УШИВАТЕЛЬ МЯГКИХ ТКАНЕЙ содержащий трубчатый корпус, расположенный в нем шток, на хвостовике которого закреплена ручка дл  перемещени  штока, иглу дл  проведени  лигатурного материала, отличающийс  тем, что, с целью повышени  удобства наложени  швов на анатомически труднодоступный центральный конец уретры и повышени  качества швов, ушиватель снабжен дополнительной иглой дл  проведени  лигатуры, корпус выполнен с продольными пазами и выступами, а иглы - S-образной формы, при этом иглы расположены в продольных пазах и шарнирно установлены на штоке с возможностью взаимодействи  с выступами корпуса. Изобретение относитс  к медицине, а именно хирургическим сшивающим устройствам . Известен ушиватель м гких тканей, содержащий трубчатый корпус, расположенный в нем щток, на хвостовике которого закреплена ручка дл  перемещени  щтока, иглу дл  нроведени  лигатурного материала 1. Недостатком известного ушивател   вл етс  то, что с его помощью неудобно накладывать швы на автоматически труднодоступный центральный конец уретры, в св зи с чем щов получаетс  недостаточно качественным . Цель изобретени  - повышение удобства наложени  швов на анатомически труднодоступный центральный конец уретры и повышение качества швов. Указанна  цель достигаетс  тем, что в ушивателе м гких тканей, содержащем трубчатый корпус, расположенный в нем щток, на хвостовике которого закреплена ручка дл  перемещени  щтока, иглу дл  проведени  лигатурного материала, устройство снабжено дополнительной иглой дл  проведени  лигатуры, корпус выполнен с продольными пазами и выступами, а иглы - S-образной формы, при этом иглы расположены в продольных пазах и шарнирно установлены на штоке с возможностью взаимодействи  с выступами корпуса. На фиг. 1 изображенушиватель м гких тканей, разрез; на фиг. 2 - ушиватель при наложении шва. Ушиватель м гких тканей содержит трубчатый корпус 1, расположенный в нем щток 2 на хвостовике 3 которого закреплена ручка 4 дл  перемещени  щтока 2, иглу 5 дл  проведени  лигатурного материала 6. Устройство снабжено дополнительной иглой 7. Корпус 1 выполнен с продольными пазами 8 и выступами 9. Иглы 5 и 7 выполнены S-образной формы при этом они расположены в продольных пазах 8 и щарнирно установлены на штоке 2 с возможностью взаимодействи  с выступами 9 корпуса 1. Устройство работает следуюшим образом . После обнажени  центрального конца уретры 10 в просвет ее ввод т трубчатый корпус до соприкосновени  ограничител  11 с краем резецированной уретры. Затем хирург , одной рукой держась за ручку 4, не сдвига  корпус 1 вдоль уретры, большим пальцем этой же руки отжимает ручку 4 от корпуса I, тем самым выдвигает щток 2 из корпуса 1 на себ . При таком движении щтока иглы 5 и 7 выход т из корпуса и благодар  выступам 9 начинают раздвигатьс , приближа сь к стенке уретры. При дальнейшем выдвижении щтока (в этом случае ручка 4 продолжает выводитьс  из корпуса 1) острые концы игл прокалывают стенку уретры с окружающими ее ткан ми в двух диаметрально противоположных местах и, незначительно раздвинувшись, концы игл за счет их S-образной формы перемещаютс  почти параллельно продольной оси устройства , протаскива  при помощи отверстий 11 через проколотые стенки уретры лигатурную нить 6 в легкодоступную зону операционного пол . Как только острые концы игл попадут в легкодоступную зону, то изогнута  часть основани  игл скользит по выступам 9 и иглы дополнительно развод тс  на угол 10-30°, обеспечива  удобный доступ к лигатурной нити. Хирург крючком-зацепом 12 (или пинцетом) выводит свободные концы нити 6 из отверстий игл.. Далее совершаютс  обратные движени , т.е. хирург перемещает ручку 4 к корпусу 1. В этом случае иглы без нити начинают складыватьс  и выходить обратно из проколов стенки уретры и утапливаютс  в продольные пазы 8. После проведени  лигатурной нити трубчатую часть корпуса устройства с убранными в прорез х иглами осторожно извлекают из просвета уретры. После извлечени  корпуса устройства из просвета уретры свободный конец лигатуры, выход щей из просвета центрального конца уретры, с помощью обычной иглы провод т через просвет периферического конца уретры. Проведение лигатурной нити в других местах конца уретры производитс  аналогичным образом, после чего хирург, подт гива  нити и концы уретры, образует швы. Как правило, дл  сщивани  концов уретры накладываетс  4-6 швов. .