder ein gutes Sehvermögen erreichen kann.
Bei der Zerkleinerung der Augenlinse durch eine mit Ultraschall schwingende Nadel ist es unvermeidbar, dass während der Operation ein relativ großes Partikel so vor der Spitze der Nadel zur Anlage kommt, dass diese Nadelspitze bzw. ihre Absaugöffhung verstopft wird. Dieser Zustand wird als Okklusion bezeichnet. In einem solchen Fall baut eine üblicherweise eingesetzte Peristaltikpumpe in der Aspirationsleitung einen mehrfach stärkeren Saugdruck im Vergleich zu einem okklusionsfreien Betrieb auf. Zusätzlich kann ein starker Energieeintrag für die Bewegung der Nadel erfolgen, so dass das die Nadel verstopfende Partikel zertrümmert wird. In einem solchen Moment wird eine Okklusion somit aufgebro- chen, wobei der zuvor anliegende hohe Unterdruck sehr schnell abnimmt. Der dadurch entstehende Sog kann dazu führen, dass nicht nur kleine Partikel und Fluid zur Aspirationsnadel gezogen werden, sondern auch ein Teil des Kapselsackes mit der Nadel in Kontakt kommt. Wenn der Kapselsack durchstochen wird, führt dies zu erheblichen Komplikationen für den Patienten, welche unbedingt vermieden werden müssen. Die Peristaltikpumpe könnte so betrieben werden, dass sie zum Beispiel mit einer hohen Umdrehungszahl Aspirationsfluid und Partikel absaugt, so dass in der Aspirationsleitung ein relativ hoher Unterdruck entsteht. Damit ließe sich zwar eine schnellere Emulsifikation der Linse erreichen, jedoch würde dann der Augeninnendruck erheblich absinken. Ein niedriger Augeninnerdruck ist gefährlich, da das Auge bei niedrigem Druck leicht kollabieren und zum Beispiel die Pupille zu einer deutlich wahrnehmbaren Schwingung angeregt werden kann. Falls zudem bei einer derartigen Betriebsweise eine Okklusion eintreten und diese später aufbrechen sollte, kommt es zu starken Schwankungen des Augeninnendruckes, welches die Gefahr erhöht, dass im Auge ein Unterdruck entsteht, welcher das Auge kollabieren lässt. Die Ursache dafür liegt im Betrieb der Peristaltikpumpe. Bei Beginn einer Okklusion muss die Peristaltikpumpe abgeschaltet und nach dem Durchbrechen der Okklusion wieder eingeschaltet werden. Bis die Pumpe nach dem Durchbrechen der Okklusion wieder ihren normalen Volumenstrom fördern kann, vergehen etwa 30 Millisekunden, in denen eine deutliche Schwankung des Augeninndruckes nicht verhindert wer- den kann.
Es gibt zwar die Möglichkeit, durch eine Änderung in der Höhe einer Flasche mit Irrigati- onsfluid den hydrostatischen Druck des Irrigationsfluides zu verändern, sodass der Augeninnendruck beeinftusst werden kann. Um die Druckschwankung nach einem Okklusions- durchbrach durch Änderung des hydrostatischen Druckes zu kompensieren, ist jedoch ein Verstellen der Flaschenhöhe um mindestens 70 cm erforderlich. Ein solcher Vorgang erfordert aus mechanischen Gründen (Zahnstangenantrieb) eine Zeitdauer von mehreren hundert Millisekunden, was viel zu lange ist, um ein Kollabieren des Auges nach einem Okklusionsdurchbruch zuverlässig zu verhindern. Außerdem erhöht sich durch eine zu- sätzliche Schlauchlänge von mindestens 70 cm der hydraulische Widerstand der Irrigationsleitung, wodurch einer dynamischen Druckänderung schlechter entgegengewirkt werden kann. Insofern wäre eine möglichst kurze Irrigationsleitung wünschenswert, die jedoch nur eine geringe Veränderung des hydrostatischen Druckes erlaubt. In US 2002/0019607 AI ist ein Irrigationssystem für eine medizinische Vorrichtung beschrieben. Das System kann eine Steuerungsvorrichtung aufweisen, welche mit einer Irri- gationspumpe und einem Drucksensor gekoppelt ist, welcher den Druck in einer Irrigationsleitung misst. Mit der Steuerungsvorrichtung kann die Geschwindigkeit der Pumpe in Abhängigkeit von dem gemessenen Druck in der Irrigationsleitung variiert werden. Es ist eine Aufgabe der Erfindung, ein Phakoemulsifikationssystem zu schaffen, mit dem es möglich ist, sehr schnell auftretende Schwankungen des Augeninnendruckes bei einer Phakoemulsifikation auf einfache und kostengünstige Weise deutlich zu reduzieren oder sogar vollständig zu kompensieren. Diese Aufgabe wird durch den Gegenstand des unabhängigen Patentanspruchs gelöst. Vorteilhafte Weiterbildungen der Erfindung sind Gegenstand der Unteransprüche.
