Abstract
The utility model relates to the field of medicine and medical technology, namely to ophthalmology and otorhinolaryngology, and relates to the surgical treatment of dacryocystitis and is aimed at ensuring the fastest and most safe for the patient summing up the hemostatic sponge soaked in diprospan and antibiotic solution into the formed anastomosis through the middle nasal passage with a minimum loss of these drugs with dacryocystorhinostomy transcanalicular approach. This is achieved by the fact that the device consists of a hollow body with an opening for a hemostatic sponge and supports for the fingers, the ventral side of which contains an opening for the stent to exit, and the dorsal side has a groove for the retainer, the latter preventing spontaneous rotation around its own axis of the stem having a ring. The utility model may find application in medical practice, in particular in ophthalmology and otorhinolaryngology, in the surgical treatment of dacryocystitis.
Description
The utility model relates to the field of medicine and medical technology, namely to ophthalmology and otorhinolaryngology, and relates to the surgical treatment of dacryocystitis.
The purpose of the utility model is to ensure that the hemostatic sponge soaked in diprospan and antibiotic solution is quickly and maximally safe for the patient into the formed anastomosis through the middle nasal passage with minimal loss of these drugs with dacryocystorhinostomy by a transcanalicular approach.
As is known, transcanalicular laser endoscopic dacryocystorhinostomy, widely used in dacryocystitis surgery, has the main advantages over other approaches: low invasiveness, relative ease of execution, and the absence of any visible cosmetic defects.
However, when performing this operation in the classical way, when a hemostatic sponge soaked in drug solutions is not fixed in the formed anastomosis, in some cases an unsuccessful outcome is observed due to scarring of the newly formed path of tear outflow and which can lead to relapse.
Currently, in order to prevent the recurrence of obstruction of the lacrimal passages after dacryocystorhinostomy, various modifications are used.
274-280).So, one of the most widely used in clinical practice is total intubation of the nasolacrimal duct with a silicone tube. However, the endotracheal material may not be correctly selected for thickness and elasticity. Also, with prolonged exposure to the lacrimal ducts, silicone can cause irritation and form bedsores in the lacrimal canaliculi. (B.F. Cherkunov // Diseases of the lacrimal organs. Samara, 2001, p. 274-280).
There is a method for the prevention of recurrence of dacryocystitis after dacryocystorhinostomy, which consists in the introduction of a gauze tampon into the cavity of the lateral anastomosis through the middle nasal passage with extraction for 4-5 days after surgery and subsequent washing of the lacrimal passages with 30% albucid solution and 1% hydrocortisone solution ( V.E. Boishtyan, A.I. Pakhomova // Restoration of lacrimation during tear duct obstruction. Chisinau, 1974, p. 48, 49). However, in some cases, albucid, when used in large doses, causes tissue irritation.
There is also known a method for the prevention of recurrence of dacryocystitis after dacryocystorhinostomy (patent RU No. 2238062, publ. 20.10.2004), which consists in washing the lacrimal and nasal canal with an antibacterial drug, characterized in that pyobacteriophage is used as an antibacterial drug in the first 3 days, followed by washing of tears nasal canal for 4 days by collagenase QC.
A common disadvantage of these methods is that when they are used, tamponade of the lacrimal sac cavity and rhinostomy walls is not performed, for example, with a fragment of a hemostatic sponge, and accordingly, the anastomosis is maintained, which would ensure the gap of the rhinostomy lumen.
The purpose of the technical solution is to create an easy-to-use, easy-to-use device that provides quick and maximum safe for the patient bringing the hemostatic sponge soaked in diprospan and antibiotic solution into the formed anastomosis through the middle nasal passage with minimal loss of these drugs with dacryocystorhinostomy with a transcanalicular approach.
8.The technical result of the goal is achieved by the fact that the device is represented by a

In FIG. 3 shows the detail of the upper part of the stem (top view).
The utility model is used as follows.
After the rhinostomy is formed by transcanalicular access and its endonasal expansion using a diode laser, bicanalicular intubation of the lacrimal passages and the formed anastomosis with a silicone stent with stainless steel guides is performed.
Next, a hemostatic sponge impregnated in a solution of diprospan and antibiotic is placed in the
Without relaxing the I, II, and III fingers, the surgeon slowly displays the device through the nasal opening.
Stainless steel guides are cut off. A sliding node is formed from the free ends of the silicone tube, with the help of which a fragment of the hemostatic sponge is fixed in the formed rhinostomy.
The free ends of the silicone tube are connected by 10 nodes, which eliminates the possibility of a dislocation of a fragment of the hemostatic sponge, up to its complete resorption.
In this case, an augmentation of the anastomosis with a hemostatic sponge is achieved, providing a gap in the lumen of the rhinostoma, and the presence of an antibiotic and a corticosteroid in the area of surgical trauma reduces the inflammatory response and prevents further excessive growth of scar tissue.
The positive effect of the claimed technical solution consists not only in ease of use, but also in quick and safe for the patient to bring the hemostatic sponge into the formed anastomosis through the middle nasal passage.
