がん又は炎症性疾患患者の予後を予測する方法
 本発明は、がん又は炎症性疾患の患者の予後を予測する方法に関する。
 フロントタンパク質は、ケモカイン受容体CCR2及びCCR5の細胞内C末端領域に結合する細胞質タンパク質であり、マクロファージなどの遊走シグナルを正に制御する(特許文献1、非特許文献1、2)。本願発明者らのグループが発見した新規分子である。
 CCR2及びCCR5は、いずれもがん及び炎症性疾患に関与することが知られており、これら疾患の新規治療剤を目指してCCR2及びCCR5の阻害剤開発が世界的に進められているものの、いずれも難航している(非特許文献3、4)。既存のアプローチは、ケモカインCCL2と受容体CCR2との結合及びケモカインCCL3~5と受容体CCR5との結合、並びに受容体下流のPI3K等によるシグナル伝達系をターゲットとしていた。ケモカイン受容体CCR2及びCCR5とフロントとの結合阻害は、新たな創薬ターゲットとして期待されている(非特許文献3、4)。
 しかしながら、フロントタンパク質、並びにこれが結合するCCR2等のCCケモカイン受容体及びそのリガンドと、がん及び炎症性疾患の予後との関連は知られていない。

 本願発明者らは、肺がん患者を対象にフロント及びCCR2等のCCケモカイン受容体/リガンドの発現と術後予後との関連を鋭意に解析した結果、フロントの発現が高い患者群は手術時の臨床ステージに違いがないにもかかわらず、発現が低い患者と比べて術後予後が有意に不良であり、フロントが予後増悪因子であることを見出した。また、CCR2、CCR5及びこれらのリガンドCCL2、CCL5が予後良好因子であることを見出した。CCR2やCCR5の発現が高い患者では、予後増悪因子であるフロントの発現が高い場合でも予後が良好であること、既知の患者群の発現データ及び予後データを解析し、CCケモカイン受容体/リガンドについてのカットオフ値を予め定めておくことにより、さらに精度の良い予後予測が可能になることを見出した。以上を見出すことにより、本願発明を完成した。

 さらにまた、本発明は、フロント阻害剤又はケモカイン阻害剤を有効成分とする医薬の有効性を予測する方法であって、前記医薬の投与が検討される患者から分離された試料におけるフロント遺伝子の発現量を測定することを含み、フロント遺伝子の発現が高いほど、当該患者において前記医薬の効果が高いと予測される、方法を提供する。さらに、本発明は、フロント阻害剤又はケモカイン阻害剤を有効成分とする医薬の効果が高い患者を選別する方法であって、上記本発明の方法によりフロント阻害剤又はケモカイン阻害剤を有効成分とする医薬の効果が高いと予測された患者を、当該医薬の効果が高い患者として選別することを含み、前記ケモカイン阻害剤は、CCR2、CCR5、CCL2及びCCL5からなる群より選択される少なくとも1種の阻害剤である、方法を提供する。さらに、本発明は、フロント阻害剤又はケモカイン阻害剤を有効成分とする医薬の投与が望まれる患者を選別する方法であって、患者から採取された試料におけるフロント遺伝子の発現を調べることを含み、前記ケモカイン阻害剤は、CCR2、CCR5、CCL2及びCCL5からなる群より選択される少なくとも1種の阻害剤であり、フロント遺伝子の発現が検出される患者が、前記医薬の投与が望まれる患者として選別される、方法を提供する。
 さらにまた、本発明は、がん又は炎症性疾患の予後と負の相関を示す予後予測マーカーとしての、フロント遺伝子産物の使用を提供する。さらに、本発明は、がん又は炎症性疾患の予後と正の相関を示す予後予測マーカーとしての、CCR2遺伝子、CCR5遺伝子、CCL2遺伝子及びCCL5遺伝子からなる群より選択される少なくとも1種の遺伝子の産物の使用を提供する。さらに、本発明は、下記(1)及び(2)の少なくともいずれか一方を含む、がん又は炎症性疾患の予後予測用キットを提供する。
(1) がん又は炎症性疾患の予後と負の相関を示す予後予測マーカーとしてのフロント遺伝子の発現量を測定可能なプライマーセット、プローブ、又は抗体。
(2) がん又は炎症性疾患の予後と正の相関を示す予後予測マーカーとしてのCCR2遺伝子、CCR5遺伝子、CCL2遺伝子及びCCL5遺伝子からなる群より選択される少なくとも1種の遺伝子の発現量を測定可能なプライマーセット、プローブ、又は抗体。
 本発明によれば、予後増悪マーカーとなるフロント遺伝子、又は予後良好マーカーとなるCCR2等のCCケモカイン受容体/リガンド遺伝子の発現量に基づき、がん及び炎症性疾患の患者の予後を予測することが可能になる。フロント遺伝子及びCCケモカイン受容体/リガンド遺伝子の両者の発現量を調べることで、さらに精度のよい予測も可能になる。予後の予測結果は、治療方針の決定や経過観察などに活用することができる。また、従来より、がんや炎症性疾患の治療目的で、各種のケモカイン阻害剤が研究、開発されており、受容体CCR2及びCCR5並びにこれらのリガンドCCL2及びCCL5を標的とした阻害剤もがんや炎症性疾患のための医薬として開発されている。また、新たな創薬標的分子であるフロントの阻害剤の開発も本願発明者らのグループにより進められている。このようなケモカイン阻害剤又はフロント阻害剤を有効成分とする医薬は、フロント遺伝子の発現量が高い患者において特に効果が高いと期待される。本発明によれば、ケモカイン阻害剤やフロント阻害剤の有効性の予測や、これら阻害剤の効果が高い患者ないしはこれら阻害剤の投与が望まれる患者の選別も可能になる。

