Description
Foreliggende oppfinnelse angår et preparat inneholdende en macrolidforbindelse, som har evne til høyst usedvanlig langvarig frigjøring, for anvendelse innenfor det medisinske område. Nærmere bestemt vedrører foreliggende oppfinnelse et preparat med langvarig frigjøring av en macrolidforbindelse som angitt i krav 1.
Et oralpreparat av en macrolidforbindelse, nemlig tacrolimus med en nyttig immunundertrykkende aktivitet, er blitt fremstilt som en fast dispersjonsblanding med hurtig frigjøring, ved anvendelse av polymerer så som hydroksypropyl-metylcellulose og nedbrytningsmiddel (se for eksempel EP 0 240 773). På grunn av tilstedeværelse av nedbrytningsmiddel er det et preparat med hurtig frigjøring. Det har vært høyt verdsatt innenfor det kliniske område på grunn av sin høye absorberbarhet. I klinisk praksis har man alternativt ventet fremkomst av et oralt tacrolimuspreparat med tilstrekkelig lang virkning og utmerket oral absorberbarhet. Det er imidlertid kjent for en fagperson på området at innenfor teknikkens stand er absorberbarheten for et farmasøytisk aktivt middel som gis oralt som et preparat med langvarig frigjøring, vanligvis redusert, og/eller det observeres en ikke ubetydelig variasjon i absorberbarheten. Oppfinnerne har ved denne oppfinnelse utført en rekke undersøkelser. Følgelig har oppfinnerne oppfunnet preparater inneholdende macrolidforbindelser med langvarig frigjøring, og representanten for disse er tacrolimus, kjennetegnet ved at en macrolidforbindelse absorberes utmerket oralt, og/eller at variasjonen av dens absorberbarhet er nedsatt.
Foreliggende oppfinnelse angår et preparat inneholdende en macrolidforbindelse, med langvarig frigjøring, hvor oppløsingen av macrolidforbindelsen skjer under langvarig frigjøring.
Mer spesielt vedrører foreliggende oppfinnelsen et preparat med langvarig frigjøring av en macrolidforbindelse, hvor tiden (T63,2%) som kreves for at 63,2% av den maksimale mengde av macrolidforbindelsen skal oppløses, er fra 0,7 til 15 timer, målt i henhold til Japanske Farmakopé, 13. utgave, Oppløsningstest nr. 2 (skovlmetoden, 50 omdr. pr. min.) under anvendelse av en testoppløsning som er en vandig 0,005 % hydroksypropylcellulose-oppløsning, justert til pH 4,5, hvor macrolidforbindelsen er tacrolimus eller et hydrat derav,
hvilket preparat omfatter en fast dispersjonsblanding, hvor tacrolimus er til stede i amorf tilstand i en vannuløselig basis, hvor den vannuløselige basisen omfattet i den faste dispersjonsblandingen er valgt fra vannuløselig polymer eller voks.
T63,2%-verdien bestemt ved oppløsningstesten i henhold til denne oppfinnelse kan beregnes ut fra frigjøringskurven som konstrueres ved avsetting av testdata på millimeterpapir. Imidlertid kan frigjøringsprofilen for et legemiddel analyseres generelt ved tilpassing av oppløsningstestdata til en frigjøringsmodell, og en slik metode kan også anvendes ved beregning av nevnte T63,2%-verdi. Modellen for tilpasning som kan anvendes, innbefatter førsteordens- eller lineær-modellen, nulteordens-modellen, kubikkrotmodellen osv., som beskrevet i K. Yamaoka & Y. Yagahara: Introduction to Pharmacokinetics with a Microcomputer, Nankodo, s. 138; men Weibull-funksjonen er kjent som en modell ved hvilken alle typer frigjøringsmønstre kan uttrykkes med høyeste validitet, noe som er beskrevet i ovennevnte bok og i L.J. Leeson & J.T. Carstensen (red.): Release of Pharmaceutical Products (American Pharmaceutical Society) (Chizin Shokan), s. 192-195.
Weibull-funksjonen er en slik funksjon at oppløsningsverdi (%) i tid (T) kan uttrykkes ved følgende likning:
representerer den maksimale oppløsningsverdi i uendelig tid, m er en skalaparameter som representerer oppløsningshastigheten, n er en formparameter som representerer formen av oppløsningskurven, Ti er en posisjonsparameter som representerer latenstiden før start av oppløsing, og oppløsnings-karakteristikken for et farmasøytisk produkt kan uttrykkes ved anvendelse av disse parametre i kombinasjon.
For å tilpasse oppløsningstestdata til Weibull-funksjonen og beregne de respektive parametre, anvendes den ikke-lineære minste kvadraters metode beskrevet i K. Yamaoka & Y. Yagahara: Introduction to Pharmacokinetics with a Microcomputer, Nankodo, s. 40, nevnt ovenfor. Mer spesielt bestemmes parametrene på det tidspunkt hvor summen av kvadratene av forskjeller mellom verdiene beregnet ved ovennevnte likning og de målte verdier på hvert tidspunkt er minimalt, og oppløsningskurven beregnet ved hjelp av ovennevnte likning under anvendelse av disse parametre er kurven hvis dose mest pålitelig representerer de målte verdier.
Betydningen av hver parameter i Weibull-funksjonen vil nå bli forklart.
