Abstract

En la presente invención se ha demostrado por primera vez el papel del receptor DC-SIGN (Dendritic cell-specific ICAM 3 grabbing non-integrin), presente en la superficie de las células dendríticas, en el proceso de unión del parásito Leishmania a células eucarióticas humanas, entre otras células dendríticas, macrófagos y células derivadas de monocitos. La invención consiste en un nuevo compuesto terapéutico frente a la infección por Leishmania capaz de bloquear o anular el proceso de unión de las formas infecciosas del parásito Leishmania con el receptor DC-SIGN, presente en células eucarióticas, entre otras, en células dendríticas humanas y ciertas poblaciones de macrófagos, y la subsecuente inhibición de internalización del parásito.

COMPUESTO TERAPÉUTICO FRENTE A LA INFECCIÓN POR Leishmania Y SU USO.
SECTOR DE LA TÉCNICA
La presente invención se encuadra dentro del sector farmacéutico y describe un compuesto terapéutico para uso frente a la infección por Leishmania, capaz de bloquear o anular la unión de las formas infecciosas de Leishmania con el receptor DC-SIGN presente en células eucarióticas.
ESTADO DE LA TÉCNICA
La leishrnaniasis es una enfermedad parasitaria transmitida por un vector con un amplio espectro de manifestaciones clínicas, desde lesiones cutáneas locales hasta formas viscerales de riesgo mortal, debido principalmente a las diferencias entre las especies de Leishmania y al estado inmunológico del hospedador.
Entre los eventos iniciales que tienen lugar en el sitio de inoculación de Leishmania se incluye la infección de células hospedadoras por parte de promastigotes metacíclicos seguido de su transformación intracelular en amastigote. Una vez liberados al medio extracelular, los amastigotes invaden nuevas células provocando la diseminación de la infección. Durante las primeras fases de este ciclo expansivo, están ausentes anticuerpos específicos, lo cual impide mecanismos opsonizantes.
El parásito Leishmania infecta fagocitos mononucleares, un factor clave de la respuesta inmune frente al parásito. Esta localización le permite al parásito subvertir desde dentro las funciones efectora y reguladora de estas células. Las células cutáneas presentadoras de antígeno (APC), específicamente las células epidérmicas de Langerhans (LC) y las células dendríticas (DC), están implicadas activamente en la vigilancia de su entorno (Banche eau, J., and R.M. Las células dendríticas expresan varios receptores de patrones moleculares asociados a patógenos (PAMP), incluyendo numerosas lectinas de tipo C y receptores tipo lectina (Figdor CG, V.K.T., Adema GJ. 2002. Teniendo en cuenta que la superficie de Leishmania spp. presenta abundantes glicoconjugados ricos en mañosa (Ilgoutz, S.C., and MJ. McConville. 2001. Int J Parasitol 31:899-908), se postuló como hipótesis razonable pensar que las interacciones lectina-oligosacáridos están implicadas en el reconocimiento del parásito.

DC-SIGN es el principal receptor de las células dendríticas involucrado en la infección por virus VIH-1 y facilita su transmisión a las células T. De este modo tanto el virus N3Η-1 como las células T en reposo presentan en su superficie un ligando con nivel de expresión elevado y que interacrúa con las células dendríticas. El dominio activo del receptor DC-SIGN corresponde principalmente al dominio lectina de su molécula, y media las interacciones con el virus HIN-1 (mediante el reconocimiento de gpl20), virus del Ebola, células T (mediante la interacción con ICAM-3) y células endoteliales (mediante la interacción con ICAM-2).
Sin embargo, los receptores utilizados por amastigotes de Leishmania para adherirse e infectar macrófagos, la diana final, no están totalmente establecidos y, según nuestro conocimiento, no se han caracterizado todavía receptores específicos para Leishmania en DCs.

Descripción breve
La presente invención tiene como objetivo buscar soluciones para el problema de infecciones por el parásito Leishmania. Esta invención se basa en la observación por parte de los inventores de que las formas amastigotes infecciosas del parásito Leishmania pifanoi se unen específicamente y con gran afinidad al receptor DC-SIGN. El bloqueo de este tipo de receptor puede modular, y más específicamente reducir, la adhesión del parásito a las células eucarióticas, y de este modo puede influenciar la respuesta inmune. Un objeto de la presente invención lo constituye un compuesto terapéutico capaz de bloquear o anular la unión de formas infecciosas del parásito Leishmania al receptor DC-SIGN en células eucarióticas en mamíferos, preferentemente humanos, y que permite una inhibición de la unión al receptor e internalización del parásito por estas células y el subsecuente desarrollo de la respuesta inmune. Un objeto particular de la presente invención es un compuesto terapéutico de la presente invención que pertenece, entre otros, al siguiente grupo: a) anticuerpo anti DC-SIGN, b) mañano soluble, c) receptor DC-SIGN o el dominio lectina de DC-SIGN, recombinantes, y isoformas y proteínas homologas de los mismos, d) receptor recombinante presente en el parásito Leishmania responsable de la unión con el receptor DC-SIGN, y sus formas homologas.
