Abstract

The subject of the invention is a fixation plate for the treatment of bone fractures, especially for connecting broken pieces of bones, which consists of an elongated carrying body with an internal side touching the bone and an external side opposite to it and openings positioned in the carrying body in an arranged way between the external side and the internal side ofthe carrying body, where at least one of the openings is a clamp opening, the first free size of the clamp opening falling in the direction of the longitudinal axis of the carrying body is larger than the second free size of the same opening perpendicular to the longitudinal axis, and the bounding surface of the clamp opening has a coupling unit for connecting the linking element. The characteristic feature of the invention is that it the coupling unit (50) is constructed from at least two rows of curved ridges (51 52) that are separated from each other but cooperate with each other, one of the rows of curved ridges (51) that are separated from each other but cooperate with each other, one of the rows of curved ridges (51) is situated in one half-zone of the bounding surface (43c) of the clamp opening (43) divided into two by the longitudinal aixs (41a), and the other row of curved ridges (52) is situated in the other half-zone of the bonding surface (43c) of the clamp opening (43) divided into two by the longitudinal axis (41a).

Description

Fixation plate for the treatment of bone fractures
The subject of the invention is a fixation plate for the treatment of bone fractures, especially for connecting broken pieces of bones, which consists of an elongated carrying body with an internal side touching the bone and an external side opposite to it and openings positioned in the carrying body in an arranged way between the external side and the internal side of the carrying body, where at least one of the openings is a clamp opening, the first free size ofthe clamp opening falling in the direction ofthe longitudinal axis of the carrying body is larger than the second free size of the same opening perpendicular to the longitudinal axis, and the bounding surface ofthe clamp opening has a coupling unit for connecting the linking element.
In the case of limb fractures, especially in the case of the fractures of arm and leg bones near joints, fixation plates are widely used for the stable immobilisation of broken pieces of bones in relation to each other. The essence of this is that in an elongated plate several openings of specific geometry are made, which are suitable for inserting bone screws with the help of which the pieces of bones are connected to the fixation plate. Such a solution is described among others in patent descriptions US 4.493.317, US 5.053.036, US 5.151.103 and EP 318.762.
These solutions are suitable for fixing pieces of bones onto the fixation plate and with the help of specially formed openings they are also suitable for compressing the fitting pieces of broken bone ends against each other to a certain determined extent. This compression promotes the union ofthe bones.
However, the disadvantage of the solutions mentioned above is that in the case of uniting pieces of bones the fixation plate being pressed against the surface ofthe bone impairs the blood supply of the cortical layer below, which inhibits the healing process and in an extreme case it may even cause further complications.
They tried to overcome this problem by giving a part of the openings of he fixation plate the shape of keyhole and creating a thread in the larger curve of the envelope surface bordering the opening in an angular domain exceeding the centre angle of 180°. The head ofthe bone fixing screw was made in a special way matching this thread.
With this fixation plate and bone screw it becomes possible - maintaining appropriate angle stability - to remove the plate from the surface ofthe pieces of bones and create an internal fixation unit. Publication document No. WO 00/53110 describes an example of this construction.
However, the disadvantage ofthe construction is that the keyhole shaped opening does not enable bi-directional compression, that is the operative solution that makes possible to compress broken bone ends against each other to the required extent during the restoration process with the help ofthe fixation plate and bone screws.
It also must be regarded as a disadvantage that the application of such plates with a stable angle, made with a keyhole-like opening containing a thread increases the number of instruments needed for the operation, and so it makes the operation more difficult and time consuming, which results in placing an undesirably greater load on the operative surgeon and the patient.
Our aim with the invention was to overcome the deficiencies ofthe known versions and by keeping the advantages to create a solution, which is suitable both for the compression fixation of broken pieces of bones and their fixation with a stable angle, which is simple to use and the construction of which does not influence either the traditional size selection nor the ordinary operative technique.
The idea behind the invention was based on the recognition that if the thread for the necessary fixation of the bone screw used as a connecting element is placed in the opening - the shape of which is also appropriately changed - in a different way than usual, then two independent pieces of threads are created, which are arranged in a novel way and are cooperating with each other, so that the task can be solved. Our experience has shown that in the case of thread segments to be in a suitable position the same fixation plate can be used for both compression fixation and fixation with a stable angle.
In accordance with the set aim the fixation plate according to the invention for the treatment of bone fractures, especially for connecting broken pieces of bones, - which consists of an elongated carrying body with an internal side touching the bone and an external side opposite to it and openings positioned in the carrying body in an arranged way between the external side and the internal side ofthe carrying body, where at least one of the openings is a clamp opening, the first free size of the clamp opening falling in the direction of the longitudinal axis ofthe carrying body is larger than the second free size of the same opening perpendicular to the longitudinal axis, and the bounding surface of the clamp opening has a coupling unit for connecting the linking element, - is constructed in a way that the coupling unit is composed from at least two rows of curved ridges that are separated from each other but cooperate with each other, one of the rows of curved ridges is situated in one half-zone ofthe bounding surface of the clamp opening divided into two by the longitudinal axis, and the other row of curved ridges is situated in the other half- zone of the bounding surface of the clamp opening divided into two by the longitudinal axis.