Dmaks(maksimal oppløsningsverdi) er den maksimale oppløsningsverdi i uendelig tid som nevnt ovenfor, og generelt er verdien av DmakSfortrinnsvis så nær 100 (%) som mulig.
m (skalaparameter) er en parameter som representerer oppløsnings-hastigheten for et farmasøytisk produkt, og jo mindre verdien for m er, jo høyere er oppløsningshastigheten, og likeledes, jo større verdien for m er, jo lavere er oppløsningshastigheten.
n (formparameter) er en parameter som representerer formen av en oppløsningskurve. Når verdien for n er 1, kan Weibull-funksjonen skrives som oppløsningsverdi (%) = DmakSx {1-eksp [-(T-Ti)/m]}, og siden dette er ekvivalent med førsteordenskinetikk, er oppløsningskurven lineær. Når verdien for n er mindre enn 1, flater oppløsningskurven ut. Når verdien for n er større enn 1, gjelder en sigmoid oppløsningskurve.
Ti (posisjonsparameter) er en parameter som representerer forsinkelses-tiden inntil start av oppløsing.
Preparatet med langvarig frigjøring omfattende en macrolidforbindelse i henhold til denne oppfinnelse kan også kjennetegnes ved hjelp av nevnte Weibull-funksjon. Det aktuelle preparat med langvarig frigjøring kan således implementeres ved at Dmaks(maksimal oppløsningsverdi) settes til 80 % eller høyere, fortrinnsvis 90 % eller høyere, mer foretrukket 95 % eller høyere, m (skalaparameter) til 0,7-20, fortrinnsvis 1 —12, mer foretrukket 1,5-8, n (formparameter) til 0,2-5, fortrinnsvis 0,3-3, mer foretrukket 0,5-1,5, og Ti (posisjonsparameter) til 0-12, fortrinnsvis 0-8, og mer foretrukket 0-4.
Verdien som finnes ved innsetting av parameterverdiene for m og n fra ovennevnte Weibull-funksjon i betegnelsen m<1/n>, representerer tiden ved hvilken 63,2 % av den maksimale mengde av oppløsning av den aktive bestanddel frigjøres fra preparatet (T63,2%). Det vil si at T63,2% (timer) = m<1/n>. Frigjø<r>ings-karakteristikaene for preparatet med langvarig frigjøring ifølge denne oppfinnelse kan evalueres ved hjelp av Oppløsningstesten, metode 2 (skovlmetoden, 50 omdr. pr. min.) ifølge JP XIII ved anvendelse av en testoppløsning som er 0,005 % vandig oppløsning av hydroksypropylcellulose justert til pH 4,5.1 preparatet med langvarig frigjøring omfattende en macrolidforbindelse i henhold til denne oppfinnelse er tiden (T63,2%) i løpet av hvilken 63,2 % av den maksimale mengde av macrolidforbindelsen som skal oppløses, frigjøres fra preparatet, 0,7-15 timer. Skjønt preparater med hurtig frigjøring omfattende macrolidforbindelse allerede er blitt fremstilt, er det aldri tidligere blitt fremstilt noen preparater med langvarig frigjøring for hvilke T63,2% er 0,7-15 timer og som ville være meget anvendelige i klinisk praksis. Foreliggende oppfinnelse har utført dette for første gang. Hvis T63,2%-verdien er kortere enn 0,7 timer, vil effektiviteten av macrolidforbindelsen etter oral administrering ikke være tilstrekkelig langvarig. Når preparatet har en T63,2%-verdi på mer enn 15 timer, vil frigjøringen av den aktive bestanddel bli forsinket slik at den aktive bestanddel vil bli utskilt fra kroppen før den effektive blodkonsentrasjon er nådd, hvorved det er uegnet som preparat ifølge denne oppfinnelse. Når T63,2% er 1,0-12 timer, kan det oppnås en mer gunstig langvarig frigjøring. Mer foretrukket er T63,2% 1,3-8,2 timer, og det mest foretrukne er et langvarig frigjørings-preparat med en T63,2%-verdi på 2-5 timer.
Betegnelsen «macrolidforbindelse» er i alminnelighet det generiske navn på forbindelser med 12 ledd eller mer, som hører til laktonene med stor ring. Tallrike macrolidforbindelser frembrakt av mikroorganismer av slekten Streptomyces, så som rapamycin, tacrolimus (FK506) og ascomycin, samt analogene og derivatene av disse, er medtatt i betegnelsen macrolidforbindelse.
Flere tricykliske macrolidforbindelser og deres farmasøytisk akseptable salter er velkjente for å ha utmerket immunundertrykkende virkning, antimikrobiell virkning og andre farmakologiske virkninger, og er som sådanne verdifulle for behandling eller forebygging av forkastningsreaksjoner ved transplantering av organer eller vev, transplantat-mot-vert-sykdommer, autoimmune sykdommer og infeksiøse sykdommer [EP-A-0 184 162, EP-A-0 323 042, EP-A-423 714, EP-A-427 680, EP-A-465 426, EP-A-480 623, EP-A-532 088, EP-A-532 089, EP-A-569 337, EP-A-626 385, WO89/05303, WO93/05058, W096/31514, W091/13889, W091/19495, WO93/5059 osv.].

Når det gjelder macrolidforbindelsen som anvendes i foreliggende oppfinnelse, må det være klart at det kan finnes konformasjonsisomerer («conformers») og én eller flere stereoisomerer så som optiske og geometriske isomerer på grunn av asymetrisk(e) karbonatom(er) eller dobbeltbinding(er), og slike konformasjonsisomerer og isomerer er også medtatt innenfor rammen av macrolidforbindelser i foreliggende oppfinnelse. Videre kan macrolidforbindelsen være i form av et solvat, som er medtatt innenfor rammen av foreliggende oppfinnelse. Solvatet innbefatter fortrinnsvis et hydrat og et etanolat.