Un objeto particular de la presente invención lo constituye el anticuerpo monoclonal anti-DC-SIGN, MR-1. Otro objeto de la presente invención lo constituye un procedimiento para la identificación y evaluación de compuestos capaces de bloquear o anular la unión de la forma infecciosa del parásito Leishmania al receptor DC-SIGN, caracterizado por los siguientes pasos: a) adición del compuesto candidato, b) determinación de la actividad bloqueadora del candidato mediante la valoración entre otros, de parámetros celulares: capacidad de unión o invasión in vitro en células K562-CD209 transfectadas con el receptor DC-SIGN, células dendríticas, células derivadas de monocitos (MDDC), u otras células que presentan en su superficie el receptor DC-SIGN, y competición en el proceso de inhibición de la unión con un anticuerpo específico anti-DC-SIGN o con mananos, c) identificación de un bloqueador de la unión de la forma infecciosa del parásito Leishmania al receptor DC-SIGN.
Un objeto adicional de la presente invención es un compuesto terapéutico de la presente invención identificado por el procedimiento anteriormente mencionado.
Un objeto adicional de la presente invención lo constituye una composición farmacéutica que contiene una cantidad terapéuticamente eficaz del compuesto terapéutico de la presente invención junto con al menos un excipiente farmacéuticamente aceptable.
Otro objeto adicional de la presente invención lo constituye el empleo de la composición farmacéutica en el tratamiento de infecciones provocadas por el parásito Leishmania en mamíferos, preferentemente en humanos. Otro objeto adicional de la presente invención lo constituye el empleo del receptor DC-SIGN como diana para el diseño de compuestos terapéuticos, capaces de bloquear o anular la unión de las formas infecciosas del parásito Leishmania al receptor DC-SIGN, o a cualquiera de sus isoformas o proteínas homologas.
Descripción detallada
Esta invención se basa en el descubrimiento que han hecho los inventores de que las formas amastigotes infecciosas del parásito Leishmania pifanoi se unen específicamente y con gran afinidad al receptor DC-SIGN, sugiriendo un papel de este receptor en el desarrollo de las respuestas inmunes frente a Leishmania. Esta es la primera vez que se describe la unión de un patógeno no viral, más específicamente un parásito, al receptor DC-SIGN.

Tal como se utiliza en esta invención, el término "receptor DC-SIGN" se refiere a una lectina de tipo C que actúa como receptor en las células dendríticas u otras células que lo presenten e identificado por la patente "Composition and Method for modulating dendritic cell-T cell mteraction" (WO0063251, 26 Octubre 2000, Universidad
Nijmegen).
Tal como se utiliza en esta invención, el término "inhibición de la unión al receptor" se refiere al bloqueo de la unión del parásito Leishmania al receptor DC-SIGN en al menos un 20%, preferentemente en al menos un 85%, ó más preferentemente en un 100%.
El termino "célula eucariótica", tal como se utiliza en la presente invención, se refiere a cualquier tipo de célula humana o animal que presente en su superficie el receptor DC-SIGN, entre otras, células dendríticas, macrófagos y células derivadas de monocitos. Tal como se utiliza en la presente invención, el término "compuesto terapéutico" se refiere a cualquier sustancia capaz de bloquear o anular la unión de las formas infecciosas de Leishmania al receptor DC-SIGN presente en células eucarióticas.
Un objeto particular de la presente invención es un compuesto terapéutico de la presente invención que pertenece, entre otros, al siguiente grupo: a) anticuerpo anti DC-SIGN, b) mañano soluble, c) receptor DC-SIGN o el dominio lectina de DC-SIGN, recombinantes, y isoformas y proteínas homologas de los mismos, d) receptor recombinante presente en el parásito Leishmania responsable de la unión con el receptor DC-SIGN, y sus formas homologas.
Como "mañano soluble", se refiere en esta invención a cualquier polisacárido cuyos azúcares componentes sean mayoritariamente residuos de mañosa, pudiendo presentar modificaciones adicionales y ramificaciones por otros residuos de azúcar, pero en los que la proporción de mañosa es mayoritaria (Haworth WN βt al. 1937. J. Chem.
Soc. Chem. Commun. 784).
Tal como se utiliza en esta invención, el término "receptor DC-SIGN recombinante" se refiere a la forma recombinante del receptor DC-SIGN, en una forma soluble, y producido en células eucarióticas o procarióticas, que tiene la capacidad de unirse al receptor presente en el parásito Leishmania, recubriendo este parásito y saturando los puntos de unión a las células eucarióticas portadoras del receptor DC- SIGN.
