Definitions
- the present invention relates to a method for improving the pharmacokinetics of hexahydrofi ⁇ ro[2,3-b]furanyl containing HIV protease inhibitors comprising administering to a human in need thereof a combination of a therapeutically effective amount of a hexahydrofuro[2,3-b]furanyl containing HIN protease inhibitor, and a therapeutically effective amount of a cytochrome P 450 inhibitor.
- HIN-1 The virus causing the acquired immunodeficiency syndrome (AIDS) is known by different names, including T-lymphocyte virus III (HTLN-III) or lymphadenopathy- associated virus (LAN) or AIDS-related virus (ARN) or human immunodeficiency virus (HIN).
- T-lymphocyte virus III HTLN-III
- LAN lymphadenopathy- associated virus
- ARN AIDS-related virus
- HIN-1 human immunodeficiency virus
- HIN-2 HIN-2.
- HIN will be used to generically denote these viruses.
- gag and gag-pol gene transcription products are translated as proteins, which are subsequently processed by a virally encoded protease (or proteinase) to yield viral ' enzymes and structural proteins of the virus core.
- a virally encoded protease or proteinase
- the gag precursor proteins are processed into the core proteins and the pol precursor proteins are processed into the viral enzymes, e.g., reverse transcriptase, integrase and retroviral protease. It has been shown that correct processing of the precursor proteins by the retroviral protease is necessary for the assembly of infectious virions.
- Retroviral protease inhibition typically involves a transition-state mimetic whereby the retroviral protease is exposed to a mimetic compound which binds (typically in a reversible manner) to the enzyme in competition with the gag and gag-pol proteins to thereby inhibit specific processing of structural proteins and the release of retroviral protease itself. In this manner, retroviral replication proteases can be effectively inhibited.
- HIN protease inhibitors are commonly administered to AIDS patients ill combination with other anti-HIN compounds such as, for instance nucleoside reverse transcriptase inhibitors ( ⁇ RTIs), non-nucleoside reverse transcriptase inhibitors
- Some antiretrovirals and, in particular, some HIN protease inhibitors are metabolized by cytochrome P 5 o, leading to sub-optimal pharmacokinetic profiles causing an undesired need for more frequent and higher doses.
- cytochrome P 5 o cytochrome P 5 o
