Claims (1)
- 【特許請求の範囲】 1.患者への正の気道圧を最適化するための方法であって、 (a)呼吸ガスに関する最初のレベルの正の気道圧を患者に加える段階と、 (b)患者への呼吸ガスの吸気流の始まりを検出する段階と、 (c)患者への呼吸ガスの吸気流を表すデータ値を記憶する段階と、 (d)記憶されたデータ値が患者の流れ制限を表すか否かを決定する段階と、 記憶されたデータ値が患者の流れ制限を表したとき、正の気道圧を増大させ る段階と、 を備えていることを特徴とする方法。 2.記憶されたデータ値が患者の流れ制限を表さないとき、正の気道圧を減少さ せる段階を更に備えていることを特徴とする請求の範囲第1項に記載の方法。 3.前記段階(d)が、記憶されたデータ値を正弦波形と比較することを含むこ とを特徴とする請求の範囲第1項に記載の方法。 4.前記段階(d)が、記憶されたデータ値を純粋な正弦波と比較することを含 むことを特徴とする請求の範囲第3項に記載の方法。 5.前記段階(d)が、記憶されたデータ値を平面度について分析することを含 むことを特徴とする請求の範囲第3項に記載の方法。 6.前記段階(d)が、記憶されたデータ値の回帰線を純粋な正弦波の回帰線と 比較することを含むことを特徴とする請求の範囲第5項に記載の方法。 7.前記段階(d)が、記憶されたデータ値を呼吸努力について分析することを 含むことを特徴とする請求の範囲第1項に記載の方法。 8.前記段階(d)が、記憶されたデータ値のピーク値を記憶されたデータ値の 導関数のピーク値と比較することを含むことを特徴とする請求の範囲第7項に記 載の方法。 9.前記段階(d)が、記憶されたデータ値を相対流量について分析することを 含むことを特徴とする請求の範囲第1項に記載の方法。 10.患者の複数の吸気について前記段階(b)〜前記段階(d)を繰り返して、 流れ制限のある呼吸について複数のピーク流値を作り、かつ、流れ制限のない 呼吸について複数のピーク流値を作ることと、記憶されたデータ値のピーク値を 、流れ制限のある呼吸については複数のピーク流値の平均値と、流れ制限のない 呼吸については複数のピーク流値の平均値と比較することとを更に含むことを特 徴とする請求の範囲第9項に記載の方法。 11.(f)患者の複数の吸気について前記段階(b)〜前記段階(d)を繰り返 して、流れ制限のある呼吸について複数のピーク流値を作り、かつ、流れ制限の ない呼吸について複数のピーク流値を作る段階と、 (g)記憶されたデータ値を純粋な正弦波と連関させて第1の指数を作る段階 と、 (h)記憶されたデータ値の回帰線を純粋な正弦波の回帰線と比較して第2の 指数を作る段階と、 (i)記憶されたデータ値のピーク値を記憶されたデータ値の導関数のピーク 値と比較して第3の指数を作る段階と、 (j)記憶されたデータ値のピーク値を、流れ制限のある呼吸については複数 のピーク流値の平均値と比較し、流れ制限のない呼吸については複数のピーク流 値の平均値と比較して、第4の指数を作る段階と、 を更に備えていることを特徴とする請求の範囲第1項に記載の方法。 12.前記段階(d)が、第1の指数、第2の指数、第3の指数および第4の指数 の関数として第5の指数を計算する段階を更に含み、第5の指数が、患者の流れ 制限の存在を示していることを特徴とする請求の範囲第11項に記載の方法。 13.前記段階(d)が、第1の指数、第2の指数、第3の指数および第4の指数 の数学的関数として第5の指数を計算する段階を更に含み、各指数が、ゼロ値を 含む範囲を備えた重み係数を有することを特徴とする請求の範囲第12項に記載 の方法。 14.患者への正の気道圧を最適化するための方法であって、 (a)呼吸ガスに関する最初のレベルの正の気道圧を患者に加える段階と、 (b)患者への呼吸ガスの吸気流の始まりを検出する段階と、 (c)患者への呼吸ガスの吸気流の波形を記憶する段階と、 (d)記憶された吸気流の波形が患者の流れ制限を表すか否かを決定する段階 と、 (e)患者の複数の吸気について前記段階(b)〜前記段階(d)を繰り返し て、流れ制限のある呼吸について複数のピーク流値を作り、かつ、流れ制限のな い呼吸について複数のピーク流値を作る段階と、 (f)吸気流の波形を純粋な正弦波と連関させて第1の指数を作る段階と、 (g)吸気流の形の回帰線を純粋な正弦波の回帰線と比較して第2の指数を作 る段階と、 (h)吸気流の波形のピーク値を吸気流の波形の導関数ピーク値と比較して第 3の指数を作る段階と、 (i)吸気流の波形のピーク値を、流れ制限のある呼吸については複数のピー ク流値の平均値と比較し、流れ制限のない呼吸については複数のピーク流値の平 均値と比較して、第4の指数を作る段階と、 (j)第1の指数、第2の指数、第3の指数および第4の指数の数学的関数と して第5の指数を計算する段階とを備え、各指数が、ゼロ値を含む範囲を備えた 重み係数を有しており、 (k)第5の指数が患者の流れ制限表すとき、正の気迫圧を増加させる段階 を更に備えていることを特徴とする方法。 15.患者への正の気道圧を最適化するための方法であって、 (a)呼吸ガスに関する最初のレベルの正の気道圧を患者に加える段階と、 (b)患者への呼吸ガスの第1の吸気流を監視する段階と、 (c)第1の吸気が患者の直前の流れ制限を示すか否かを表示する第1の指数 を決定する段階と、 (d)患者への呼吸ガスの第2の吸気流を監視する段階と、 (e)第2の吸気流が患者の現在の流れ制限を示すか否かを表示する第2の指 数を決定する段階と、 (f)第1の指数および第2の指数を使用して多数関数を計算する段階と、 (g)第2の指数および多数関数が患者の気道での継続的な流れ制限を示すと き、正の気道圧の量を減少させる段階と、 を備えていることを特徴とする方法。 16.第2の指数および多数関数が患者の気道での流れ制限の欠如を示すとき、正 の気道圧の量を減少させる段階を更に備えていることを特徴とする請求の範囲第 15項に記載の方法。 17.正の気道圧の量が以前に上昇され、下降され、或いは同じ値に保持されたか 否かを決定することによって、正の気道圧の量を更に調整することを特徴とする 請求の範囲第16項に記載の方法。 18.患者への吸気流の少なくとも2つの波形を表すデータ値から第1の指数を決 定することによって、多数関数を計算することを特徴とする請求の範囲第15項 に記載の方法。 19.患者への正の気道圧を最適化するための呼吸装置であって、 呼吸ガスに関する最初のレベルの正の気迫圧を患者に加えるための手段と、 患者への呼吸ガスの吸気流を表すデータ値を記憶するための手段と、 記憶されたデータ値が患者の流れ制限を表すか否かを決定するための手段と 、 記憶されたデータ値が患者の流れ制限を表すとき、正の気道圧を増加させる ための手段とを備えていることを特徴とする装置。 20.前記決定手段が、流れ制限のある呼吸について複数のピーク吸気流値を決定 し、流れ制限のない呼吸について複数のピーク吸気流値を決定し、第1の指数を 作るため、記憶されたデータ値を純粋な正弦波と相関させ、第2の指数を作るた め、記憶されたデータ値の回帰線を純粋な正弦波の回帰線と比較し、第3の指数 を作るため、記憶されたデータ値のピーク値を記憶されたデータ値の導関数のピ ーク値と比較し、第4の指数を作るため、記憶されたデータ値のピーク値を、流 れ制限のある呼吸については複数のピーク流値の平均値と、流れ制限のない呼吸 については複数のピーク流値の平均値と比較するための処理手段を有しているこ とを特徴とする請求の範囲第19項に記載の装置。 21.前記処理手段が更に、第1の指数、第2の指数、第3の指数および第4の指 数の数学的関数として第5の指数を計算するための手段を有し、各指数が、ゼロ 値を含む範囲を備えた重み係数を有していることを特徴とする請求の範囲第20 項に記載の装置。 22.前記決定手段が、記憶されたデータ値を正弦波形と相関させ、記憶されたデ ータ値を平面度について分析し、記憶されたデータ値を呼吸努力について分析し 、記憶されたデータ値を相対流量について分析するための処理手段を有すること を特徴とする請求の範囲第19項に記載の装置。 23.前記処理手段がマイクロプロセッサを有することを特徴とする請求の範囲第 22項に記載の装置。 24.前記記憶手段が、マイクロプロセッサと関連したランダムアクセスメモリを 有することを特徴とする請求の範囲第23項に記載の装置。 25.前記気道圧を加えるための手段がモータによって駆動されるブロワを有し、 前記気道圧を増加させるための手段がモータ速度コントローラを有することを特 徴とする請求の範囲第24項に記載の装置。 26.