Цель исследования показать, что своевременное введения внутривенно беременным человеческого иммуноглобулина при обострении во втором триместре герпес-вирусной инфекции снижает к концу беременности содержание в периферической крови содержание перекисей жирных кислот.The aim of the study is to show that timely administration of a human immunoglobulin intravenously to pregnant women during exacerbation of herpes virus infection in the second trimester reduces the content of fatty acid peroxides in peripheral blood by the end of pregnancy.
Установлено, что с нарушением метаболизма в организме появляется большое количество перекисей жирных кислот [1, 2, 3]. Инфекционные процессы в организме приводят к повреждению клеточных мембран во многих органах; появляется снижение содержания в них фосфатидилэтаноламина и возрастает фракция лизофосфатидилхолина [5]. Герпес-вирусная инфекция нарушает устойчивость мембран эритроцитов, в них увеличивается содержание перекисей жирных кислот [4, 5].It has been established that with a metabolic disorder, a large number of fatty acid peroxides appears in the body [1, 2, 3]. Infectious processes in the body lead to damage to cell membranes in many organs; a decrease in the content of phosphatidylethanolamine appears in them and the fraction of lysophosphatidylcholine increases [5]. Herpes-virus infection violates the stability of erythrocyte membranes, they increase the content of fatty acid peroxides [4, 5].
Нарастание неустойчивости мембран эритроцитов приводит к увеличению в периферической крови беременных процентного содержания дегенеративных форм этих клеток, что вызывает опасность развития гипоксии. Необходимы меры, предотвращающие развитие герпес-вирусной инфекции в период беременности, приводящие к снижению в периферической крови перекисей жирных кислот. Поэтому задачей нашего исследования явилось применение средства, направленного на снижение активности герпес-вирусной инфекции и его влияния на выработку перекисей жирных кислот.The increase in the instability of erythrocyte membranes leads to an increase in the percentage of degenerative forms of these cells in the peripheral blood of pregnant women, which causes the risk of hypoxia. Measures are needed to prevent the development of herpes virus infection during pregnancy, leading to a decrease in fatty acid peroxides in the peripheral blood. Therefore, the objective of our study was the use of a drug aimed at reducing the activity of herpes virus infection and its effect on the production of fatty acid peroxides.
Исследования проводились на протяжении II и III триместров гестации у 20 беременных, у которых отмечалась вспышка герпес-вирусной инфекции на 12 неделе гестации. После лечения проводилось повторное обследование беременных с целью выявления содержания в крови перекисей жирных кислот.Studies were conducted during the second and third trimesters of gestation in 20 pregnant women who had an outbreak of herpes virus infection at 12 weeks of gestation. After treatment, a repeated examination of pregnant women was carried out in order to detect the content of fatty acid peroxides in the blood.
Заявленный способ имеет следующие приемы:The claimed method has the following techniques:
1. Исследования проводились на базе стационара акушерского отделения ФГБУ Дальневосточного научного центра физиологии и патологии дыхания СО РАМН. Обследовано 20 беременных, у которых отмечалось обострение герпес-вирусной инфекции во втором триместре на 12 неделе гестации; титр антител к вирусу герпеса 1:6400. Контролем служили 10 беременных, которые не болели на протяжении всего периода гестации. Все исследования были проведены с учетом требований Хельсинской декларации Всемирной ассоциации «Этические принципы проведения научных медицинских исследований с участием человека» с поправками 2000 г. и «Правилами клинической практики в Российской Федерации», утвержденными Приказом Минздрава РФ от 19.06.2003 г. №266.1. The studies were carried out on the basis of the hospital of the obstetric department of the Federal State Budgetary Institution of the Far Eastern Scientific Center for Physiology and Respiratory Pathology SB RAMS. We examined 20 pregnant women who exacerbated herpes virus infection in the second trimester at 12 weeks of gestation; herpes virus antibody titer 1: 6400. The control was 10 pregnant women who were not ill during the entire gestation period. All studies were carried out taking into account the requirements of the Helsinki Declaration of the World Association “Ethical Principles for Conducting Scientific Medical Research with Human Participation” as amended in 2000 and the “Rules of Clinical Practice in the Russian Federation”, approved by Order of the Ministry of Health of the Russian Federation of June 19, 2003 No. 266.
2. Титр антител к вирусу герпеса анализировался с позиций активности герпес-вирусной инфекции в организме беременной по величине четырехкратного и более нарастания титра антител IgG к ВПГ-1, выявляемого в динамике через 10 дней в парных сыворотках. Титр антител определяли иммуноферментным методом на спектрофотометре «Stat-Fax» (USA).2. The titer of antibodies to the herpes virus was analyzed from the standpoint of the activity of herpes virus infection in the pregnant woman’s body by the magnitude of four or more increase in the titer of IgG antibodies to HSV-1, detected in dynamics after 10 days in paired sera. The antibody titer was determined by enzyme immunoassay on a Stat-Fax spectrophotometer (USA).
3. Активность перекисей жирных кислот в периферической крови беременных определяли путем определения активности моноаминоксидазы (МДА) по методу Стальной И.Д. и Гариашвили Т.Г. [6].3. The activity of fatty acid peroxides in the peripheral blood of pregnant women was determined by determining the activity of monoamine oxidase (MDA) according to the method of Steel ID and Gariashvili T.G. [6].
4. Для лечения применяли стандартно выпускаемый человеческий иммуноглобулин, который вводили внутривенно капельно по 25 мл трижды через день на 16 неделе беременности и повторно на 24 неделе также по 25 мл внутривенно капельно трижды через день. Применяли иммуноглобулин человека, выпущенный Нижегородским предприятием «МинБио», филиал ФГУП НПО «Микроген» МЗ РФ.4. For treatment, a standardly released human immunoglobulin was used, which was administered intravenously 25 ml three times every other day at 16 weeks of gestation and again at 24 weeks also 25 ml intravenously three times every other day. We used human immunoglobulin produced by the Nizhny Novgorod enterprise "MinBio", a branch of the FSUE NPO "Microgen" of the Ministry of Health of the Russian Federation.
Исследования показали, что после обострения герпес-вирусной инфекции у беременной на 12 неделе гестации определяли в периферической крови титр антител к вирусу герпеса 1:6400, а содержание моноаминооксидазы составляло 1,08±0,09 мкмоль/л после лечения иммуноглобулином на 16 и на 24 неделях титр антител к вирусу герпеса на 36 неделе гестации снизился до 1:1600, а количество МДА в периферической крови составило 0,64±0,02 мкмоль/л. Таким образом, применяя своевременно для лечения беременной человеческий иммуноглобулин при обострении герпес-вирусной инфекции во II триместре, можно понизить не только агрессивность герпес-вирусной инфекции, но и снизить содержание в крови перекисей жирных кислот, уменьшая тем самым риск повреждения клеточных мембран органов фетоплацентарной системы.Thus, applying human immunoglobulin in a timely manner to treat a pregnant woman with exacerbation of herpes virus infection in the second trimester, it is possible to reduce not only the aggressiveness of herpes virus infection, but also reduce the content of fatty acid peroxides in the blood, thereby reducing the risk of damage to the cell membranes of the organs of the fetoplacental system .
