突起 3はほぼ球状に形成され、 図示例では 3個設け られているが、 数は任意であ,る。 突起 3の外径はワイ ャ本体 2の直径より も約 O . i 〜2. 0 ram大きい。 各突起 3の間隔は例えば 0 . 5 〜 5 m mで、 すべての突起 3の 全長は 5〜 4 0 である。 先端側の突起 3とワイヤ本 体 2の先端との間隔は、 例えば 0〜 5 O HIIIU 好ま しく は 1 0〜 2 0 mmである。
心線は血管閉塞部開通用ワイヤ 1の使用時に、 外力 を受けて変形した後、 元の形に復帰する材料 (弾性材 料) 、 例えばステンレス鋼、 合金 (ニッケル一チタン 合金など) 、 ピアノ線材料、 アモルフ ァス金属、 金、 プラスチック等からなっている。 この心線 5はワイヤ 本体 2を補強する作用、 あるいはワイヤ本体 2の弾性 を高める作用がある。 心線 5はワイヤ本体 2の先端ま で達しておらず、 先端と突起 3の間のところで終わつ ている。 心線 5の直径は例えば 0 . i〜 1 ramである。
第 2図に示した第 2実施例による血管閉塞部開通用 ワイヤ 1 Aは、 突起 3 Aが円柱形に形成され、 そして 閉塞している血管へのワイヤの挿入を容易にするため に、 先端と突起 3 Aの間のところで角度 だけ曲げら れている。 この場合、 ワイヤ本体 2 Aは、 血管閉塞部 開通用ワイヤ 1 A使用時に、 外力を受けて変形し、 そ れ自身では元の形に復帰しない材料 (可撓性材料) 、 あるいは変形後、 元の形に復帰する材料 (弾性材料) からなり、 心線 4 Aは血管閉塞部開通用ワイヤ 1 A使 用時に、 外力を受けて変形し、 元の形に復帰する材料 (弾性材料) からなつている。 この心線 4 Aは更に、 使用前に術者等が弾性限度以上の外力を加えて適当な 角度 a (角度 αは 9 0 ° 以下の任意の角度) に塑性変 形または適当な形状に塑性変形し得る材料からなって いると好都合である。 なぜなら、 血管の閉塞部にワイ ャを揷入しやすいように、 閉塞部の部位に応じて、 術 者が血管閉塞部開通用ワイヤ 1 Αの先端範囲を変形で きるからである。 心線 4 Aはワイヤ本体 2 Aの先端近 く まで達している。
第 3図に示した第 3実施例による血管閉塞部開通用 ワイヤ 1 Bは、 突起 3 Bがらせん体によって形成され ている点だけが第 1実施例の血管閉塞部開通用ワイヤ 1と異なる。 この突起 3 Bはワイヤ本体 2 Bと同じ材 料で一体に形成してもよく、 他の材料で別体に形成し てもよい。 例えば人体親和性の金属線 (心線 4 Bでも よい) またはプラスチック線で形成する場合には、 そ の両端をヮィャ本体 1 B内に埋込んで露出しないよう にする必要がある。 らせん体 3 Bのピッチは例えば 0 . 5〜 5 mmでめる。
第 4図に示した第 4実施例による血管閉塞部開通用 ワイヤ 1 Cは、 突起の代わりにらせん体 5が心線 4 C と同心的に設けられ、 このらせん体 5の範囲のワイヤ 本体 2 Cが切除されている。 このらせん体 5は人体親 和性の金属線 (心線 4 Bでもよい) 、 高電気低抗性の 金属線、 強磁性の金属線、 またはプラスチック線で形 成されている。 らせん体 5の外径はワイヤ本体 2 じょ り も大き く てもよいし、 小さ く てもよい。 らせん体 5 の素線の径は 0 . 1〜 1 mmで、 ピッチは 0 . 5〜 5關であ る O
第 5図に示した第 5実施例による血管閉塞部開通用 ワイヤ 1 Dは、 ワイヤ本体 2 Dが弾性.材料からなり、 突起の代わりに、 リ ング状の溝 6を備えている。
第 6図の第 6実施例による血管閉塞部開通用ワイヤ 1 Eは、 突起 3 Eが円錐台の形をしている。 この場合 も、 第 5図の第 5実施例と同様に、 ワイヤ本体 2 Eは 弾性材料からなっている。
