Description
The present invention relates to oral suspension compositions containing a therapeutically effective amount of amoxicillin or amoxicillin and a mixture of clavulanic acid or salts thereof as a main component.
Amoxicillin is a penicillin-based oral antibiotic and is widely used for upper and lower infectious diseases such as acute bronchitis or pneumonia or infectious diseases such as sinusitis or otitis media. Recently, in order to inhibit the degradation of amoxicillin by enzymes in vivo, it has been widely used in combination with a mixture of amoxicillin and beta lactamase inhibitor clavulanic acid or salts thereof. For example, as a formulation containing amoxicillin and clavulanic acid, Augmentin ™ tablets and dry syrups of GlaxoSmithKlein are commercially available.
However, clavulanic acid or salts thereof are particularly susceptible to heat and moisture, and have a property of lowering the titer due to moisture impregnation during storage and discoloration to yellowish brown. Therefore, formulations containing this ingredient are subject to particularly stringent criteria for their manufacturing workplace environment (humidity 25% RH and temperature below 20 ° C), airtightness of packaging containers and storage conditions (cold and dark below 25 ° C). And management is known to be essential. In particular, in the case of a dry syrup, the potency drops rapidly after being suspended in water, and the shelf life of the syrup is only 7 days even when refrigerated, and when left at room temperature, there is a problem of yellowing with severe titer in a few hours. have. Therefore, water must be weighed and suspended immediately before use, which is troublesome in preparation and administration, and there is a problem of misuse because it is difficult to take an accurate dose.
In order to solve the weak stability problem of such dry syrup formulations, WO 119/19227 discloses suspension tablets comprising intragranular and extragranular disintegrants in amoxicillin and clavulanate potassium. However, the suspension tablets do not spontaneously disintegrate in water and have a problem in that they need to be stirred separately.
In addition, WO 91/15197 discloses an effervescent tablet comprising an amoxicillin and clavulanic acid as main components and an alkaline component and an acid component which produce carbon dioxide upon contact with water. However, the boiling tablets are severely bubbled during suspension, so that the functionality is poor, the taste in the oral cavity is not good and is very irritating. In addition, in the case of boiling tablets, the foaming action is started even with a small amount of moisture due to moisture storage during storage, and the main component itself is decomposed as well as the tablets or packaging are collapsed, which is particularly problematic in terms of stability during the distribution period. It is not suitable as a technique applied to suspension tablets.
Accordingly, an object of the present invention is an oral suspension tablet which contains a therapeutically effective amount of amoxicillin or amoxicillin and a mixture of clavulanic acid or salts thereof as a main component, and maintains the stability of the main component even after prolonged storage and spontaneously disintegrates in contact with water. It is to provide a composition.
According to one embodiment of the present invention, the present invention provides a therapeutically effective amount of amoxicillin or amoxicillin and clavulanic acid or salts thereof as a main component, surface-modified sodium bicarbonate, and optionally one or more medicaments An oral suspension tablet composition is provided which contains a pharmaceutically acceptable excipient.
A suspension tablet composition is provided.
As used herein, the term "therapeutically effective amount" refers to an amount representing a desired therapeutic response upon oral administration, and can be easily determined by those skilled in the art according to the composition ratio of amoxicillin and potassium clavulanate and the age and symptoms of the patient. Dosage and frequency can be varied.
As used herein, the term "spontaneous disintegration" means that when the suspension tablet of the present invention is dropped into a small amount of water, for example, about 5 ml of water, no physical force is exerted. Even if it disintegrates or disperses itself within at least 60 seconds to become a suspension.
As used herein, "surface-modified sodium bicarbonate" refers to sodium bicarbonate in which the particle surface is processed with sodium carbonate. The composition of the surface-processed sodium bicarbonate preferably consists of about 88% sodium bicarbonate and about 12% sodium carbonate. In addition, surface-processed sodium bicarbonate is also commercially available (eg Effer ™ soda; SPI Pharma, etc.).
A therapeutically effective amount of amoxicillin may be used alone or a mixture of amoxicillin and clavulanic acid or a salt thereof may be used as the main component used in the composition of the present invention, preferably a mixture of amoxicillin and clavulanic acid or a salt thereof is used. Preferably a mixture of amoxicillin trihydrate and clavulanate potassium salt is used. The content of the main component is preferably 10 to 60% by weight of the total weight of the composition. In the case of mixtures of amoxicillin and clavulanic acid or salts thereof, the mixing ratio of amoxicillin and clavulanic acid or salts thereof is preferably mixed at 1: 1 to 30: 1.
Suspension tablets of the present invention include surface-processed sodium bicarbonate, the surface-processed sodium bicarbonate not only stabilizes the main components susceptible to moisture, but also acts to promote disintegration of tablets. More specifically, sodium carbonate, which surrounds the particle surface of sodium bicarbonate, acts like a silica gel, adsorbing free moisture present in the air and in the formulation, and is stable at temperatures of 70 ° C. Sodium sesquicarbonate ) To effectively inhibit the decomposition reaction of the main component by free moisture during the distribution period. In addition, surface-modified sodium bicarbonate generates carbon dioxide upon contact with water, and the carbon dioxide generated thereby provides a physical driving force inside the tablet to allow spontaneous disintegration of the tablet. In addition, by using the surface-processed sodium bicarbonate as described above, it is not necessary to use the fine silicon dioxide, which has been additionally added for water adsorption in the prior art, and thus the improvement and the simplification of the manufacturing process can be achieved together. The content of the surface-processed sodium bicarbonate is preferably 0.1 to 10% by weight of the total weight of the tablet, more preferably 0.5 to 2% by weight of the total weight of the tablet.