前記気道圧を加えるための手段が、呼吸ガスの正圧源と、該正圧源と流体連 通する、ベント付きの鼻取付具とを有し、前記気道圧を増加させるための手段が 、圧力制御弁を有することを特徴とする請求の範囲第24項に記載の装置。 27.患者への正の気道圧を最適化するための呼吸装置であって、 一定の正圧の呼吸ガスを患者の気迫に供給する源と、 患者への呼吸ガスの吸気流を表す第1のデータ値を発生させるように形成さ れた流れセンサと、 流れセンサによって発生された第1のデータ値を記憶するように形成された コンピュータメモリと、 第1のデータ値が患者の流れ制限を表すとき、第1の信号を発生させるよう に形成されたマイクロプロセッサと、 29.患者への呼吸ガスの一定の正圧を表す第2のデータ値を発生させるように形 成された圧力センサを更に備え、前記コンピュータメモリが、第2のデータ値 を記憶するように形成されていることを特徴とする請求の範囲第27項に記載の 装置。 30.前記マイクロプロセッサが、第1のデータ値を純粋な正弦波と相関させるた めの手段を有していることを特徴とする請求の範囲第27項に記載の装置。 31.前記マイクロプロセッサが、第1のデータ値の回帰線を純粋な正弦波の回帰 線と比較するための手段を有していることを特徴とする請求の範囲第27項に記 載の装置。 32.前記マイクロプロセッサが、第1のデータ値のピーク値を第1のデータ値の 導関数のピーク値と比較するための手段を有していることを特徴とする請求の範 囲第27項に記載の装置。 33.前記マイクロプロセッサが、流れ制限のある呼吸について複数のピーク流値 を決定し、流れ制限のない呼吸について複数のピーク流値を決定し、第1のデー タ値のピーク流値を、流れ制限のある呼吸について複数のピーク流値の平均値と 比較し、流れ制限のない呼吸について複数のピーク流値の平均値と比較するため の手段を有することを特徴とする請求の範囲第27項に記載の装置。 34.呼吸ガスを患者の気迫に供給するための装置であって、 一定の正圧の呼吸ガスを患者の気迫に供給する源と、 患者への呼吸ガスの吸気流を表すデータ値を発生させるように形成されたセ ンサと、 センサによって発生された第1のデータ値を記憶し、センサによって発生さ れた第2のデータ値を記憶するように形成されたコンピュータメモリと メモリ装置に接続され、第1のデータ値が患者の第1の流れ制限を示すか否 かを表す第1の指数を決定するための処理手段とを備え、該処理手段が、第2の データ値が患者の第2の流れ制限を示すか否かを表す第2の指数を決定し、かつ 、第1および第2の指数を使用して多数関数を計算するように形成されており、 呼吸ガス源と作動関係をなした圧力コントローラを更に備え、該圧力コント ローラが、第2の指数および多数関数が患者の気道の継続的な流れ制限を示すと き、患者の気道への正圧を増加させるように、前記処理手段に応答すること を特徴とする装置。 35.前記処理手段が、第2の指数および多数関数が患者の気道での流れ制限の欠 如を示すとき、圧力コントローラにより、患者の気道への正圧を減少させるよう に形成されていることを特徴とする請求の範囲第34項に記載の装置。 36.前記処理手段が、正圧が以前に上昇され、下降され、或いは同じ値に保持さ れたか否かを決定することによって、患者の気道への正圧を更に調整するように 形成されていることを特徴とする請求の範囲第35項に記載の装置。 37.患者の気道での流れ制限を検出するための方法であって、 (a)患者への周囲空気の吸気流を測定するための手段を有する鼻取付具を、 患者の気道と連通するように準備する段階と、 (b)患者の気への吸気流の始まりを検出する段階と、 (c)患者への呼吸ガスの吸気流を表すデータ値を記憶させる段階と、 (d)記憶されたデータ値が患者の流れ制限を示すか否かを決定する段階と、 を備えていることを特徴とする方法。 38.患者からの周囲空気の呼気流を測定する段階と、呼気流を表す第2のデータ 値を記憶させる段階とを更に備えていることを特徴とする請求の範囲第37項に 記載の方法。 39.記憶されたデータ値が患者の気道での流れ制限を示すとき、患者の治療を指 示する段階を更に備えていることを特徴とする請求の範囲第37項に記載の方法 。 40.