Das erfmdungsgemäße Phakoemulsifikationssystem weist auf:
einen Irrigationsfluidbehälter zur Aufnahme von Irrigationsfluid,
- eine Irrigationsleitung, durch welche das Irrigationsfluid fließen kann,
- eine Strömungspumpe, durch welche das Irrigationsfluid strömen kann,
eine Aspirationsleitung, durch welche Aspirationsfluid fließen kann,
- eine Aspirationspumpe zum Absaugen von Aspirationsfluid,
- . eine Vorrichtung zum Erfassen eines Drucksignals basierend auf einem Druck in der Aspirationsleitung oder eine Vorrichtung zum Erfassen eines Volumenstromsignals basierend auf einem Volumenstrom in der Aspirationsleitung, und eine Steuerungsvorrichtung, welche eingerichtet ist, das Drucksignal oder das Volumenstromsignal zu empfangen und aufgrund des Drucksignals oder des Volumenstromsignals die Strömungspumpe so anzusteuern, dass eine Änderung des Druckes oder des Volumenstromes in der Irrigationsleitung erfolgt, wobei die Steuerungsvorrichtung eingerichtet ist, die Drehzahl der Strömungspumpe mit einer Frequenz im Bereich von 0 bis 10 Hertz einzustellen, wobei die Strömungspumpe einen dynamischen Überdruck von bis zu 0,5 bar aufbaut. Das Phakoemul si fikationssystem weist somit unter anderem eine Strömungspumpe auf, durch welche das Irrigationsfluid von dem Irrigationsfluidbehälter durch die Irrigations- fluidleitung zur Strömungspumpe strömen kann. Eine Strömungspumpe ist vorteilhaft, da mit ihr sehr schnell eine Drehzahländerung und damit Volumenstromänderung bis zu einem Maximalwert erreicht werden kann. Ein für eine Phakoemulsifikation erforderlicher maximaler Volumenstrom von 150 ml/s kann durch Einsatz einer Strömungspumpe in weniger als 10 Millisekunden erreicht werden. Dies bedeutet, dass durch Ansteuemng der Strömungspumpe sehr schnell eine Änderung des Volumenstromes in der Irrigationsleitung und sehr schnell eine Änderung des Druckes innerhalb des Auges möglich ist. Zudem ist eine Strömungspumpe vorteilhaft, da keine aufwändigen pneumatischen oder hydraulischen Druckregelungen, Druckkammern oder Ventile angesteuert und betätigt werden müssen.