Tal como se utiliza en esta invención, el término "receptor recombinante presente en el parásito Leishmania" se refiere a la forma recombinante del receptor presente en el parásito Leishmania, en una forma soluble, y producido en células eucarióticas o procarióticas, que tiene la capacidad de unirse al receptor DC-SIGN presente en células eucarióticas, recubriendo estas células y saturando los puntos de unión a las formas infecciosas del parásito Leishmania portadoras del receptor.
Como formas homologas, se refiere a todas las proteínas que, con respecto al receptor DC-SIGN y al receptor presente en el parásito Leishmania, independientemente de su origen, presentan una homología de al menos, un 20% en la zona de reconocimiento del parásito y del receptor DC-SIGN. Un objeto particular de la presente invención lo constituye el anticuerpo monoclonal anti-DC-SIGN, MR-1.
Otro objeto adicional de la presente invención lo constituye un procedimiento para la identificación y evaluación de compuestos capaces de bloquear o anular la unión de la forma infecciosa del parásito Leishmania al receptor DC-SIGN, caracterizado por los siguientes pasos: a) adición al medio del compuesto candidato, b) determinación de la actividad bloqueadora del candidato mediante la valoración entre otros, de parámetros celulares: capacidad de unión o invasión in vitro en células K562-CD209 transfectadas con el receptor DC-SIGN, células dendríticas, células derivadas de monocitos (MDDC), u otras células que presentan en su superficie el receptor DC-SIGN, y competición en el proceso de inhibición de la unión con un anticuerpo específico anti-DC-SIGN o con mananos, c) identificación de un bloqueador de la unión de la forma infecciosa del parásito Leishmania al receptor DC-SIGN.
Un objeto particular de la presente invención es un compuesto terapéutico de la presente invención identificado por el procedimiento anteriormente mencionado.
Un objeto particular de la presente invención lo constituye una composición farmacéutica que contiene una cantidad terapéuticamente eficaz del compuesto terapéutico de la presente invención junto con al menos un excipiente farmacéuticamente aceptable. Tal como se utiliza en esta invención el término "cantidad terapéuticamente eficaz" se refiere a la cantidad de compuesto terapéutico de la presente invención suficiente para inhibir o reducir el proceso de unión entre el parásito
Leishmania y el receptor DC-SIGN. Por otro lado, un objeto adicional de la presente invención lo constituye el empleo de la composición farmacéutica en el tratamiento de infecciones provocadas por el parásito Leishmania en mamíferos, entre otros, en humanos.
Otro objeto adicional de la presente invención lo constituye el empleo del receptor DC-SIGN como diana para el diseño de compuestos terapéuticos, capaces de bloquear o anular la unión de las formas infecciosas del parásito Leishmania al receptor
DC-SIGN, o a cualquiera de sus isoformas o proteínas homologas. DESCRIPCIÓN DE LAS FIGURAS
Figura 1.- Amastigotes de L. pifanoi se unen a células K562 establemente transfectadas con DC-SIGN. Se incubaron células no transfectadas (K562) o células K562 transfectadas con DC-SIGN (K562-CD209) con amastigotes marcados con CFSE a distintas proporciones durante 2h a 37°C. Se cuantificó por citometría de flujo la proporción de células con amastigotes unidos mediante "gating" en K562 y determinación del porcentaje de células fluorescentes. Dos regiones al azar fueron establecidas: K562 con parásitos no unidos y K562 fluorescentes debido a la unión a las células de formas amastigotes marcadas con CFSE. Para efectos de comparación, las escalas fueron establecidas con relación al numero de células sin parásitos unidos. Figura 3.- La unión de amastigotes de L. pifanoi a células dendríticas derivadas de monocitos (MDDC) se ve afectada por la presencia de un anticuerpo anti-DC- SIGN (MR-1). MDDC previamente no tratadas (control), pre-tratadas durante 10 min a temperatura ambiente con un anticuerpo monoclonal anti-DC-SIGN MR-1 (columna izquierda), otros anticuerpos irrelevantes (inmunoglobulinas humanas, anti-CD18), mañano soluble o EGTA, fueron incubadas con formas amastigotes marcadas con CFSE en la proporción 1:5 célula:parásito, durante 2 horas a 37°C. EJEMPLOS DE REALIZACIÓN DE LA INVENCIÓN
