Abstract

PROBLEM TO BE SOLVED: To obtain the subject preparation useful for treating bed sores, skin ulcers, etc. by including a base agent, a pharmaceutically active ingredient and insulin. SOLUTION: This preparation comprises a base agent (a higher aliphatic alcohol, a hydrophilic polymer, a lower alcohol, natural rubber, etc.), a pharmaceutically active ingredient (a disinfectant, an antibiotic, etc., preferably a skinforming agent or a bulbil-forming agent) and further insulin (prompt, semi- prompt, intermediate, long-acting or biphasic isophane insulin aqueous suspension) preferably in an amount of 0.05-20 units per g of the skin preparation containing zinc for external use, and can stimulate the formation of bulbil and skin in a diseased site without producing a side effect. The preparation can be formed in the form of ointments, creams or adhesive coatings to apply to diseased sites for long times.

The external preparation for skin of the present invention may be in any form such as ointment, cream, solution, gel, aerosol, and sheet (adhesive) attached to a tacky substrate. In other words, it refers to a preparation that is applied outside the skin, which is applied to the skin and causes the medicinal ingredient to act directly on the skin. Ointments and adhesives are preferred because they can be applied to the affected area for a long time. Pharmaceutical components of skin external preparations for skin diseases such as pressure ulcers and skin ulcers (burn ulcers, leg ulcers) include, for example, disinfectants, antibiotics, enzymes, etc., sterilization, improvement of blood flow, promotion of granulation formation, promotion of epidermal formation. Used for this purpose contain one or more conventionally known components. As an example of a conventionally known drug, as a disinfectant, povidone iodine, iodoform,
Chlorhexidine gluconate, and the like.Examples of antibiotics include erythromycin, tetracycline, chromium fenule, sodium fusidate, gentamicin sulfate, kanamycin sulfate, fradiomycin sulfate, and gramicidin.Sulfa drugs include sulfadiazine, silver sulfadiazine, Examples include nadifloxacin and the like, and examples of the enzyme include lysozyme chloride, alcloxa, tocoretinate, and pucladecin sodium.

[0010] Insulin is used in an amount of 0.
In order to contain 0.5 to 20 units, the maintenance amount is 0.4 units /
kg-day. When used for the treatment of diabetes, which is the original use of insulin, the maintenance dose is about 0.4 units / kg · day by subcutaneous intramuscular injection. That is, for a person weighing 50 kg, 20 units / day can be said to be a maintenance amount, that is, a safe amount that does not cause a low blood sugar level.
Since it acts on the skin surface as an external preparation, it has a composition with little transdermal absorption of insulin. However, even if the safety factor is large and about 50% absorption occurs, 40 units /
It is unlikely that hypoglycemic levels will occur on topical day use. In addition, skin diseases such as pressure ulcer, skin ulcer (burn ulcer, leg ulcer),
It affects almost all adults and the elderly, and the size of the lesion is up to 10
cm × 10 cm. Apply 10 to this lesion
When applying an ointment to a gauze of cm × 10 cm, an ointment of 2 g is required when applied thinly and 20 g when applied thickly. When 40 units of insulin are contained in 2 g of ointment, skin external preparation 1
Insulin amount of 20 units per gram, insulin 4
When 0 units are contained in 20 g of ointment in the case of thick coating, the amount of insulin becomes 2 units per 1 g of the skin external preparation. If the percutaneous absorption of an ointment-like external preparation for skin is suppressed, a larger amount of insulin can be contained. For skin external preparations that are applied to substrates such as cloths, sheets, meshes, band-shaped members, or films and adhered and used, the amount of insulin applied to the substrate should be 20 units or more per g of the preparation because the amount of application to the substrate is small. Can also. However, the effect can be exerted in a low concentration range when a preparation having a strong skin permeation action is used with a cream base or the like, or when the skin permeation action is enhanced by a sealing treatment. The reason for containing 0.05 unit or more of insulin per 1 g of the skin external preparation is that
If the amount is less than 0.05 units, the effect is not remarkable. The insulin content of an ointment base, a cream base, or other bases is preferably about 0.1 to 10 units per gram of an external preparation for skin, and 0.1 to 10 units per gram of a zinc-containing preparation.
About 1 to 1 unit is more preferable.

(Use Example 1) It was used for treatment of a post-burn skin ulcer formed due to a low-temperature burn due to a hottie of a bedridden person. Patients who had been treated with conventional treatments had poor nutrition and were unable to move at all, and the ulcer was 12 cm long.
The short part was about 7 cm, and gradually shrank and began to be locally mummified. The above-mentioned ointment-like preparation for external use on the skin was spread on a gauze and applied at a rate of once a day on the affected part. One week later, formation of pink granulation was observed, and two weeks later, the ulcer site was reduced to about 8 cm in the long part and about 4 cm in the short part, and the depressed ulcer floor was raised to the level of the skin surface, Has become epithelial. After 8 weeks the ulcer was gone.

(Use Example 2) It was used for treatment of pressure sores on the buttocks of a bedridden elderly person. Patients receiving conventional therapies were unlikely to cure, despite prolonged treatment. In this affected area, the formation of pink granulation was gradually observed at the site of the pressure ulcer, and thereafter, the bottom of the affected area gradually rose to the height of the skin surface, and the peripheral area became epithelialized, and finally healed completely.