第 7図に示した第 7実施例による血管閉塞部開通用 ワイヤ 1 Fの場合には、 突起 3 Fがワイヤ本体 2 Fの 片側にだけ設けられ、 そして薬剤 (血管拡張剤、 血栓 溶解剤等) を供給するための通路 1 1がワイヤ本体 2 F内に形成されている。 この通路 1 1は突起 3 Fの 範囲とワイヤ先端で、 ワイヤ表面に開口している。 薬 剤の流れ方向は矢印で示してある。
第 8図に示した第 8実施例による血管閉塞部開通用 ワイヤ 1 Gは、 第 4図の第 4実施例の場合と同様に、 らせん体 5 Gを備えている。 しかし、 内部に心線を備 えておらず、 通路 1 1 Gが形成してある。 この通路 1 1 Gはらせん体 5 Gの手前で、 ワイヤ本体 2 Gの表 面に開口している。 この通路にガラスファイバーを内 揷し、 外部からレーザー光線を照射可能としてもよい, 次に、 上記血管閉塞部開通用ワイヤ 1を用いた冠状 動脈閉塞部開通方法について、 第 9図と第 1 0図を参 照して説明する。 この開通は次のような順序で行われ O
1 ) 先ず、 図示していない動脈を突刺し、 動脈内に 図示していないシースを留置する。
2 ) 動脈内に心臓カテーテル 7を挿入し、 心臓 8に 導く。
3 ) カテーテル 7の先端を冠状動脈 9に揷入し、 造 影剤を注入し、 レン トゲンで透視して閉塞部 1 0を確 認する
4 ) 血管閉塞部開通用ワイヤ 1をカテーテル 7内に 挿入し、 冠状動脈 9内に入れ、 冠状動脈閉塞部 1 0に 導く。 その際、 ワイヤ先端と突起 3の間の部分が案内 の役目をするので、 閉塞部に容易に揷入可能である。
5 ) 血管閉塞部開通用ワイヤ 1を操作して、 その突 起 3を閉塞部 1 0の前と後の間で数度往復させ、 閉塞 部を削りながら内腔を拡げる。 この時、 心線を通じて 高抵抗材に高周波電流の付与、 強磁性体に高周波磁場 の付与を行ない、 閉塞部を焼灼、 開拡してもよい。
6 ) 最後に、 血管閉塞部開通用ワイヤ 1を力テーテ ル 7内に引込めてカテーテル 7を動脈から抜去する。 上記のように、 血管閉塞部開通用ワイヤ 1の突起 3 を閉塞部に位置させ、 往復させることにより、 閉塞部 に存在する血栓、 肥厚した血管内膜、 コレステロール などを削り除去し、 心臓などへの血流を再開させるこ とができる。 また心線を通しての高周波電流の付与、 高周波磁場の付与により、 上記血栓等が焼灼、 除去さ れる。
従来のバルーンカテーテルでは、 このような肥厚部 を削ることなく 、 単に拡大させるにとどまるため、 時 間が経つと、 内腔が押され、 再狭窄を生じる確率が非 常に高い。 しかし、 本発明の血管閉塞部開通用ワイヤ 1を用いると、 血管内に肥厚した部分が除去されるた め、 再狭窄を生じる確率が非常に低く なる。 また、 様 々のサイズのワイヤを用いることにより、 閉塞した血 管を開通させ、 その内怪を段階的に削除、 拡大してい く ことが可能となる。 ワイヤ先端部分は非常に弾性に 富んでいるため、 心臓血管の損傷、 心原性ショ ックな どの合併症が生じにく く 、 いかなる細い直径の血管に も有効性を発揮する。
更に、 従来の血栓溶解剤を用いた方法と比較して、 出血傾向を生じることがなく 、 成功率も非常に高い。
更に、 ワイヤ先端と突起 3の間の弾性の部分が案内 の役目をするので、 血管閉塞部に容易に挿入可能であ る。 また、 ワイヤ本体 2の表面の滑りが良好であるの で、 血管内壁を傷つけることがない。 更に、 上述のよ うに、 ワイヤ 1を血管に挿入して往復動作させるだけ でよいので、 手術操作が簡単かつ容易である。 更に、 ワイヤ 1の構造が簡単であるため、 製造コス トが安価 でめる。
以上、 本発明の実施例について説明したが、 本発明 は上記実施例に限定されるものではない。 例えば、 第
