Description
2016年6月20日に提出された「経心尖除去装置」(Transapical Removal Device)と題する米国仮特許出願第62/352235号の優先的利益が主張され、その内容全体が、参照により本明細書に援用される。
僧帽弁逆流を治療するために、投薬、開胸手術及びカテーテル処置を含めた多様な治療の選択肢が開発されてきた。1つのカテーテル処置は、僧帽弁の機能を改良するために僧帽弁の弁尖を一緒にクリップ留めすることを含む。アレルギー反応、クリップの移動又は感染症など特定の状況において、僧帽弁クリップの除去が必要である。残念ながら、僧帽弁クリップ留置後、僧帽弁クリップは、外科的にしか除去できず、将来の経皮的僧帽弁置換の可能性がなくなって、将来の罹患率及び死亡率を高める。僧帽弁クリップは、開胸手術を必要としないカテーテル処置によって除去できると好ましい。更に、僧帽弁逆流を患う患者の一部は、経カテーテル僧帽弁置換手術を受け、その後、左心室流出路(LVOT)閉塞を生じる。僧帽弁前尖は、多くの患者のLVOT閉塞の病因において大きな役割を果たすものとして特定されている。僧帽弁クリップを除去するためのカテーテル処置は、LVOT閉塞などの将来の合併症を防止するために経カテーテル僧帽弁置換への第1段階として僧帽弁前尖を切除、除去又は修復できることが好ましい。
なお、本開示の背景技術として、特許文献1に記載の「枢動防止機構を伴う人工弁」、特許文献2に記載の「僧帽弁固定装置の取り外し装置と方法」、特許文献3に記載の「全面的な心肺保持を要さない心臓弁や近傍の組織の修理用や交換用の装置と手段」などが知られている。

本発明には、実施態様として、以下の参考例22ないし43も含まれる。

僧帽弁クリップを除去する又は前記僧帽弁の前尖を除去又は改変する経心尖方法であって、
左心室の中へその後左心房の中へ挿入するためのガイドカテーテルを提供することと、
前記左心室の中へ挿入するための送達カテーテルを提供することと、
(i)前記ガイドカテーテルを通過して前記送達カテーテルを移動するための折畳み状態と、(ii)前記僧帽弁の少なくとも一部分を束縛するために前記僧帽弁に事前配置された僧帽弁クリップ又は前記僧帽弁の前尖を捕捉するための展開状態と、を有するスネアヘッドを提供することと、