Предложенное устройство по сравнению с базовым объектом позвол ет производить наложение швов на центральный конец уретры , расположенный в труднодоступном месте , причем качество наложени  швов всегда бывает высокое. Использование данного устройства позвол ет существенно облегчить работу хирурга за счет лучшего совмещени  концов уретры и значительно снизить процентр рецидивов структур.This invention relates to medicine, namely surgical stapling devices. This goal is achieved by the fact that in a soft tissue stitcher, comprising a tubular body, located therein a brush, on the shank of which a handle for moving the brush is fixed, a needle for carrying out a ligature material, the device is equipped with an additional needle for carrying out a ligature, the body is made with longitudinal grooves and protrusions, and the needles are S-shaped, while the needles are located in the longitudinal grooves and pivotally mounted on the stem with the ability to interact with the body projections. FIG. 1 shows a soft tissue blower, incision; in fig. 2 - stitcher when stitching. The soft tissue stitcher contains a tubular body 1, the brush 2 located therein, on the shank 3 of which the handle 4 is fixed to move the brush 2, the needle 5 for holding the alloy material 6. The device is equipped with an additional needle 7. The body 1 is made with longitudinal grooves 8 and protrusions 9 The needles 5 and 7 are S-shaped, while they are located in the longitudinal grooves 8 and are hingedly mounted on the stem 2 with the ability to interact with the projections 9 of the housing 1. The device works as follows. After exposing the central end of the urethra 10, the tubular body is inserted into the lumen until the limiter 11 contacts the edge of the resected urethra. Then the surgeon, holding the handle 4 with one hand, does not move the body 1 along the urethra, squeezes the handle 4 from the body I with the thumb of the same hand, thereby pushing the brush 2 out of the body 1 towards itself. With this movement of the flap, the needles 5 and 7 emerge from the housing and, thanks to the projections 9, begin to move apart, approaching the wall of the urethra. With further extension of the schock (in this case, the handle 4 continues to be removed from the housing 1), the sharp ends of the needles pierce the wall of the urethra with its surrounding tissues in two diametrically opposite places and, slightly apart, the ends of the needles move almost parallel to the longitudinal the axis of the device, pulling through the holes 11 through the punctured walls of the urethra, the ligature thread 6 into the easily accessible area of the surgical field. As soon as the sharp ends of the needles fall into an easily accessible area, the part of the base of the needles is bent and slides along the protrusions 9 and the needles are further divorced by an angle of 10-30 °, providing convenient access to the ligature thread. The hook-and-hook surgeon 12 (or tweezers) removes the free ends of the thread 6 from the needle holes. Next, reverse movements are performed, i.e. the surgeon moves the handle 4 to the housing 1. In this case, the needles without a thread begin to fold and exit from the punctures of the urethral wall and sink into the longitudinal grooves 8. After the ligature thread has been drawn, the tubular part of the device with the needles removed in the slots are carefully removed from the urethral lumen. After removing the device body from the lumen of the urethra, the free end of the ligature exiting from the lumen of the central end of the urethra is guided through a lumen of the peripheral end of the urethra using an ordinary needle. The ligature thread is carried out in other places at the end of the urethra in a similar manner, after which the surgeon, suture the thread and the ends of the urethra, forms sutures. Typically, 4-6 sutures are applied to suture the ends of the urethra. The proposed device, compared with the base object, allows suturing of the central end of the urethra located in a hard-to-reach place, and the quality of suturing is always high. The use of this device significantly simplifies the work of the surgeon due to better alignment of the ends of the urethra and significantly reduce the percentage of recurrences of the structures.