Die Änderung des Volumenstromes in der Irrigationsleitung oder des Druckes im Auge erfolgt bei dem erfmdungsgemäßen Phakoemulsifikationssystem nur durch eine Änderung der Drehzahl der Strömungspumpe. Die Ansteuerung einer solchen Strömungspumpe kann zum Beispiel mittels eines digitalen Drehzahlmessers einfach und sicher gesteuert und überwacht werden. Außerdem kann eine Strömungspumpe direkt in eine Kassette eingebaut werden, welche in einer Konsole eines Phakoemulsifikationssystems eingesetzt ist. Damit ist der Abstand zu einem zu behandelnden Auge relativ kurz, sodass eine nur geringe Schlauchlänge bis zu einem Handstück erforderlich ist, welches eine schwingende Na- del zur Emulsifikation des Auges aufweist und Irrigationsfluid zum Auge zuführt. Statt einer üblichen Schlauchlänge vom Irrigationsfluidbehälter zum Handstück von etwa drei Metern ist somit nur noch eine Schlauchlänge von der Konsole bis zum Handstück von etwa einem Meter erforderlich. Der hydraulische Widerstand im Schlauch, der einer schnellen Druckänderung entgegenwirkt, ist damit mindestens um die Hälfte kleiner als bisher. Eine rasch auftretende Schwankung des Augeninnendruckes bis zu einem gefährlich niedrigen Druckniveau kann durch das erfindungsgemäße Phakoemulsifikationssystem mittels einer schnellen Zufuhr von Irrigationsfluid durch Einsatz einer Strömungspumpe kompensiert werden. Die Steuerungsvorrichtung ist gemäß der Erfindung eingerichtet, die Drehzahl der Strömungspumpe mit einer Frequenz im Bereich von 0 bis 10 Hertz einzustellen, wobei die Strömungspumpe einen dynamischen Überdruck von bis zu 0,5 bar aufbaut. Damit können auch die bei kurzen Okklusionsvorgängen auftretenden Druckschwankungen in der Irrigationsleitung ausgeglichen werden.
Vorzugsweise ist die Strömungspumpe in der Irrigationsleitung integriert. Damit fließt direkt und ohne Umwege Irrigationsfluid durch die Strömungspumpe. Es ist also kein Bypass oder eine ähnliche Abzweigung mit Ventilen und zugehöriger Ansteuerung erforderlich. Dies ist zusätzlich vorteilhaft, da keine Zeit für das Betätigen von Ventilen benötigt wird, sodass mit einer noch höheren Sicherheit einer gefährlichen Druckschwankung im Auge eines Patienten entgegengewirkt werden kann.
Gemäß einer weiteren Ausführungsform ist der Irrigationsfluidbehälter während einer Phakoemu 1 si fikati on in seiner Höhe nicht verstellbar. Damit ist sichergestellt, dass stets eine vorher eingestellte Höhe und damit ein vorher eingestellter hydrostatischer Druck gegeben ist. Wenn während der Phakoemulsifikation keine Okklusion und daher auch kein anschließender Okklusionsdurchbruch vorliegt, genügt ein Volumenstrom, wie er sich aus dem hydrostatischen Druck bei einer vorbestimmten Höhe des Irrigationsfluidbehälters ergibt. Die Strömungspumpe ist dann auch ohne Ansteuerung für diesen relativ kleinen Volumenstrom durchlässig. Falls ein Okklusionsdurchbruch vorliegt, kann zu diesem durch den hydrostatischen Druck vorhandenen Volumenstrom mittels der Strömungspumpe ein zusätzlicher Volumenstrom in sehr kurzer Zeit hinzugefügt werden. Der hydrostatische Druck wirkt als Offset für die Strömungspumpe. Durch die fest eingestellte Höhe des Irrigationsfluidbehälters kann somit auf einen aufwändigen Zahnstangenantrieb mit Dämpfung verzichtet werden, wie es bei einem Phakoemulsifikationssystem nach dem Stand der Technik üblich ist.
Es ist vorteilhaft, wenn die Strömungspumpe mittels einer Magnetkupplung ansteuerbar ist. Eine Magnetkupplung ermöglicht ein Antreiben eines Strömungsrades der Strömungspumpe ohne direkte mechanische Verbindung. Damit lassen sich Probleme mit eine Abdich- tung geschickt umgehen. Ferner erlaubt eine Magnetkupplung einen sicheren nicht- invasiven Betrieb der Strömungspumpe, was im Hinblick auf eine Sterilität des Irrigations- fluides sehr vorteilhaft ist.