En la presente invención se ha demostrado por primera vez que el proceso de unión e internalización de las formas amastigotes del parásito Leishmania se realiza por medio del receptor DC-SIGN presente en las células dendríticas humanas. De este modo, se empezó por demostrar que los amastigotes axénicos se unen a células dendríticas de forma dependiente de DC-SIGN, tal como hemos demostrado utilizando tanto células K562 transfectadas con DC-SIGN (K562-CD209) como células dendríticas derivadas de monocitos (MDDC) (Figuras 1-3). Los amastigotes axénicos de Leishmania son formas amastigotes del parásito que no están recubiertos de anticuerpos opsonizantes, no quedando de este modo enmascarada la participación de otros receptores distintos, y pudiendo analizarse la participación del receptor DC-SIGN en la unión e internalización de amastigotes de Leishmania. Esta unión es bloqueada por medio del anticuerpo monoclonal anti-DC-SIGN (MR-1) que inhibe la unión del parásito a las células dendríticas. El siguiente paso consistió en determinar si el bloqueo del receptor DC-SIGN con determinados compuestos, tales como el anticuerpo monoclonal MR-1, mañano soluble o un anticuerpo anti CD18 (TS1/18) alteraba el número de células dendríticas infectadas con amastigotes. Se observó que el número de DCs con formas infecciosas del parásito unidas era menor en el grupo tratado con el anticuerpo MR-1. Este hecho sugiere un papel importante de este receptor en la infección inicial de células dendríticas por Leishmania. Contrariamente a la unión de MDDC a ICAM-3 (presente en las células T), los mananos bloquearon parcialmente la unión de amastigotes a células dendríticas, resultado que se asemeja a resultados previos sobre la incapacidad de compuestos constituidos por mañano soluble de inhibir la interacción entre amastigotes de Leishmania y macrófagos (Kane, M.M., and D.M. Mosser. 2000. Leishmania parasites and their ploys to disrupt macrophage activation. Curr Opin Hemaíol 7:26-31). El efecto inhibidor incompleto de mañano sobre la interacción DC-SIGN-Leishmania puede deberse a diversas razones alternativas. En primer lugar, los sitios de unión de Leishmania en DC-SIGN pueden ser diferentes a los que están implicados en la unión de mañano. En este sentido, experimentos de mutagénesis dirigida han indicado que los lugares de unión de HIV gpl20 e ICAM-3 no son completamente idénticos, y de hecho, el reconocimiento de HIV gpl20 no es dependiente de su glicosilación (Geijtenbeek, T.B., G.C. van Duijnhoven, S.J. van Vliet, E. Krieger, G. Vriend, C.G. Figdor, and Y. van Kooyk. 2002. Identification of different binding sites in the dendritic cell- specific receptor DC-sIGN for ICAM-3 and HIN-1. J Biol Chem 17:17). En segundo lugar, la afinidad del los ligando(s) de Leishmania por DC-SIGΝ puede ser superior a la del mañano. De hecho, sutiles cambios en la estructura oligosacárida de moléculas relacionadas ricas en mañano da lugar a diferencias sustanciales en la afinidad por DC-SIGΝ (Mitchell, D.A., A.J. Fadden, and K. Drickamer. 2001. A novel mechanism of carbohydrate recognition by the C-type lectins DC- SIGN and DC-SIGNR. Subunit organization and binding to multivalent ligands. J Biol Chem 276:28939-28945). En cualquier caso, no se puede descartar la posibilidad de un mecanismo cooperativo en la interacción entre Leishmania y DC- SIGN, ya que el parásito podría actuar como un ligando multivalente.
Puesto que la composición de la membrana del parásito varía a lo largo de su ciclo vital (Wright, E. P., and E. R. el Amin. 1989. Leishmania infection: surfaces and immunity. Biochem Cell Biol 67:525; Bahr, V., Y. D. Stierhof, T. Ilg, M. Demar, M. Quinten, and P. Overath. 1993. Expression of lipophosphoglycan, high-molecular weight phosphoglycan and glycoprotein 63 in promastigotes and amastigotes of Leishmania mexicana. Mol Biochem Parasitol 58:107), la implicación de DC-SIGN como receptor para las tres formas principales del ciclo vital del parásito (promastigotes logarítmicos y estacionarios , conjuntamente con amastigotes) fue analizada. Los amastigotes mostraron la interacción más fuerte, y dentro de los promastigotes, aquellos de fase estacionaria (metacíclicos e infectivos) poseen una mayor avidez por DC-SIGN que los promastigotes de fase logarítmica ( procíclicos y no infectivos).
