Description
本願は、米国仮特許出願第61/348,963号(名称「Energy−Optimal Biphasic Waveform Shapes for Neural Stimulation」、2010年5月27日出願)の利益を主張し、この出願は、本明細書に参照することによって援用される。

本発明は、哺乳類、特に、ヒトにおける神経を刺激するためのシステムおよび方法に関する。
図3は、中央神経系の組織を刺激するためのシステム10である。システムは、中央神経系組織と接触して、所望の位置に留置された導線12を含む。例示される実施形態では、導線12は、脳深部刺激の目的のために、視床、視床腹部、または淡蒼球等の脳の領域内に移植される。しかしながら、導線12は、治療目的を達成するための選択的刺激の目的のために、脊髄内、上、またはその近傍に、あるいは末梢神経(感覚または運動)内、上、またはその近傍に、筋組織(心組織を含む)または脂肪組織等の任意の皮下組織内に移植され得ることを理解されたい。加えて、導線12は、電極が皮下ではなく、外側皮膚表面上に留置される経表皮的刺激のために利用されてもよい。
(A.概説)
発明者らは、末梢神経刺激の計算モデルにおいて、遺伝的アルゴリズムを実装し、神経刺激のためのエネルギー最適の波形形状を決定した。GA波形のエネルギー効率を軸索集団の計算モデルにおける従来の波形形状のもの、および末梢神経ファイバの生体内刺激の間のものと比較した。
(1.概要)
遺伝的アルゴリズムは、生物学的進化の原理に基づくプロセスを介して最適解を求める。図4Aに示されるように、GAの第1の生成は、候補解の集団から開始する。図4Aにおいて、それぞれ、異なる波形(上昇ランプおよび正方形)を有する2つの候補刺激パラメータが存在する。候補解は、自然有機体に類似し、各候補を特徴付ける刺激パラメータは、その「遺伝子」である。
特異的遺伝的アルゴリズム(GA)は、図5Aおよび5Bに示される単一の有髄哺乳類末梢軸索の細胞外刺激の計算モデルにおけるエネルギー最適の波形形状を求めるために導出された。
図6Aは、GA波形導出プロセスの結果の概説を示す。
GAの各試験は、ランダム波形の異なる集団から開始したが、各試験の終了までに、GAは、一貫した高エネルギー効率の波形形状に収束した(図6Aおよび6Bが示すように)。生成エネルギーは、PW≦0.5msの場合、5000回の生成によって、PW=1および2msの場合、9000回の生成によって、最終生成エネルギーの1%以内に収束した。図7が示すように、各PWに対して、GA波形は、試験全体において非常に類似し、PWにわたってGA波形の形状は非常に類似していた。図6が示すように、PW≦0.2msに対して、GA波形は、パルスの中央近傍にピークを有する切断ガウス曲線に類似していた。PW≧0.5msに対して、GA波形の形状はまた、ガウス曲線に類似したが、無視可能な振幅の先頭および/または末尾を有する。
(1.集団モデル)
(i)方法論
GA波形は、3−mm直径を有するシリンダ内に一様に分布した100個の平行MRG軸索(11.5−μm直径)の集団モデルにおいて評価された。細胞外刺激は、シリンダの中心に設置された点電流源を介して送達された。各PW(0.02、0.05、0.1、0.2、0.5、1、および2ms)に対して、10個のランダムに配置された軸索の集団が選択された。各集団に対して、活性化されたファイバの数対E、および活性化されたファイバの数対Qについて入力/出力(I/O)曲線(図8参照)が作成された。波形の刺激振幅を調節するために、波形全体がスケーリングされた。各I/O曲線に対して、集団全体の50%を活性化するために必要とされるEおよびQが算出され、10個の軸索集団にわたるこれらの値の平均および標準誤差が計算された。同一の軸索集団を使用して、神経刺激において使用される従来の波形に対するI/O曲線が計算された。すなわち、矩形、上昇/降下ランプ、上昇/減衰指数関数、および正弦波形である(従来の波形に対する式については、付録を参照)。
(a)概説
GA波形は、集団モデルにおける全PWに対して、従来の刺激波形形状よりもエネルギー効率的であった。GA波形のエネルギー持続時間曲線は、上に凹であって(図9A参照)、PWにわたるGA波形に対する最小Eは、従来の波形形状に対する最小Eよりも小さかった。これらの他の形状のうち、GA波形に最も類似した形状、すなわち、正弦波は、PWにわたって最低の最小エネルギーを有していた。PW≦0.2msに対して、GA波形は、他の波形形状よりも若干エネルギー効率的(<20%)であった(図9B参照)。PW=0.2msと0.5msとの間では、GA波形と従来の形状との間のエネルギー効率差は、大幅に増加し、これらの差は、指数関数波形を除いて、全PWに関して、さらに増加した。軸索の位置は、集団モデルにおいてランダム化されたので、これらの結果は、GA波形の優れたエネルギー効率が、軸索に対する電極の場所から独立していたことを実証する。
