Abstract
Description
Изобретение относится к медицине, а именно к абдоминальной хирургии, и может быть использовано для восстановления проходимости главного панкреатического протока (ГПП) поджелудочной железы (ПЖ).The invention relates to medicine, namely to abdominal surgery, and can be used to restore patency of the main pancreatic duct (GLP) of the pancreas (pancreas).
- с.107).There is a method of restoring patency of GLP of the pancreas by endoscopic retrograde stenting of GLP (Mantserov M.P. Features of retrograde cholangiopancreatography in the diagnosis and treatment of pathology of the pancreatobiliary system and ways to increase its effectiveness: Dis .... Candidate of Medical Sciences. - Moscow, 2004. - p.107 )
Недостатком способа является ограниченное его применение в силу анатомических особенностей зоны большого дуоденального сосочка (БДС), таких как стриктуры БДС, дивертикулы, инфильтрация, сдавление извне двенадцатиперстной кишки, а также после перенесенных ранее операций на желудке (резекция по Бильрот II).The disadvantage of this method is its limited use due to the anatomical features of the zone of the large duodenal papilla (BDS), such as strictures of the BDS, diverticula, infiltration, compression from the outside of the duodenum, as well as after previous operations on the stomach (resection by Billroth II).
- с. 225-226).A known method of restoring patency of the GLP of the pancreas by percutaneous antegrade drainage and subsequent stenting under ultrasound (ultrasound) and radiological (Rg) control with access to the body or tail of the pancreas. The method is minimally invasive and in cases of expanded GLP in the region of the body or tail of the pancreas (about 1 cm or more), surgical intervention in the experience of percutaneous puncture-draining operations is not difficult (Pogrebnyakov V.Yu., Ivanov P.A., Goncharov S.A. X-ray surgical puncture stenting of cicatricial strictures of the Wirsung duct // Scientific Medical Bulletin of the Central Chernozem Region, No. 30. - Voronezh, 2007. - pp. 225-226).
The disadvantage of this method is that in cases where there is no sufficient expansion of GLP in the body and tail of the pancreas, often due to sclerotic changes in pancreatic tissue in chronic pancreatitis, puncture access and subsequent manipulations present certain technical difficulties and, as a rule, are not feasible.
The technical result of the invention is the creation of a method that allows drainage and subsequent stenting of the GLP in cases where the expansion of the GLP in the body and tail of the pancreas is insufficient (less than 1 cm) and does not allow the necessary manipulations.
The specified technical result is achieved by the fact that in the method of restoring patency of the pancreatic hyperplasia of the pancreas by percutaneous antegrade drainage and stenting under ultrasound and radiological control with access to the pancreas body or tail, according to the invention, in the absence of sufficient expansion of the GLP in the pancreatic body or tail region, puncture is preliminarily performed and install drainage in the expansion of the GLP in the area of the pancreatic head or in the retention cyst in the area of the pancreatic head, then through the installed drainage carry out hydra vlicheskoe extension in the area of the body and tail of the pancreas by GLP tight filling water soluble contrast agents, followed by percutaneous drainage and antegrade stenting with access to areas of the body or tail of the pancreas.
The tight filling of the GLP of the pancreas with a water-soluble contrast agent provides sufficient expansion of the GLP in the body and tail of the pancreas and allows drainage and subsequent stenting according to the standard method with access to the body or tail of the pancreas. At the same time, the introduction of a contrast agent provides x-ray control during the operation.
The method is illustrated by the operation diagrams in FIG. 1, 2, where:
На фиг. 1 представлено чрескожное дренирование регенционной кисты в области головки ПЖ;In FIG. 1 shows percutaneous drainage of a regenerative cyst in the area of the pancreatic head;
In FIG. Figure 2 shows transdermal antegrade drainage of a previously hydraulically expanded pancreatic GLP under ultrasound and Rg control.
В качестве водорастворимого контрастного препарата используют 38% раствор Урографина (20,0-30,0 мл).The method is carried out, for example, as follows. In the absence of sufficient expansion of
Then, under the control of ultrasound and Rg, a puncture is performed and
The method is confirmed by the following clinical examples.