 本発明の第1の方法では、がん又は炎症性疾患の患者から採取された試料を用いて、フロント遺伝子の発現量を測定する。フロント遺伝子は、本願発明者らのグループが初めて発見した、CCR2及びCCR5に結合しマクロファージなどの遊走シグナルを正に制御する分子である(Nature Immunology, 6: 827-835, 2005, Journal of Immunology, 183:6387-6394, 2009)。フロント遺伝子及びフロントタンパク質の配列情報は、NCBIのデータベースGenBankにAF498261やNM_024844のアクセッション番号で登録されている。このうちAF498261として登録されている塩基配列及びアミノ酸配列を配列表の配列番号1及び2に示す。フロントは予後増悪因子であり、フロント遺伝子の発現量が低い患者ほど予後が良好であると予測することができる。すなわち、フロント遺伝子産物は、がん又は炎症性疾患の予後と負の相関を示す予後予測マーカーとして利用可能である。なお、遺伝子産物という語には、該遺伝子から発現されるmRNA及びタンパク質の両者が包含される。
 本発明の第2の方法では、がん又は炎症性疾患の患者から採取された試料を用いて、CCケモカイン受容体/リガンド遺伝子の発現量を測定する。本発明において、「CCケモカイン受容体/リガンド遺伝子」とは、CCR2遺伝子、CCR5遺伝子、CCL2遺伝子及びCCL5遺伝子からなる群より選択される少なくとも1種の遺伝子であり、例えば、CCR2遺伝子及びCCR5遺伝子から選択される少なくとも1種であり得る。あるいは、CCケモカイン受容体/リガンド遺伝子は、CCR2遺伝子及びCCL2遺伝子の2つの遺伝子の組み合わせ、又はCCR5遺伝子及びCCL5遺伝子の2つの遺伝子の組み合わせであり得る。CCケモカイン受容体/リガンドは予後良好因子であり、CCケモカイン受容体/リガンド遺伝子の発現量が高い患者ほど予後が良好であると予測することができる。すなわち、CCケモカイン受容体/リガンド遺伝子産物は、がん又は炎症性疾患の予後と正の相関を示す予後予測マーカーとして利用可能である。CCR2高発現かつCCL2高発現の患者、及びCCR5高発現かつCCL5遺伝子高発現の患者では、特に予後が良好である。
 CCR2にはC末端領域の異なるCCR2A及びCCR2Bの2つのアイソフォームが存在するが、細胞に広く発現する主要なアイソフォームはCCR2Bである。本発明において測定対象となるCCR2とはCCR2Bである。以下、単にCCR2といった場合には、文脈からそうではないことが明らかな場合を除き、CCR2Bを意味する。GenBankにNM_001123396.1のアクセッション番号で登録されているCCR2Bの塩基配列及びアミノ酸配列を配列番号3及び4に示す。
 CCR5には5'-UTR領域が相違するCCR5A及びCCR5Bの2つのアイソフォームが存在するが、いずれもコード領域の配列は同一であり、タンパク質のアミノ酸配列も同一である。CCR5遺伝子の発現をmRNA発現量により調べる場合、コード領域を対象にプライマー等を設定すれば両方のアイソフォームを検出できる。CCR5遺伝子の配列として、GenBankにNM_000579.3で登録されているCCR5Aの塩基配列及びアミノ酸配列を配列番号5及び6に示す。
 配列番号7~10には、CCL2(NM_002982.3)及びCCL5(NM_002985.2)の塩基配列及びアミノ酸配列をそれぞれ示す。
 本発明の第3の方法では、がん又は炎症性疾患の患者から採取された試料を用いて、フロント遺伝子の発現量及びCCケモカイン受容体/リガンド遺伝子の発現量を測定する。フロント遺伝子発現量とCCケモカイン受容体/リガンド遺伝子発現量との間には正の相関があり、フロント発現量が高い患者ではCCケモカイン受容体/リガンド遺伝子の発現量も高い傾向があるが、CCケモカイン受容体/リガンド遺伝子の発現量が一定以上高い患者では、フロント発現量が高くても予後が良好であることが、既知の肺がん患者集団の生存データの解析により明らかとなった。従って、第3の方法では、既知の患者集団の発現データ及び予後データの解析により予め定められたCCケモカイン受容体/リガンド遺伝子の発現量の参照値と、患者由来試料において測定されたCCケモカイン受容体/リガンド遺伝子発現量を対比する。患者のCCケモカイン受容体/リガンド発現量が参照値以上である場合、当該患者の予後は良好と予測することができる。
 なお、上述の通り、CCケモカイン受容体/リガンド遺伝子の発現量が一定以上高いがん患者では、フロント発現量が高くても予後が良好であるが、このCCケモカイン受容体/リガンド高発現群ではステージIの患者が占める割合が極めて多い。従って、第1の方法をがん患者に対して実施する場合、ステージII以上の患者を予後予測対象として選択してもよい。ステージII以上のがん患者に対して第1の方法を実施した場合、フロント遺伝子の発現が高いほど予後が不良であるという予測も可能である。
 発現解析に用いる試料は、例えば、患者から採取された病変部の組織試料である。がん患者の場合、手術により切除された手術検体から病変部組織試料(がん細胞を含む微小環境)を得ることができる。炎症性疾患の場合、炎症病変部(炎症細胞を含む炎症性微小環境)の生検試料などから組織試料を得ることができる。がん細胞又は炎症細胞とその周辺の微小環境構成細胞とを含む組織断片を用いればよい。後述の通り、発現量データとしては標準遺伝子に対してノーマライズした値を用いるので、組織試料からRNAを抽出してmRNAの発現を調べる場合でも、組織試料の重量を厳密に揃える必要はない。
