Изобретение относится к области медицины, а именно инфекционным болезням, и может быть использовано для профилактики анемии, развивающейся у больных хроническим гепатитом С (ХГС) в результате проведения комбинированной противовирусной терапии (КПТ).The invention relates to medicine, namely to infectious diseases, and can be used to prevent anemia developing in patients with chronic hepatitis C (CHC) as a result of combined antiviral therapy (CBT).
ХГС является, как известно, одним из наиболее распространенных инфекционных заболеваний человека и характеризуется выраженной склонностью к формированию цирроза печени и гепатоцеллюлярной карциномы [8, 9]. Лишь применение современной КПТ с использованием препаратов интерферона-α (ИФН-α) и рибавирина позволяет добиться у многих пациентов полной элиминации вируса гепатита С (HCV) и обеспечить тем самым предупреждение неблагоприятных исходов заболевания [1, 2, 7].CHC is, as you know, one of the most common infectious diseases in humans and is characterized by a pronounced tendency to form liver cirrhosis and hepatocellular carcinoma [8, 9]. Актуальной проблемой противовирусной терапии ХГС остается, однако, развитие такого нежелательного явления, как КПТ-ассоциированная анемия [3-6, 10, 12]. В зависимости от концентрации гемоглобина (Hb) в крови различают легкую (Hb 10,0-11,9 г/дл), умеренную (Hb 8,0-9,9 г/дл) и тяжелую (Hb<8,0 г/дл) степени КПТ-ассоциированной анемии [4]. Согласно современным данным развитие тяжелой анемии является одним из лимитирующих факторов достижения устойчивого вирусологического ответа у лиц, страдающих хронической HCV-инфекцией [3].The actual problem of antiviral therapy of chronic hepatitis C, however, remains the development of such an undesirable phenomenon as CBT-associated anemia [3-6, 10, 12]. До настоящего времени способы профилактики КПТ-ассоциированной анемии у больных ХГС не разработаны. Существующие же способы коррекции данного нежелательного явления направлены на борьбу с уже развившейся КПТ-ассоциированной анемией [1, 4-6, 10, 11, 13]. При этом первый метод заключается либо в уменьшении терапевтической дозы, либо же в полном прекращении приема препаратов рибавирина, что в обоих случаях приводит к резкому снижению эффективности КПТ [1, 4-6]. Второй способ предусматривает назначение пациентам фармакологических средств, относящихся к группе кроветворных факторов роста, в частности - препаратов рекомбинантного эритропоэтина [4, 10, 11, 13]. Недостатками данного метода являются отсутствие четких алгоритмов по применению препаратов рекомбинантного эритропоэтина у больных ХГС [11], развитие дополнительных побочных эффектов и повышение общей стоимости курсового лечения.To date, methods for the prevention of CBT-associated anemia in patients with chronic hepatitis C have not been developed. Existing methods for correcting this undesirable phenomenon are aimed at combating the already developed CBT-associated anemia [1, 4-6, 10, 11, 13]. In this case, the first method consists in either reducing the therapeutic dose, or completely stop taking ribavirin preparations, which in both cases leads to a sharp decrease in the effectiveness of CBT [1, 4-6]. The second method involves prescribing to patients pharmacological agents belonging to the group of hematopoietic growth factors, in particular, recombinant erythropoietin preparations [4, 10, 11, 13]. The disadvantages of this method are the lack of clear algorithms for the use of recombinant erythropoietin drugs in patients with chronic hepatitis C [11], the development of additional side effects and an increase in the total cost of course treatment.
Цель изобретения - разработать способ профилактики КПТ-ассоциированной анемии у больных хроническим гепатитом С.The purpose of the invention is to develop a method for the prevention of CBT-associated anemia in patients with chronic hepatitis C.
Техническим результатом предлагаемого изобретения является использование гемолизата аутокрови (ГАК) у больных ХГС для предотвращения развития анемии в ходе КПТ.The technical result of the invention is the use of autologous hemolysate (HAC) in patients with chronic hepatitis C to prevent the development of anemia during CBT.
Поставленная задача достигается тем, что, в соответствии с изобретением, больным ХГС ровно за 10 дней до начала КПТ проводят ежедневное внутривенное введение 20,0 мл гемолизата аутокрови (ГАК); с момента начала и на протяжении всего курса КПТ внутривенное введение ГАК продолжают в той же дозе, но уже один раз в 7 дней, при этом в процессе внутривенного введения ГАК у пациента из локтевой вены забирают 10,0 мл крови в шприц объемом 20,0 мл, предварительно заполненный на 50% дистиллированной водой, а затем, не вынимая пункционной иглы, образовавшийся в шприце ГАК, вводят обратно в сосудистое русло.The task is achieved in that, in accordance with the invention, patients with chronic hepatitis C exactly 10 days before the start of CBT spend daily intravenous administration of 20.0 ml of hemolysate autologous blood (HAC); from the beginning and throughout the course of CBT, intravenous administration of HAC continues at the same dose, but once every 7 days, while during the intravenous administration of HAC, 10.0 ml of blood is taken from the cubital vein into a 20.0 syringe ml, pre-filled with 50% distilled water, and then, without removing the puncture needle, formed in the syringe HAC, injected back into the vascular bed.