Ejemplo 1.- La unión de amastigotes a DC-SIGN es específica y de gran afinidad. Los resultados preliminares indicaron que los amastigotes axénicos de Leishmania pifanoi eran eficazmente internalizados por MDDCs humanas. Con el objetivo de explorar si DC-SIGN estaba implicado en la unión de amastigotes, primero analizamos la interacción de amastigotes axénicos con células K562 transfectadas con DC-SIGN (K562-CD209). Los amastigotes axénicos de L. pifanoi fueron capaces de unirse de forma dosis dependiente a las células transfectadas, pero no a las no transfectadas, según se evaluó por citometría de flujo usando parásitos marcados con CSFE (Figura 1). Mediante el empleo de distintas proporciones de células K562- CD209: amastigotes (1:1, 1:5 y 1:10), se determinó que la unión era máxima dentro del rango 1:10-1:5. Consecuentemente, para evitar el exceso de ligandos del parásito, en los demás experimentos de cinética y competición, se empleó la proporción 1:5. La unión de amastigotes a K562-CD209 fue rápida y alcanzó la saturación en lh (Figura 2). El anticuerpo monoclonal MR-1 bloqueó completamente la unión de amastigotes a células K562-CD209 (Figura 2), confirmándose la participación de DC-SIGN en esta interacción específica. Con relación al mañano soluble (300 μg/ml), este redujo en un 30% la unión de amastigotes al receptor DC-SIGN (Figura 2). No se observó inhibición empleando GLPLs de Leishmania, que son moléculas con un alto contenido en mañosa presentes en elevado número en las membranas plasmáticas de las formas promastigotes y amastigotes, a una concentración de 5 μg/ml (aproximadamente equivalente a 2x10 amastigotes/ml) (Zawadzki, J., C. Scholz, G. Currie, G.H. Coombs, and M.J. McConville. 1998. The glycoinositolphospholipids from Leishmania panamensis contain unusual glycan and lipid moieties. J Mol Biol 282:287-299) (Figura 2). Por lo tanto, DC-SIGN funciona como un receptor para amastigotes de L. pifanoi, cuya unión a DC-SIGN puede ser total o parcialmente desplazada por mañano, dependiendo del tipo celular ensayado.
Ejemplo 2.- DC-SIGN está implicado en la unión de amastigotes a DC.
Para evaluar el papel desarrollado por el receptor DC-SIGN en el proceso de intemalización de las formas amastigotes de Leishmania por DC, se pre-incubaron MDDC humanas inmaduras con un exceso de anticuerpo anti-DC-SIGN (MR-1), inmunoglobulinas humanas, anticuerpo anti-CD18 TS1/18, EGTA o mañano soluble antes de la adición de amastigotes axénicos marcados con CFSE, en la proporción 1:5 DC:parásito, por 2 horas a 37°C. La incubación inicial de MDDC con el anticuerpo MR- 1 reduce el número de parásitos unidos a las células en una forma dosis dependiente, observándose inhibición con diluciones inferiores a 1/40000 del fluido ascítico del hibridoma (figura 3). De este modo, el efecto inhibidor dependiente de la dosis del anticuerpo MR-1 demuestra la especificidad de la interacción entre DC-SIGN - Leishmania. La unión del parásito a las células eucarióticas fue completamente inhibida por EGTA, que es un inhibidor de las interacciones mediadas por lectinas de tipo C, pero no por las inmunoglobulinas humanas, ni por el anticuerpo anti-CD18 TS1/18 (Figura 3). La pre-incubación de MDDC humanas inmaduras con mañano soluble tuvo poca influencia en el proceso de unión del parásito a las células.
Debido a la amplia diversidad de posibles receptores en DC para amastigotes, el papel de DC-SIGN se evaluó mediante la preincubación de MDDC humanas inmaduras con un anticuerpo anti-DC-SIGN o mañano soluble, previamente a la adición de amastigotes axénicos marcados con CFSE (proporción 1:5, 2h, 37°C). El número de amastigotes unidos a DC, determinado microscópicamente, fue 17.7 (± 4.9) y 7.25 (± 2.0) en el control y en las MDDC tratadas con MR-1 respectivamente. En contraste, el mañano tuvo un efecto menor en la reducción del número de amastigotes unidos por DC (Figura 4A), pero en cambio suprimió la unión de MDDC dependiente de DC-SIGN a ICAM-3 inmovilizado (Figura 4B). Estos resultados se complementaron mediante análisis por citometría de flujo (Figura 3).
Para evaluar el significado fisiológico de la interacción DC-SIGN-amastigotes de L. pifanoi, se incubaron MDDC humanas inmaduras con amastigotes axénicos, en la proporción 1:5 DC:parásito, después de la previa incubación con el anticuerpo MR-1, anticueφo TS1/18 o mañano soluble (500μg/ml). Al cabo de 48h, la infección se evaluó mediante el examen de al menos 200 células teñidas con Giemsa. Como se resume en la Tabla I, los amastigotes axénicos de L. pifanoi fueron eficazmente internalizados por MDDC humanas inmaduras en ausencia de anticueφos opsonizantes (anticueφos específicos que recubren el parásito y que tienen un papel primordial entre los mecanismos desarrollados por este para sobrevivir). Se observó que en el control, el 82.5% de DC infectadas presentaba 2.3 parásitos internalizados por célula (Tabla I). Sin embargo, la pre-incubación de MDDC humanas inmaduras con el anticueφo MR-1 reducía significativamente el numero de células infectadas (82.5% vs. 7.4%) (Tabla I). Como se observó en ensayos de unión, la pre-incubación con el anticueφo anti-CD18 no produjo una reducción del porcentaje de células infectadas (91.0%). Una vez más, el mañano soluble solo inhibió parcialmente la internalización del parásito Leishmania por las MDDC.

