Automated blood pressure measurements are rapidly gaining acceptance and, in many cases, becoming an essential feature for human and veterinary procedures. Blood pressure monitors of this type have now become a routine part of the patient environment in emergency rooms, CCUs, ICUs, and surgical settings. The so-called oscillometric method of blood pressure measurement is one of the most popular methods in commercially available devices. This method relies on measuring the changes in pressure applied to an artery, such as that exerted by an elastic cuff that is pressurized and released in a controlled manner. In some cases, the cuff pressure change is continuous, and in other cases it is stepwise. In almost all cases, a transducer detects arterial pressure amplitudes, and a processor converts selected parameters of these amplitudes into blood pressure data.

Of particular importance with respect to the principles of the invention, M.
M. Ramsey's U.S. Patent No.
This is a concept described in Nos. 4360029 and 4394034. The Ramsey patents come from a common source, the former disclosing an apparatus and the latter disclosing a method. Both patents have a common disclosure of apparatus and methods for artifact rejection in oscillometric systems, which were developed into the commercially successful ``DINAMAP'' system. It has been implemented in the brand Monitor, which is manufactured and marketed by Critikon, Inc., Tampa, Fla., the assignee of the present application. According to the Ramsey patent, an elastic cuff is properly placed on a patient's limb and air is delivered by a pump to a predetermined pressure. Thereafter, the cuff pressure is decreased by a predetermined constant value and pressure fluctuations at each level are detected. These generally consist of a DC voltage with a small, superimposed fluctuating component (referred to herein as an "oscillatory complex") caused by arterial blood pressure oscillations. Therefore, after appropriate filtering to remove the DC component and amplification, the pulse peak amplitude above a given threshold level is measured and stored. As the attenuation continues, the peak amplitude will increase from a lower than normal amount to a relative maximum amount, and then decrease. The lowest cuff pressure at which the oscillations have the highest peak value represents the mean arterial pressure. Once the vibrational complex pulse peak amplitude is stored, the resulting cuff pressure has a predetermined fractional relationship to the maximum stored peak corresponding to the subject's systolic and diastolic pressures.

The Ramsey patent represents significant achievement and disclosure in the rejection of artifact data in order to obtain accurate blood pressure data. In fact, as is clear from Figure 2 of the Ramsey patent, the most important part of the measurement cycle (referred to as "T3") consists of various pressure levels,
It is directed to performing complex detection in the measurement of signal peaks of genuine complexes and peak processing according to an algorithm of artifact rejection. Despite these efforts, as the cuff pressure at which the artery is occluded decreases at a constant rate, the signal peak data collected may sometimes be erroneous, i.e., consistent with the subject's general physiological response pattern described above. Contains data patterns that do not.

Further, U.S. Patent Application No. 751,840 to M. Ramsay et al., filed on the same day as the present application, entitled "Oscillometric Blood Pressure Monitor Utilizing Non-Uniform Pressure Attenuation Steps," describes a method for oscillometric blood pressure measurement. is performed by a non-uniform pressure decrease depending on the cuff pressure between the intervals of oscillatory complex peak measurements. The method of performing such oscillometric blood pressure measurements is facilitated by previously unutilized systolic, diastolic, and mean blood pressure measurement algorithms.

A blood pressure cuff is placed around the subject's artery and pressurized above systolic levels so that the artery is sufficiently occluded for the maximum cardiac cycle. after that,
The cuff pressure is decreased to allow blood flow to increase by gradually loosening the degree of occlusion of the artery, and measurements of the peak amplitudes of successive vibrational complexes are stored in memory. The cuff pressure obtained for each stored complex peak is also stored.

Briefly, an artery occlusion cuff is placed on a subject, eg, around the subject's upper arm and over the brachial artery. At the beginning of a measurement cycle, the cuff is pressurized to a pressure sufficient to occlude the brachial artery, ie, to prevent blood from flowing through it at any point during the cardiac cycle. The cuff is then deflated in stages as in the attenuation step. A pressure transducer is connected to detect the internal pressure of the cuff and is used when a blood pressure oscillatory complex begins to occur (i.e., at an arterial occlusion where maximum cardiac pressure corresponding to contraction of the left ventricle of the heart is instantaneously obtained). (when the cuff pressure is exceeded), it generates an analog signal characterizing the blood pressure oscillatory complex. The peak value of the complex signal is determined by hardware or software.

As the measurement cycle progresses, the peak amplitude of the blood pressure complex generally increases monotonically to a maximum value and then decreases monotonically as the cuff pressure continues to decrease toward deflation. The peak amplitude of the cuff pressure oscillation complex and the corresponding occlusion cuff pressure value are maintained in computer memory.
The above-mentioned Ramsey patents and patent applications exemplify algorithms previously used to process stored blood pressure complex peak values and accompanying pressure values to obtain a subject's mean arterial pressure. ing. These patents and patent applications describe detailed methods for oscillatory complex peak measurements and the elimination of bad data and complexity related to complexes and measurement-interfering artifacts (e.g. motion) during the measurement cycle. It also provides methods for testing.