Заявляемый способ осуществляется следующим образом. Больным ХГС, нуждающимся в проведении КПТ и имеющим при этом высокий риск развития КПТ-ассоциированной анемии, ровно за 10 дней до начала противовирусной терапии начинают проводить ежедневное внутривенное введение 20,0 мл ГАК. С момента начала КПТ внутривенное введение ГАК продолжают в той же дозе, но уже один раз в 7 дней на протяжении всего курса противовирусной терапии. Методика внутривенного введения ГАК заключается в том, что пациенту на область нижней трети плеча накладывают жгут, пунктируют локтевую вену и в шприц объемом 20,0 мл, предварительно заполненным на 50% дистиллированной водой, забирают 10,0 мл крови. После этого, не вынимая пункционной иглы, снимают жгут и образовавшийся в шприце ГАК вводят обратно в сосудистое русло.The inventive method is as follows. Для объективной оценки эффективности предложенного нами способа методом рандомизации, по случайному признаку были сформированы две группы больных ХГС, нуждавшихся в противовирусном лечении. 1-я группа включала в себя 94, вторая - 113 человек. По своим исходным клинико-лабораторным показателям обе группы, как и следовало ожидать, были вполне сопоставимы.For an objective assessment of the effectiveness of our proposed method by randomization, two groups of patients with chronic hepatitis C who needed antiviral treatment were formed on a random basis. Пациентам 1-й группы этиотропная терапия ХГС проводилась по стандартной схеме. Во 2-й группе больных ХГС, назначение препаратов ИНФ-α и рибавирина осуществлялось на фоне комплексного приема ГАК в соответствии с заявляемым способом.Patients of the 1st group etiotropic therapy of chronic hepatitis C was carried out according to the standard scheme. Как видно на представленной ниже таблице, удельный вес больных ХГС с развившейся КПТ-ассоциированной анемией в 1-й и во 2-й группе оказался практически идентичным (>0,05). Однако процентное содержание больных ХГС с легкой степенью КПТ-ассоциированной анемии оказалось достоверно выше во 2-й группе испытуемых лиц (р<0,01). Частота же развития умеренной и тяжелой степени КПТ-ассоциированной анемии у пациентов, получавших ГАК, наоборот, была значительно ниже (р<0,05 и р<0,01 соответственно).As can be seen in the table below, the proportion of patients with chronic hepatitis C with developed CBT-associated anemia in the 1st and 2nd group was almost identical (> 0.05). Таким образом, проведенный статистический анализ показал, что клиническая эффективность ГАК заключается, прежде всего, в достоверном снижении удельного веса пациентов с тяжелой КПТ-ассоциированной анемией, которая и оказывает негативное влияние на частоту достижения устойчивого вирусологического ответа у больных ХГС.Thus, the statistical analysis showed that the clinical efficacy of HAC consists, first of all, in a significant decrease in the specific gravity of patients with severe CBT-associated anemia, which negatively affects the frequency of achieving a stable virologic response in patients with chronic hepatitis C.
В результате выполненного сравнительного анализа было установлено, что частота развития умеренной и тяжелой степени анемии в группе пациентов, получавших КПТ параллельно с ГАК, оказалась достоверно ниже, нежели у лиц, получавших КПТ по стандартной схеме (р<0,05 и р<0,01 соответственно).As a result of the comparative analysis, it was found that the incidence of moderate to severe anemia in the group of patients receiving CBT in parallel with HAC was significantly lower than in individuals receiving CBT according to the standard scheme (p <0.05 and p <0, 01 respectively).
Практическая реализуемость изобретения иллюстрируется следующим клиническим примером.The feasibility of the invention is illustrated by the following clinical example.
Больная К, 59 лет, поступила в Гепатологический центр г. Ростова-на-Дону с верифицированным диагнозом - ХГС, генотип HCV - 1b, для проведения начального этапа противовирусной терапии. Исходные показатели (до начала лечения) оказались равны: индекс массы тела 37, уровень вирусной нагрузки - 3 200000 МЕ/мл; индекс фиброза - 4 балла (F4). Больной была назначена КПТ по схеме: ребетол - 1200 мг в сутки перорально, пегасис - 180 мкг в неделю подкожно. Планируемая продолжительность противовирусного лечения - 48 недель. Концентрация гемоглобина (Hb) в крови на старте терапии составила 12,1 г/дл. Учитывая высокую степень вероятности развития у данной пациентки тяжелой степени КПТ-ассоциированной анемии, противовирусная терапия выполнялась ей параллельно с внутривенным введением ГАК в соответствии заявляемым способом. Минимальное значение Нb, которое было зафиксировано у больной за весь период проведения КПТ, составило 11,3 г/дл, что соответствовало легкой степени анемии. Таким образом, благодаря разработанным профилактическим мероприятиям, нам удалось предотвратить развитие у пациентки ожидаемой тяжелой степени КПТ-ассоциированной анемии, что позволило провести ей полный курс КПТ и получить в итоге устойчивый вирусологический ответ через 24 недели после завершения противовирусного лечения.Patient K, 59 years old, was admitted to the Hepatology Center of Rostov-on-Don with a verified diagnosis of CHC, the HCV genotype is 1b, for the initial stage of antiviral therapy. Given the high likelihood of developing a severe degree of CBT-associated anemia in this patient, antiviral therapy was carried out in parallel with the intravenous administration of HAC in accordance with the claimed method. The minimum value of Hb, which was recorded in the patient for the entire period of CBT, was 11.3 g / dl, which corresponded to mild anemia. Thus, thanks to the developed preventive measures, we were able to prevent the patient from developing the expected severe degree of CBT-associated anemia, which allowed her to undergo a full course of CBT and finally obtain a stable virological response 24 weeks after completion of the antiviral treatment.
