Description
본 발명은 의료 장치, 보다 상세하게는 경피 채널을 형성하기 위한 방법 및 장치에 관한 것이다. 하나의 적용예에 있어서, 본 발명은 정위치에 형성된 척수 안정화 로드(spinal stabilization rod)와 같은 신체 내 정형외과적 고정 또는 안정화 임플란트를 삽입하는 최소 침습법(minimally invasive procedure)에 관한 것이다.
척추 및 관련 연결요소(connective elements)는 통증이나 장애를 일으키는 다양한 질병 및 상태와 관련된다. 이들 질병 및 상태로는 척추관절강직증, 척추전방전위증, 척추 불안정, 척추관협착증 및 퇴행성 탈출증(degenerated herniated) 또는 퇴행성 추간판 탈출증(degenerated herniated intervertebral discs) 등이 있다. 또한, 척추 및 관련 연결요소는 골절을 포함하는 손상, 열상인대, 및 척추후궁절제술을 포함하는 외과적 처치와 관련된다.
이러한 질병, 상태, 상처 및 처치와 관련된 통증 및 장애는 척주의 잔여부로부터의 척추의 전부 또는 일부분의 변위에 기인한다. 변위된 척추 또는 변위된 척추의 일부분을 정상위치로 복원시키고 그들을 척주 내에 고정시키기 위한 각종 방법이 개발되어 왔다. 예컨대, 나사고정을 이용한 개방정복술은 현재 사용되는 방법 중 하나이다. 핀, 나사, 로드 및 플레이트로 뼈의 2개 이상의 부분을 붙이는 외과적 처치는, 뼈를 둘러싼 조직의 절개, 접합되는 뼈 부분을 통한 하나 이상의 구멍의 드릴링(drilling)을 필요로 한다. 종래기술에서는 뼈 크기, 구조 및 부하 요건(load requirements)의 현저한 변화로 인해 다양한 종류의 뼈 고정 장치가 개발되어 왔다. 일반적으로, 현재의 치료 표준은 다양한 종류의 금속 와이어, 나사, 로드, 플레이트 및 클램프에 의존하여 치유 또는 유합 과정동안 골편을 안정화시킨다. 그러나, 이러한 방법은 이환율, 고비용, 긴 병상치료기간 및 개방처치에 수반되는 통증과 같은 많은 단점을 갖고 있다.
따라서, 통증 및 잠재적인 합병증을 덜 유발하는 변위된 척추 또는 변위된 척추의 일부분을 재위치 및 고정시키기 위한 장치 및 방법이 요구되고 있다. 바람직하게는, 상기 장치는 최소 침습법을 통해서 이식될 수 있다.

발명의 요약
본 발명의 하나의 양상에 따라 경피 접근 시스(percutaneous access sheath)가 제공된다. 하나의 적용예에서, 경피 접근 시스는 척추 안정화 로드(spinal stabilization rod)와 같은 정위치에 형성된 정형외과적 고정화 또는 안정화 임플란트(stabilization implant)의 삽입을 촉진시키는데 사용된다.
경피 접근 시스는 그의 원위 말단에 풍선(balloon)이 제공되는 전개 카테터와 병용하여 사용될 수 있다. 상기 경피 접근 시스는 하나의 근위 구역 및 다양한 직경의 원위 구역을 갖는다. 상기 전개 카테터는 풍선이 경피 접근 시스의 원위 구역 내에 위치하도록 경피 접근 시스 내에 배치될 수 있다.

2개의 뼈 앵커를 갖는 구현예를 기술한다. 상기 경피 접근 시스(100)은 제 1 뼈 앵커(410)의 상기 개구(422)를 통하여 그리고 계속해서 제 2 뼈 앵커(410)의 상기 개구(422)를 통하여 연장된다. 상기 제 1 뼈 앵커(410)는 제 2 뼈 내로 이식될 수 있다. 상기 뼈들은 인접한 척추골 본체들 또는 척추골들, 또는 하나 또는 그 이상의 중간의 척추골들에 의해 이격된 제 1 및 제 2 척추골이 될 수 있다. 임상적 절차들을 이하에서 상세하게 기술한다.