A further criterion of the fixation plate according to the invention may be that the one row of curved ridges and the other row of curved ridges are separated from each other on the one side by a separating zone belonging to the bounding surface of the clamp opening and on the other side by another separating zone belonging to the bounding surface of the clamp opening.
In a different construction of the fixation plate the covering surface of the one row of. curved ridges of the coupling unit occupied from the bounding surface of the clamp opening and the covering surface of the other row of curved ridges of the coupling unit occupied from the bounding surface of the clamp opening is positioned symmetrically to the longitudinal axis ofthe carrying body. In a further different construction of the invention the one row of curved ridges of the coupling unit is a conical metric double thread-piece and/or the other row of curved ridges ofthe coupling unit is a conical metric double thread-piece.
The most important advantage of the fixation plate according to the invention is that due to its novel construction it is suitable for both compression and stable-angle binding of broken pieces of bones.
A further advantage is that although its construction is different from the known constructions it is suits the size selection of traditional compression fixation plates, and its application is also the same as those ofthe traditional operative techniques.
An advantage deriving from the above is that its use does not require new auxiliary operating equipment, so the operative time does not increase, which has a favourable effect on the loading of the operating surgeon and the condition and recovery period of the patient.
In the case when the axis of the coupling units and the axis of the opening are collinear, there is a further advantage, since the operating surgeon can change over to stable-angle screwing even after placing in a traditional bone screw. It must be pointed out that as compared to the known solutions this possibility is completely unique, which increases the possibilities of the operating surgeon even further, and contributes to performing optimal surgical interventions and to an even more complete recovery of patients even in the case of serious splintered fractures.
In the following the invention is presented in detail in connection with construction examples on the basis of drawings. On the drawing
Figure 1 shows a version of the fixation plate according to the invention in side view in partial cross-section, Figure 2 is a view of figure 1 in the direction II, Figure 3 is a cross-section of figure 1 along the plane III-III, Figure 4 is another version ofthe fixation plate in top view, Figure 5 is a cross-section of figure 4 along the plane N-N.
Figures 1, 2 and 3 show a self-compression version ofthe fixation plate 40 according to the invention of a lightened construction. It can be observed that the fixation plate connects the one piece of bone 10 and the other piece of bone 20 to each other with the help of linking elements 30, in the present case with bone screws, in a way known in itself. The carrying body 41 ofthe fixation plate 40 has an elongated shape, in its cross-section it has a slightly curved form in the complete length of the longitudinal axis 41a of the carrying body 41. The internal side 41b ofthe carrying body 41 facing the one piece of bone 10 and the other piece of bone 20 is concave from the direction ofthe one piece of bone 10, similarly to the external side 41c further away from the one piece of bone 10.
The openings 42 through which the linking units 30 can be driven are positioned along the longitudinal axis 41a of the carrying body 41 in an arranged way. So the openings 42 run through the complete material width of the carrying body 41 connecting the internal side 41b and the external side 41c in this way. One or more of the openings 42, all of them in the present case, are clamp openings 43. The first free size 43 a of the clamp opening 43 falling in the longitudinal axis 41a of the carrying body 41 is larger than the second free size 43b perpendicular to the longitudinal axis 41a, that is the clamp opening 43 has an oval shape elongated along the longitudinal axis 41a ofthe carrying body 41. However, the bounding surface 43 c ofthe oval clamp opening 43 is reshaped in accordance with the aim, in the case ofthe present construction with symmetrically positioned conical countersinks.
The coupling unit 50 is situated in the bounding surface 43c of the clamp opening 43, and it consists of a row of curved ridges 51 and another row of curved ridges 52. The covering surface of the one row of curved ridges 52 and the other row of curved ridges 52 - in the present construction - is in a symmetrical position to the longitudinal axis 41a of the carrying body 41. Here the one row of curved ridges 51 and the other row of curved ridges 52 is a metric double thread-piece each. The one row of curved ridges 51 and the other row of curved ridges 52 cooperate with each other, and in the present case they together, each form a section of one single virtual metric double female thread. Between the one row of curved ridges 51 and the other row of curved ridges 52 there is a separating zone 44 and another separating zone 45, which completely separate the two constituents ofthe coupling unit 50.
Figures 4 and 5 show another - not lightened - self-compression version of the fixation plate 40 according to the invention. This construction also has openings 42 positioned along the longitudinal axis 41 a of the carrying body 41 , and these openings 42 also connect the internal side 41b and external side 41c ofthe carrying body 41. In this construction the openings 42 are created as clamp openings 43 also. In this case the first free size 43 a ofthe clamp opening 43 is larger than the second free size 43b, so these clamp openings 43 are also oval shaped. However, the bounding surface 43 a of the clamp opening 43 is formed differently than in figure 2. Obviously the coupling unit 50 consisting of the one row of curved ridges 51 and the other row of curved ridges 52 can be found in the bounding surface 43 c of this clamp opening 43 too, and in this case it is a symmetrically positioned part of a metric double conical female thread with respect to its two covering surfaces also. Also in this version too the one row of curved ridges 51 and the other row of curved ridges 52 are separated from each other by the one separating zone 44 and the other separating zone 45.
In the case of the use of the fixation plate 40 according to the invention the fixation plate
40 is placed on the appropriately restored one piece of bone 10 and other piece of bone 20, and then the linking elements 30 are driven through the openings 42 of the carrying body
41 of the fixation plate 40, which linking elements 30 are screwed in the one piece of bone 10 and the other piece of bone 20 in a way known in itself.
The fixation plate according to the invention can be favourably used for binding together the broken parts of different bones, especially limb bones. List of references