In the present invention, the diluent is not particularly limited but may optionally be one or more from water-soluble sugars or polyols, for example, cellulose derivatives such as microcrystalline cellulose, lactose and sugars. ) (Eg Ludipress ™ , Nutab ™, etc.) or dextrose (eg Emdex ™, etc.), mannitol, xylitol, sorbitol, maltitol or lactitol, or mixtures thereof Can be. In addition, the diluent may be used in the form of a spray dried (eg, Cellactose ™ , Microcellac ™ , Starlac ™ ). Among them, microcrystalline cellulose can be preferably used because it quickly absorbs the surrounding water, wets it, and swells itself, thereby promoting physical disintegration of the tablet.
Can be selected and used. The disintegrant improves the degree of dispersion when the suspension tablet of the present invention is disintegrated, thereby acting to improve the properties after disintegration.
In the present invention, as the organic acid, citric acid, tartaric acid, malic acid, succinic acid, fumaric acid, malic acid and the like may be used, but among them, it is most preferable to use succinic acid having little hygroscopicity and no interaction with the main component. desirable. As the sweetener, artificial sweeteners such as aspartame or conventional water-soluble sugars and polyols can be used.

Suspension tablets according to the present invention can be prepared in a variety of conventional tablet manufacturing method such as direct tableting method or dry granulation compression method, there is no particular limitation in the production method. In addition, the composition of the present invention does not delay spontaneous disintegration in water due to the characteristics of the composition even if the hardness at the time of tableting is higher than the hardness used in the manufacture of a conventional suspension tablet. It can be tableted to withstand hardness.
Tablets prepared according to each of the above Examples and Comparative Examples were tested and compared according to the items shown in Table 5 below.
1 Test Example 1
The disintegration rate, disintegration phase and functionality of the suspension tablets prepared according to Examples 1 to 12 and the suspension tablets prepared according to Comparative Examples 1 to 8 were evaluated. Testing of the suspension after suspension of the tablets was compared with a commercially available dry syrup of the same ingredient (Control: Ogumentin ™ Dry Syrup, GlaxoSmithKlein). Each test result is shown in Table 6 below.
As shown in Table 6 above, the suspension tablets prepared according to Examples 1 to 12 of the present invention spontaneously disintegrated and completely suspended within 25 seconds without applying physical force in contact with water. On the other hand, the suspension tablets prepared according to Comparative Examples 1 to 7 did not spontaneously disintegrate without physical strength (stirring) and remained intact as the appearance of the tablet. In addition, the comparative example 8 (boiling tablet) prepared by using a large amount of boiling base and acid rapidly generated bubbles at the same time as swelling (wetting) and disintegrated relatively quickly, but the appearance of bubbles was very bad and not only poor appearance However, the pH of the suspension is relatively very high and the taste is very irritating and is not suitable for practical use. Suspension tablets prepared according to Examples 1 to 12 of the present invention showed little foaming during suspension and showed comparable pharmaceutical properties with the ogumentin TM dry syrup, which is a control drug in suspension.
2 Test Example 2
The suspension tablets prepared according to Example 8 and the suspension tablets prepared according to Comparative Example 9 were each packaged in aluminum bags and then stored for 6 months under accelerated conditions (40 ° C./75% RH) over time with amoxicillin trihydrate and claus The content, hardness, friability, and pH of the potassium fluorate salt were measured to compare the stability. The evaluation results are shown in Table 7 below.
As shown in Table 7, Example 8 exhibited a much better stability profile in all the items compared to Comparative Example 9 under accelerated conditions. In particular, the content of clavulanic acid was reduced by about 5% in Example 8 while in the case of Comparative Example 9 was reduced by about 11%, the suspension tablet according to the present invention showed an excellent stabilizing effect of about 2 times or more. The degree of stabilization of clavulanic acid can also be compared with the degree of expansion of the packaging material due to the gas evolution accompanying the decomposition reaction of clavulanic acid. In the accelerated condition test, the aluminum bag of Example 8 hardly expanded, The aluminum bag of Comparative Example 9 was inflated taut. In terms of hardness and wear and tear, Example 8 showed little change, but in Comparative Example 9, the hardness decreased by about 2 kp over time and the wear and tear also increased by about twice. As a result of the accelerated condition test, Example 8 was almost unchanged, whereas Comparative Example 9 was relatively changed. From these results, it was confirmed that the suspension tablet containing surface-processed sodium bicarbonate significantly improved the stability under high temperature and humidity conditions than otherwise.
Suspension tablets containing amoxicillin and / or clavulanic acid or salts thereof according to the present invention not only significantly improve the stability of the main component, but also spontaneously disintegrate without contacting water with water for administration. The state is switched.