도 8 내지 도 11의 상기 뼈 앵커(410)는 티타늄 또는 스테인레스강 등과 같은 생체적합성 물질로 만들어진다. 달리, 상기 뼈 앵커(410)는 합성물질로 제조될 수 있다. 상기 뼈 앵커(410)는 또한 적절한 의료용 등급의 중합체로 만들어질 수 있다. 하나의 구현예에서, 상기 뼈 앵커(410)들은 40 내지 60㎜, 바람직하게는 약 50㎜의 길이를 갖는다. 그러나, 실제 길이는 환자 등의 손상 위치 및 크기에 따른다.
상기 뼈 앵커(410)은 근위 말단(414)를 갖는 근위 부분(412) 및 원위 말단(418)을 갖는 원위 부분(416)을 포함한다. 상기 근위 부분(412)는 전형적으로 헤드(420) 및 문맥(422)를 포함한다. 바람직한 구현예에서, 상기 헤드(420)는 스크류드라이버의 선단과 일치하도록 배열된 근위 부분(424)를 포함한다. 상기 헤드(420)는 나사 드라이버와 일치하는 표준의, 또는 필립스 홈(Phillips slot)을 포함한다. 육각형, 토크스(Torx), 직사각형, 삼각형, 곡선형 또는 다른 적절한 형상의 다양한 슬롯 배열형 또한 적절하게 사용될 수 있다. 도 11의 상기 뼈 앵커는, 스크류 드라이버의 원위 말단에에서 그에 대응하는 압축에 일치하도록 배열된 육각형의 플랫폼(platform) 등과 같이, 다수의 실질적으로 편평한 면들을 갖는 승강된 플랫폼(434)을 갖는다. 플랫폼(434)은 나사 드라이버와 일치되도록 하기에 적절한 다양한 형상으로 될 수 있다.
상기 뼈 앵커(410)의 상기 개구(422)는 상기 헤드(420)을 통하여 연장되고, 일반적으로 4 내지 8㎜의 직경, 바람직하게는 6 내지 8㎜의 직경을 갖는다. 상기 개구(422)는 임의의 적절한 형상이 될 수 있으나, 그러나, 상기 개구(422)는 바람직하게는 둥글게 처리되어 상기 경피관(100)의 삽입 뿐만 아니라 정형외과적 척추 안정화 임플란트의 동시적인 성형을 용이하게 할 수 있다.

본 발명의 한 측면에 따라 형성된 정형외과적 척추 안정 및 임플란트의 삽입을 용이하게 하기 위한 경피 접근 시스(100)의 사용방법에 이하 도면에서 설명된다. 이러한 구현예에서, 그 원위 말단에서 팽창식 정형외과적 고정장치를 운반하는 다른 전개 카테터의 통로를 사용가능하게 하기 위해 평활한 채널이 먼저 2개 이상의 이웃하는 뼈 앵커들 사이에 구축된다. 이러한 방법은 단지 두개의 척추체를 기준으로 하여 설명되고 묘사되는데, 그 중 하나가 불안정, 분리 또는 변위되지 않으면서, 다른 하나도 불안정, 분리 또는 변위되지 않으므로, 셋 이상의 척추체에도 동시에 적용될 수 있다. 추가적으로, 이러한 방법은 L5가 앵커링되는 "척추체"와 같은 엉치뼈의 두개방향 부분을 이용하여 L5 척추체를 안정화하는데 사용될 수 있다. 그 방법은 척주의 좌측면에서 적용되는 것으로 설명되고 묘사되는데, 본원을 참조로 할 때 당업자에게 이해될 수 있는 것처럼 척주의 우측면, 또는 바람직하게는 척주의 양측 모두에 적용될 수 있다. 다른 적용예들은 신체의 다른 뼈 및 골격 요소의 안정화를 포함한다.