Preparatet med langvarig frigjøring i henhold til foreliggende oppfinnelse er et preparat omfattende en fast dispersjonsblanding, hvor macrolidforbindelsen er til stede i amorf tilstand i en fast basis, som viser at dens T63,2-verdi er fra 0,7 til 15 timer. Tilstedeværelse eller fravær av en diffraksjonstopp påvist ved hjelp av røntgenkrystallografi, termiske analyser osv. viser hvorvidt en macrolidforbindelse er til stede i amorf tilstand i en fast basis i den faste dispersjonsblanding eller ikke.
Den faste basis ifølge oppfinnelsen er en vannuløselig farmasøytisk
akseptabel basis som kan bevare macrolidforbindelsen i amorf tilstand og som er i fast tilstand ved omgivelsestemperatur, valgt fra voks og vannuløselige polymerer.
Foretrukne eksempler på voks innbefatter spesifikt glyserolmonostearat og sakkarosefettsyreestere [for eksempel mono-, di- eller triestere av sakkarose med moderate til høyere fettsyrer, med 8-20 karbonatomer, for eksempel kaprylsyre, kaprinsyre, laurinsyre, myristinsyre, palmitinsyre, stearinsyre, arachinsyre, behensyre, oljesyre, linolsyre osv.). Ytterligere eksempler på voks innbefatter polygyserolfettsyreester. Hvilken som helst polyglyserolfettsyreester innbefattende monoester, diester eller polyester av polyglyserol med fettsyre er tilfredsstillende. Foretrukne polyglyserolfettsyreestere er for eksempel behenatheksa(tetra)glyserid [for eksempel Poem J-46B under et handelsnavn, fremstilt av Riken Vitamin Co., Ltd.], stearatpenta(tetra)glyserid [for eksempel PS-310 under et handelsnavn, fremstilt av Sakamoto Yakuhin Kogyo Co., Ltd.], stearatmono(tetra)glyserid [for eksempel MS-310 under et handelsnavn, fremstilt av Sakamoto Yakuhin Kogyo Co., Ltd.], stearatpenta(heksa)-glyserid [PS-500 under et handelsnavn, fremstilt av Sakamoto Yakuhin Kogyo Co., Ltd.], stearatseskvi(heksa)glyserid [SS-500 under et handelsnavn, fremstilt av Sakamoto Yakuhin Kogyo Co., Ltd.], stearatmono(deka)-glyserid, og en blanding av disse. Mer foretrukne voksarter er glyserolmono-stearat og lav-HLB-sakkarosefettsyreester [for eksempel F-50, F-20, F-10 osv., fremstilt av Dai-ichi Kogyo Seiyaku, Co., Ltd.].
Vektforholdet mellom macrolidforbindelsen og voks er fortrinnsvis fra 1:10 til 1:100, mer foretrukket fra 1:40 til 1:60, når voksen for eksempel er glyserolmonostearat; vektforholdet mellom dem er fortrinnsvis fra 1:0,2 til 1:20, mer foretrukket fra 1:0,5 til 1:5 når voksen for eksempel er sakkarosefettsyreester; vektforholdet mellom dem er fortrinnsvis fra 1:0,1 til 1:100, mer foretrukket fra 1:0,5 til 1:50 når voksen er polyglyserolfettsyreester.
Foretrukne vannuløselige polymerer innbefatter for eksempel etylcellulose, metakrylat-kopolymerer (for eksempel Eudragiter så som Eudragit E, R, S, RS, LD osv.). I det tilfelle hvor vannuløselige polymerer er etylcellulose, kan en farmasøytisk akseptabel av denne anvendes i foreliggende oppfinnelse. Dens foretrukne viskositet er imidlertid fra 3 til 110 cp, mer foretrukket 6-49 cp, mest foretrukket 9-11 cp, når viskositeten av 5 % etylcellulose-toluen/etanol (80/20)-oppløsning måles ved hjelp av en viskositetstest beskrevet i USP 23, NF18. Den foretrukne polymer er for eksempel ETHOCELL (viskositet: 10) (varemerke, Dow Chemical (US)).
Vektforholdet mellom macrolidforbindelsen og den vannuløselige polymer er fortrinnsvis fra 1:0,01 til 1:10, mer foretrukket fra 1:0,1 til 1:5, mest foretrukket fra 1:0,1 til 1:1 når den vannuløselige polymer er etylcellulose; vektforholdet mellom dem er mest foretrukket fra 1:0,5 til 1:5 når den vannuløselige polymer er en metakrylat-kopolymer.
Preparatet med langvarig frigjøring ifølge oppfinnelsen kan ytterlige inneholde en vannløselig basis; og mer foretrukket er denne tilsatte basisen én av de følgende vannløselige polymerer: polyvinylpyrrolidon (PVP), cellulosepolymer [hydroksypropylmetylcellulose (HPMC), hydroksypropylmetylcelluloseftalat, metylcellulose (MC), karboksymetylcellulosenatrium (CMC-Na), hydroksyetylcellulose, hydroksypropylcellulose (HPC), osv.], pektin, cyklodekstriner, galaktomannan, polyetylenglykol (PEG) med en gjennomsnittelig molekylvekt på 4000 eller høyere, gelatin osv.
For anvendelse tilsettes videre de vannløselige polymerer individuelt eller i en blanding av to eller flere derav. En mer foretrukket vannløselig basis er cellulosepolymer eller PVP; og den mest foretrukne vannløselige basis er HPMC, PVP eller en kombinasjon av disse. Spesielt kan HPMC av en type med lav viskositet utøve en mer ønskelig langvarig frigjøringsvirkning ved anvendelse; en vandig 2 % oppløsning av typen av HPMC har en viskositet på fra 1 til 4 000 cp, fortrinnsvis fra 1 til 50 cp, mer foretrukket fra 1 til 15 cp, målt ved 20 °C ved hjelp av et viskosimeter av Brookfield-typen; spesielt er HPMC 2910 med en viskositet på 3 cp (TC-5E, EW, Shin-estu Chemical Co., Ltd.) foretrukket.