Mañano soluble 34.0 2.3 * El porcentaje de DCs infectadas y el número de amastigotes internalizados por célula infectada se determinó por microscopía tras el examen (200 células) de preparaciones teñidas con Giemsa. Ejemplo 3.- Los promastigotes y amastigotes de Leishmania infantum y L.pifanoi se unen vía DC- SIGN a las células dendríticas.
La metaciclogénesis es un proceso mediante el cual Leishmania se transforma de la fase procíclica del promastigote, con muy baja infectividad, a la fase metacíclica, muy infectiva . En la Naturaleza, dicho proceso tiene lugar dentro del tubo digestivo del insecto vector. Dicha transformación conlleva un incremento en la capacidad para infectar y sobrevivir dentro del huésped vertebrado, donde el parásito se enfrenta al sistema immune del hospedador .
Para identificar las formas infectivas del parásito reconocidas por DC-SIGN, los inventores analizaron la capacidad de las células dendríticas inmaduras humanas para unir promastigotes metacíclicos y amastigotes tanto de L. pifanoi como de L.infantum. Conforme a los gráficos de la figura 5, ambas especies se unen a células dendríticas(54.1% y 65.0% para los amastigotes de L.pifanoi y L.infaníum respectivamente), independientemente de qué forma infectiva del parásito se ensaye. En todos los casos , la unión del parásito se inhibe drásticamente si las células de mamífero se preincuban con el anticueφo monoclonal MR-1, que bloquea DC-SIGN (64% de reducción para ambas formas infectivas de L. pifanoi y de 55% para las formas de L. infaníum). En las células dendríticas inmaduras humanas, el porcentaje de las mismas con parásitos unidos fue siempre inferior para promastigotes que para amastigotes, lo que sugiere una menor afinidad de unión de la forma flagelada del parásito hacia DC- SIGN . Por consiguiente, DC-SIGN une tanto promastigotes metacíclicos y amastigotes de L. pifanoi (causante de la leishmaniasis cutánea) como de L. infantum (agente de leishmaniasis visceral).
Con el fin de profundizar en el papel de DC-SIGN en la interacción de Leishmania con las células dendríticas, los experimentos fueron repetidos en células K562 DC-SIGN. Como sucedió con las células dendríticas inmaduras humana, el porcentaje de células con promastigotes metacíclicos unidos (58.9% y 34.1% para L. pifanoi y L.infantum respectivamente ) son más bajos que los correspondientes para amastigotes de ambas especies (65.6% y 51.8% para L. pifanoi y L. infantum, respectivamente) (Figura 6). Estos resultados confirman las diferencias en la unión relativa a DC-SIGN para promastigotes y amastigotes. Por otra parte, el efecto bloqueante del mañano, que bloquea DC-SIGN , es mayor sobre promastigotes (reducción de 40.3% para L. pifanoi y 84.8% para L. infantum) que sobre los amastigotes (13.0% y 27.8% de reducción para L. pifanoi y L.infaníum, respectivamente), independientemente de la especie de parásito (Figura 6). En conjunto, estos resultados avalan una diferente avidez de ambas formas infectivas hacia DC-SIGN, que es confirmada mediante los ensayos de desplazamiento, mediante utilización de un exceso de amastigotes sin marcar: La adición de amastigotes de L. pifanoi reduce la unión de promastigotes logarítmicos (30%) y de fase estacionaria (32%), con porcentajes muy similares, pero no desplazan los amastigotes previamente unidos a DC-SIG(6.2% de reducción).