도 12는 뼈막 표면을 통해서 척추체(500, 502)의 전방 척추체 또는 다른 적합한 부위에 삽입되어 왔던 뼈 앵커(410)을 도시하고 있다. 상기한 바와 같이, 하이드록시아파타이트 조제와 같은 뼈 매트릭스는 뼈의 내증식을 촉진하기 위해서 뼈 앵커(410)의 중심 루멘(430) 내 및 그 천공(도시되지 않음) 속으로 삽입될 수 있다. 뼈 앵커(410)은 그 문맥(422)가 실질적으로 서로에 대하여 동축이 되도록 조절된다.
중공침(436)은 투시검사를 이용하여 경피를 통해 삽입되어 뼈 앵커(410)의 문맥(422) 안으로 진입한다. 중공침(436)은 16 또는 18 게이지일 수도 있다. 중공침(436)이 두개 방향 척추체(502) 내에서 뼈나사(410)을 체결하는 것을 보여주는데, 그것은 본원을 참조로 하여 당업자에게 이해되는 바와 같이, 미부 방향 척추체(500) 내에서 뼈 나사(410)를 체결할 수 있다. 도 13은 중공침(436)의 원위 말단의 관점에서 확장된 것이다. 반(半)경질 가이드 와이어(semi-rigid guide wire)(438)은 2개의 방향 척추체(502) 내에서 중공침(436)의 루멘 및 뼈 앵커(410)의 문맥(422)를 통해 도입된다. 중공침(436)은 바람직하게 가이드 와이어(438)가 중공침(436)이 뼈 앵커(410)의 중심 루멘(430)쪽으로 수직으로 나가거나, 뼈 앵커(410)의 문맥(422)의 축을 동축으로 하여 나가도록 하는 투오이 침 선단(Tuohy needle tip)을 갖는다. 한편으로, 가이드 와이어(438)의 뼈 앵커(410)의 문맥(422)를 통한 벤딩(bending)은 개시된 것을 참조로 하여 당업자에게 이해될 수 있는 바와 같이, 각진 선단으로 변형된 로스 침(Ross needle) 또는 다른 적합한 구조에 의해 수행될 수 있다.
도 14는 본 발명의 한 측면에 따른, 뼈 앵커(410) 사이에 경피를 통해 삽입된 선택적인 가이드 와이어 조절 장치(440)을 도시한다. 가이드 와이어 조절 장치(440)은 미부 방향 척추체(500) 내에서 뼈 앵커(410)의 문맥(422)를 통해 가이드 와이어(438)을 조절하기 위해 사용되는 포크 형태의 말단을 가질 수도 있다. 다른 구현예에서, 올가미 또는 겸자와 같은 가이드 와이어 캡쳐 장치(442)가 미부 방향 척추체(500)의 문맥(422)에 대하여 경피를 통해 미부로 삽입될 수도 있다. 가이드 와이어 캡쳐 장치(442)는 미부 방향 척추체(500) 내에서 뼈 앵커(410)의 문맥(422)를 통해 가이드 와이어(438)이 투과한 이후에, 가이드 와이어(438)의 원위 말단과 체결하고, 그것을 피부를 통해 등쪽으로 끌어당겨 가이드 와이어(438)의 양쪽 말단이 고정된다(secured).
도 15는 도 12-14에 도시된 절차 이후에 대체되는 가이드 와이어(438)을 도시한 것이다.
가이드 와이어(438)은 경피 접근 시스 어셈블리(150)의 전개 카테터(300)의 가이드 와이어 루멘(304)에 삽입될 수도 있다. 전체 어셈블리(150)은 그 원위의 테이퍼진 부분이 뼈 앵커(410)의 문맥(422)를 통해 삽입될 때까지 가이드 와이어(438) 위로 움직인다. 경피 접근 시스 어셈블리(150)의 외부에 위치한 삽입 시스(200)은 매끄럽고, 낮은 프로파일의 외면으로 인하여 삽입을 용이하게 한다. 상기한 바와 같이, 그것은 PTFE로 만들어질 수도 있다.

