Description

The invention relates to a medical material based on natural polysaccharides containing an interpolymer polyelectrolyte complex of a cationic or amphoteric linear polysaccharide with an anionic linear polysaccharide chemically cross-linked by polyfunctional aldehydes or epoxy compounds, which has a hemostatic and reparative effect and can be used both for therapeutic and therapeutic purposes. The claimed material may be a composition for creating various types of dressings (dressings, wound dressings, dressing bags, tampons) in the form of gels, films, sponges, colloidal solutions, which may additionally contain compositions for treating wounds, burns, medicines, dermatological compositions, plant extracts, as well as serve as the basis for cosmetic masks and pads for compresses.

In vitro experiments revealed nonspecific hemostatic properties of polysaccharides, which are confirmed by experiments when stopping parenchymal bleeding from liver and spleen wounds, bone bleeding from sternotomic wounds and ulcerative bleeding from stomach ulcers, without causing a reaction from surrounding tissues.

The stimulating effect of polysaccharides on regenerative processes in the wound has also been established, which is expressed in accelerating the growth of granulation tissue, stimulating marginal and islet epithelization, creating favorable conditions for the migration of epithelial cells, vascular germination and scarless wound healing.

Known compositions for the treatment of skin lesions based on natural polysaccharides (RU 2091082 C1, RU 1814764 C1). However, they do not have a wide area of use, namely, sufficient effectiveness in stopping bleeding (hemostasis) and at the same time in stimulating the reparative processes that occur in wounds.

Interpolymer polyelectrolyte complexes, the stability of which depends on the pH level and ionic strength of the medium, are also known (Zezin A.B., Kabanov V.A. Advances in Chemistry, 1982, volume LI, issue 9, p. 1447). The claimed polysaccharide complexes in a non-crosslinked state under real conditions of the course of the wound process, accompanied by bleeding and exudation of the wound discharge, were prone to destruction with a complete loss of the therapeutic effect.

SUMMARY OF THE INVENTION

The present invention is directed to the creation of improved biocompatible compositions, which can be obtained in the form of: film, sponge, gel, colloidal solution. The material can be used as a basis for various types of dressings, introduced into the composition of composite dressings, including for drug delivery, used as cosmetic masks, cosmetic compresses. This material based on natural polysaccharides contains an interpolymer polyelectrolyte complex of a cationic or amphoteric linear polysaccharide with an anionic linear polysaccharide chemically crosslinked by polyfunctional aldehydes or epoxy compounds in certain ratios. Fixing a predetermined ratio of cationic and anionic polymers in an interpolymer polyelectrolyte complex by means of chemical crosslinking ensures stabilization and prolongation of the therapeutic effect of a wound agent.

The interpolymer polyelectrolyte complex as a cationic linear polysaccharide contains chitosan, as an amphoteric linear polysaccharide - 6-O-carboxymethylchitosan, and the anionic linear polysaccharide is selected from the group of alginate, carrageenan, hyaluronic acid, carboxymethylchitose, their carboxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxymethoxyethoxy mixtures.

The interpolymer polyelectrolyte complex is structurally chemically stabilized by chemical crosslinking using polyfunctional aldehydes, such as glyoxal, glutaraldehyde, etc., polyfunctional epoxides, such as diglycidyl ethers of di- and triethylene glycol, oligoethylene oxide, etc. as crosslinking agents.

п.The polymer molecules can be crosslinked by any method suitable for the formation of medical materials. According to the present invention, for example, polymer molecules can be crosslinked using bi- or polyfunctional crosslinking agents that covalently attach to two or more chains of polymer molecules. Typical bifunctional crosslinking agents include aldehydes, epoxides, succinimides, carbodiimides, maleimides, azides, carbonates, isocyanates, divinyl sulfone, anhydrides, halides, silanes, diazoacetate, aziridines and the like.

Crosslinking can be achieved using oxidizing or other agents, for example periodates, which activate side chains or specific segments in the polymer. Thus, they can react with other side chains or segments with the formation of cross-links. An additional crosslinking method is to irradiate the polymers, for example with gamma radiation, in order to activate the polymer so that crosslinking reactions are carried out. Dehydrothermal crosslinking methods can also be used.

The degree of crosslinking of the polymer affects some functional properties of the medical material, including its ability to model, adhere to wound or skin tissues, absorption of surrounding biological fluids, etc. The degree of crosslinking of the polymer composition can be controlled, for example, by the concentration of the crosslinking agent.

The material may include, if necessary, a plasticizer, usually in an amount of from 0.1 to 10% by weight. The plasticizer is selected from the group of polyethylene glycol, sorbitol, glycerol, dimethyl sulfoxide, and the like. Depending on the method of isolation of the target material from composite mixtures, a film, sponge, gel, colloidal solution can be obtained.

The hemostatic and reparative effects of the claimed material are regulated depending on the composition of the polyelectrolyte complex. The optimal ratio of the two types of action is achieved with a certain (mainly stoichiometric) molar ratio of cationic anionic polysaccharides, which are fixed due to the reaction of the functional groups of the crosslinking reagent with the corresponding functional groups of the complex of biopolymers. By changing the molar ratio of cationic and anionic polysaccharides in the polyelectrolyte complex, materials were obtained that have a predominantly hemostatic or reparative effect. The required molar ratio in them was also fixed by the crosslinking reaction.