En la siguiente sección de Materiales y Métodos se identifican los reactivos, anticueφos, parásitos y células, así como una descripción detallada de los ensayos utilizados en dichos Ejemplos. MATERIALES Y MÉTODOS Reactivos y anticuerpos. El anticueφo monoclonal específico para DC-SIGN (MR-1) ha sido descrito anteriormente (Relloso, M., Puig-Kroger, A., Muniz Pello, O., Rodriguez-Fernandez, JL., de la Rosa, G., Longo, N., Navarro, J., Muñoz-Fernandez, MA., Sánchez-Mateos, P., Corbi AL. 2002. Eur J Immunol 29:235-244), se analizaron por TLC y se guardaron como solución stock estéril a 10 mg/ml en PBS (1% DMSO). ICAM-3/Fc recombinante humana se obtuvo de R&D Systems (cat. 715-IC). Parásitos. Los amastigotes axénicos de Leishmania pifanoi MHOM VE/60Ltrod fueron cultivados a 31°C en medio F29 simplificado suplementado con 20% de suero bovino fetal inactivado por calor (HIFBS) (Pan, A.A. 1984. Los amastigotes axénicos de Leishmania infantum (MCAN/ES/89/TPZ229/1/89 son cultivados conforme a los protocolos descritos a 37°C (Pan, A. A. 1984. Leishmania mexicana: serial cultivation of intracellular stages in a cell-free médium. Exp Parasiiol 58:72; Armson, A., S. W. Kamau, F. Grirnm, J. A. Reynoldson, W. M. Best, L. M. MacDonald, and R. C. Thompson. 1999. A comparison of the effects of a benzimidazole and the dinitroanilines against Leishmania infantum. Acta Trop 73:303). Los promastigotes de L.pifanoi y L.infantum son crecidos a 27°C en medio RPMI 1640 suplementado con 20% de suero fetal bovino inactivado por calor. Los promastigotes de la fase de crecimiento logarítmica y estacionaria se obtienen a los 3 ó 7-8 días de crecimiento de un cultivo con un inoculo inicial de 106 células/ml. Las MDDC inmaduras se prepararon a partir de monocitos de sangre periférica mediante la utilización de IL-4 (1000 U/ml) y GM-CSF (1000 U/ml), según lo ya descrito (Relloso, M., Puig-Kroger, A., Muniz Pello, O., Rodriguez-Fernandez, JL., de la Rosa, G., Longo, Ν., Navarro, J., Muñoz-Fernandez, MA., Sánchez-Mateos, P., Corbi AL. 2002. Efficient presentation of soluble antigen by cultured human dendritic cells is maintained by granulocyte/macrophage colony-stimulating factor plus interleukin 4 and downregulated by tumor necrosis factor alpha. J Exp Med 179:1109- 1118). Las células K562 se cultivaron en RPMI 1640 suplementado con 10% suero de ternera fetal (FCS), 2 uiM glutamina y 10 μg/ml gentamicina (medio completo). Las células K562 transfectadas con DC-SIGN (K562-CD209) (Relloso, M., Puig-Kroger, A., Muniz Pello, O., Rodriguez-Fernandez, JL., de la Rosa, G., Longo, N., Navarro, J., Muñoz-Fernandez, MA., Sánchez-Mateos, P., Corbi AL. 2002. DC-SIGN (CD209) expression is IL-4 dependent and is negatively regulated by IFN, TGF-β, and anti- inflammatory agents. J Immunol 168: 2634-2643) se crecieron en medio completo suplementado con 300 μg/ml de G418. Ensayos de unión del parásito. Se resuspendieron células MDDC inmaduras, K562 o K562-CD209 en medio completo y se alicuotaron en placas de 24 pocilios (2xl05 células/pocilio). A las células se añadieron parásitos marcados con CFSE, en las proporciones indicadas, y se incubaron a 37°C durante en tiempo reseñado en cada experimento. Posteriormente, las células se fijaron (1% paraformaldehyde en PBS) durante lh a temperatura ambiente, se montaron en Mowiol (Calbiochem, 475904) y se analizaron en un microscopio Nikon Eclipse E800 (Nikon, Melville, NY) equipado para epifluorescencia o mediante citometría de flujo en un EPICS-CS (Coulter Científica, Madrid, Spain). Los porcentajes de células sin o con parásitos unidos se resuelven en dos poblaciones de baja (B) y alta (C) fluorescencia. Para los ensayos de inhibición, las células se lavaron con PBS-lmM EDTA y antes de la adición de parásitos, se preincubaron durante 10 min a temperatura ambiente con el anticueφo MR-1 (sobrenadante de hibridoma diluido 1/1000), anticueφos irrelevantes (100 μg/ml de inmunoglobulinas humanas o sobrenadante del hibridoma anti-CD18 TS1/18, diluido 1/500), EGTA (5 mM), mañano soluble (300 μg/ml) o GJ LS purificados de amastigotes de L. pifanoi amastigotes (5 μg/ml) en medio completo. Para los ensayos de desplazamiento, las células K562 son primeramente incubadas durante 2 minutos con parásitos marcados con CSFE, y a continuación se añade un exceso (4: 1) de amastigotes de L. pifanoi no marcados. Ensayos de adhesión de DC-SIGN. La adhesión de MDDC dependiente de DC-SIGN fue evaluada mediante la utilización de ICAM-3-Fc. Placas EIA II-Linbro de 96 pocilios se recubrieron durante una noche con ICAM-3-Fc a 3 μg/ml en 100 mM NaHCO3 pH 8,8 y a 4° C, y se bloquearon con albúmina sérica