 また、試料は血液試料または咯痰であり得る。病変部の微小環境を構成する細胞の中でフロントの発現が特に顕著にみられる細胞としては、第一にマクロファージを挙げることができるが、末梢血や咯痰は患者体内のフロント発現マクロファージの量を反映し得るため、血液試料や喀痰を用いて本発明を実施することも可能である。
 発現量の測定方法は特に限定されず、数値データ等として定量的又は半定量的に発現量を評価できる方法であればいかなる方法でもよい。リアルタイムPCRやノーザンブロッティング等の、mRNAの発現量を調べる方法のほか、測定対象分子に対する抗体を用いたウエスタンブロッティングや免疫組織染色等の免疫学的測定方法も採用することができる。本発明では、mRNAの発現量を調べる方法、特にリアルタイムPCRが好ましく用いられ得る。本発明において、発現量の測定値といった場合には、標準遺伝子に対してノーマライズした後の相対値や、その対数値(log gene expression level)も包含する。標準遺伝子としては、通常、Gapdh遺伝子等のハウスキーピング遺伝子を用いる。
 組織標本の免疫染色により発現量を測定する場合、染色の程度によって測定対象分子の発現量を定量的又は半定量的に評価すればよい。目視による半定量的な評価でもよいし、染色像をコンピューターにより解析して染色の程度を数値化して定量的に評価してもよい。染色像から目視により発現量を評価する場合には、例えば、発現量が低い例及び高い例を含めた複数のサンプル画像との対比により、発現量が高いかどうかを評価してもよい。特に、発現量が低いほど又は高いほど予後良好と予測する第1及び第2の方法は、発現量に関する参照値を定めることなく実施することもできるので、mRNAの発現量を調べる方法に加え、免疫組織染色等によりタンパク質の発現量を調べる方法も好ましく用いることができる。第3の方法におけるフロント遺伝子の発現量の測定も同様であり、例えば、第3の方法において、フロントタンパク質の発現量を免疫組織染色等により調べ、CCケモカイン受容体/リガンドmRNAの発現量をリアルタイムPCR等により調べることも可能である。
 リアルタイムPCR等の発現解析に用いるプライマーセットやプローブは、当業者であれば、公知のフロント遺伝子及びCCケモカイン受容体/リガンド遺伝子(CCR2、CCR5、CCL2又はCCL5)の配列情報をもとに適宜設計することができる。フロント及びCCケモカイン受容体/リガンドを免疫学的に測定するための抗体も、当業者であれば周知の常法により作出することができ、また適当な市販品を用いることも可能である。
 なお、本発明において、遺伝子の発現という語には、mRNAの発現及びタンパク質の発現の両者が包含され、遺伝子の発現量の測定という語には、発現したmRNAの測定及び発現したタンパク質の測定の両者が包含される。
第3の方法における参照値の決定方法
 CCケモカイン受容体/リガンド遺伝子発現量についての参照値は、既知の患者集団のフロント及びCCケモカイン受容体/リガンドの発現データと予後データを統計学的に解析して関連を調べることにより求めることができる。予後データは、がんの場合、生存期間データ(術後生存期間、又は無病生存期間)を予後データとして用いるのが一般的である。炎症性疾患の場合は、再燃や増悪などについてのデータが予後データとなる。再燃や増悪が見られるまでの期間(日数、月数又は年数)、あるいは一定期間内での症状のスコア評価の合計値など、適宜数値データとして得ることができる。
参照値決定方法の具体例1
 既知の患者集団の発現データと予後データを再帰分割分析に付し、フロント高発現で予後が不良な患者群(FNT-H)、フロント低発現で予後が良好な患者群(FNT-L)、及びCCケモカイン受容体/リガンド高発現で予後が良好な患者群(CC-H)に分割する。FNT-H及びFNT-LとCC-Hとの間を区別するCCケモカイン受容体/リガンドの発現量を参照値として用いることができる。一例として、下記実施例に記載された68名の肺がん患者集団のうちのステージI症例(28症例)に対してこの手法を適用すると、CCR2についてはlog gene expression levelで1.42、CCR5については2.23、CCL5については1.34という数値が参照値として得られる。従って、様々ながん及び炎症性疾患の患者、好ましくはがん患者に対し、log gene expression levelでCCR2は約1.27~約1.57の間、例えば約1.32~約1.52の間の数値を、CCR5は約2.08~約2.38の間、例えば約2.13~約2.33の間の数値を、またCCL5は約1.19~約1.49の間、例えば約1.24~約1.44の間の数値を、それぞれ参照値として用いることができる。もっとも、これらの数値は一例であり、より多くの患者集団のデータを集めてさらに望ましい数値を求めることができるので、本発明の範囲はこのような具体的数値に限定されるものではない。
参照値決定方法の具体例2
 予後予測の対象ががん患者である場合、ステージIの既知の患者群とステージII以上の既知の患者群とを有意に分けるCCケモカイン受容体/リガンド遺伝子発現量のカットオフ値を求め、これを参照値とする。ステージIか(Yes)、ステージII以上か(No)の問題になるので、二値変数に対する各種の統計解析手法を用いることができる。例えば、ロジスティック解析、サポートベクターマシン等の解析手法を挙げることができるが、これらに限定されない。
