Il est parfois nécessaire de retirer une prothèse osseuse pour la remplacer par une autre plus adaptée, notamment en cas de descellement de cette prothèse ou de dégradation osseuse. La gaine de ciment ayant servi au scellement de la prothèse d'origine doit être éliminée afin de permettre la mise en place adéquate de la nouvelle prothèse. Le brevet américain n° 5 047 035 décrit une pièce de perçage permettant une telle élimination. Cette pièce présente une forme et des dimensions correspondant d'une manière générale à celles d'une tige fémorale, et comprend un conduit aménagé en elle, parallèlement à son axe longitudinal. Ce conduit permet le perçage d'un trou coaxial à l'os pour la mise en place d'une broche, cette broche servant au guidage d'alésoirs canules de diamètre allant en augmentant, utilisés pour réaliser l'élimination de la gaine. La pièce de perçage selon ce document antérieur présente l'inconvénient majeur de rendre possible un alésage ou une perforation de la corticale fémorale. En effet, il est très fréquent que l'axe longitudinal de la prothèse, et donc de la gaine de ciment, soit orienté de manière oblique par rapport à l'axe de la diaphyse fémorale. Cette orientation oblique conduit à une orientation correspondante de ladite pièce de perçage, et donc du conduit, lorsque cette pièce est mise en place dans le canal médullaire de l'os, de sorte que ce conduit dirige l'outil de perçage en direction de la corticale. Ce problème est particulièrement important dans le cas d'un fémur puisque la fragilisation de l'os résultant de l'alésage ou de la perforation de la corticale se produit à un niveau de l'os où s ' exercent des contraintes importantes . En outre, les tiges fémorales de prothèses de hanche ont des formes très variables d'un fabricant à un autre, auxquelles la pièce de perçage selon ce document antérieur ne correspond pas parfaitement.
