(項目1)
患者の体内の血管からの血栓の除去のための方法であって、前記方法は、
一体型有窓構造から形成されている近位自己拡張式部材と、前記一体型有窓構造に取り付けられている網状フィラメントメッシュ構造から形成されている遠位略円筒形部分と、前記網状フィラメントメッシュ構造の遠位端に接続されている内側シャフト部材とを備えている、血栓抽出デバイスを提供することと、
前記血栓抽出デバイスを拘束するカテーテルを血管血栓を通して前進させることと、
前記血栓抽出デバイスを前記カテーテルの遠位端を越えて前進させるか、または前記カテーテルを前記血栓抽出デバイスに対して後退させるかのいずれかによって、前記血栓抽出デバイスを展開し、したがって、前記血栓抽出デバイスを前記血栓の一部を過ぎて遠位に露出し、前記血栓抽出デバイスの拡張が前記血管の壁に係合することを可能にすることと、
前記血栓抽出デバイスを後退させ、前記血栓の一部を前記脈管壁から分離し、前記血栓の一部を前記網状フィラメントメッシュ構造内に捕捉することと、
前記血栓抽出デバイスを前記体から引き抜き、血栓を前記患者から除去することと

(項目2)
前記カテーテルを前進させることは、前記カテーテルの放射線不透過性遠位先端が前記血栓部分を遠位に過ぎるまで、前記カテーテルを前記血管の中に挿入することを含む、項目1に記載の方法。

(項目5)
前記血栓部分は、前記ステント部分の近位端に位置する少なくとも1つの開口または開口部を介して、前記網状フィラメントメッシュ構造に進入することによって、前記網状フィラメントメッシュ構造を用いて捕捉される、項目4に記載の方法。

前記血栓抽出デバイスは、前記カテーテルを通して中間シャフトを遠位に前進させることによって、前記カテーテルの遠位端を越えて前進させられ、前記中間シャフトは、前記カテーテルを通してスライド可能に延び、前記中間シャフトの遠位端は、前記有窓構造の近位端に結合されている、項目1~5のいずれかに記載の方法。

アクセス部位を通して、前記導入器シースを用いて、患者の静脈血管に経皮的にアクセスすることであって、前記アクセス部位は、膝窩アクセス部位、大腿アクセス部位、または内頸アクセス部位を備えている、ことと、
前記導入器シースの遠位端を血栓の近位の位置まで前進させることと、
前記自己拡張式漏斗を前記捕捉シース内に拘束された構成から前記捕捉シースから解放された拡張された構成に展開することと、
血栓を前記自己拡張式漏斗内に捕捉することと、
前記捕捉された物質を前記細長いシースの管腔を通して吸引することと
を含む、方法。

前記自己拡張式漏斗を展開することは、前記閉塞具を前記細長いシースに対して遠位に前進させることにより、前記自己拡張式漏斗を前記拘束された構成から前記拡張された構成に抜き出すことと、前記閉塞具を前記展開された自己拡張式漏斗を通して、および前記細長いシースの管腔を通して近位に後退させることによって前記閉塞具を前記導入器シースから除去することとを含む、項目1~44のいずれかに記載の方法。

前記自己拡張式漏斗を展開することは、前記シースを前記閉塞具を覆って近位に後退させることにより、前記自己拡張式漏斗を前記拘束された構成から前記拡張された構成に抜き出すことと、前記閉塞具を前記展開された自己拡張式漏斗を通して、および前記細長いシースの管腔を通して近位に後退させることによって前記閉塞具を前記導入器シースから除去することとを含む、項目1~45のいずれかに記載の方法。

固有の対のワイヤを前記形成点の各々に通すことであって、前記固有の対のワイヤは、第1のワイヤと、前記第1のワイヤに重なる第2のワイヤとを備えている、ことと、
前記第1のワイヤおよび前記第2のワイヤのうちの少なくとも一方が前記第1のワイヤおよび前記第2のワイヤが通されている形成点の周りにループを形成しないように、かつ、前記第1のワイヤおよび前記第2のワイヤのうちの他方が前記第1のワイヤおよび前記第2のワイヤが通されている形成点の周りにループを形成するように、前記網状フィラメントメッシュ構造を前記固有の対のワイヤから編組することと
を含む、方法。

Claims (8)
- 患者の血管から血管血栓を除去するための血栓切除術システムにおいて使用される導入器シースアセンブリであって、前記導入器シースアセンブリは、
近位端部分と遠位端部分とを有する細長いシースであって、前記細長いシースは、前記近位端部分と前記遠位端部分との間に延びている管腔を画定する、細長いシースと、
前記細長いシースの前記遠位端部分に結合されている自己拡張式漏斗であって、拘束された構成と拡張された構成とを有する自己拡張式漏斗と、
前記細長いシースの前記管腔内に位置付けられるように構成されている細長い閉塞具であって、前記細長い閉塞具は、細長いシャフトと、前記細長い閉塞具の遠位端において前記細長いシャフトに接続されている捕捉シースとを含む、細長い閉塞具と
を備え、
前記捕捉シースは、前記捕捉シースの遠位端から前記捕捉シースの近位端まで延びている捕捉シェルを含み、
前記捕捉シェルは、前記捕捉シェルを通って延びている前記細長いシャフトの部分の直径よりも大きい内径を有し、これにより、前記捕捉シェルと前記捕捉シェルを通って延びている前記細長いシャフトの前記部分との間に受容空間が作成され、
前記受容空間は、前記自己拡張式漏斗を前記拘束された構成に保持し、
前記自己拡張式漏斗は、前記受容空間から前記自己拡張式漏斗を解放するように前記細長い閉塞具が遠位方向に前進する場合には、前記拘束された構成から前記拡張された構成に拡張するように構成されている、導入器シースアセンブリ。 - 前記導入器シースアセンブリは、密閉されたハブをさらに備え、前記密閉されたハブは、前記細長いシースの前記近位端部分における吸引ポートを含み、前記吸引ポートは、前記血管血栓を吸引するためのものであり、前記吸引ポートは、弁を介して、前記細長いシースの前記管腔に流体接続されている、請求項1に記載の導入器シースアセンブリ。
- 前記自己拡張式漏斗が前記拘束された構成にあるとき、前記自己拡張式漏斗は、最大の直径を有し、前記最大の直径は、前記細長いシースの直径に等しいかそれより小さい、請求項1に記載の導入器シースアセンブリ。
- 前記細長い閉塞具は、前記細長い閉塞具の遠位端部分に位置付けられている先端を含む、請求項1に記載の導入器シースアセンブリ。
- 前記細長い閉塞具の遠位端部分は、前記受容空間が前記自己拡張式漏斗を前記拘束された構成に保持しているとき、前記細長いシースの前記遠位端部分をシールするように構成されている接続継手を含む、請求項1に記載の導入器シースアセンブリ。
- 前記自己拡張式漏斗は、血液に対して浸透性である、請求項1に記載の導入器シースアセンブリ。
- 前記自己拡張式漏斗は、前記拡張された構成において円錐形形状を有する、請求項1に記載の導入器シースアセンブリ。
- 前記自己拡張式漏斗は、レーザ切断ニチノール、レーザ切断ポリマー管、膨張可能バルーンのうちの少なくとも1つから形成されている、請求項1に記載の導入器シースアセンブリ。