When using this material, a significant decrease in the formation of rough scarring (adhesions) of the tissue was noted. In addition, this material can be used in combination with appropriate hemostatic agents, for example, thrombin, fibrinogen, blood coagulation factors, hemostatic drugs, such as iron lactate, ferracryl, etc.

Also, this material can be used for transdermal drug administration. Compositions according to the present invention can also be combined with other materials and components, for example, a bioactive component (s), administered to a patient with viscosity modifiers, for example carbohydrates and alcohols, as well as with other materials intended for other purposes, for example, to regulate desorption. Typical bioactive components include, but are not limited to, proteins, nucleic acids, as well as inorganic and organic biologically active molecules, such as enzymes, antimicrobials, antineoplastic agents, antiviral agents, local anesthetics, anti-flammable agents, hormones, antiangiogenic substances, antibodies, neurotransmitters, psychoactive drugs, drugs affecting the genitals, oligonucleotides, plant extracts. Such bioactive components are usually present in a relatively low concentration, typically less than 10% by weight of the compositions, substantially less than 5% by weight, and often below 1% by weight.

The material as dressings easily repeats the relief of the injury, fits snugly to it, but is also easily removed.

This polymer composition can be applied to an additional layer, for example atraumatic. In addition, the material may have an additional upper layer, for example in the form of a protective polymer and / or perforated film, as well as the lower and / or upper layer of fabric, non-woven material, mesh fabric.

The material is applied so that the required area, such as a wound, is completely covered.

Medical material is obtained by partial crosslinking of natural polysaccharides, which are an interpolymer polyelectrolyte complex of a cationic or amphoteric polysaccharide with an anionic linear polysaccharide.

For example, without limitation, to obtain the material according to this invention take an aqueous solution of chitosan acetate or 6-O-carboxymethylchitosan (5-95%) and sodium alginate (5-95%) with the addition of polyfunctional epoxide as a crosslinking agent (0.1 -10% of the total polymer) and maintain the resulting composite mixture with stirring for at least 30 minutes until a hydrophilic gel is formed.

Time and temperature do not play a significant role. The process can be carried out at room temperature 20 ± 2 ° C. The term “gel” means a viscous, semi-solid, or jelly-like state. The gel according to the invention is stable.

Chitosan is one of the few cationic hydrocolloids. This is a multifunctional aminopolysaccharide containing two hydroxyl (primary and secondary) and one primary amino group in the monomer unit.

When alkylating chitosan with monochloracetic acid in a heterogeneous medium, a mixture of 6-O-carboxymethylchitosans with a hemostatic effect is obtained (Krieger A.G., Fronchek E.V., et al. Materials of the 5th conference "New Perspectives in the Study of Chitin and Chitosan", M. - Schelkovo, 25-27.05.99, p. 155)

The film can be easily removed after a certain period of time, with no traces of the film remaining.

To obtain a sponge, the gel (composite mixture) is freeze-dried. The medical material in the form of a sponge has a loose porous structure and according to this invention can also contain all of the above active components and be used according to the above purposes.

п.Upon receipt of the dressings according to the invention in the form of gels, films, sponges, the medical material can be applied to the substrate and coated with a layer that prevents the adhesion of the material. This outer layer is removed before application to the wound. The substrate can perform one or more functions, including hardening, creating a gas or liquid barrier, regulating air permeability, creating protection for the processing area, etc.

The choice of substrate to impart desired properties is well known. Suitable substrates include, but are not limited to, films of natural or synthetic polymers, textile materials, metals, or a combination thereof. Preferably, the substrates include a polyurethane or polyester mesh in combination with an outer peelable layer.

The medical material may be applied to a substrate or substrate by any known means. It can be combined with an unlimited variety of substrates or substrates, including polymer films, metal foil, fabrics and non-woven materials made from natural and synthetic fibers, etc. The substrate providing barrier properties for gas and liquid may be a polymer film, for example, of polyurethane. Necessary composite materials can be made using films of polyester, polyvinyl alcohol or polyvinylidene chloride. When a material has a protective backing, such a structure is used as a dressing for wounds and burns. Moisture is retained, and excess exudate is absorbed, helping to heal, bacteria are prevented from entering the wound or burn area, microbial stasis can be maintained by introducing an antimicrobial agent into the composition. For ease of use, the material in the form of a sticky gel on a substrate is coated with an outer layer, which may be a polymer film coated with silicone or polyethylene.

Medical material can be applied to the substrate so that it occupies all or part of the surface of the substrate. If it occupies part of the surface of the substrate, those parts of it that do not contain material can be additionally provided with glue.

The medical material may also contain tackifiers, for example polyacids, polyols and polyamines, which promote tack. A dressing of this type is located on the skin so that the additional adhesive is in contact with the skin, and the medical material is on the wound. An additional adhesive provides a dressing that retains the adhesion property when the medical material is saturated with exudate from the wound and loses to some extent the ability to adhere due to the dilution effect.

Medical material can be used in dermatologically compatible compositions for cosmetic preparations and cosmetic compresses. It can be used, for example, in cosmetic face masks and nail pads. The material performs a hydrating function with or without a substrate, and the cosmetic effect can be enhanced by the introduction of other ingredients, for example hydrating, aromatic substances, etc. In use, the components can be mixed and applied. After application, the material can be easily removed. It should be borne in mind that the term “cosmetic” means a drug designed to preserve or improve appearance.
