Среди урологических осложнений, возникающих в послеоперационном периоде у больных с пересаженной почкой, основное место занимают осложнения, связанные с мочеточником трансплантата - стриктуры мочеточника и мочевые свищи. С недавнего времени применяются при лечении этой категории больных чрезкожные оперативные вмешательства, которые заключаются в проведении струны-проводника после пункции чашечно-лоханочной системы трансплантата через суженный участок мочеточника в мочевой пузырь, дальнейшее его бужирование или балонную дилятацию с последующей установкой внутреннего стента. Недостатками таких вмешательств является проведение струны-проводника только под рентгеновским контролем, что не всегда удается, высокая рентген-нагрузка на медицинский персонал и больного. Установка внутреннего стента и нефростомического дренажа (нефростомы) в трансплантат раздельно вызывает определенные технические сложности, часто нет возможности установить необходимый уровень локализации стента, восстановить его проходимость в раннем послеоперационном периоде.Recently, percutaneous surgical interventions have been used in the treatment of this category of patients, which consist of holding a conductor string after puncture of the pyelocaliceal system of the transplant through a narrowed section of the ureter into the bladder, its further bougienage or balloon dilatation, followed by the installation of an internal stent. The installation of an internal stent and nephrostomy drainage (nephrostomy) in the graft separately causes certain technical difficulties; it is often not possible to establish the necessary level of localization of the stent and restore its patency in the early postoperative period.

Для этого в способе лечения послеоперационных стриктур мочеточника у больных с пересаженной почкой, включающем чрезкожную пункцию чашечно-лоханочной системы трансплантата, проведение струны до места стриктуры, ликвидацию стриктуры при помощи бужирования или баллонной дилятации и установку раздельно внутреннего стента и наружной нефростомы, предложено дополнительно проводить рассечение стриктуры под контролем эндоскопа, а затем устанавливать на 3-5 суток дренажное устройство, выполненное как одно целое в виде стента-нефростомы, после чего удалять его нефростомический конец путем распрямления почечного кольцу, а внутренний оставлять в мочевых путях трансплантата на 12-16 недель, после чего его также удалять. Затем эндоскоп удаляют и по оставленной струне проводят дренажное устройство, выполненное в виде стента-нефростомы. Через 3-5 суток удаляют нефростомическую часть дренажного устройства и по оставшемуся стенту трансплантат дренируется в мочевой пузырь. Через 12-16 недель производят удаление стента. Было проведено чрезкожное оперативное вмешательство по ликвидации стриктуры мочеточника. Произведена антеградная пиелоуретерография - мочевые пути пересаженной почки расширены до места стриктуры, контрастное вещество с трудом проходит в мочевой пузырь через стриктуру. Попытка провести под рентген-контролем струну-проводник через стриктуру мочеточника не увенчалась успехом. После бужирования свищевого хода в трансплантат введен гибкий эндоскоп, который затем по мочеточнику подведен к месту стриктуры. Под визуальным контролем струна-проводник прошла через область стриктуры в мочевой пузырь. Произведено рассечение стриктуры, которое затем было дополнено баллонной дилятацией в месте рассечения суженного участка мочеточника. Эндоскоп удален. По оставленной струне-проводнику установлена в мочевые пути пересаженной почки стент-нефростома, под рентген-контролем отрегулирован уровень ее локализации. Послеоперационный период проходил без осложнений. На 5-е сутки после проверки проходимости дренажа нефростомическая часть стента-нефростомы была удалена, а внутренний стент оставлен в чашечно-лоханочной системе трансплантата, который затем был удален после выписки больной из стационара в амбулаторных условиях через 16 недель. При контрольном обследовании состояние больной и функция пересаженной почки удовлетворительные, расширения мочевых путей трансплантата не выявлено. A stricture was dissected, which was then supplemented by balloon dilatation at the dissection site of the narrowed portion of the ureter. The endoscope is deleted. A stent nephrostomy was installed in the urinary tract along the left conductor string, and the level of its localization was adjusted under X-ray control. The postoperative period was uneventful. On the 5th day after checking the patency of the drainage, the nephrostomy part of the stent-nephrostomy was removed, and the internal stent was left in the pyelocaliceal graft system, which was then removed after the patient was discharged from the hospital on an outpatient basis after 16 weeks. During the control examination, the patient's condition and the function of the transplanted kidney are satisfactory, no enlargement of the urinary tract transplants was detected.