Claims

1. Fixation plate according to the invention for the treatment of bone fractures, especially for connecting broken pieces of bones, which consists of an elongated carrying body (41) with an internal side (41b) touching the bone (10, 20) and an external side (41c) opposite to it and openings (42) positioned in the carrying body (41) in an arranged way between the external side (41c) and the internal side (41b) ofthe carrying body (41), where at least one of the openings (42) is a clamp opening (43), the first free size (43 a) of the clamp opening (43) falling in the direction of the longitudinal axis (41a) of the carrying body (41) is larger than the second free size (43b) of the clamping opening (43) perpendicular to the longitudinal axis (41a), and the bounding surface (43c) of the clamp opening (43) has a coupling unit (50) for connecting the linking element (30), characterised by that it the coupling unit (50) is constructed from at least two rows of curved ridges (51, 52) that are separated from each other but cooperate with each other, one of the rows of curved ridges (51) is situated in one half-zone of the bounding surface (43c) of the clamp opening (43) divided into two by the longitudinal axis (41a), and the other row of curved ridges (52) is situated in the other half-zone of the bounding surface (43 c) ofthe clamp opening (43) divided into two by the longitudinal axis (41a).
2. Fixation plate as in claim 1, characterised by that the one row of curved ridges 51 and the other row of curved ridges 52 are separated from each other on the one side by a separating zone 44 belonging to the bounding surface 43 c ofthe clamp opening 43, and on the other side by another separating zone 45 belonging to the bounding surface 43 c of the clamp opening 43.

Fixation plate as in claim 1 or 2, characterised by that the covering surface ofthe one row of curved ridges 51 of the coupling unit 50 occupied from the bounding surface 43c ofthe clamp opening 43 and the covering surface ofthe other row of curved ridges 52 ofthe coupling unit 50 occupied from the bounding surface 43 c ofthe clamp opening 43 is positioned symmetrically to the longitudinal axis 41a ofthe carrying body 41.
4. Fixation plate as in any of claims 1-3, characterised by that one row of curved ridges 51 ofthe coupling unit 50 is a conical metric double thread-piece.
5. Fixation plate as in any of claims 1-4, characterised by that other row of curved ridges 52 ofthe coupling unit 50 is a conical metric double thread-piece.