Claims (21)
- ガイドカテーテルを使用して僧帽弁付近に留置されるように構成された送達カテーテルと、
前記送達カテーテルの遠位端のスネアヘッドであって、前記スネアヘッドが、
(i)前記ガイドカテーテルを通過して前記送達カテーテルを移動するための折畳み状態と
(ii)前記僧帽弁の少なくとも一部分を束縛するために前記僧帽弁に事前配置された僧帽弁クリップを捕捉するための、又は前記僧帽弁の前尖の改変又は除去のために捕捉するための、展開状態と、を有する、スネアヘッドと、
前記送達カテーテルの近位端に接続されかつ前記折畳み状態と前記展開状態との間の前記スネアヘッドの移行を制御するように構成されたスネアヘッドコントローラと、
を備え、
前記スネアヘッドが、少なくとも部分的に前記事前配置された僧帽弁クリップ又は前記前尖を取り囲むためのスネアバスケットを有し、かつ前記事前配置された僧帽弁クリップを取り囲む組織又は前記前尖を切除するように構成された1つ又は複数の切除用送達カテーテルを有し、
前記スネアヘッドコントローラが、前記展開状態において前記スネアバスケットの位置及び/又はサイズを制御するように構成され、前記スネアヘッドコントローラが、前記展開状態において前記組織への切除源送達を制御するように構成される、
経心尖除去装置。 - 更に、前記送達カテーテルを留置するために使用される前記ガイドカテーテルを備える、請求項1に記載の経心尖除去装置。
- 前記1つまたは複数の切除用送達カテーテルの各々の近位端が、前記僧帽弁クリップの除去又は前記前尖の除去又は改変を可能にするために、前記僧帽弁クリップに隣接する組織又は前尖を切除するための高周波エネルギーを供給するための電極を備える、請求項1又は2に記載の経心尖除去装置。
- 更に、
電極と電気的に連絡する電源と、
スイッチであって、前記スイッチが、前記電源から前記電極への電流の流れを交互に許容し停止する、スイッチと、
を備える、請求項1~3のいずれかに記載の経心尖除去装置。 - 前記1つ又は複数の切除用送達カテーテルの各々の近位端が、前記僧帽弁クリップの除去又は前記前尖の除去又は改変を可能にするために、前記僧帽弁クリップに隣接する組織又は前尖を切除するためのレーザー切除信号を送るために配置された光ファイバを備える、請求項1~4のいずれかに記載の経心尖除去装置。
- 更に、切除源を備え、前記切除源が高周波源、レーザー源及び寒剤熱源の1つである、請求項1~5のいずれかに記載の経心尖除去装置。
- 更に、除去された僧帽弁クリップを置換するために経カテーテル弁を前記僧帽弁の中へ留置するための留置機構を備える、請求項1~6のいずれかに記載の経心尖除去装置。
- 前記切除源が250~500kHzの範囲の高周波信号である、請求項6に記載の経心尖除去装置。
- 前記スイッチが遠隔的に制御される、請求項4~8のいずれかに記載の経心尖除去装置。
- 前記電源がバッテリである、請求項4~9のいずれかに記載の経心尖除去装置。
- 前記留置機構が前記送達カテーテルを備え、前記送達カテーテルが、前記経カテーテル弁を収容しかつ弁置換モードにおいて前記除去された僧帽弁クリップを置換するために前記僧帽弁の中へ前記経カテーテル弁を送達するように構成される、請求項7~10のいずれかに記載の経心尖除去装置。
- 前記スネアバスケットが、医療用プラスチック及び医療用金属の少なくとも1つを含む、請求項1~11のいずれかに記載の経心尖除去装置。
- 前記スネアヘッドが形状記憶材料を含む、請求項1~12のいずれか記載の経心尖除去装置。
- 前記形状記憶材料がニチノールである、請求項13に記載の経心尖除去装置。
- 更に、前記スネアヘッドを前記展開状態から前記折畳み状態へ強制的に戻すように構成された前記送達カテーテルの近位端の引込み漏斗を備える、請求項1~14のいずれかに記載の経心尖除去装置。
- 前記スネアヘッドが、前記スネアヘッドが前記折畳み状態のとき圧縮されかつ前記スネアヘッドが前記展開状態のとき静止するバネを備え、前記バネが、前記スネアバスケットが前記展開状態ではない限り前記スネアバスケット内で圧縮されるように構成される、請求項1~15のいずれかに記載の経心尖除去装置。
- 前記スネアバスケットが、第1バスケット側と第2バスケット側とを備える、請求項1~16のいずれかに記載の経心尖除去装置。
- 前記僧帽弁クリップ又は前記前尖の周りでの前記スネアバスケットの閉鎖を容易にするために前記スネアバスケットに磁石が設置される、請求項1~17のいずれかに記載の経心尖除去装置。
- 前記スネアバスケットが、前記僧帽弁クリップ又は前記前尖の周りで前記スネアバスケットを縛るためのコードを備える、請求項1~17のいずれかに記載の経心尖除去装置。
- 前記スネアヘッドが、閉鎖状態と開放状態との間で移動可能でありかつ前記スネアヘッドコントローラによって制御される把握ツールを備える、請求項1~19のいずれかに記載の経心尖除去装置。
- 前記把握ツールが、チューブ内部の位置と前記チューブ外部の位置との間で前記スネアヘッドコントローラによって制御可能に移動できる、請求項20に記載の経心尖除去装置。