(e)患者の複数の吸気について前記段階(b)〜前記段階(d)を繰り返 して、流れ制限のある呼吸について複数のピーク流値を作り、かつ、流れ制限の ない呼吸について複数のピーク流値を作る段階と、 (f)記憶されたデータ値を純粋な正弦波と連関させて第1の指数を作る段階 と、 (g)記憶されたデータ値の回帰線を純粋な正弦波の回帰線と比較して第2の 指数を作る段階と、 (h)記憶されたデータ値のピーク値を記憶されたデータ値の導関数のピーク 値と比較して第3の指数を作る段階と、 (i)記憶されたデータ値のピーク値を、流れ制限のある呼吸については複数 のピーク流値の平均値と比較し、流れ制限のない呼吸については複数のピーク流 値の平均値と比較して、第4の指数を作る段階と、 を更にそなえいることを特徴とする請求の範囲第37項に記載の方法。 41.前記段階(d)が、第1の指数、第2の指数、第3の指数および第4の指数 の数学的関数として第5の指数を計算する段階を更に含み、各指数が、ゼロ値を 含む範囲を備えた重み係数を有することを特徴とする請求の範囲第40項に記載 の方法。 42.患者の気道での流れ制限を検出するための呼吸装置であって、 患者の気迫と流体連通する鼻取付具を備え、該鼻取付具が、入口と、入口と 患者の気迫との間に位置決めされた流れ制限器とを有し、 鼻取付具と流体連通し、患者への周囲空気の吸気流を表す第1のデータ値を 発生させるように形成された流れセンサと、 第1のデータ値が患者の流れ制限を示すか否かを決定し、かつ、流れ制限を 表す第2のデータ値を発生させるように形成されたマイクロプロセッサと、 流れセンサによって発生された第1のデータ値、及びマイクロプロセッサに よって発生された第2のデータ値を記憶するように形成されたコンピュータメモ リとを備えていることを特徴とする装置。 43.患者の物理的位置を表す第3のデータ値を発生させるように形成された位置 センサを更に備え、コンピュータメモリが、第3のデータ値を記憶するように形 成されていることを特徴とする請求の範囲第42項に記載の装置。 44.患者の血液中の酸素のヘモグロビンの飽和量を表す第4のデータ値を発生さ せるように形成された酸素濃度計を更に備え、コンピュータメモリが、第4のデ ータ値を記憶するように形成されていることを特徴とする請求の範囲第42項に 記載の装置。 45.流れセンサが、患者からの周囲空気の呼気流を表す第5のデータ値を発生さ せるように形成されており、コンピュータメモリが、第5のデータ値を記憶する ように形成されていることを特徴とする請求の範囲第42項に記載の装置。 46.マイクロプロセッサが、記憶されたデータ値を正弦波形と相関させ、記憶さ れたデータ値を平面度について分析し、記憶されたデータ値を呼吸努力について 分析し、記憶されたデータ値を相対流量について分析するための処理手段を有す ることを特徴とする請求の範囲第42項に記載の装置。 47.閉塞性睡眠無呼吸の患者を診断するための方法であって、 患者の睡眠中に患者の家庭で使用するための診断用呼吸装置を患者に提供す る段階と、 患者が診断用呼吸装置を使用している際、患者の気道への吸気流を表す第1 のデータ値を記憶させる段階と、 第1のデータ値を分析して、患者の気道での流れ制限の存在を決定する段階 と、 患者の睡眠中に患者の家庭で使用するための自動調整式治療用呼吸装置を患 者に提供する段階と、 患者が自動調整式呼吸装置を使用している際、患者の気道への吸気流を表す 第2のデータ値と患者の気迫圧を表す第3のデータ値を記憶させる段階と、 第2のデータ値を分析して、患者の気道での流れ制限の量を決定する段階と 、 第3のデータ値が大きな圧力変動を示すとき、家庭での睡眠時の使用のため 、自動調整式治療用呼吸装置を提供する段階と、 を備えていることを特徴とする方法。 48.第3のデータ値が大きな圧力変動を示さないとき、家庭での睡眠時の使用の ため、一定の正の気道圧の呼吸装置を提供する段階を更に備えていることを特徴 とする請求の範囲第47項に記載の方法。 49.第1のデータ値が、患者に流れ制限が存在することを示さないとき、患者を 睡眠研究室にまわす段階を更に備えていることを特徴とする請求の範囲第47項 に記載の方法。 50.第2のデータ値および第3のデータ値が、患者が正常呼吸に戻ったことを示 さないとき、患者を睡眠研究室にまわす段階を更に備えていることを特徴とする 請求の範囲第47項に記載の方法。
