Description

本明細書は、超高純度一酸化窒素の送達のためのシステム及び方法に関する。
ニトロシルラジカルとしても知られる一酸化窒素(NO)は、フリーラジカルであり、重要なシグナル伝達分子である。例えば、NOは、血管の平滑筋を弛緩させ、これにより血管の拡張をもたらし、血流を増大させることができる。NOは、僅か数秒の寿命で極めて反応性が高く、体内で急速に代謝され得るため、これらの効果は、小さい生物学的領域に限定され得る。
治療的使用のために一酸化窒素(NO)を哺乳動物へ送達する場合は、哺乳動物への二酸化窒素(NO2)の送達を回避することが重要となり得る。二酸化窒素(NO2)は、酸素(O2)による一酸化窒素(NO)の酸化によって生じ得る。二酸化窒素(NO2)の生成率は、酸素(O2)濃度に一酸化窒素(NO)濃度の二乗を乗じた値に比例し得る、すなわち、(O2)*(NO)*(NO)=NO2である。NO送達システムは、二酸化窒素(NO2)を一酸化窒素(NO)に変換することができる。
1/T(Tは絶対温度である)に対するlog(NO)のプロットの傾きは、直線のはずである。一酸化窒素送達システムを使用して得られた典型的なプロットが図15に示されている。直線に対する小さな変動は、主に不十分な温度管理による実験誤差に起因し得る。流量は、毎分空気1リットルとした。
一酸化窒素送達システムは、著しい変動又は劣化なしに続けて何日も作動することができる。例えば、約36時間に亘る1回の実験における時間に対するNO、NO2、及びNO+NO2(ppm)の典型的なプロットが、図16に示されている。この実験では、NO2をNOに変換するカートリッジは存在しなかった。図16は、分単位でのリザーバの出力を示し、緑色の線がNOレベルを示し、黄色の線がNO2レベルを示し、そして黒色の線がNO+NO2応答を示している。いかなる理論にも拘束されるものではないが、最初のスパイクは、ロケット燃料酸化剤として、従来の使用の際に腐食割れを低減するために時としてN2O4に添加される約1%のNO不純物によるものである可能性が高かった。リザーバが、高い蒸気圧を有するため、NOが、初期段階で液体から酸化剤を除去する。
図17は、NO2を変換するためにNO変換カートリッジがシステムに含められたときの出力を示している。この実験では、データを、780分(13時間)に亘って収集した。データは、多少のドリフトを示しているが、このドリフトは、臨床用途に必要とされる±20%の十分に範囲内であった。
図18は、24時間に亘るNO及びNO2の出力を示している。ガス流がカートリッジを通過した後のNO2濃度は、本質的に0である。
Claims (13)
- 治療量の一酸化窒素を送達するためのシステムであって:
金属シースで覆われたリストリクタ及び加熱要素を備えたリザーバを含むリザーバアセンブリであって、ここで、該リストリクタが、第1の端部及び第2の端部を備え、該第1の端部が、該リザーバに接続されている、前記リザーバアセンブリ;
二酸化窒素源を含む透過管を含む保存チャンバであって、該リザーバアセンブリに接続されている該保存チャンバ;
ガス供給源;
表面活性物質及び還元剤を含む少なくとも1つのカートリッジを含む、治療量の一酸化窒素を送達するための送達管;及び
患者インターフェイス
を含む、前記システム。 - 前記リストリクタが毛細管を備えている、請求項1記載のシステム。
- 前記リストリクタ第2の端部が、前記送達管に接続されている、請求項1又は2のいずれか一項記載のシステム。
- 前記送達管が、入口及び出口を備えている、請求項1〜3のいずれか一項記載のシステム。
- 前記ガス供給源が、前記送達管の入口に接続されており、かつ前記患者インターフェイスが、前記送達管の出口に接続されている、請求項4記載のシステム。
- 前記二酸化窒素源が、液体四酸化二窒素を含む、請求項1〜5のいずれか一項記載のシステム。
- 前記ガス供給源が空気供給源である、請求項1〜6のいずれか一項記載のシステム。
- 前記空気供給源が空気ポンプである、請求項7記載のシステム。
- 前記ガス供給源が、前記送達管の入口に接続され、かつリストリクタの第2の端部が、該送達管に接続されている、請求項4〜8のいずれか一項記載のシステム。
- 前記リザーバ、前記リストリクタ及び前記カートリッジが、システムから除去することが可能な使い捨てモジュールを形成する、請求項1〜9のいずれか一項記載のシステム。
- 少なくとも1つの一酸化窒素センサ、二酸化窒素センサ、流量センサ、圧力センサ、及び大気圧センサを更に含む、請求項1〜10のいずれか一項記載のシステム。
- 32オンス(0.91kg)未満の重さである、請求項1〜11のいずれか一項記載のシステム。