Vektforholdet mellom macrolidforbindelsen og en slik vannløselig basis er fortrinnsvis fra 1:0,05 til 1:2, mer foretrukket fra 1:0,1 til 1:1, mest foretrukket fra 1:0,2 til 1:0,4.
Ved fremstilling av den faste dispersjonsblanding ifølge foreliggende oppfinnelse kan ovennevnte vannuløselig basis anvendes enkeltvis eller i kombinasjon med den vannløselige basis. Fordi den vannuløselige basis er valgt som den essensielle faste basis i foreliggende oppfinnelse, kan egnet oppløsningsprofil for den faste dispersjonsblanding oppnås ved blanding av en egnet mengde vann-løselig basis, så som en vannløselig polymer (f.eks. HPMC). Hvis ønskelig, tilsettes det, ut over den faste basis beskrevet ovenfor, egnede eksipienser (laktose osv.) bindemidler, fargestoffer, søtningsstoffer, smaksstoffer, fortynningsmidler, antioksidanter (vitamin E osv.) og smøremidler (for eksempel syntetisk aluminiumsilikat, magnesiumstearat, kalsiumhydrogenfosfat, kalsiumstearat, talk osv.) for vanlig anvendelse, for fremstilling av en fast dispersjonsblanding.
Avhengig av typen av den faste basis, er dessuten oppløsningshastigheten for macrolidforbindelsen fra den faste dispersjonsblanding noen ganger for langsom, eller det fordres noen ganger at den innledende oppløsningshastigheten for denne er forhøyet. I dette tilfelle kan oppløsningshastigheten for macrolidforbindelsen fra den faste dispersjonsblanding justeres ved tilsetting av passende nedbrytningsmidler [for eksempel krysskarmellosenatrium (CC-Na), karboksymetylcellulosekalsium (CM-Ca), lavsubstituert hydroksypropylcellulose (L-HPC), stivelsesnatriumglykolat, mikrofin krystallcellulose, krysspovidon osv.] eller passende overflateaktive midler [for eksempel herdet polyoksyetylen-ricinusolje, polyoksylstearat 40, polysorbat 80, natriumlaurylsulfat, sakkarosefettsyreester (HLB er høyere enn 10) osv.] til den faste dispersjonsblanding.
Partikkelstørrelsen i den faste dispersjonsblanding hvor macrolidforbindelsen er til stede i amorf tilstand i den faste basis, er fortrinnsvis lik eller mindre enn 500 |xm. Mer foretrukket har blandingen en partikkelstørrelse som går gjennom en 350 nm, mest foretrukket en 250fim sikt.
Videre kan den faste dispersjonsblanding av en macrolidforbindelse som finnes i preparatet med langvarig frigjøring ifølge oppfinnelsen, femstilles ved hjelp av metoder beskrevet i EP 0 240 773 og WO 91/19495 og liknende; metodene er beskrevet mer spesifikt nedenfor.
Macrolidforbindelsen oppløses i et organisk løsningsmiddel (for eksempel etanol, diklormetan eller en vandig blanding av disse, osv.), fulgt av tilsetting av en passende mengde av en fast basis, og den resulterende blanding oppløses eller suspenderes tilstrekkelig sammen eller får svelle. Deretter blir blandingen eltet tilstrekkelig sammen. Etter fjerning av det organiske løsningsmiddel fra blandingen, blir residuet tørket og malt og blir deretter underkastet størrelses-reduksjon, hvorved det kan fremstilles en fast dispersjonsblanding, hvor macrolidforbindelsen er tilstede i amorf tilstand i den faste basis. Under elteprosessen kan det videre tilsettes smøremidler så som kalsiumhydrogenfosfat, eksipienser så som laktose og liknende til blandingen, hvis nødvendig.
Preparatet med langvarig frigjøring omfattende en macrolidforbindelse i henhold til denne oppfinnelse, kan fremstilles ved anvendelse av et findelt pulver av macrolidforbindelsen. Partikkelstørrelsesreguleringen for macrolidforbindelsen kan utføres ved hjelp av malingsmaskineri som anvendes rutinemessig innenfor den farmasøytiske industri, så som en stavmølle, hammermølle, jetmølle og tørr-eller våtkulemølle, for å angi bare noen få eksempler. Det fine pulver av macrolidforbindelsen bør ha en partikkeldiameterfordeling i området 0,1-50 nm, fortrinnsvis 0,2-20 nm og mer foretrukket 0,5-10 nm, og/eller en middelpartikkeldiameter på 0,2-20 nm. fortrinnsvis 0,5-10 nm og mer foretrukket 1-5 nm.
Den faste dispersjonsblanding og det fine pulver av macrolidforbindelsen som således fremstilles ved hjelp av ovennevnte metoder, kan anvendes som sådan som et preparat med langvarig frigjøring. Idet man tar hensyn til håndter-barhet som preparat, dispergerbarhet i vann og dispergerbarhet etter oral dosering, blir blandingen mer foretrukket fremstilt som et preparat med langvarig frigjøring i form av pulver, fint pulver, granuler, tabletter eller kapsler ved hjelp av rutineutformingsmetoder (f.eks. trykkforming).
