Description

The invention relates to medicine, namely to ophthalmology, and can be used for antiglaucomatous operations.

Known drainage for anti-glaucomatous operations, containing a tube with a metered valve [1]
The disadvantages of this drainage include liquid filtration only at a certain level of IOP, the inability to regulate pressure in the late postoperative period, complications in the form of choroid detachments in the early postoperative period, the shape of the drainage mismatching the shape of the eye, and the need to replace drainage in case of blockage of the tube lumen.

The closest in its technical essence and the achieved result is a drainage containing a tube and a support platform made of a biologically inert material [2]
However, the disadvantages of this drainage include:
the presence of operational and postoperative complications in the form of detachment of the choroid, prolonged postoperative hypotension, etc. requiring additional conservative or surgical intervention and arising from the fact that the filtration of fluid from the anterior chamber is limited only by the diameter of the inner lumen of the tube of a sharp drop in intraocular pressure.

The aim of the invention is to reduce postoperative complications and stabilize hydrodynamic parameters in the late postoperative period.

The invention is illustrated by drawings, where figure 1 shows a General view of the drainage; figure 2 is a view along arrow A in figure 1; FIG. 3 is a view along arrow B in FIG. 2; Fig.5.5 the position of the drainage in the eye.

The drainage for antiglaucomatous operations contains a tube 1 and a housing 2 connected to it with a lower disk-shaped surface. The tube 1 and the housing 2 are made of a hydrophobic biologically inert material. On the upper surface of the housing 2, a recess 3 is made from the side opposite the tube. The housing 2 is made with at least three channels diverging from the tube 4. The sum of the diameters of the channels 4 does not exceed the inner diameter of the tube 1, the opposite ends of the channels 4 are placed in the end part 5 of the recess 3 and are made with removable plugs 6. One of the channels 4a is placed along the longitudinal tube axis 1. Using hole 7, the drainage is fixed to the sclera.

Implant a drain as follows.

Opening of the contact 5-6 mm long 5 mm from the limb and around the incision zone. Marking and separation to the limb of the sclera area 4x4 mm in size. Depth 2/3 sclera. Opening of the anterior chamber of the eye, basal iridectomy. The end of tube 1 is inserted into the anterior chamber of the eye by 1-1.5 mm at the site of the basal iridectomy. When implanting the drainage channel 4A, placed along the longitudinal axis of the tube, is not closed by a removable plug, because through it, intraocular fluid should be filtered into the subconjunctival space. The housing 2 is fixed to the episclera with two sutures behind the openings 7. Next, the sclera is fixed over the tubular part of the drainage with two sutures. The conjunctiva is sutured with a continuous suture. Subsequently, dynamic monitoring of the level of intraocular pressure is carried out. With a persistent increase in intraocular pressure through the incision of the conjunctiva 1-2 mm, remove one more plug 6. With one pair of sutures, put one stitch on the conjunctiva. With a persistent increase in intraocular pressure, which is not compensated by the opening of additional channels 4, the reason for which is mainly a blockage of the lumen of the drainage tube 1, the conjunctiva is opened at the projection point of the exit of the directly located channel 4a and the lumen of the channel 4a is cleaned by the introduction of a metal rod, for example, which is smaller the diameter of the channel 4A. On the conjunctiva impose 1-2 sutures. With persistent hypotension, which is not stopped by conservative treatment, after opening the conjunctiva at the projection site of the output of channel 4 in the end part 5 of the recess 3, the channel may be closed with a removable plug (provided that at least two channels are open). Suture the conjunctiva.

The use of this drainage makes it possible to stabilize the hydrodynamic indices in the late postoperative period by approximately 90% and to reduce postoperative complications by 80%. This effect was obtained due to the fact that the design of the drainage provides for the regulation of the opening and closing of channels of the support platform diverging from the tube.
