Description
本発明は、鎖骨近傍の位置において、頸部内の閉塞された大脈管を通して、患者の中心静脈系へのアクセスを得るために使用されるカテーテルシステムに関する。従来のやり方とは対照的に、中心静脈系は、鼠径部領域内の初期進入点と鎖骨近傍(通常、鎖骨の上方)の出射位置とを用いて、内外からアプローチされる。
本発明は、例えば、以下を提供する。

Claims (5)
- 患者の上胸部領域内の大脈管内の閉塞を通して、前記患者の中心静脈系へのアクセスを得るために、ユーザによって使用されるカテーテルシステムであって、前記カテーテルシステムは、
鎖骨近傍において前記患者の外部に適用される放射線不透過性標的であって、所望の出射点を規定する放射線不透過性標的と、
近位端と遠位端とを有する細長い作業カテーテルと、
前記作業カテーテルの近位端に結合されたユーザ操作ハンドルであって、前記作業カテーテルは、前記作業カテーテルの遠位端に近接する作業区分を有し、前記作業区分は、閉鎖された鈍的切開先端において終端をなし、前記作業カテーテルは、前記ハンドルに印加される、ユーザによって供給される力を介して、前記閉鎖された鈍的切開先端を閉塞内に進入するために十分な剛性押動能力およびトルク能力を有し、前記ユーザによる前記ハンドルの回転は、前記閉鎖された鈍的切開先端を回転させ、回転位置を規定し、前記ハンドルの平行移動は、前記閉鎖された鈍的切開先端を前進させ、前記ハンドルの平行移動は、前記閉塞内での前記閉鎖された鈍的切開先端の遠位端位置を規定する、ユーザ操作ハンドルと、
前記閉鎖された鈍的切開先端の近傍であるが、前記閉鎖された鈍的切開先端の近位の前記遠位先端における発射角ガイド管であって、前記発射角ガイド管は、湾曲部として形成された形状記憶区分を有し、前記ハンドルにおける設定に応答して、前記閉鎖された鈍的切開先端上の側面ポートから出現し、それによって、前記ハンドルの調節によって選択される可変発射角を提供し、それによって、針ワイヤのための定常発射角を規定し、前記針ワイヤ出射平面を規定するように適合されている、発射角ガイド管と
を備え、前記作業区分および標的は、ともに視覚化されることにより、望ましい発射角の決定を可能にし、出射平面および端部位置はともに、前記針ワイヤの狙いを前記標的に定めるように選択され、前記針ワイヤは、前記発射角ガイド管内に含まれ、前記発射角ガイド管内に位置し、前記発射角ガイド管に対する相対的な動きのために適合され、前記ハンドルが前記ユーザによって操作され得る場合に前記ハンドルを通過し、前記針ワイヤは、鋭的切開先端を有し、前記ユーザは、前記閉塞、脈管壁介在組織、および皮膚を通して、前記鋭的切開先端を押し込むことにより、前記患者の表面の前記標的位置において露出させ得る、カテーテルシステム。 - 前記閉鎖された鈍的切開先端は、
前記先端の回転角度の関数として、X線下のコントラストを変動させる放射線不透過性特徴を含み、
前記ユーザは、蛍光透視指導の下、前記出射平面に狙いを定め、前記出射平面に向けることができる、請求項1に記載のカテーテルシステム。 - 前記放射線不透過性特徴は、前記遠位先端の本体内に形成された開口である、請求項2に記載のカテーテルシステム。
- 前記ハンドルに対する回転のために適合された、前記ハンドル上に位置するノブであって、前記ノブは、キャプティブナットを回す、ノブと、
前記発射角ガイド管に取り付けられたねじ形態アセンブリであって、前記ねじ形態アセンブリは、前記ノブの回転が前記ガイド管の前進および後退を制御するように、前記キャプティブナットによって動作させられる、ねじ形態アセンブリと
をさらに含む、請求項1に記載のカテーテルシステム。 - 前記遠位先端に対する前記発射角ガイド管の相対位置を読み取るための目盛をさらに含む、請求項4に記載のカテーテルシステム。
