Abstract
Description
The invention relates to medicine, in particular to dentistry, and is intended to restore the crown of multi-rooted teeth.
Anatomical restoration in the lateral group of teeth is more important for the functioning of the dentition, although the aesthetic component of the work performed is also important. Functionally, the lateral sector is largely responsible for fixing the height of the bite and for its stabilization due to fissure-tubercular contact, which affects the function of the paired temporomandibular joint system. Equally important is the importance of the surface topography for the mechanical processing of the food lump, its preparation for further fermentation in the digestive system. Thus, one of the significant areas in which modern dentistry is developing is the development and implementation of techniques that allow the most complete restoration of the anatomical structure of the chewing surface. Modeling of the masticatory surface should be considered as one of the parts of the restoration of the shape of the crown of the tooth.
A known method of restoration of tooth crowns, including modeling stereotypes of the crown parts of the teeth [1]. This method of modeling the crown of the tooth does not take into account the individual microrelief of the restored surface, which affects the aesthetic and functional results. The implementation of restoration methods by constant rotation of the manufactured model is carried out only in the conditions of a denture laboratory. It is impossible to perform appropriate manipulations in the patient’s oral cavity.
Known research in the field of processes of separation and cutting food by the frontal group of teeth [2, 3, 4]. But the existing methods of restoration of the crown part of the incisors do not provide the full restoration of the function of this group of teeth.
[5].Closest to the proposed method is a method of restoration of the crown of the teeth, using modeling of the anatomical features of the tooth surface, in which a modular reconstruction of the tooth monolith is carried out, while the modules are formed in the form of fangs - odontomers, each of which is built within the boundaries of a cell of a conditional mesh that simulates the fissure of the first the order that is previously applied to the chewing surface of the simulated tooth [5].
The disadvantages of the prototype method are the low functional qualities of the restoration of multi-rooted teeth, the high complexity of the process.
The objective of the invention is to reduce the complexity while improving the functional qualities of the restoration of multi-rooted teeth.
The problem is achieved in that in the method of restoration of the crown part of molars based on modular technologies, including the operation of modular reconstruction of the tooth monolith, which consists in forming the main modules in the form of a fractal unit of the canine odontomer, according to the invention, additional modules are formed in the form of a part of the fractal units of the canine odontomer, the volumetric relief of the chewing surface of the molar is divided into fragments consisting of basic and additional modules, they are made from a composite material by industrial method, each fragment is placed for storage in a sterile packaging corresponding to a certain shape of the part of the molar that needs to be restored, then in the process of restoration of the missing fragment within the crown part of the molar, hard tissues are prepared, their medication, adhesive technology, in place a defect impose a layer of fluid composite, within which the desired pre-made fragment is placed, adapt it to existing Verdi the tooth and polymerized further step is carried out grinding and polishing.
The technical result achieved is the improvement of the aesthetic and functional qualities of the restored tooth, as well as the reduction of time spent during restoration.
4.6.In FIG. 1 shows a model of tooth 4.6 with a prepared cavity. In FIG. Figure 2 shows the layered arrangement of the modules, the first layer of which is fixed colored modules. In FIG. 3. presents the location of the modules of the second layer, fixed on top of the first layer. Selected modules are placed along the fissure of the first order. In FIG. 4 presents a form corresponding to the morphology of the tooth 4.6.
The proposed method works as follows. When restoring missing tooth tissues based on modular technologies, the canine odontomer is used as the main module, or part of it is an additional module manufactured in advance by an industrial method. When modeling the crown of the tooth, it is necessary to place on its basis not only fractal units corresponding to the main tubercles, but also fill the remaining space in the area of contact surfaces by operating with additional fractal units or individual parts of this fractal unit. The volumetric relief of the chewing surface of the molar is represented by fragments made of composite material, stored in a specially sterile package, corresponding to the specific shape of any part of the molar that needs to be restored.
When restoring the missing fragment within the coronal part of the molar, the operator carries out the preparation of hard tissues, drug treatment, adhesive technology. Within the defect, a thin layer of a fluid composite is superimposed, within which a fragment of a composite material previously manufactured by the industrial method is located, adapts to existing hard tooth tissues and polymerizes. Then, as usual, the grinding and polishing stage is carried out.
1-4.A graphical representation of the proposed method is presented in FIG. 1-4.
Clinical case.
Patient S. Turned to the clinic of therapeutic dentistry for the purpose of rehabilitation of the oral cavity. Objectively observed the presence of hidden carious cavities in the tooth 4.6. During the preparation of tooth 4.6, a defect is formed, which must be replenished. Preforms are prepared that are made in advance from the filling material in the form of basic modules - odontomer fangs, the number of basic modules depends on the number of tooth tubercles, in this case there are 5 of them, but additional modules can be added if necessary. The blanks are fixed in the prepared cavity on the composite. Polymerization and bite testing take place, followed by the grinding and polishing process.