CCケモカイン受容体/リガンド発現量が参照値未満の患者についての予後予測
 参照値以下の患者については、フロント発現量が高いほど予後が悪いという予測が可能である。上記具体例1の方法で参照値を定めると、FNT-H群とFNT-L群とを分けるカットオフ値となるフロント参照値も得ることができる。以下、フロント参照値との区別のため、CCケモカイン受容体/リガンド発現量の参照値をCC参照値という。患者のCCケモカイン受容体/リガンド発現量がCC参照値以下であった場合、患者のフロント発現量がこのフロント参照値未満であれば予後が良好である可能性が高く、また参照値以上であれば予後が不良である可能性が高いと予測することができる。もっとも、フロント発現量が高いほど予後が悪いという傾向に変わりはない。
 CC参照値未満の患者について、予後良好因子としてのCCケモカイン受容体/リガンドの発現量も考慮してより精度の良い予測を行ないたい場合、例えば下記の式1を用いてリスク指標hを求めてもよい。log値は、底が10の対数値である。

 βCCは、既知の患者集団について、予後データに対しCCケモカイン受容体/リガンド遺伝子の発現量のlog値を変量としたコックス比例ハザードモデルを適用して求められた係数、
 βFROUNTは、既知の患者集団について、予後データに対しフロント遺伝子の発現量のlog値を変量としたコックス比例ハザードモデルを適用して求められた係数
である。
 h≧0のとき、当該患者は予後不良(予後不良のリスク大)と予測され、h<0のとき、当該患者は予後良好(予後不良のリスク小)と予測される。また、例えばh = 1.5であればリスクは4.5倍(exp(1.5) = 4.48)というように評価することで、患者や医療者のさまざまな価値観を反映させることも可能である。なお、上記のように評価した場合、リスクが2倍になるのはh = 0.7(ln(2) = 0.69)、リスクが3倍になるのはh = 1.1である。
 式1において、発現量は、いずれも、Gapdh遺伝子等の標準遺伝子に対してノーマライズしたlog gene expression levelを用いる。[CC]m及び[FROUNT]mは、平均値でも中央値でもいずれでもよい。
 係数βCC及びβFROUNTは、既知の患者集団の全症例を対象として求めてもよいし、CCケモカイン受容体/リガンド遺伝子の発現量がCC参照値未満の症例を対象として求めてもよい。がん患者の予後予測においては、上記の他、既知の患者集団のうちステージII以上の症例を対象として係数を求めることも可能である。下記実施例には、68名の肺がん患者集団のうちステージII以上の症例(40症例)に対してコックス比例ハザードモデルを適用し、各係数を求めた具体例(式1-1~1-4)が記載されている。もっとも、下記式1-1~1-4で採用されている係数及び中央値は一例であり、より多くの患者集団のデータを集めてさらに望ましい係数及び中央値(又は平均値)を求めることができるので、本発明の範囲はこれらの具体的数値に限定されるものではない。コックス比例ハザードモデルを用いる場合に使用可能な統計処理ソフトは各種のものが公知であり、いずれを用いてもよい。当業者であれば、適当な患者集団のデータ及び適当な統計処理ソフトウェアを用いて、式1中の係数及び中央値(又は平均値)を求めることができる。
 式1は、予測対象の患者がCC参照値未満であった場合に適用可能であるが、予測対象ががん患者の場合、CC参照値未満で且つステージII以上の患者についての予測精度が特に高いと期待される。
 本発明の方法で予後の予測可能ながん及び炎症性疾患の種類は特に限定されず、フロント、CCR2、CCR5、CCL2及びCCL5の少なくともいずれかの関与が知られている様々ながん及び炎症性疾患が包含される。CCR2、CCR5、CCL2及びCCL5の少なくともいずれかが関与するがん及び炎症性疾患は、フロントが関与しているがん及び炎症性疾患でもある。対象となるがんには、固形がん及び液性がんの両者が包含される。また、原発性がん及び転移性がんの両者が包含される。
 がんの具体例として、肺がん、メラノーマ、胃がん、大腸がん、乳がん、肝臓がん、膵臓がん、子宮がん、食道がん、前立腺がん、悪性リンパ種、白血病などを挙げることができるが、これらに限定されない。例えば、CCR2が関与するがんとしては、メラノーマ、乳がん、前立腺がん、肺がん、骨髄腫、脳腫瘍などが知られており、CCR5が関与するがんとしては、乳がん、前立腺がん、肺がん、膵臓がん、骨髄腫などが知られている(Scholten DJ, et al., Br J Pharmacol, 165:1617-1643, 2012)。さらに、臨床試験に進んでいるケモカイン分野の抗がん剤としては、CCL2を標的とする転移性去勢抵抗性前立腺がんに対する抗がん剤、CCL5を標的とする非小細胞肺がんに対する抗がん剤、CCR2を標的とする転移性がんに対する抗がん剤、CCR5を標的とする進行大腸がんに対する抗がん剤などがある(がん基盤生物学-革新的シーズ育成に向けて-, 南山堂, 2013年, p.130-136)。これらのがんは、本発明で予後予測の対象となる好ましい具体例である。