bovina (BSA) al 0.4 % en la misma solución durante 2h a 37°C. Las MDDC se marcaron en medio completo con el colorante fluorescente 2',7'-bis-(2-carboxyethyl)-5-(and-6)-carboxyfluorescein, acetoxymethyl ester (BCECF) (Molecular Probes, The Netherlands), y entonces se preincubaron durante 20 min a 37°C en RPMI + 0.4% BSA y con anticuerpos bloqueantes contra CD209 (MR-1), CD18 (TS1/18) o con mañano a 100 μg/ml. Entonces se dejó que las células se adhiriesen a cada pocilio durante 15 min a 37° C. Se eliminaron las células no unidas mediante tres lavados con RPMI atemperado y las células adheridas se cuantificaron en un analizador de fluorescencia. Infección de DC con Leishmania. Se ajustó la concentración de DC a 10 células/pocilio en placas de 24 pocilios y se incubaron con distintas proporciones de parásitos durante lh a 37°C. Para los ensayos de inhibición las células fueron lavadas con PBS-lmM EDTA, previamente a la adición del parásito, y pre-incubadas durante 10 min a temperatura ambiente con el anticueφo MR-1, el anticueφo TS1/18 o mañano soluble en medio completo. Después de eliminar por lavado los parásitos no unidos, las muestras OC-Leishmania se incubaron durante 48h a 34°C. Posteriormente se lavó con PBS, se fijó con metanol y se tino con Giemsa para poder visualizar las DC infectadas con amastigotes. El porcentaje de DC infectadas y la relación de parásitos-DCs se determinó por microscopía tras el examen de al menos 200 células.

Claims

REIVINDICACIONES
1.- Compuesto terapéutico capaz de bloquear la unión de formas infecciosas del parásito Leishmania a células eucarióticas en mamíferos, entre otros, humanos, caracterizado porque permite bloquear o anular la unión del parásito Leishmania al receptor DC- SIGN presente en estas células y la internalización del parásito por las mismas.
2.- Compuesto terapéutico según la reivindicación 1 caracterizado porque las formas infecciosas de Leishmania son amastigotes y promastigotes, entre otras, de las especies de L. amazonensis, L. major, L. pifanoi, L. infantum.
3.- Compuesto terapéutico según las reivindicaciones 1 y 2 caracterizado porque la célula eucariótica pertenece, entre otras, al siguiente grupo: células dendríticas, macrófagos y células derivadas de monocitos.
4.- Compuesto terapéutico según reivindicaciones 1 a 3 caracterizado porque permite bloquear o anular la unión de las formas infecciosas del parásito Leishmania al receptor DC-SIGN en al menos un 20%, preferentemente en al menos un 85%, ó más preferentemente en un 100%.
5.- Compuesto terapéutico según una cualquiera de las reivindicaciones 1 a la 4 caracterizado porque pertenece, entre otros, al siguiente grupo: a) anticueφo anti DC-SIGN, b) mañano soluble, c) receptor DC-SIGN o el dominio lectina de DC-SIGN, recombinantes, y isoformas y proteínas homologas de los mismos, d) receptor recombinante presente en el parásito Leishmania responsable de la unión con el receptor DC-SIGN, y sus formas homologas.
6.- Compuesto terapéutico según la reivindicación 5 caracterizado porque las formas homologas, independientemente de su origen, presentan una homología de al menos, un 20%, en la zona de reconocimiento del parásito y del receptor DC-SIGN.
7 - Compuesto terapéutico según la reivindicación 5 caracterizado porque el anticueφo anti-DC-SIGN es el anticueφo monoclonal MR-1.
8.- Procedimiento para la identificación y evaluación de compuestos según las reivindicaciones 1 a la 4 caracterizado porque engloba los siguientes pasos: a) adición al medio del compuesto candidato, b) determinación de la actividad bloqueadora del candidato mediante la valoración entre otros, de parámetros celulares: capacidad de unión o invasión in vitro en células K562-CD209 transfectadas con el receptor DC-SIGN, células dendríticas, células derivadas de monocitos (MDDC), u otras células que presentan en su superficie el receptor DC-SIGN, y competición en el proceso de inhibición de la unión con un anticueφo específico anti-DC-SIGN o con mananos, c) identificación de un bloqueador de la unión de la forma infecciosa del parásito Leishmania al receptor DC-SIGN.
9.- Composición farmacéutica caracterizada porque contiene una cantidad terapéuticamente eficaz del compuesto terapéutico según una cualquiera de las reivindicaciones 1 a la 7, junto con al menos un excipiente farmacéuticamente aceptable.
10.- Uso de la composición farmacéutica según la reivindicación 9 para el tratamiento de infecciones provocadas por el parásito Leishmania en mamíferos, preferentemente en humanos.
11.- Uso del receptor DC-SIGN como diana para el diseño de compuestos terapéuticos según las reivindicaciones 1 a la 7.
