Одним из наиболее эффективных, доступных и современных путей хирургического решения проблемы коксартроза стало эндопротезирование тазобедренного сустава. Оно связано со значительным уровнем послеоперационной боли, возникающей в результате повреждения тканей при хирургическом доступе. Послеоперационная боль препятствует ранней мобилизации и реабилитации пациента, а также восстановлению в целом [1], в связи с чем в клинической практике активно применяют различные методики обезболивания: системное применение НПВП и анальгетиков, а при выраженном болевом синдроме - опиоидов, как в монотерапии, так и в комбинации [2]. Последнее время все большее распространение получает локальная инфильтрационная анестезия, являющаяся составной частью протокола мультимодальной анестезии, что обеспечивает эффективную анальгезию, большую удовлетворенность пациентов, более раннюю выписку из стационара по сравнению с традиционными методами аналгезии [3].One of the most effective, affordable, and modern ways of surgical solution to the problem of coxarthrosis is endoprosthesis replacement of the hip joint. Recently, local infiltration anesthesia, which is an integral part of the multimodal anesthesia protocol, is becoming more widespread, which provides effective analgesia, greater patient satisfaction, earlier discharge from the hospital compared to traditional methods of analgesia [3].

Лекарственным коктейлем, состоящим из 50 мл 0,75% раствора ропивакаина и 2 мл 0,25% раствора дексаметазона, используя аспирационную пробу, были последовательно болюсно инфильтрированы капсула и надкостница, сухожилие большой ягодичной мышцы, верхний глубокий слой отводящих мышц, передние участки надкостницы бедренной кости, латеральная прямая мышца бедра, малая и средняя ягодичные мышцы, напрягатель широкой фасции бедра, большая ягодичная мышца, подкожная жировая клетчатка, кожа. Рана послойно ушита. Время манипуляции составило 160 сек.