図10Aに示されるように、エネルギー最適の波形形状は、概して、GAのパラメータにおける変動に対して鈍感であった。次の世代まで残存した波形の数または各世代における波形の数を2倍または半分にしても、GA波形の形状またはそのエネルギー効率に及ぼす実質的な影響はなかった(<0.1%差)。また、初期世代における波形の振幅は、元の振幅の0.4−1.6倍の間でスケーリングされたが、スケーリング係数>0.8は、GA波形の形状およびエネルギー効率に影響をほとんど及ぼさなかった(<0.1%差)。しかしながら、0.6を下回るスケーリング係数は、すべて閾値を下回る初期波形をもたらし、GAは、エネルギー効率的な波形に収束しなかった。加えて、突然変異において使用された正規分布の分散は、元の分散の0−4倍の間でスケーリングされた。分散=0(突然変異が無い)において、GAは、あるエネルギー効率的な波形に急速に収束した。しかしながら、分散の他の全ての値に対して、GAは、略同一のエネルギー効率を有する略同じGA波形を生成した(<0.4%の差)。
エネルギー最適化波形の厳密な形状のさらなる理解を得るために、GA波形を区分一般化正規分布に適合させた。
(i)手術準備
全動物の世話および実験手順は、Institutional Animal Care and Use Committees of Duke Universityによって承認され、The Guide to the Care and Use of Laboratory Animals,1996 Edition,National Research Councilに従って行なわれた。
EおよびQの関数として調整されたEMGの積分の動員曲線は、ランダムな順序で、種々のPW(0.02、0.05、0.1、0.2、0.5、および1ms)における矩形、減衰指数関数(時定数[τ]=132、263、および526μs)、およびGA波形に対して測定された。実験の過程中、頻繁な間隔で、固定PWの矩形波形による刺激が、閾値におけるシフトを監視するために提供された。閾値シフトは、1匹の動物においてのみ生じ、EおよびQの値は、適宜、スケーリングされた。動員曲線は、計算モデルにおけるように、類似手順を使用して生成された。すなわち、刺激振幅が増分され、3つの刺激パルスが、各増分において、〜1Hzで送達され、E、Q、およびEMG積分の平均値が記録された。各動員曲線から、最大EMGの50%を生成するために要求されるEおよびQの値を計算し、矩形波形に対するPW=0.02msにおける値が基礎値として定義された。続いて、EおよびQの全値が、そのそれぞれの基礎値に正規化され、実験中の平均および標準誤差を計算した。
GA波形の効率を矩形および減衰指数関数波形と比較する生体内測定値は、概して、集団モデルの結果を裏付けた。PW≧0.05msに対して、GA波形は、矩形および減衰指数関数波形の大部分よりも有意にエネルギー効率的であった(p<0.05、FPLSD)(図12Aおよび12B参照)。τ=132μsを有する減衰指数関数は、PW≧0.5msに対するGA波形よりもエネルギー効率的であると考えられたが、この結果は、異なっていた。すなわち、長いPWの場合、指数関数波形に対してPWを増加させることは、単に、興奮に及ぼす無視可能な影響を有する低振幅の末尾を延在させるだけである。その結果、指数関数波形に対するエネルギー持続時間曲線は、長いPWにおいて横ばい状態になった一方、GA波形に対するエネルギー持続時間曲線は、集団モデルにおけるように、PWとともに増加した。正規化されたEが正規化されたQに対してプロットされると、GA波形は、正規化されたQ>2に対して、矩形波形よりもエネルギー効率的であると考えられた(図12C参照)。しかしながら、GA波形は、実質的に、減衰指数関数波形よりもエネルギー効率的ではなかった。
元のGAは、単相性刺激に対してエネルギー最適の波形を呈した。しかしながら、神経刺激のために使用されるほとんどの波形は、二相性である。電荷回復パルスは、一次パルスの閾値に影響を及ぼし得るので(van den Honert and Mortimer 1979)、従来、単相性GA波形が、二相性刺激に対して依然としてエネルギー最適であるか否かは明白ではなかった。最初に、全波形形状に対する単一ファイバモデルにおいて、矩形電荷平衡アノード相を加えることによって閾値を再計算した。持続時間は、電荷平衡相のタイミング(カソード相前または後)と同様に、変動した(PWanodic/PWcathodic=1、5、または10)。アノード相の振幅は、波形全体に対してゼロ正味電荷を生成するように調節され、Eは、波形の両方の相から計算された。