Hvis ønskelig, kan preparatet med langvarig frigjøring så fremstilles ved blanding av den faste dispersjonsblanding eller det fine pulver av macrolidforbindelser med for eksempel fortynningsmidler eller smøremidler (så som sakkarose, laktose, stivelse, krystallcellulose, syntetisk aluminiumsilikat, magnesiumstearat, kalsiumstearat, kalsiumhydrogenfosfat og talk) og/eller fargestoffer, søtningsstoffer, smaksstoffer og nedbrytningsmidler for rutine-anvendelse. Den resulterende blanding blir så blandet grundig sammen under fremstilling av et preparat med langvarig frigjøring. Preparatet med langvarig frigjøring eller den faste dispersjonsblanding eller det fine pulver av macrolidforbindelse ifølge foreliggende oppfinnelse kan dispergeres innledningsvis i vann og væske for å gis oralt som et flytende preparat.
Den effektive dose av macrolidforbindelsen varierer, avhengig av typen forbindelse, pasientens alder, hans (hennes) sykdom, graden av denne eller andre faktorer. Den effektive bestanddel anvendes vanligvis i en dose på ca. 0,001-1 000 mg, fortrinnsvis 0,01-500 mg, mer foretrukket 0,1-100 mg pr. dag, for terapeutisk behandling av sykdommen; vanligvis er en gjennomsnittelig enkelt dose ca. 0,01 mg, 0,1 mg, 0,5 mg, 1 mg, 5 mg, 10 mg, 50 mg, 100 mg, 250 mg og 500 mg.
Etter oral administrering frigjør preparatet med langvarig frigjøring av macrolidforbindelsen ifølge oppfinnelsen karakteristisk macrolidforbindelsen på langvarig måte, og den farmasøytiske aktivitet vedvarer i et langvarig tidsrom. I henhold til denne oppfinnelse kan administreringshyppigheten for tacrolimus eller et hydrat derav reduseres. Mer spesielt er det blitt mulig å tilveiebringe et macrolidholdig farmasøytisk preparat som kan administreres bare én gang pr. dag. Videre er det for tiden mulig å tilveiebringe en farmasøytisk blanding som er fri for risikoen for uønskede virkninger forårsaket av en forbigående usedvanlig stor konsentrasjon, og som sikrer en ekspresjon av en farmakologisk effektivitet over et tilstrekkelig langvarig tidsrom.
Preparatet med langvarig frigjøring ifølge foreliggende oppfinnelse er nyttig til behandling og eller forebygging av følgende sykdommer og tilstander på grunn av de farmakologiske aktiviteter som nevnte macrolidforbindelser har. Forkastningsreaksjoner ved transplantasjon av organer eller vev så som hjerte, nyre, lever, benmarg, hud, hornhinne, lunge, bukspyttkjertel, tynntarm, lemmer, muskler, nerver, mellomvirvelskiver, luftrør, myoblast, brusk osv.; transplantat-mot-vert-reaksjoner etter benmargtransplantasjon;
autoimmune sykdommer så som reumatoid artritt, systemisk lupus erythematosus, Hashimotos tyreoiditt, multippel sklerose, myasthenia gravis, diabetes type I osv.; og infeksjoner forårsaket av patogene mikroorganismer (f.eks. Aspergillus fumigatus, Fusarium oxysporum, Trichophyton asteroides osv.);
inflammatoriske eller hyperproliferative hudsykdommer eller kutane manifestasjoner av immunologisk medierte sykdommer (f.eks. psoriasis, atopisk dermatitt, kontaktdermatitt, eksematoid dermatitt, seboréisk dermatitt, lichen planus, pemfigus, bulløst pemfigoid, epidermolysis bullosa, urticaria, angioødem, vaskulitter, erytem, dermal eosinofili, lupus erythematosus, akne og alopecia areata);
autoimmune sykdommer i øyet (f.eks. keratokonjunktivitt, vernal konjunktivitt, uveitt forbundet med Behcets sykdom, keratitt, herpetisk keratitt, konisk keratitt, hornhinneepitel-dystrofi, keratoleukom, okularpemfigus, Moorens ulcer, skleritt, Graves oftalmopati, Vogt-Koyanagi-Harada-syndrom, keratoconjunctivitis sicca (tørt øye), flyktenule, iridosyklitt, sarkoidose, endokrin oftalmopati osv.); reversible obstruktive luftveissykdommer [astma (f.eks. bronkial astma, allergisk astma, intrinsisk astma, ekstrinsisk astma og støvastma), spesielt kronisk eller inveterert astma (f.eks. sen astma og luftveis-hyperresponsivitet) bronkitt osv.]; mukosale eller vaskulære inflammasjoner (f.eks. magesår, iskemisk eller trombotisk vaskulær skade, iskemiske tarmsykdommer, enteritt, nekrotiserende enterokolitt, intestinale ødeleggelser forbundet med termiske forbrenninger, leukotrien B4-medierte sykdommer);
intestinal-inflammasjoner / allergier (f.eks. cøliakiske sykdommer, proktitt, eosinofil gastroenteritt, mastocytose, Crohns sykdom og ulcerøs kolitt);
matvareforbundne allergiske sykdommer med symptomatisk manifestasjon fjernt fra det gastrointestinale system (f.eks. migrene, rhinitt og eksem); nyresykdommer (f.eks. interstitiell nefritt, Goodpastures syndrom, hemolytisk uremisk syndrom og diabetisk nefropati);

endokrine sykdommer (f.eks. hypertyreoidisme og Basedows sykdom); hematiske sykdommer (f.eks. ren rødcelle-aplasi, aplastisk anemi, hypoplastisk anemi, idiopatisk trombocytopenisk purpura, autoimmun hemolytisk anemi, agranulocytose, perniciøs anemi, megaloblastisk anemi og anerytroplasi); bensykdommer (f.eks. osteoporose);
respiratoriske sykdommer (f.eks. sarkoidose, lungefibrose og idiopatisk interstitiell pneumoni);
hudsykdommer (f.eks. dermatomyositt, leukoderma vulgaris, ichthyosis vulgaris, fotosensitivitet og kutant T-cellelymfom);