 対象となる炎症性疾患は、典型的には慢性炎症性疾患である。炎症性疾患の具体例としては、関節リウマチ、線維症、腹膜炎、多発性硬化症、動脈硬化症、糖尿病、喘息、アルツハイマー、乾癬、アトピー等を挙げることができるが、これらに限定されない。CCR2、CCR5、CCL2及びCCL5の少なくともいずれかの関与が知られている炎症性疾患としては、動脈硬化症、多発性硬化症、関節リウマチ、乾癬、2型糖尿病、炎症性腸疾患、慢性肝炎、腎炎、移植片対宿主病、慢性閉塞性肺疾患、喘息、後天性免疫不全症候群などが挙げられる(Scholten DJ, et al., Br J Pharmacol, 165:1617-1643, 2012; 臨床免疫・アレルギー科, 59(3):386-391, 2013)。その他、CCR2、CCR5、CCL2及びCCL5の少なくともいずれか、又はフロントが関与する炎症性疾患として、肺線維症及び肝線維症等の各種の線維化疾患、腹膜炎、アレルギー性気道過敏症なども挙げられる(日本臨牀70巻, 増刊号8, 365-371, 2012; 及び下記実施例)。これらの炎症性疾患は、本発明で予後予測の対象となる好ましい具体例である。
 上記の通り、従来より、がんや炎症性疾患の治療目的で、各種のケモカイン阻害剤が研究、開発されており、受容体CCR2及びCCR5並びにこれらのリガンドCCL2及びCCL5を標的とした阻害剤もがんや炎症性疾患のための医薬として開発されている。また、新たな創薬標的分子であるフロントの阻害剤の開発も本願発明者らのグループにより進められている。このようなケモカイン阻害剤又はフロント阻害剤を有効成分とする医薬は、フロント遺伝子の発現量が高い患者において特に効果が高いと期待される。すなわち、CCR2、CCR5、CCL2及びCCL5より選択される少なくとも1種を阻害するケモカイン阻害剤やフロント阻害剤を有効成分とする医薬の投与が検討される患者(典型的にはがん患者又は炎症性疾患患者)について、該患者から分離された試料におけるフロント遺伝子の発現量を測定することで、上記阻害剤を有効成分とする医薬の有効性を予測することができる。フロント遺伝子の発現量が高い患者ほど、上記医薬の効果が高いと予測することができる。当該方法により、上記阻害剤を有効成分とする医薬の有効性が高い患者を選別することができる。
 フロントの発現がみられない、又はごく低い患者においては、フロント阻害剤やCCケモカイン受容体/リガンドの阻害剤を投与しても、これらの阻害剤の効果は十分に得ることができないと考えられる。一方、フロント遺伝子の発現が検出される患者は、上記した阻害剤を有効成分とする医薬の投与により、症状が大幅に改善されると期待されるので、そのような医薬の投与が特に望まれる。すなわち、患者由来の試料におけるフロント遺伝子の発現を調べることで、上記阻害剤を有効成分とする医薬の投与が望まれる患者(典型的にはがん患者又は炎症性疾患患者)を選別することも可能である。
 予後予測マーカーとしてのフロント遺伝子の発現量を測定可能なプライマーセット、プローブ、又は抗体、並びに、予後予測マーカーとしてのCCケモカイン受容体/リガンド遺伝子の発現を測定可能なプライマーセット、プローブ、又は抗体は、適宜他の試薬類及び取扱説明書等と組み合わせて、予後予測用キットとして提供することができる。当該キットは、フロント発現測定用のプライマーセット、プローブ又は抗体と、CCケモカイン受容体/リガンド発現測定用のプライマーセット、プローブ又は抗体のうち、いずれか一方を含んでいてもよいし、両者を含んでいてもよい。免疫組織染色のためのキットにおいては、フロントの発現が低い例と高い例を含めたサンプル染色画像も併せて提供してよい。
 以下、本発明を実施例に基づきより具体的に説明する。もっとも、本発明は下記実施例に限定されるものではない。なお、全ての動物実験は東京大学の動物実験委員会のガイドラインに従って実施した。
1. フロント欠損マウスではがん増生及び転移が低下する
 腫瘍微小環境におけるフロントの役割を調べるため、cre/loxPシステムを用いてフロントをノックアウトしたマウスを作出した。フロントは完全欠損で胎生致死となるため、タモキシフェン依存的に組み換え反応が誘導されるシステムを用いてコンディショナルなノックアウトを行なった。
 フロント遺伝子のエクソン15~19を含むゲノム領域をLoxPで挟み込んだターゲティングベクターをマウスに導入し、ヘテロのFNTfloxマウスを交配してホモのFNTflox/floxマウスを作出した。次いで、Creリコンビナーゼと変異エストロゲン受容体の融合タンパク質Cre-ERが導入されたB6.Cg-Tg (CAG-cre/Esr1*)5Amc/Jマウス(Jaxon Laboratory)とFNTflox/floxマウスを交配し、タモキシフェン誘導性のフロントコンディショナルノックアウトマウスFNT-cKOを得た。FNT-cKOマウスをタモキシフェン処理することで、ゲノムDNA及びmRNAいずれにおいてもフロントの欠失が誘導されること、タモキシフェン処理後のFNT-cKOマウスではフロントmRNAの発現が半分以下に抑制されることを確認した。
 FNT-cKOマウスを実験に用いる際は、実験の6日前又は14日前より、クエン酸タモキシフェン(和光純薬)を0.4mg/1gCE-2の量で添加したCE-2粉末飼料(日本クレア)を8~16週齢のFNTflox/floxマウス及びFNT-cKOマウスに与えることでCreの発現を誘導し、サザンブロッティングにより組換えを確認した。
(1) FNT-cKOマウスにおけるがん増生の低下