Oscillometric blood pressure measurement, as featured by the Ramsey disclosure cited above, uses a read-only memory (ROM or PROM) and a read-only memory (ROM or PROM) to measure cuff pressure, oscillatory complex peak amplitude, and other information under stored program control. The processing is performed via a microprocessor operating in conjunction with a program that includes variable content random access memory (RAM) for storing operand variables. The microprocessor accepts cuff pressure readings as processed by a pressure transducer, e.g., as processed by a peak detector, an amplifier, and an analog-to-digital converter, and generates the required full output control signal. for example, to open and close one or more cuff deflation valves.

The oscillometric method described in more detail in the Ramsey patents and patent applications mentioned above may be practiced with several variations. Thus, for example, the cuff may be directly pressurized by an air pump under microprocessor control and deflated in predetermined discrete steps. Alternatively, the cuff is partially or fully pressurized by pressurized air within the container, and/or deflation is performed via selected one or more of a plurality of deflation valves to increase the cuff pressure. It may also proceed variably with dependent steps. These latter alternatives achieve the desired result of reducing the time required for multiple measurement cycles of operation.

Alternatively, methods exist for measuring vibrational peak amplitudes at all prevailing cuff pressures. According to one embodiment already utilized, at each cuff pressure step during cuff deflation,
Multiple (eg, two) complex peaks are measured and their average is used as the peak value. The peaks should be approximately equal, so any significant discrepancies (e.g. >20%)
indicates that some artifact error has occurred and the data is rejected. In fast (stat) mode, after several intervals qualifying companion complexes (close or equal peak values) have been detected and confidence in the measurement has been established, the required Only one pulse is applied, thereby speeding up the composite measurement period. In this regard,
See the aforementioned application, filed July 9, 1984.

As mentioned above, when testing blood pressure complexes for peak amplitude at all occlusion pressure levels, inadequate data can sometimes occur. There are causes of variation for such abnormalities. Perhaps the most common are disturbances by the subject (e.g. moving);
This can create inappropriate pressure impulses within the kuff, which can be sensed by the pressure transducer and then inaccurately reflected in blood pressure measurements. Other causes include fluctuating sources of interfering electrical noise or changes in the subject's internal heart or breathing. When an erroneous complex peak amplitude value occurs, it is rejected by the complex measurement device and this rejected signal value (eg, +1) is held in place in the storage device.

We now turn our attention to the data processing under stored program control to purify the data collected by the blood pressure measuring device. Additionally, one example of an effective algorithm for actually measuring a subject's systolic, diastolic, and mean arterial pressures is specifically discussed. This type of data processing can be performed by computing devices, preferably digital microprocessors, commercially available from many vendors. The program instructions and sequences set forth below are for illustrative purposes only. In practice, such an instruction
It is actually possible in different programming languages and sequences that will be readily apparent to those skilled in the art.

The cuff pressure is rapidly increased to a maximum value above the subject's systolic pressure, and then is decreased to some point below the diastolic pressure in each sequence of steps. The sequence of cuff deflation steps is indicated by time intervals 1, 2, . . . (bottom row 18 in the data table section of FIG. 1). The internal pressure characterizing the cuff pressure at each step i is a data array.
It is given by CP1, CP2, ... (upper column 12 of the data table).

Each step (time interval) is long enough to encompass at least two heart beats.
Therefore, after such a pulse is initiated, at least two cuff pressure complex pulses 21i and 22i are measured during each interval. In FIG. 1, the processing performed on some of the pulses occurring during contraction steps 6 and 9 in order to eliminate defective pulses is explained as follows. No pulses are measured during the first and second pressure steps (time intervals), but this is due to the cuff pressure [CP1 = 201 Torr and CP2 = 194 Torr].
[Toll] is estimated to be sufficient to eliminate blood flow through the subject's arteries for the entire cardiac cycle during these periods. During the next intervals 3, 4..., the two oscillometric complexes
Pulses 21i and 22i are generated and measured, these two pulses having an average peak amplitude 23i
(The processor variable array values are initially stored in ΦA(I)). This measured vibration amplitude array (ΦA(I)) is shown in the second column 14 of the data table of FIG. 1 for each time interval.