aktivt oksygen-medierte sykdommer [f.eks. organskade (f.eks. iskemiske sirkulasjonsforstyrrelser i organer (f.eks. hjerte, lever, nyre, fordøyelseskanalen osv.) forbundet med konservering, transplantering eller iskemiske sykdommer (f.eks. trombose, hjerteinfarkt osv.)];
tarmsykdommer (f.eks. endotoksin-sjokk, pseudomembranøs kolitt og legemiddel-eller bestrålingsbevirket kolitt);
nyresykdommer (f.eks. iskemisk akutt nyreinsuffisiens, kronisk nyresvikt); lungesykdommer (f.eks. toksikose forårsaket av lungeoksygen eller legemidler (f.eks. paracort, bleomycin osv.), lungekreft og lungeemfysem); okularsykdommer (f.eks. katarakt, jernlagringssykdom (siderosis bulbi), retinitt, pigmentose, senile plakk, glasslegeme-arring, hornhinne-alkaliforbrenning); dermatitt (f.eks. erythema multiforme, lineær immunglobulin A-bulløs dermatitt, sement-dermatitt);
og andre sykdommer [f.eks. gingivitt, periodontitt, sepsis, pankreatitt, og sykdommer forårsaket av miljøforurensning (f.eks. luftforurensning), aldring, karsinogener, metastase av karsinomer, samt hypobaropati)];
sykdommer forårsaket av histaminfrigjøring eller leukotrien C4-frigjøring; restenose av kransarterie etter antioplasti og forebygging av postkirurgiske adhesjoner;
autoimmune sykdommer og inflammatoriske tilstander (f.eks. primært mukosalt ødem, autoimmun atrofisk gastritt, for tidlig menopause, sterilitet hos menn, juvenil diabetes mellitus, pemphigus vulgaris, pemfigoid, sympatiserende oftalmitt, linseindusert uveitt, idiopatisk leukopeni, aktiv kronisk hepatitt, idiopatisk cirrhose, diskoid lupus erythematosus, autoimmun orchitt, artritt (f.eks. arthritis deformans) eller polykondritt);
infeksjon med humant immunsviktvirus (HIV-infeksjon), AIDS;
allergisk konjunktivitt;
hypertrofisk cicatrix og keloid på grunn av traume, forbrenning eller kirurgi.
Dessuten har nevnte tricykliske macrolider leverregenererende virkning og/eller virkninger med stimulerende hypertrofi og hyperplasi av hepatocytter. Den farmasøytiske blanding ifølge foreliggende oppfinnelse er derfor nyttig til øking av effekten av behandlingen og/eller forebyggingen av leversykdommer [f.eks. immunogene sykdommer (f.eks. kroniske autoimmune leversykdommer så som autoimmune hepatiske sykdommer, primær gallecirrhose eller skleroserende kolangitt), partiell leverreseksjon, akutt levernekrose (f.eks. nekrose forårsaket av toksiner, virushepatitt, sjokk eller anoksi), hepatitt B, ikke-A,- ikke-B-hepatitt, hepatokirrose og hepatisk svikt (f.eks. fulminant hepatitt, sent inntredende hepatitt og «akutt-ved-kronisk» leversvikt (akutt leversvikt ved kroniske leversykdommer))].
Videre er foreliggende blanding også nyttig til øking av effekten av forebygging og/eller behandling av forskjellige sykdommer på grunn av den nyttige farmakologiske aktivitet av nevnte tricykliske macrolider, så som forøking av aktiviteten av kjemoterapeutisk effekt, aktiviteten av cytomegalovirus-infeksjon, anti-inflammatorisk aktivitet, inhiberende aktivitet overfor peptidyl-prolyl-isomerase eller -rotamase, antimalari-aktivitet, antitumor-aktivitet og så videre.
Det har videre blitt funnet en oppløsningstestmetode for et fast preparat omfattende macrolidforbindelse, som anvender en testoppløsning inneholdende en egnet mengde cellulosepolymer. Vanligvis utføres oppløsningstesten for testing av et frigjøringskarakteristikum for en medisinsk aktiv bestanddel oppløst fra et fast preparat inneholdende den, i henhold til Oppløsningstesten, metode 2 (skovlmetoden, 50 omdr. pr min.), JP XIII, eller Oppløsningstest vist i USP 23, NF18 eller i Europeisk Farmakopé (3. utgave). Ved utførelse av en oppløsnings-test når det gjelder et preparat inneholdende en liten mengde av en macrolidforbindelse, vil frigjøringen av macrolidforbindelsen, basert på det iboende innhold av den, kanskje ikke nå 100 % selv etter flere timer. Dette skyldes at når mengden av macrolidforbindelsen er liten, vil adsorpsjon av macrolidforbindelsen på over-flatene av testbeholderen, filteret osv. utøve en virkning med øket styrke. Etter mye undersøkelse fant man at ved tilsetting av en egnet mengde cellulosepolymer (så som HPMC, hydroksypropylcelluloseftalat, MC, CMC-Na, hydroksyetylcellulose, hydroksypropylcellulose (HPC) og så videre) til testoppløsningen, og, hvis nødvendig, tilsetting av fosforsyre eller liknende til testoppløsningen for å bringe pH i den til ikke mer enn 7 for unngåelse av den uheldige virkning av den derav følgende økning i pH på stabiliteten av macrolidforbindelsen, kan inn-virkningen av adsorpsjon av macrolidforbindelsen på overflater av testapparatet inhiberes under oppnåelse av en gjenvinningshastighet på praktisk talt 100 %. En foretrukket cellulosepolymer er hydroksypropylcellulose eller dens ekvivalent, hvis foretrukne viskositet er slik at når 5,0 g av den oppløses i 95 ml vann, og etter sentrifugering for fjerning av skummet hvor nødvendig, måles oppløsningens viskositet med et roterende viskosimeter ved 25 ± 0,1 °C, hvorved oppløsningen viser en viskositet på 75-150 cp. For eksempel svarer hydroksypropylcellulosen med en gjennomsnittlig molekylvekt på ca. 100 000, levert fra Aldrich, til dette.
