The turn-over section 5 6 has its rear face provided with printed matter identical in substance and arrangement with that on the front face of the data section, so thatl when turned under the latter by folding it along the line of scoring y-Q any particular space or zone of the data section will lie immediately over the corresponding space or zone of the turn-over section.

Under conditions of use the blanks above vertisement over the counter, writes or permits the advertiser to write, the desired copy oradvertisement upon the ruled lines vin the space 1 and thereafter notes in the spaces 25 and 26 the name and address of the advertiser, also filling in, it desired, the

By rny invention the advertiser is given a duplicate of the data required and utilized by the newspaper in displaying his advertisement on any particular day or number of days and this record not onlyv serves as a receipt for the payment of the charge nwhich it indicates but shows both the classi- ,licatiom name and number under which the -advertisement will appear 1n any particular issue ot the paper. By reason of this construction and arrangement of the material on the form or blank, the chances of errorare reduced to a minimum and all information required for any particular adv vertisement lis concentrated in a form Where the reception of a written ad and the. other end section and the middle section each being vertically divided and provided "with printed matter to constitute a receipt. section and a bookkeepefs section, the said printed matter on the tivo sections being similar in subst-ance and arrangement but upon opposite *faces ot the sheet, a dividing line between the latter two sections such that when the end section is folded back thereon against the middle section the corresponding printed matter upon the respective sections is brought into registration, and carbonized material upon the back ot vthe middle sectiont'or transferring to the said end section matter Written on the middie section Twhen' the said end section is folded thereunder.
