Abstract

The utility model relates to medicine, namely laboratory diagnostics, and can be used in dentistry for blood sampling from the gums for its laboratory (cytological, biochemical, immunological, etc.) diagnostics. The proposed device has a hollow body with a volume of 9.0-10.0 cm, equipped at the outlet with a vacuum pump with a manometer and an automatic valve for maintaining negative pressure in the body at a level of 200.0 mm Hg, and at the outlet with a flexible nozzle transparent tightly adjacent open nozzle with a volume of 4.0-5.0 mm for the formation of a blister on the gum and the collection of blood with a syringe. The device allows to provide a reliable possibility of stable obtaining the necessary blood volume for the study.

Description

The proposed utility model relates to medicine, namely to laboratory diagnostics, and can be used in dentistry when taking blood from the gums for cytological, biochemical, immunological diagnostics.

The relevance of this development is explained by the fact that the information obtained during laboratory blood tests helps to obtain material for a more accurate diagnosis, the choice of the most effective treatment method and monitoring its implementation (Bezrukova I.V. Due to the fact that blood sampling from the gums even in a small volume (4-5 cubic mm) is difficult due to limited blood supply (a network of small vessels is located in the dense gum tissue). According to the traditional method (Aksenova T.V. The disadvantage of this method is a significant (up to 40%) number of unsuccessful attempts to take blood, requiring repeated punctures, and in 10-15% of cases, blood sampling from this place cannot be carried out.

As the closest analogue adopted "Sample of the contents of the gingival pocket and (or) the gingival sulcus for microbiological research" (Utility model patent No. 85332. The technical result of the proposed device is the formation of a hemorrhagic blister under the action of a metered vacuum in the gingival region and ensuring its stable state, preventing its transformation into a hematoma, the significance of this is explained by the fact that the name hemorrhagic blister, which is a local edema of the gingival mucosa with the formation of significant the number of dilated blood vessels, provides the ability to stably obtain the necessary volume of blood for research blowing. Experimentally selected vacuum intensity parameter 200.0 ± 15.0 mm Hg allows you to get exactly hyperemic blister. At a lower vacuum intensity, its formation is either impossible, or its condition will be unstable, and blood supply (hyperemia) is insufficient to ensure blood sampling. A more intense vacuum will lead to the destruction of blood vessels in the blister, multiple hemorrhages and the formation of a blister, the blood obtained from which will already have different characteristics, which will lead to a distortion of the research results.

To ensure a stable vacuum of a given intensity in the working area (hollow body with a nozzle), a device (valve) is arranged in the form of a hollow cylinder, and a replaceable nozzle made of transparent elastic material allows corresponding visual control of the state of the blister and provides the opportunity to enter syringe needle blister.

Example.

Patient D., 28 years old, diagnosis: chronic periodontitis 24 teeth. After treating the surface of the gum in the base area of the distal papilla, the mucosa was treated with heparin ointment, after 2 minutes the nozzle was applied and the vacuum pump was turned on, after 30 seconds the side wall of the nozzle was punctured with an insulin syringe and the needle was inserted into the blister, blood was taken in volume 5 mm3.

To study the effectiveness of the developed device, it was used to collect blood from the gums in 50 patients. The number of successful cases of obtaining the blood volume necessary for the study was 96.0 ± 3.5% at p <0.05 (48 people). In the comparison group of 50 people, in which the blood sampling was carried out in the traditional way, the number of successful cases was 76.0 ± 4.2% at p <0.05 (38 people). Thus, it was found that the effectiveness of the proposed device is 1.25 ± 0.2 (p <0.05) times higher.