Den «egnede mengde» av cellulosepolymer for tilsetting til testopp-løsningen er 0,001-0,1 %, fortrinnsvis 0,002-0,01 %, og mest foretrukket 0,005 %, alt basert på den totale mengde av testoppløsningen.
Oppløsningstest, metode 2 (skovlmetoden), JP XIII, og oppløsningstest vist i USP 23, NF18 eller i Europeisk Farmakopé (3. utgave) er velkjente metoder for testing av frigjøringskinetikken for den aktive bestanddel fra et fast farmasøytisk produkt. De er oppløsningstester som anvender den spesifiserte beholder, skovl og annet utstyr, med regulering av mengden av testoppløsning, temperatur i testoppløsning, rotasjonshastighet og andre betingelser. Hvor nødvendig, utføres testen med testoppløsningen justert til en egnet pH. I foreliggende oppfinnelse er pH fortrinnsvis ikke høyere enn 7. I foreliggende oppfinnelse er «Oppløsningstest, metode 2 (skovlmetoden, 50 omdr. pr. min.) JP XIII» ensbetydende med «Oppløsningstest, metode 2 (skovlmetoden) JP XIII», som utføres med omrøring ved 50 omdr. pr. min. De tilsvarende beskrivelser i JP XIII, USP 23 (NF18) og Europeisk Farmakopé (3. utgave) er medtatt i det foreliggende som referanse.
Oppfinnelsen vil nå bli beskrevet i følgende eksempler, men er ikke begrenset til disse. I følgende eksempler blandes FK506 som sitt monohydrat ved fremstilling av blandinger inneholdende det, skjønt mengden av det uttrykkes som vekten av FK506, og ikke av dens monohydrat.
FK506 ble oppløst i etanol, og HPMC 2910 ble tilsatt til den resulterende oppløsning for at FK506 skulle kunne svelle tilstrekkelig. Deretter ble blandingen eltet sammen. Den resulterende eltede blanding ble overført til et rustfritt kar, tørket i vakuum og malt med en kaffemølle. Deretter ble det resulterende pulver underkastet størrelsesreduksjon ved hjelp av følgende prosesser, under fremstilling av fast dispersjonsblanding (i det følgende omtalt som SDCer fra 1 -1) til 1-6). På liknende måte som for SDC 1-2) i eksempel 1 ble en SDC fremstilt ved anvendelse av FK506 (1,0 mg) og HPMC 2910 (1,0 mg). På liknende måte som i eksempel 2 ble følgende additiver deretter henholdsvis tilsatt til SDC for fremstilling av kapsler 5-1) til 5-4), hver med totalt 60,0 mg.
FK506 ble oppløst i etanol, og HPMC 2910 ble tilsatt til den resulterende oppløsning for å muliggjøre tilstrekkelig svelling. Deretter ble blandingen eltet sammen. Eksempel 12
Glyserolmonostearat ble oppvarmet og smeltet ved 80 °C, og til dette ble det tilsatt FK506 med agitasjon under oppløsing av FK506 i det. Til den resulterende blanding ble det tilsatt HPMC 2910 for tilstrekkelig blanding, og den resulterende blanding ble deretter overført til et kar for å stå i ro for spontan avkjøling. Den faste substans som ble oppnådd ved avkjøling, ble malt med en kaffemølle og ble deretter siktet med en sikt på 500 \xm. En fraksjon av dem som gikk gjennom sikten, ble definert som SDC 12. SDC 12 ble blandet med magnesiumstearat under fremstilling av preparat 12, og 60,6 mg av dette ble fylt i kapsel nr. 5. Den resulterende kapsel er definert som preparat E.
Eksempel 13
I etanol ble det oppløst FK506 og aminoalkylmetakrylat-kopolymer, fulgt av tilsetting av kalsiumhydrogenfosfat, og den resulterende blanding ble blandet tilstrekkelig sammen. Blandingen ble tørket i vakuum over natten, knust i en morter og sortert ved anvendelse av sikter på 150 og 106 nm under fremstilling av en fraksjon på 106-150 nm som SDC 13. SDC 13 ble blandet med laktose og fremstilt som preparat 13, og 70 mg av dette ble deretter fylt i gelatinkapsel nr. 5 for tillaging som preparat F.

Eksempel 22
FK506 ble oppløst i etanol/aceton (1/1). Etter oppvarming av oppløsningen ved 75 °C ble sakkarosefettsyreester tilsatt under oppløsing og deretter avkjøling til romtemperatur. Blandingen ble tørket i vakuum over natten, knust i en morter og sortert ved anvendelse av sikter på 150 og 106 nm, under frembringelse av en fraksjon på 106-150 nm som SDC 22. SDC 22 ble blandet med laktose og fremstilt som preparat 22, og deretter ble 70 mg fylt i gelatinkapsel nr. 5 for tillaging som preparat Q.

Testmetode:
I henhold til den Japanske Farmakopé, 13. utgave, Oppløsningstest nr. 2 (skovlmetoden, 50 omdr. pr. min.) med anvendelse av en vandig 0,005 % hydroksypropylcellulose-oppløsning, justert til pH 4,5 som testoppløsning, ble det utført en test. De oppnådde data er vist i det følgende.

Oral absorberbarhet
Testprøve:
(1) Preparat B og D, som ble fremstilt i eksemplene nevnt foran.