삽입 시스(200)의 제거 이후에, 풍선(310)은 완전히 팽창되어 경피 접근 시스의 근위 부위 110이 그 전체 단면 프로파일까지 확장되도록 팽창될 수도 있다. 이후에, 풍선(310)은 전개 카테터(300)의 제거를 용이하도록 공기가 빠질 수도 있다. 상기한 바와 같이, 풍선(310)의 팽창 및 수축은 전개 카테터(300)의 문맥 320에 연결된 펌프를 통해서, 바람직하게는 풍선의 상태를 보다 잘 전달하기 위해 펌핑되는 대조 메디아를 이용하여 이루어질 수도 있다.
따라서 경피 접근 시스(100)은 뼈 앵커(410)의 문맥(422)를 통해 삽입된다. 뼈 앵커(410)의 문맥(422)를 통한 이러한 매끄러운 채널의 구축은 팽창식 정형외과적 고정 장치를 운반하는 다른 전개 카테터의 통행을 용이하도록 한다. 관련 앵커 및 방법들 뿐만 아니라 그 말단에 팽창식 정형외과적 고정 장치를 갖는 전개 카테터와 같은 예들은 그 전체가 본원에 참조로 포함되는 것으로서 2002년 5월 31일에 출원된 미국특허 제10/161,544호에 개시되어 있다.
본 발명은 특정의 바람직한 구현예의 관점에서 설명되고 있음에도 불구하고, 치수, 구조 및 재료의 변형을 포함하는 본 발명의 다른 구현예들도 본원에 개시된 관점에서 당업자들에게 명백할 것이다. 뿐만 아니라, 본원에서 임의의 한 구현예의 관점에서 논의된 모든 특징은 본원의 다른 구현예에서 사용되기 위해 용이하게 채택될 수 있다. 다른 구현예에서 유사한 특징에 대한 다른 용어 또는 참조 치수의 사용은 명백히 설명하기 위한 것 이상의 어떤 것도 포함하지 않는다. 따라서, 본 발명은 단지 청구 범위에 의해 설명되는 것이고, 본원에 개시된 바람직한 구현예에 의하여 한정되는 것은 아니다.
Claims (14)
- 연장된 관형 구조(elongate tubular structure) 및 제거가능한 삽입 시스(removable insertion sheath)를 포함하되, 상기 연장된 관형 구조의 일부분 또는 전체가 보다 작은 단면의 제 1 단면 프로파일로부터 보다 큰 단면의 제 2 단면 프로파일까지 확장될 수 있으며 상기 제거가능한 삽입 시스는 상기 제 1 단면 프로파일에서 상기 연장된 관형 구조의 일부분 또는 전체를 저지하도록 경피 접근 시스에 의해서 운반되는 것임을 특징으로 하는 경피 접근 시스(percutaneous access sheath).
- 제 1 항에 있어서, 상기 연장된 관형 구조의 일부분 또는 전체를 상기 제 1 단면 프로파일로부터 상기 제 2 단면 프로파일까지 확장하기 위한 팽창가능한 풍선(balloon)과 조합된 것을 특징으로 하는 경피 접근 시스.
- 제 2 항에 있어서, 상기 풍선은 경피 접근 시스 내에서 축으로 움직일 수 있도록 위치한 풍선 카테터(balloon catheter)에 의해 운반되는 것을 특징으로 하는 경피 접근 시스.
- 연장된 관형 구조 및 제거가능한 저지부(removable restraint)를 포함하되, 상기 연장된 관형 구조는 보다 작은 단면의 제 1 단면 프로파일로부터 보다 큰 단면의 제 2 단면 프로파일까지 확장될 수 있는 원위 구역을 가지며, 상기 제거가능한 저지부는 상기 제 1 단면 프로파일에서 상기 연장된 관형 구조의 상기 원위 구역을 저지하는 것임을 특징으로 하는 경피 접근 시스.
- 제 4 항에 있어서, 상기 제거가능한 삽입 시스는 절개가능한 시스(tearable sheath)를 포함하는 것을 특징으로 하는 경피 접근 시스.