Gemäß einer Weiterbildung der Erfindung ist die Steuerungsvorrichtung des Phakoemulsi- fikationssystems eingerichtet, die Strömungspumpe so anzusteuern, dass der von der Strömungspumpe erzeugte Volumenstrom in der Irrigationsleitung proportional zum Volumenstrom in der Aspirationsleitung ist. Damit ist eine besonders einfache Steuerung bei sehr gutem Ergebnis bezüglich eines Ausgleiches des Augeninnendruckes möglich.
Bevorzugt ist die Strömungspumpe eine Kreiselpumpe. Bei einer Kreiselpumpe wird Fluid zu einer Nabe eines Laufrades der Kreiselpumpe zugeführt und durch Fliehkraft zu einem Rand des Laufrades geschleudert. Es erfolgt eine Umwandlung der kinetischen Energie in eine Druckenergie. Eine solche Pumpe eignet sich dazu, den Volumenstrom und den Druck des zu fördernden Fluids zu ändern.
Eine Kreiselpumpe ist bei dem erfindungsgemäßen Phakoemul si fikationssystem vorteilhaft, da sie einfach und robust ist und zudem Drehzahlen bis zu 10.000 Umdrehungen pro Minute und eine sehr schnelle Änderung des Volumenstromes und des Druckes des zu fördernden Fluides ermöglicht.
Weitere Vorteile und Merkmale der Erfindung werden mit Bezug auf die nachfolgenden Zeichnungen erklärt, in welchen zeigen:
Fig. 1 eine schematische Darstellung eines erfindungsgemäßen Phakoemulsifikations- systems;
Fig. 2 eine schematische Darstellung von Signalverläufen in Abhängigkeit von der Zeit für einen Volumenstrom in einer Aspirationsleitung, einen Saugdruck in einer Aspirationsleitung, einen Augeninnendruck bei einem Phakoemulsifikationssystem nach dem Stand der Technik, eine Drehzahl einer Strömungspumpe und einen zugehörigen Volumenstrom der Strömungspumpe und einen Fluiddruck der Strömungspumpe bei einer Ausfuhrungsform des erfindungsgemäßen Phakoemulsifi- kationssystems, und einen Augeninnendruck beim Einsatz des erfindungsgemäßen Phakoemulsifikationssystems;
Fig. 3 ein Diagramm mit einem Verlauf des Augeninnendruckes in Abhängigkeit von der Drehzahl der Strömungspumpe;
Fig. 4 ein Diagramm mit einem Verlauf bezüglich der Drehzahl der Strömungspumpe in Abhängigkeit von der Zeit bei einer bevorzugten Ausführungsform ; und
Fig. 5 ein Diagramm mit einem Verlauf bezüglich des von der Strömungspumpe erzeugten Druckes in Abhängigkeit von der Zeit bei der bevorzugten Ausführungsform.
Fig. 1 zeigt eine schematische Darstellung einer Ausfuhrungsform des erfindungsgemäßen Phakoemulsifikationssystems 200. In einem Irrigationsfluidbehälter 1 ist ein Irrigationsfluid 2 enthalten, welches durch eine Irrigationsleitung 3 abfließen kann. Das Irrigationsfluid 2 erreicht eine Strömungspumpe 4, welche in der Irrigationsleitung 3 integriert ist. Das Irrigationsfluid 2 strömt durch die Strömungspumpe 4 hindurch und erreicht ein Handstück 5 mit einer mit Ultraschall in Longitudinalrichtung schwingenden Nadel 6 zur Emul- sifikation einer zu behandelnden Augenlinse 7 eines Auges 8. Die emulsifizierten Linsenpartikel und zugehöriges Fluid werden durch eine Aspirationsleitung 9 abtransportiert, wobei der Saugdruck von einer Aspirationspumpe 10 erzeugt wird. Die abgesaugten Linsenpartikel und das Fluid werden in einem Sammelbehälter 11 gesammelt, der in Strömungsrichtung nach der Aspirationspumpe 10 angeordnet ist.