1 , 3, 4実施例の血管閉塞部開通用ワイヤ 1 , 1 B, 1 Cの場合には、 第 5, 6, 7, 8実施例の血管閉塞 部開通用ワイヤ 1 D, 1 E, I F, 1 Gのように、 必 ずしも心線を設ける必要がない。 心線を設ける場合に は、 ワイヤ本体の先端まで達するようにしてもよいし、 ヮィや本体の先端の少し手前まで達するようにしても よい。 更に、 通路 1 1 , 1 1 Gを備えていない第 1〜 6実施例による血管閉塞部開通用ワイヤ 1 , 1 A, 1 B , 1 C , 1 Dの場合にも、 通路を設けることがで きる。 この場合、 心線と通路はワイヤ本体内に互いに 平行に設けてもよく、 また通路を心線より も大きく形 成して、 この通路内に心線を配置してもよい。 更に、 通路 1 1 , 1 1 Gの開口個所は、 ワイヤ先端範囲のど こでもよく、 また、 開口 (出口) の数も任意である。 更に、 突起 3, 3 A, 3 B, 3 E , 3 Fと溝 6はいろ いろな形に形成可能であり、 突起と溝の両方を設けて もよい。 更に、 突起と溝の個数は任意であり、 その配 置場所は、 ワイヤ先端範囲であればどこでも (先端ま たは先端近くでも) よい。 以上説明したように、 本発明による血管閉塞部開通 用ワイヤは、 術後の再閉塞を生じる確率が非常に低く、 心筋梗塞、 心原性シ ョ ッ ク、 冠状動脈損傷等の合併症 を生じにく い。 更に、 出血傾向が小さく、 成功率が非 常に高い。 更に、 製造コス 卜が安く、 かつ手術操作を 簡単かつ容易にするという利点がある。

Claims

請 求 の 範 囲
1 . カテーテルに内挿されるワイヤにして、 可撓性 または弾性のワイヤ本体の先端範囲に、 複数の突起ま たは溝を形成したことを特徵とする血管閉塞部開通用 ワイヤ。
2 . 前記突起が、 プラスチック材料、 高電気抵抗材 および強磁性材からなる.群から選択される 1種の材料 からなることを特徴とする請求の範囲第 1項に記載の 血管閉塞部開通用ワイヤ。
3 . カテーテルに内挿されるワイヤにして、 可撓性 または弾性のワイャ本体の先端から間隔をおいてらせ ん体を設け、 このらせん体の範囲のワイヤ本体を切除 したことを特徵とする血管閉塞部開通用ワイヤ。
4 . 前記らせん体が、 高電気抵抗材または強磁性材 から選択される材料からなることを特徴とする請求の 範囲第 3項に.記載の血管閉塞部開通用ワイヤ。
5 . ワイヤ本体内に、 弾性の心線が埋込まれている ことを特徴とする請求の範囲第 1 〜 4項のいずれか一 項に記載の血管閉塞部開通用ワイヤ。
6 . ワイヤ本体がその先端と突起、 溝またはらせん 体との間で曲がっていることを特徴とする請求の範囲 第 1 〜 5項のいずれか一項に記載の血管閉塞部開通用 ワイヤ。
7 . 通路がワイヤ本体内に形成され、 この通路がヮ ィャ本体の先端範囲でワイヤ本体の表面に開口してい ることを特徴とする請求の範囲第 1〜 6項のいずれか 一項に記載の血管閉塞部開通用ワイヤ。
8 . 前記,通路が薬剤供給用通路であることを特徵と する請求の範囲第 7項に記載の血管閉塞部開通用ワイ ャ。
9 . 前記通路がガラスフ ァイバ内挿用通路であり、 このガラスファイバを通じてレーザを付与可能と して いることを特徴とする請求の範囲第 7項記載の血管閉 塞部開通用ワイヤ。
1 0 . カテーテルに内挿されるワイヤにして、 可撓 性または弾性のワイヤ本体内に心線を設け、 ワイヤ本 体の先端から間隔を開けて前記心線に接続して高電気 抵抗材からなるらせん体を設けて、 高周波電流を付与 することにより加熱可能と していることを特徴とする 血管閉塞部開通用ワイヤ。
1 1 . カテーテルに内挿されるワイヤにして、 可撓 性または弾性のワイャ本体の先端部を強磁性材によつ て形成し、 高周波磁場により加熱可能と していること を特徴とする血管閉塞部開通用ワイャ。