 5×105個のB16メラノーマ細胞をFNTflox/floxマウス及びFNT-cKOマウスの右側腹部に移植し、その後週2回、腫瘍サイズをノギスで測定し、腫瘍体積を算出した。腫瘍体積は下記の式により算出した。
腫瘍体積=(腫瘍短径)2×腫瘍長径/2
 その結果、非ノックアウト(FNTflox/flox)マウスに比べてFNT-cKOマウスでは腫瘍の増殖が有意に抑制され、また生存率も向上することが明らかとなった(図1a, b)。
(2) FNT-cKOマウスにおけるがん転移の低下
 1×106個のB16F10メラノーマ細胞を200μLのPBSに懸濁し、FNTflox/floxマウス及びFNT-cKOマウスに尾静脈より投与した。投与8日目にマウスを安楽死させ、左心室からPBSを灌流させた後に肺を分離した。左葉中の転移結節の数及びサイズを目視で確認した。
 その結果、転移結節の数もサイズもFNT-cKOマウスで有意に低下することが明らかとなった(図2a, b)。
2. フロントはマクロファージに高発現している
[方法]
フロント-gfp-ノックインマウスの作出
 常法により、マウスのゲノム上のフロントプロモーターの下流にGFP遺伝子を組み込み、フロント-gfpノックインマウスを作出した。
フローサイトメトリー
 2 mLの4%チオグリコレートを腹腔内投与して腹膜炎を誘導したマウスより、腹膜の浸潤細胞を採取した。2%ウシ胎仔血清を添加したPBSにて細胞を洗浄、再懸濁し、70μmのストレイナーで濾過した。抗マウスCD16/32抗体(BD biosciences)でインキュベーションすることでFcレセプターをブロッキングした後、蛍光標識抗体で細胞を染色した。抗マウスCD11b-Pacific Blue、抗マウスLy6C-APC-Cy7、抗マウスLy6G-Alexa Fluor 700、抗マウスCD4-FITC及び抗マウスB220-PE-Cy7はBiolegendより購入した。抗マウスCD8-Pacific BlueはBD biosciencesより購入した。
[結果]
 フロント-GFPノックインマウス由来の細胞のフローサイトメトリー解析によると、腹膜炎モデルの炎症部位にリクルートされた免疫細胞の中でも特にマクロファージにおいてフロントが高発現していた(図3)。CCL2で動員された細胞集団ではフロント高発現の単球/マクロファージが濃縮されており、その受容体であるCCR2を発現している細胞においてフロントが高発現していることが示された(データ省略)。
3. フロント欠損マウスでは腫瘍部位へのマクロファージ集積が低下する
[方法]
フローサイトメトリー
 上記1(2)に記載したFNT-cKOマウス及びFNTflox/floxマウスの肺転移モデルの肺細胞を、右下葉よりコラゲナーゼ及びDNaseで消化させて得た。2%ウシ胎仔血清を添加したPBSにて細胞を洗浄、再懸濁し、70μmのストレイナーで濾過した。抗マウスCD16/32抗体(BD biosciences)でインキュベーションすることでFcレセプターをブロッキングした後、蛍光標識抗体で細胞を染色した。抗マウスCD11b-Pacific Blue、抗マウスLy6C-APC-Cy7、抗マウスLy6G-Alexa Fluor 700はBiolegendより購入した。染色した細胞は、Galliosフローサイトメーター(Beckman coulter)を用いて解析した。
免疫組織染色
 マウスをPBSで灌流し、左肺を分離した。気管からOptimal Cutting Temperatureコンパウンド(OCT)(サクラファインテック)を注入し、OCT中に包埋して液体窒素で凍結させた。8μm厚の新鮮凍結切片を調製し、4%パラホルムアルデヒド-PBSで固定した。0.05% Tween 20-PBSで洗浄後、切片をBlocking One試薬(ナカライテスク)でブロッキングし、抗マウスF4/80抗体(BioLegend)及びAlexa Fluor 594 抗ラットIgG (Life technologies)で順次染色した。SP5共焦点顕微鏡(Leica Microsystems)により蛍光像を取得した。
[結果]
 フロントを欠損したFNT-cKOマウスでは、フロント非欠損のFNTflox/floxマウスと比較してがん転移結節へのマクロファージの集積が低下していた(図4)。
4. フロント欠損マウスでは炎症部位へのマクロファージ浸潤が抑制される
[方法]
in vivo走化性アッセイ
 FNTflox/floxマウス及びFNT-cKOマウスに4%チオグリコレート培地(Difco社)2mLを腹腔内投与することで腹膜炎を誘導させた。腹腔中に5mLの氷冷PBSを注入して穏やかにマッサージすることで、腹膜の浸潤細胞を収集した。収集した細胞集団を、0.1%FBSを含むPBSで洗浄し、フローサイトメトリー解析に付して細胞数及び細胞ポピュレーションを調べた。
[結果]
 結果を図5に示す。フロント欠損マウスではマクロファージ数が低下しており、腹膜炎症部位へのマクロファージの浸潤が低下することが確認された(図5上段)。好中球数はフロント欠損マウスと非欠損マウスの間で差異がなく、フロント欠損は好中球の浸潤には影響しないことが確認された(図5下段)。
5. 肺腺癌患者におけるフロント発現量並びにCCR2、CCR5、CCL2及びCCL5発現量と術後予後の関連
<方法>
患者
