Description

The invention relates to medicine, namely to the field of dentistry, and can be used for dental prosthetics.

Prosthetics with fixed bridge-like all-cast and metal-ceramic prostheses due to their high reliability and aesthetics are firmly established in the practice of orthopedic dentistry. In the conditions of insurance medicine, it is very important to increase the resource for using these prostheses. Reducing the life of prostheses can be associated with periodontal complications that can occur during and after orthopedic treatment.

[4 - 7]. First of all, this can be associated with the processing of teeth under the crowns - preparation, during which the main negative effect is directed to the periodontal vessels, its trophism and function. This is especially pronounced in conditions of increased masticatory load, when the microcirculatory bed of the periodontal membrane provides hydro-absorbing mechanisms of the supporting teeth when using bridges [4 - 7].

As is known [4] under the influence of the main damaging factors of preparation (temperature and vibration), an inflammatory reaction occurs in periodontal tissues, proceeding with a violation of microcirculation (short-term arterial hyperemia is replaced by venous congestion).

[2]. Periodontal complications arise from the expansion of the gingival groove, when with gentle surface conduct there is aseptic inflammation of the gums (gingivitis), which completely disappears only on the 20th day [1]. With deep retraction, the consequences are more severe, since the periodontal ligament and the tops of the alveoli are involved in the inflammatory process. There are techniques that allow not to open the gingival groove [1,3,5,6,14], and during preparation, to form a ledge with a smooth bevel to the gum, with the help of which the gingival groove is displayed on the impression. With this method, as a rule, injury to the gingival margin with abrasive tools occurs. There are descriptions of gentle methods for the preparation of hard tooth tissues [2].

A known method for the prevention of periodontal complications associated with gum retraction during tooth preparation, based on the application of the gingival margin after teeth preparation using a solution consisting of alum-potassium alum, ephedrine hydrochloride, antipyrine, 0.025% androxone, 96 g of ethyl alcohol, polyvinyl alcohol, distilled water [1]. However, this method does not prevent periodontal complications in the long term after prosthetics, but acts only in a short period and does not have a broad pathogenetic effect.

It is known that the light of low-intensity lasers with a wavelength of 0.85 - 0.95 μm has a wide range of preventive and therapeutic actions [7,9]: it stimulates microcirculation, anti-inflammatory and decongestant, analgesic effects, etc. Therefore, they have been successfully used in the treatment of periodontal diseases .

In order to more effectively and reliably prevent the probable periodontal complications that may occur when prosthetics are made by fixed bridge-like whole-cast and metal-ceramic prostheses in the form of vascular disorders, trophic disorders and gum retraction with exposure of the necks of supporting teeth, the use of laser light is proposed.

When studying the effectiveness of laser light, a traditional clinical examination was carried out, the Diast computer system was used [8], and in the presence of treated abutment teeth for pulpitis and periodontitis, Endodontia was used [11]. Based on the obtained examination data, the conditions for laser prophylaxis of probable periodontal complications and enhancement of the functional properties of the periodontal support teeth were determined accordingly.

The method is as follows. When processing periodontal teeth with laser light, the Optodan semiconductor physiotherapeutic apparatus with a wavelength of 0.85 - 0.95 μm [12] was used after preparation and removal of the impression, fitting of the frame, fixing of crowns and prostheses. Using a Optodan laser semiconductor device, the periodontium of each prepared tooth was irradiated in the field from the gum edge to the level of the root projection along the transition fold at a power of 2–4 W and a purity of 80–100 Hz. The exposure time of one tooth ranged from 90 to 120 seconds. Particular attention was paid to teeth restored with solid cast pin inserts, the channels of which were treated with the resorcinol-formalin method, which underwent surgical treatment (tooth-preserving operations).

In periodontal diseases, the number of laser procedures was increased, depending on the severity of the pathology (up to 10 - 12 procedures).