Example 1. Patient F., 52 g. Diagnosis: chronic recurrent pancreatitis with symptoms of pancreatic hypertension. Indications for the drainage of GLP were the need to eliminate ductal hypertension, as well as the restoration of patency of the GLP of the pancreas. Due to the lack of the possibility of retrograde manipulations due to the manifestations of duodenitis and deformation in the BDS zone, percutaneous antegrade external-internal endopancreatic drainage under ultrasound, Rg-control is recommended.
The expansion of the GLP in the area of the pancreatic head to 1.3 cm, retention cysts in the area of the pancreatic head 4.3 × 4.9 cm.
Given the insufficient expansion (0.3-0.4 cm) of GLP in the area of the pancreas tail and the presence of retention cysts in the area of the pancreas head, the first step was the puncture and percutaneous drainage of the cyst under ultrasound control (it was technically easier to drain the retention cyst in the region of the pancreas head) . Then, through the installed drainage, the main pancreatic duct of the pancreas was tightly filled by introducing 30.0 ml of a 38% solution of Urografin. After hydraulic expansion of the GLP in the region of the body and tail (more than 1 cm), percutaneous antegrade external-internal endopancreatic drainage was performed.
The immediate postoperative period was uneventful. The outer end of the drainage is blocked. During the control ultrasound, the expansion of GLP in the area of the pancreatic head decreased from 1.3 to 0.4 cm. After drainage, an improvement in well-being was noted, the pain syndrome was stopped.
Six months later, with a control ultrasound, expansion of the GLP and retention cysts in the area of the pancreatic head was not detected. Instead of endopancreatic drainage, percutaneous antegrade stenting (self-expanding nitilone stent) was performed with Rg-controlled GLP, the control drainage was removed, the patient was discharged.
Example 2. Patient, 48 years old, with a diagnosis of chronic calcifying pancreatitis (pain form). MRI proved the presence of stricture in the terminal section of GLP, signs of pancreatic hypertension. With MRI and ultrasound of the abdominal cavity in the area of the pancreas head, GLP is expanded to 0.9 cm, in the area of the body and tail of the pancreas - 0.3-0.4 cm. Given the presence of pancreatic hypertension, pain, and the lack of the possibility of retrograde endoscopic manipulations (peripapillary diverticulum); indications for percutaneous antegrade decompression of GPP have been made.
In connection with the peculiarities of the expansion of the GLP, the first stage was performed under ultrasound control to puncture the GLP in the area of the pancreatic head. Through the installed needle under Rg control, the main pancreatic duct of the pancreas was tightly filled with 38% Urografinum in a volume of 25.0 ml. After hydraulic expansion of the GLP in the region of the body and tail (0.8-0.9 cm), transdermal antegrade external endopancreatic drainage was performed through the GLP at the border of the body and tail of the pancreas (an attempt to guide the conductor through stricture and turging of the latter remained without effect, and therefore left external endopancreatic drainage with internal pig drainage fixation system).
The immediate postoperative period was uneventful. Drainage products 250.0-300.0 ml of pancreatic juice per day. Under the control of ultrasound, the expansion of GLP in the region of the pancreatic head decreased from 0.9 to 0.3 cm. After drainage, an improvement in well-being was noted, the pain syndrome was stopped.
1 month after external drainage, given the impossibility of passing through the stricture of the GLP, as well as the transgastric course of the established drainage, in order to form pancreaticogastric anastomosis, the outer end of the drainage under endoscopic control was transferred to the stomach.
At the follow-up visit after 3 months, ultrasound - there are no signs of pancreatic hypertension, the pain syndrome did not resume. The drainage was not removed (in order to form an anastomosis, it is necessary to stand for a long time frame drainage - about 1 year).
The method was applied in 4 patients. All operations were successful, no complications were noted.
The proposed method is minimally invasive and allows you to restore patency of the GLP of the pancreas in cases where, due to insufficient expansion of the GLP in the body or tail of the pancreas, drainage and subsequent stenting of the GLP cannot be carried out.