 千葉県がんセンターにおいて1997~2004年の間に肺腺癌に対する肺切除術(肺葉の完全切除、又は門部と縦隔リンパ節の切開を伴う区域切除)を受けた患者120名(男性患者52名、女性患者68名)を対象とし、フロント発現量並びにCCR2、CCR5、CCL2及びCCL5発現量と術後予後の関連を調べた。本研究は千葉県がんセンターの施設内審査委員会及び倫理委員会に承認された。全ての患者から書面によるインフォームドコンセントを得た。患者の臨床的特徴を下記表1に示す。

組織試料
 全ての組織サンプルは、採取後直ちに液体窒素で凍結し、使用時まで-80℃で保存した。がんの進行度(ステージ)はTNM(tumor node metastasis)分類システムを用いて評価した。サンプルは千葉県がんセンターの病理学者2名により評価し、肺病理学専門の病理学者1名が確証した。
リアルタイムPCRによるmRNAレベルの定量
 手術検体の癌病変部(癌細胞を含む微小環境)より、RNA-Bee試薬(Tel-Test社)又はTRIzol試薬(Invitrogen社)を製造者の指示書に従い使用して全RNAを抽出した。SuperScript synthesis kit(Life Technologies社)を用いて1μgの全RNAよりcDNAを調製した。SYBR Green PCR Master Mix(Qiagen社)を用いてcDNAを増幅し、リアルタイム定量RT-PCRを行なった。フォワードプライマー及びリバースプライマーはそれぞれ終濃度0.5μM、20μLの反応系とした。ABI 7500 real-time PCR system(Life Technologies社)を製造者の指示書に従い使用し、2回反復してアッセイを行なった。結果はGapdh遺伝子のRNA発現レベルに対してノーマライズした。使用したプライマーを下記表2に示す。

 リアルタイムPCRにより発現量を定量解析し、フロント及びケモカイン受容体/リガンドの発現量(log gene expression level)を下記式により求めた。

臨床データの解析
 CCR2、CCR5、CCL2及びCCL5の発現量と予後との関連については、遺伝子発現量の中央値をカットオフ値として、120名の患者を各遺伝子につき高発現群と低発現群に分けて予後との関連を評価した。有意性の検定はカイ二乗検定により行なった。術後生存率はカプラン-マイヤー法(マンテル-コックス検定)により解析し、群間の差はログランク検定により評価した。統計学的計算はPrism 6(GraphPad Software社)を用いて行なった。p<0.05を有意差ありとした。
 フロント遺伝子及びCCR2遺伝子の発現量と術後生存期間との関連は、多変量Cox比例ハザード解析で評価した。計算は、オープンソースの統計解析環境R(R Core Team (2013). R: A language and environment for statistical computing. R Foundation for Statistical Computing, Vienna, Austria. <URL http://www.R-project.org/>.)のsurvivalパッケージ("survival" package in R version 3.0.1., Therneau T (2014). _A Package for Survival Analysis in S_. R package version 2.37-7, <URL http://CRAN.R-project.org/package=survival>.)を用いて行なった。結果はhazard ratio (95% confidence interval)で示した。術後生存率は、上記と同様にカプラン-マイヤー法(マンテル-コックス検定)により解析し、群間の差はログランク検定により評価した。p<0.05を有意差ありとした。再帰分割分析はRのmvpartパッケージ("mvpart" package in R version 3.0.1., rpart by Terry M Therneau, Beth Atkinson. (2014). mvpart: Multivariate partitioning. R package version 1.6-2. <URL http://CRAN.R-project.org/package=mvpart>)によって行なった。
<結果>
CCR2, CCR5, CCL2及びCCL5のmRNAの高発現は肺腺癌の術後経過の有意な予後良好因子である
 ケモカインリガンド及び受容体(CCR2、CCR5、CCL2、CCL5)の発現状態の予後的意義を評価するため、各遺伝子のmRNA発現量の中央値をカットオフ値として、120名の患者集団を高発現群及び低発現群に分けた。CCR2、CCR5、CCL2及びCCL5いずれも、高発現群では低発現群よりも有意に予後良好であった(図6A, B, C, D)。CCR2-highかつCCL2-highの患者群、及びCCR5-highかつCCL5-highの患者群では、特に予後が良好であった(図7A, B)。
フロント発現は予後増悪因子である
 女性肺がん患者68名の手術検体を用いたフロントおよびCCR2の遺伝子発現解析の結果、フロントおよびCCR2の発現の間には正の相間があった(Fig. 3a, r = 0.51, p < 0.01)。フロントおよびCCR2の発現量と予後の関連を多変量コックス比例ハザード分析により評価した結果、フロントおよびCCR2の発現量に対するハザード比はそれぞれ7.8(1.8-34, p < 0.01)、0.21(0.088-0.49, p < 0.01)であった(表3)。すなわち、フロントの高発現は有意に高い死亡リスクと関連しており、CCR2の高発現は有意に低い死亡リスクと関連していた。言い換えると、CCR2は予後良好因子(発現が10倍でリスクは0.21倍)であり、フロントは予後増悪因子(発現が10倍でリスクは7.8倍)であることが明らかとなった。