Mittels einer Vorrichtung 12 wird ein Volumenstrom in der Aspirationsleitung 9 oder ein Druck in der Aspirationsleitung 9 erfasst, sodass von dieser Vorrichtung 12 ein Volumenstromsignal 13 oder ein Drucksignal 14 erzeugt wird. Das Volumenstromsignal 13 oder Drucksignal 14 wird daraufhin zu einer Steuerungsvorrichtung 15 geleitet und von dieser empfangen. Die Steuerungsvorrichtung 15 ist eingerichtet, ein Steuersignal 16 an die Strömungspumpe 4 zu senden, so dass eine Änderung des Volumenstromes oder des Druckes in der Irrigationsleitung 3 erfolgt. Das Volumenstromsignal 13 kann mittels eines Volumenstromsensors oder mittels der Aspirationspumpe 10 ermittelt werden. Das Drucksignal kann bevorzugt mittels eines Drucksensors ermittelt werden. Die Strömungs- pumpe 4 kann bevorzugt maximal 150 ml/s fördern, besonders bevorzugt kann sie maximal 120 ml/s fördern.
Fig. 2 zeigt die relevanten Signalverläufe bei einem Phakoemulsifikationssystem nach dem Stand der Technik und bei dem erfindungsgemäßen Phakoemulsifikationssystem. In einem Diagramm 20 ist ein Verlauf eines Volumenstromes (ASP) in der Aspirationsleitung dargestellt. Wenn die Aspirationspumpe 10 mit dem Absaugen beginnt, steigt der Volumenstrom in der Aspirationsleitung 9 an, siehe Bezugszeichen 21, bis ein konstanter Wert gemäß der Saugleistung der Aspirationspumpe erreicht ist, siehe Bezugszeichen 22. Wenn eine Okklusion auftritt, siehe Diagramm 40 und dort Bezugszeichen 41, sinkt der Volumenstrom in der Aspirationsleitung sehr schnell auf den Betrag Null ab, siehe Bezugszeichen 23. Mit dem Aufbrechen der Okklusion, siehe Bezugszeichen 42 in Diagramm 40, steigt der Volumenstrom in der Aspirationsleitung wieder an, siehe Bezugszeichen 24, wobei nachfolgend ein starkes Schwanken des Volumenstromes auftreten kann, siehe Be- zugszeichen 25, bis sich der Wert wieder auf ein Niveau wie vor der Okklusion einpendelt, siehe Bezugszeichen 26.
Diagramm 30 zeigt den zugehörigen Verlauf des Druckes p(ASP) in der Aspirationsleitung, Mit dem Beginn des Abpumpens steigt der Saugdruck Unterdruck) auf einen bestimmten Wert, siehe Bezugszeichen 31 , der während des Abpumpens konstant bleibt, wenn die Nadel 6 des Phakohandstückes 5 nicht verstopft ist. Wenn die Okklusion auftritt, siehe Bezugszeichen 41, saugt die Aspirationspumpe 10 die Aspirationsleitung 9 leer, bis in der Aspirationsleitung 9 ein relativ hoher Unterdruck 33 besteht. Wenn aufgrund dieses Druckes und eventuell gleichzeitig anliegender Ultraschallschwingung der Nadel 6 nach eini- ger Zeit die Okklusion aufbricht, siehe Bezugszeichen 42, fällt der Unterdruck in der Aspirationsleitung 9 sehr schnell ab, siehe Bezugszeichen 34, und pendelt sich daraufhin auf den Betrag vor der Okklusion ein, siehe Bezugszeichen 35.