(2) Kontrollpreparat (det samme som kontrollen i eksempel 31) Testmetode: Testprøvene ble gitt oralt til 6 cynomologe aper (med 1 mg/ape som en FK506-dose), under analysering av blodkonsentrasjonen av FK506 etter administrering. Sytten timer før administreringen ble maten fjernet fra et matbord for cynomologe aper med kroppsvekt rundt 6 kg. Deretter ble dyrene satt på sultekur til det hadde gått 12 timer etter administrering. Vann ble tilført etter behag før igangsetting av testen gjennom hele administreringen av testprøvene og deretter. Ved doseringen ble vann (20 ml) gitt samtidig til dyrene. Ved forbestemte mellomrom etter dosering ble det uttatt 1 ml blod fra underarmsvenen ved anvendelse av en steril sprøyte i et plastrør inneholdende heparin, og lagret ved ca. -80 °C til analysen av legemiddelkonsentrasjonen startet. Fullblods- legemiddelkonsentrasjonen av FK506 ble analysert ved hjelp av den FK506-spesifikke enzym-immunanalyse (EIA) kjent i JP-A-1-92659. Beskrivelsen ifølge dette er angitt i det foreliggende og omfattet i beskrivelsen.
Den maksimale blodkonsentrasjon (Cmaks) er definert som den maksimale verdi av fullblod-legemidlet. Tmaks er den tid som kreves for oppnåelse av den maksimale blodkonsentrasjon. MRT er definert som middel-retensjonstiden. Arealet under kurven for blodkonsentrasjon/tid (AUC) ble beregnet ved hjelp av trapesoidmetoden. Som en indikator på variasjonen av oral absorberbarhet ble CV (standardavvik/middel i %) beregnet.
Eksempel 34
I henhold til en liknende måte som for eksempel 33 ble det utført undersøkelse av oral absorberbarhet for de forskjellige preparater ifølge foreliggende oppfinnelse.
Ovenstående resultater viser at preparatet som ble valgt i ovenstående forsøk, etter oral administrering har mindre Cmaks, samt tilstrekkelig forlenget Tmaks og M RT i forhold til for preparatet med hurtig frigjøring (kontroll). Sammenliknet med preparatet med hurtig frigjøring er AUC, vist ved ovenstående preparater, nesten det samme eller større. Eller de ovennevnte preparater med langvarig frigjøring har små variasjoner hos individer når det gjelder Cmaks og/eller AUC, sammenliknet med et preparat med hurtig frigjøring.
I henhold til oppfinnelsen ifølge foreliggende patentsøknad kan den lille variasjon hos individer av den maksimale blodkonsentrasjon, eller arealet under kurven for blodkonsentrasjon/tid for macrolidforbindelsen etter oral dosering, sammenliknet med et preparat av denne med hurtig frigjøring, bestemmes ved anvendelse av en indikator for variasjonen av blod-absorberbarhet for macrolidforbindelsen, nemlig standardavvik/middel (CV i %) for den maksimale blodkonsentrasjon eller arealet under kurven for blodkonsentrasjon/tid. Betegnelsen «liten variasjon» angir en liten CV-verdi for denne; mer spesifikt angir betegnelsen at CV-verdien er mindre enn for et preparat med hurtig frigjøring som beskrevet ovenfor.

Claims (27)
1. Preparat med langvarig frigjøring av en macrolidforbindelse, hvor tiden (T63,2%) som trenges for at 63,2% av den maksimale mengde av macrolidforbindelsen skal oppløses, er fra 0,7 til 15 timer, målt i henhold til Japansk Farmakopé, 13. utgave, Oppløsningstest nr. 2 (skovlmetoden, 50 omdr. pr. min.) under anvendelse av en testoppløsning som er en vandig 0,005 % hydroksypropylcellulose-oppløsning justert til pH 4,5, hvor macrolidforbindelsen er tacrolimus eller et hydrat derav,
hvilket preparat omfatter en fast dispersjonsblanding, hvori tacrolimus er til stede i amorf tilstand i en vannuløselig basis, hvor den vannuløselige basisen omfattet i den faste dispersjonsblandingen er valgt fra vannuløselig polymer eller voks.
2. Preparat med langvarig frigjøring ifølge krav 1, hvori den vannuløselige basis er en vannuløselig polymer.
3. 22. Preparat med langvarig frigjøring ifølge krav 21, hvori mengden etylcellulose er 0,1 -1 til tacrolimus (1,0) basert på vektforhold.
23. Preparat med langvarig frigjøring ifølge krav 21, hvori mengden laktose er 2, 3 eller 5 til tacrolimus (1,0) basert på vektforhold.
24. Preparat med langvarig frigjøring ifølge krav 21, hvori intet nedbrytningsmiddel inneholdes i den faste dispersjonsblandingen.
25. Preparat med langvarig frigjøring ifølge krav 21, hvor partikkelstørrelsen til den faste dispersjonsblandingen er lik eller mindre enn 212 um.
26. Preparat med langvarig frigjøring ifølge krav 9 omfattende en fast dispersjonsblanding som erkarakterisert vedat 1) tacrolimus eller et hydrat derav er til stede i en amorf tilstand i en blanding av etylcellulose og hydroksypropylmetylcellulose i en mengde på henholdsvis 0,3 og 0,3 til tacrolimus eller et hydrat derav (1,0) basert på vektforhold, 2) laktose inneholdes som en eksipiens, 3) partikkelstørrelsen til den faste dispersjonsblandingen er lik eller mindre enn 212 um.
27. Preparat med langvarig frigjøring ifølge et hvilket som helst av kravene 21 til 24, som er i form av pulver, fint pulver, granul, tablett eller kapsel.