 再帰分割アルゴリズムにより患者をサブグループに分けたところ、フロント低発現(FNT-L)、フロント高発現(FNT-H)、CCR2高発現(CCR2-H)患者の3グループに分割された(図8)。このフロント低発現(FNT-L)とフロント高発現(FNT-H)の間では手術の時点では臨床ステージに差が確認できないにもかかわらず、Kaplan-Meier及びログランク検定にてフロント低発現(FNT-L)は高発現(FNT-H)患者と比較し予後が良好であることがわかった(図9、図10)。そしてこの結果はコックス比例ハザード分析の結果と一致する結果であり、患者においてもフロントは予後増悪因子として機能していることが明らかとなった。
 フロント及びCCR5の発現と術後予後との関連を評価しても、CCR2と同様の結果であった(データ省略)。
CCケモカイン受容体/リガンド発現量の閾値の検討
 68症例のうちのステージI症例(28症例)に対し、再帰分割法を適用してCCケモカイン受容体/リガンド発現量の閾値(参照値)を求めたところ、CCR2については1.42(すなわち、CCR2のlog gene expression level>1.42で予後良好)、CCR5については2.23、CCL5については1.34の値が得られた。これらの値の±0.15以内の値、例えば±0.10以内の値、又は±0.05以内の値を、様々ながん及び炎症性疾患の患者、好ましくはがん患者に対し、各遺伝子の発現量の閾値として用いることができる。
予後予測式の検討
 68名の肺がん患者集団のうちステージII以上の症例(40症例)に対してコックス比例ハザードモデルを適用し、上述の式1における各遺伝子発現量に対する係数を求めた。[CC]m及び[FROUNT]mは40症例における中央値を採用した。その結果、4種のCCケモカイン受容体/リガンド遺伝子について、それぞれ下記の式が得られた。

 CCR2遺伝子とフロント遺伝子の発現量により予後を予測する場合には式1-1を、CCR5遺伝子とフロント遺伝子の発現量により予後を予測する場合には式1-2を、CCL2遺伝子とフロント遺伝子の発現量により予後を予測する場合には式1-3を、CCL5遺伝子とフロント遺伝子の発現量により予後を予測する場合には式1-4を用いることができる。これらの式は、例えば、ステージII以上の症例、及びステージIかつ[CC]が参照値未満の症例に好ましく適用することができる。
まとめ
 以上の解析結果より、フロントはがんおよび炎症性疾患の発症/増悪における鍵分子であることが明らかとなった。フロント発現の定量解析はがんおよび炎症性疾患の診断、モニタリングおよび予後予測に有用な新規技術である。CCケモカイン受容体/リガンド遺伝子では、4種の中でもCCR2、CCR5及びCCL5が、特にCCR2及びCCR5が、予後予測因子としてとりわけ好ましく利用可能であると考えられる。
6. 免疫組織染色によるフロント発現マクロファージの検出
[方法]
フロント抗体の作出
 フロントタンパク質(配列番号2)の第23番~第141番残基に対応する領域を組換えタンパク質として大腸菌で発現し、この組換えフロントタンパク質を常法により精製してウサギへ複数回免疫した。免疫したウサギ及び未免疫ウサギの血清からそれぞれIgGを常法により精製し、抗フロントポリクローナル抗体及びコントロール抗体を得た。
免疫組織染色
 肺がん患者より、腫瘍が存在する肺葉を完全切除し、肺門リンパ節と縦隔リンパ節を摘出した。手術検体は10%ホルマリン中で固定し、パラフィン包埋して4μm厚切片を作製した。上記の通り作出したウサギ抗フロントポリクローナル抗体及びDAKO Envision FLEX/HRPを使用して切片の免疫染色を行なった後、核の染色を行なった。
[結果]
 免疫組織染色像の一例を図11に示す。フロント陽性の細胞は、がん細胞部位の周辺に多数、褐色に染色されて検出された。フロントの発現量は染色強度や染色細胞数に基づいて評価できるので、手術検体の免疫組織染色による患者の予後予測も可能である。

  1.  がん又は炎症性疾患の患者の予後を予測する方法であって、前記患者から採取された試料におけるフロント遺伝子の発現量を測定することを含み、フロント遺伝子の発現量が低いほど、前記患者の予後が良好であると予測される、方法。
  2.  がん又は炎症性疾患の患者の予後を予測する方法であって、前記患者から採取された試料におけるCCケモカイン受容体/リガンド遺伝子の発現量を測定することを含み、CCケモカイン受容体/リガンド遺伝子の発現量が高いほど、前記患者の予後が良好であると予測され、前記CCケモカイン受容体/リガンド遺伝子は、CCR2遺伝子、CCR5遺伝子、CCL2遺伝子及びCCL5遺伝子からなる群より選択される少なくとも1種である、方法。
  3.  前記CCケモカイン受容体/リガンド遺伝子として、CCR2遺伝子及びCCL2遺伝子の発現、又はCCR5遺伝子及びCCL5遺伝子の発現を調べる、請求項2記載の方法。
  4.  発現量の測定が免疫組織染色により行なわれる、請求項1ないし3のいずれか1項に記載の方法。