Bei einem Phakoemulsifikationssystem nach dem Stand der Technik stellt sich bei diesem Verhalten des Volumenstroms und des Druckes in der Aspirationsleitung ein Augeninnendruck p(IOPl) ein, wie er in Diagramm 50 dargestellt ist. Vor der Operation hat das Auge einen normalen Augeninnendruck, siehe Bezugszeichen 51. Mit Beginn des Absaugens durch die Aspirationspumpe sinkt trotz Zufluss von Irrigationsfluid in die Augenkammer der Augeninnendruck auf einen niedrigeren Betrag ab, siehe Bezugszeichen 52, und bleibt zunächst konstant. Mit Eintreten der Okklusion, siehe Bezugszeichen 41, kann kein Fluid mehr durch die Nadel 6 und die Aspirationsleitung 9 abgesaugt werden, sodass der Augeninnendruck p(IOPl) wieder auf den vorherigen Wert ansteigt, siehe Bezugszeichen 53. Mit dem Okklusionsdurchbruch, siehe Bezugszeichen 42, saugt die Aspirationspumpe 10 wieder sehr schnell und sehr viel Fluid ab, sodass der Augeninnendruck p(IOPl) sehr stark abfallen kann, siehe Bezugszeichen 54, bis er sich auf den vorherigen Wert wieder ein- schwingt, siehe Bezugszeichen 55.
Unerwünscht ist ein starkes Absinken oder Ansteigen des Augeninnendruckes in kurzer Zeit. Durch Einsatz des erfindungsgemäßen Phakoemulsifikationssystems 200 kann dies wirksam verringert oder sogar völlig verhindert werden. Das Diagramm 60 zeigt den Ver- lauf der Drehzahl n(PP) der Strömungspumpe 4 in Abhängigkeit von der Zeit t. Mit dem Einsetzen der Absaugung durch die Aspirationspumpe 10 und dem zunehmenden Volumenstrom in der Aspirationsleitung, siehe Bezugszeichen 21, wird die Strömungspumpe 4 so angesteuert, dass ihre Drehzahl ansteigt, siehe Bezugszeichen 61, so dass der Volumenstrom in der Irrigationsleitung 3 zunimmt. Wenn der Volumenstrom in der Aspirationslei- tung 9 einen konstanten Betrag erreicht hat, siehe Bezugszeichen 22, bleibt die Drehzahl der Strömungspumpe 4 ebenfalls konstant, siehe Bezugszeichen 62. Damit wird dem Absinken des Augeninnendruckes gemäß Bezugszeichen 52 entgegengewirkt, so dass der Augeninnendruck p(IOP2) nahezu konstant bleibt, siehe Diagramm 90. Mit dem Einsetzen einer Okklusion nimmt der Volumenstrom in der Aspirationsleitung schnell ab, siehe Bezugszeichen 23, wobei die S trömungspumpe 4 so angesteuert wird, dass Ihre Drehzahl auf Null absinkt, siehe Bezugszeichen 63. Wenn die Okklusion aufbricht und aufgrund des hohen Unterdruckes in der Aspirationsleitung ein starker Sog im Auge auftritt, wird sofort die Strömungspumpe auf eine höhere Drehzahl angesteuert, siehe Bezugszeichen 64, um das Schwanken des Augeninnendruckes, siehe Bezugszeichen 54, zu kompensieren. Dass daraufhin der Augeninnendruck nicht mehr schwankt, ist aus Dia- gramm 90 ersichtlich. Während der weiteren Emulsifikation des Linse wird ständig von der Strömungspumpe 4 weiteres Irrigationsfluid 2 zur Verfügung gestellt, was durch eine vorbestimmte Drehzahl der Strömungspumpe 4 erreicht wird, siehe Bezugszeichen 65. Der Volumenstrom in der Irrigationsleitung 3 wird durch den Einsatz der Strömungspumpe 4 beeinflusst, siehe Diagramm 70: Der Verlauf des Volumenstromes in der Irrigationsleitung, wie er durch die Strömungspumpe erzeugt wird, ist proportional zu dem Verlauf der Drehzahl der Strömungspumpe 4. Wenn das durch die Strömungspumpe 4 fließende Irrigationsfluid 2 mit einem konstanten hydrostatischen Druck strömt, ergibt sich ein Druckver- lauf p(IR ) in der Irrigationsleitung 3 gemäß Diagramm 80, wobei der Offset 81 durch den hydrostatischen Druck gebildet wird. Ohne Berücksichtigung des Offsets ist der restliche Verlauf des Drucksignals proportional zur Drehzahl n der Strömungspumpe, siehe Vergleich des Diagramms 80 mit Diagramm 60. Fig. 3 zeigt ein Diagramm 100 mit einer Änderung des Augeninndruckes p(IOP2) in Abhängigkeit von der Drehzahl der Strömungspumpe 4 bei dem erfindungsgemäßen Phako- emulsifikationssystem, wenn ein hydrostatischer Druck von 29 mmHg aufgrund der fixen Höhe des Irrigationsfluidbehälters vorliegt. Mit zunehmender Drehzahl n der Strömungspumpe nimmt der Augeninnendruck p(IOP2) linear zu.