The data flow to accomplish the data purification algorithm described above is shown in the program flowchart of FIG. FIG. 2 operates on the measured average vibration amplitude (second data table column 14 in FIG. 1) and shows the correction ΦA(I ) value. For this purpose, start block 1
Step 15 following 0 (Fig. 2) is the next value ΦA(I)
(proceeding to the right along data table column 14 of FIG. 1) and test 18 determines whether the value stored in ΦA(I) is equal to the error signal value +1. If it is not +1, as is usually the case (the measured value inferentially indicates that there was no function with artifacts, etc.), control proceeds to an equality test. However, if the content of ΦA(I) is equal to +1 (the "yes" branch of test 18), then
Function block 23 implements Equation 1. In other words, the content of the memory cell ΦA(I), which is +1 corresponding to the cuff pressure CP(I), is changed to the next lower value (ΦA(I−
1)) and the next higher non-plus one (ΦA(I
+1))) by the average value of the vibration amplitudes measured in each contraction step. processing step
18 and 23 thus remove all +1 values of the measured pressure peak amplitude memory contents (column 2 of the data table in Figure 1) and transfer them to the immediately adjacent contraction step (corrected ΦA(I) The content of is replaced by the average value of the measurements made in column 15).

That is, the modified array ΦA(I), as shown by the third column 15 of FIG. Contains values by which each of the systolic, diastolic, and mean arterial pressures can be measured. The data purification described above provides more accurate measurements than in the traditional case, and also allows blood pressure to be measured more quickly, eliminating repeated dilation when unacceptable artifacts or noise-contaminated data are detected. Eliminates the need for steps.

(b) Find the amplitude level (LVL), which is the value obtained by multiplying the peak value ΦA (MAX) by a predetermined coefficient.
We obtained a satisfactory coefficient of 0.5 for normal processing. For high speed (rapid inflation and/or single pulse) operation, a slightly lower value (e.g., 0.45) is better; (c) starting at the MAX interval and moving toward higher cuff pressure (i.e., Figures 1 and 3); left)

is calculated, and the amount of linear dispersion of vibration amplitude and cuff pressure between intervals L and L+1 is estimated. Linear trapezoidal interpolation, which is known per se, is illustrated in the graph of FIG. The interpolated cuff pressure corresponds directly to the subject's systolic pressure (SYS). To explain the methodology of systolic pressure measurement mentioned above in more detail,
The cuff pressure interval I=MAX at which the maximum vibration amplitude peak occurs is determined by essentially any known method (step 40 of the flowchart of Figure 4 corresponding to interval MAX of Figure 3). .

An initial assumption is made to load a temporary peak value that stores . For an assumed N time interval measurement, the loop between Equations 6 and 10 sequentially tests all elements of the ΦA(I) array from 2 to N, with the value ΦA(K) (K being the loop index) first. The above procedure is illustrated in the blood pressure complex display flowcharts of FIGS. 6 and 7. The peak ΦA(MAX) is first located by processing Equation 4-10. The upper and lower peak amplitude fractional parts DIAU and DIAL are then determined (steps 64 and 65 in FIG. 7 correspond to the labeled horizontal dash lines in FIG. 6). Then,
In step 69, starting from MAX, the peak amplitude ΦA
Find the first time interval (CD) at the point where (UD) is lower than the value stored as DIAU (“MAX
-J" is replaced by "MAX+J" by processing similar to Equations 12 to 15. ). Then step 72
Then, a trapezoidal interpolation similar to FIG. 5 is performed to determine the sleeve cover pressure (DIAU) corresponding to the UDLVL complex amplitude value. The time interval UD-1 is the interval when the peak complex value occurs.
It is observed that it is consistent with MAX. It is less than 0.69×ΦA(MAX) for the illustrated data.
This is because the first pulse complex following MAX occurred during the next time interval MAX+1.

Function steps 73 and 74 in FIG.
and 72 are performed in a very similar manner. That is, the peak complex amplitude is ΦA
When an LDLVL value equal to 0.55 times the (MAX) is formed, the cuff pressure is determined by interpolation for that interval.
Decide on the DIAL. This latter search starts with ΦA(i) at the lowest CP and then works its way to higher CPs. Finally, the subject's diastolic pressure (DIA) is calculated as the average value of the contents stored in DIAU and DLAL (step 82).

The above algorithm for measuring mean arterial pressure is illustrated in FIGS. 8 and 9. Step 101 (FIG. 9) obtains the peak spacing MAX (eg, by an implementation comparable to Equation 4-10).
The process variable AMP is set equal to the peak value of the complex following interval MAX (step 105), and the interval MN1 is set equal to the value of AMP (i.e., ΦA) to the left of time MAX in FIG.

Thus, the foregoing discussion indicates that measurement data can be enhanced by replacing lost data or deviations from the proper data pattern, such as due to measurement artifacts, with approximate values. Specific data processing algorithms were presented and discussed for calculation of a subject's measured systolic, diastolic, and mean arterial pressures.

The above configuration is merely illustrative of the principles of the present invention. Numerous variations and adaptations thereof will be readily apparent to those skilled in the art without departing from the spirit and scope of the invention. For example, the pressure measurement mode is described above as a gradual deflation from an initial inflation above the subject's systolic pressure. Measurements of the present invention can be selectively performed by stepwise inflation from an initial sub-diastolic cuff pressure or by sequential inflation or deflation of separate independent cuffs.
