COMBINED LABEL AND PRESCRIPTION BLANK.

Another object of my invention is the provision of a blank or strip which can be used with facility upon boxes or bottles, which can be made in any size or shape desired, and which will provide a record for the physician, the druggist and the purchaser, and thus avoid accidents and mistakes which are constantly occurring, and which will enable a purchaser to have the prescription renewed with case and facility.

Another object of my invention is the provision of a combined label and prescription blank which will preserve a record for the prescribing physician, for the druggist and for the purchaser, and which will provide means for indicating to the purchaser or person for whom the medicine is intended, the exact amount of the dose to be taken as well as the time during the day or night, to positively prevent any error in taking the dose at the proper and exact time.

Another object of my invention is the provision of a combined label and prescription blank which will be of comparatively'small size and of the desired shape while being ample to carry all printed matter desired, to justify the use of the article for the purposes intended.

To attain the desired objects, my inven tion consists of a strip or sheet of paper of the desired size and shape providing a combined label and prescription blank as shown substantially in the accompanying drawing, which represents a plan view of a complete label and prescription blank embodying my improvements.

The drawing shows a sheet or strip of paper of rectangular form, although any desired. form or size of sheet may be used which will answer the purpose, which sheet at the upper portion is scored at A, to provide the blank or strip B which is adapted to receive the date C. number D, of the prescription, space E for the person who prepares the prescription, and space F for the name and address of the person for whom the medicine is prescribed. This forms a ticket or strip which is retained by the druggist and which carries a record as to the number of the prescription, its date of prep aration, the person who has compounded the preparation, and the name and address of the person ordering the medicine, thus making a complete record which is retained by the druggist. The said strip B is centrally scored, as shown, to provide two record portions which may be divided on the scored line. 5 7

Below said upper ticket is the labelcarrying ticket G, which has at one side the indication H, for the daytime and the indication H for the night-time at the other edge thereof, while between said day and nightindicating spaces is the space J, in which is placed the number of the prescription and date K, the space for advertising L, and the amount of the dose M, thus making a label which carries in addition to the advertisement and record, the amount of the dose and the time day or night when the medicine mustbe taken, and to the side of the night indication is a space N for any desired purpose, such as a cautionary paragraph, or other matter of importance.

This construction provides the strip or ticket for the druggist and the label to be Cir placed upon the bottle or box, and below said label space is the ticket 0, which carries the number 0 the street and address 0 and name of the person Q as well as whether the medicine has been paid for, to be collected, or charged as at Q this forming the purchasers ticket and record to be delivered and retained by the purchaser, while below the purchasers ticket is the prescription blank or ticket P, which carries the date P, number of the prescription P and the name and address of the doctor prescribing the medicine P 7 It will thus be seen that I provide in a single article and at about the price of the usual label or prescription blank, a blank. which provides a ticket with full particulars for the druggist, a label to be attached to the box or bottle carrying full particulars and information, a ticket to be retained by the purchaser carrying a record, and finally a prescription ticket or blank which carries complete information and record, thus the label places with the druggist, purchaser and doctor a complete record and further provides a label which is attached to the bottle or box containing the medicine and provides a record, the amount of the dose, and the I exact time day or night when the dose is to be taken, thus producing a useful and desirable label which will positively prevent mistake or accident, and will prove a saving of time and expense to all persons concerned.

I claim:

1. A combined label and prescription blank consisting of a strip divided to proride a druggists ticket, a label to be attached to the bottle or box containing the medicine, a purchasers ticket, and a prescription blank, said tickets, label and prescription blank carrying the same record and information.

2. A combined label and prescription blank consisting of a strip divided to provide a druagists ticket, a label to be attached to the bottle or box containing the medicine, a purchasers ticket, and a prescription blank, said strip having on the label portion thereof means to indicate day and night time for taking the medicine.

3. A combined label and prescription blank, providing a label to be attached to the box or bottle containing the medicine, and tickets for the druggist, purchaser and doctor having a record similar to that upon the label portion of the blank.

4. A combined label and prescription blank, having an upper pair of strips, an intermediate label strip to be attached to the bottle, a record slip joined to said lab-cl strip and a lower strip forming a prescription blank, the five strips forming the blank each carrying the record number of the prescription and other identifying" matter.