Die Fig. 4 zeigt einen Verlauf der Drehzahl n(PP) der Strömungspumpe 4 in Abhängigkeit von der Zeit. Es ist erkennbar, dass die Ansteuerung der Strömungspumpe 4 mit einem Rechtecksignal erfolgt, welches mit einer Frequenz von 10 Hertz (Periodendauer 0,1 Sekunden) anliegt. Aus Fig. 5 ist der zugehörige Druckverlauf p(PP) der Strömungspumpe erkennbar. Die Strömungspumpe kann bei konstanter Drehzahl einen Überdruck von bis zu 0,5 bar (5 x 104 N/m2), bevorzugt von bis zu 0,3 bar (3 x 104 N/m2) erzeugen. Je kleiner die Frequenz ist, umso länger ist die Strömungspumpe 4 je Periode ohne Unterbrechung in Betrieb. Durch eine Pulsweitenmodulation der Ansteuerung und Variation der Frequenz kann die zugeführte Menge an Irrigationsfluid bei gleicher Drehzahl variiert werden.

Claims

Patentansprüche:
1. Phakoemulsifikationssystem, aufweisend:
einen Irrigationsfluidbehälter zur Aufnahme von Irrigationsfluid,
eine Irrigationsleitung, durch welche das Irrigationsfluid fließen kann, eine Strömungspumpe, durch welche das Irrigationsfluid strömen kann, eine Aspirationsleitung, durch welche Aspirationsfluid fließen kann, eine Aspirationspumpe zum Absaugen von Aspirationsfluid,
eine Vorrichtung zum Erfassen eines Drucksignals basierend auf einem Druck in der Aspirationsleitung oder eine Vorrichtung zum Erfassen eines Volumenstromsignals basierend auf einem Volumenstrom in der Aspirationsleitung, und eine Steuerungsvorrichtung, welche eingerichtet ist, das Drucksignal oder das Volumenstromsignal zu empfangen und aufgrund des Drucksignals oder des Volumenstromsignals die Strömungspumpe so anzusteuern, dass eine Änderung des Druckes oder des Volumenstromes in der Irrigationsleitung erfolgt, wobei die Steuerungsvorrichtung eingerichtet ist, die Drehzahl der Strömungspumpe mit einer Frequenz im Bereich von 0 bis 10 Hertz einzustellen, wobei die Strömungspumpe einen dynamischen Überdruck von bis zu 0,5 bar aufbaut.
2. Phakoemulsifikationssystem nach Anspruch 1, wobei die Strömungspumpe in der Irrigationsleitung integriert ist.
3. Phakoemulsifikationssystem nach Anspruch 1 oder 2, wobei der Irrigationsfluidbehälter während einer Phakoemulsifikation in seiner Höhe nicht verstellbar ist.
4. Phakoemulsifikationssystem nach einem der Ansprüche 1 bis 3, wobei die Strömungspumpe mittels einer Magnetkupplung ansteuerbar ist.
5. Phakoemulsifikationssystem nach einem der vorherigen Ansprüche, wobei die Steuerungsvorrichtung eingerichtet ist, die Strömungspumpe so anzusteuern, dass der von der Strömungspumpe erzeugte Volumenstrom in der Irrigationsleitung proportional zum Volumenstrom in der A spirationsl ei tung ist.
6. Phakoemulsifikationssystem nach einem der vorherigen Ansprüche, wobei die Strömungspumpe eine Kreiselpumpe ist.
