Abstract

FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, namely to cardiology. Independent two-channel stimulation of the right pulmonary veins and the ipsilateral diaphragmatic nerve is performed using the Astrocard - CardioEfi II system. Stimulation of the Achieve electrode in the right pulmonary veins was performed by means of the main stimulator of the Astrocard - CardioEfi II system. Stimulation of ipsilateral diaphragmatic nerve - using 2 built-in stimulator of Astrocard - CardioEfi II system.
EFFECT: method enables simplifying the independent two-channel stimulation in pulmonary vein cryoballoon ablation, reducing the length of the procedure, improving the ablation efficiency and thereby reducing the need for repeated procedures, hospitalization rate and improving the patients' quality of life.

В настоящее время в патогенезе ФП наибольшее значение имеют данные об участии очагов с высокочастотной электрической активностью в муфтах легочных вен (ЛВ), электрическая изоляция которых посредством катетерной аблации позволяет устранить ФП (Haïssaguerre et al., 1998). Катетерная изоляция устьев ЛВ (ИЛВ) – основной метод лечения пациентов с симптомной ФП (Kirchhof et al., 2016)(Calkins et al., 2017). Достижение двунаправленной блокады проведения в области левое предсердие – легочная вена (ЛП-ЛВ) (блокада выхода – отсутствие проведения импульса от ЛВ в ЛП; блокада входа – отсутствие проведения импульса от ЛП в ЛВ) является электрофизиологической конечной точкой всех методик ИЛВ. На сегодняшний день радиочастотная аблация (РЧА) и криобаллонная аблация (КБА) ЛВ - 2 стандарта катетерного лечения ФП (Calkins et al., 2017) со сопоставимой эффективностью и безопасностью. Криобаллонная аблация ЛВ позволяет достичь полного циркулярного повреждения в устье ЛВ единой аппликацией (стандартный протокол) с помощью хладагента (оксид азота). Процесс изоляции контролируется во время процедуры в реальном времени. Метод КБА основан на принципах криобиологии. Под местной анестезией sol. Lidocaini 0.5% - 20 мл проводится пункция правой и левой бедренных вен. По методике Сельдингера[http://info-farm.ru/alphabet_index/m/metod-seldingera.html] устанавливается интродьюсер для транссептальной пункции SL0 (St. JudeMedical 8 Fr.) с транссептальной иглой BRK-1 (St. JudeMedical) и интродьюсер 10 Fr., через который в сердце проводится датчик для внутрисердечной эхокардиографии (ВСЭхоКГ). Под флюроскопическим (PhilipsAlluraXperFD 20 / Inova 4100 GEHealthcare) и ВСЭхоКГ (VividiSafelockCart, GEHealthcare) контролем (AcuNav 10-French, SiemensAG, Germany) в области овальной ямки выполняется транссептальная пункция. В/в вводится гепарин с расчетом 100Ед/кг. Далее датчик для ВСЭХО удаляется и в правый желудочек (ПЖ) проводится 10-ти полюсный диагностический электрод Webster (WebsterDecapolarDeflectableCatheter, BiosenseWebster, USA). На фоне сверхчастой желудочковой стимуляции (система Астрокард® - КардиоЭфи II - стимулятор) (длительность цикла (ДЦ) 250 мс) через интродьюсер для транссептальной пункции SL0 проводится прямое контрастирование ЛП и ЛВ. Далее 10-ти полюсный электрод Webster устанавливается в коронарный синус (КС). Интродьюсер для транссептальной пункции заменяется на сосудистый интродьюсер с гемостатическим клапаном FlexCath (Medtronic, USA, 12Fr) через который проводится криобаллон (ArcticFrontAdvance) с диагностическим электродом Achieve(AC, Medtronic, USA, 25mm), который устанавливается поочередно в правые и левые легочные вены. После регистрации потенциалов в устье ЛВ электрод позиционируется более дистально для поддержания криобаллона. Наличие окклюзии подтверждается ангиографически - вводится контраст дистальнее места окклюзии баллоном для изучения степени окклюзии. По стандартному протоколу выполняется криоаблация легочных вен в следующем порядке: левая верхняя легочная вена (ЛВЛВ), левая нижняя легочная вена (ЛНЛВ), правая нижняя легочная вена (ПНЛВ), правая верхняя легочная вена (ПВЛВ), по одной аппликации в каждой вене с длительностью 180-240 сек. Одновременно происходит регистрация биофизических и электрофизиологических параметров процедуры, позволяющих оценить эффективность воздействия. При аблации в правых ЛВ с 10-ти полюсного диагностического электрода, установленного в ВПВ выполняется стимуляция ипсилатерального диафрагмального нерва (2000 мс, 20 мА) с целью контроля функции диафрагмального нерва (2-й стимулятор системы Астрокард® - КардиоЭфи II, фиг.№4). Пример №1. Пациентка К., 63 лет, с частыми, плохо переносимыми, резистентные к ААТ приступами ФП поступила в клинику для проведения катетерного лечения ФП. При позиционировании криобаллона в устье ПВЛВ на электроде Achieve регистрировалась спайковая активность. После достижения стабильной окклюзии криобаллоном начата аппликация. Несмотря на удовлетворительные параметры криоаблации (стабильная окклюзия, снижение температуры криобаллона до - 40°C), изоляция ПВЛВ до 100 сек. воздействия не наступила (неэффективное воздействие) и аппликация была остановлена. При более дистальном положении Achieve потенциалы отсутствовали, при стимуляции отмечался захват муфты ПВЛВ с предсердным ответом. После репозиционирования баллона и достижения стабильной окклюзии в ПВЛВ была выполнена эффективная криоаблация с развитием блокады выхода на 52-й сек. с использованием способа параллельной стимуляции диафрагмального нерва и электрода в ПВЛВ с применением системы Астрокард® - КардиоЭфи II (фиг.6).Example No. 1. Patient K., 63 years old, with frequent, poorly tolerated, AAT-resistant attacks of AF, was admitted to the clinic for catheter treatment of AF. Способ одномоментной двухканальной стимуляции при криобаллонной аблации легочных вен, включающий независимую двухканальную стимуляцию правых легочных вен и ипсилатерального диафрагмального нерва с применением системы Астрокард - КардиоЭфи II, отличающийся тем, что стимуляция электрода Achieve в правых легочных венах проводилась посредством основного стимулятора системы Астрокард - КардиоЭфи II, а стимуляция ипсилатерального диафрагмального нерва - с применением 2-го встроенного стимулятора системы Астрокард - КардиоЭфи II.A method of simultaneous two-channel stimulation during cryoballon ablation of pulmonary veins, including independent two-channel stimulation of the right pulmonary veins and ipsilateral phrenic nerve using the Astrocard-CardioEfi II system, characterized in that the Achieve electrode in the right pulmonary veins was stimulated using the main Astrocardi-Cardi system, and stimulation of the ipsilateral phrenic nerve - using the 2nd built-in stimulator of the Astrocard system - CardioEfi II.
